Candida glabrata infection of urinary bladder in a Chinchilla Persian cat

  • A 5-year-old castrated male Chinchilla Persian cat weighing 4.84 kg was referred for hematuria. The cat had a history of urethrostomy and bacterial cystitis. In urine culture, Candida glabrata was cultured on Sabouraud dextrose agar. Based on these results, the cat was diagnosed with Candida cystitis. Subsequently, oral administration of fluconazole was initiated. Urine culture was negative at 31 days after administration. This case describes the diagnosis and treatment of Candida glabrata infection of urinary bladder in a cat with a history of urethrostomy.

실험적 방광염 유발 토끼에서의 초음파 및 방사선학적 연구 (Ultrasonographic and radiographic study on experimental cystitis in rabbit)

  • To investigate diagnostic imaging of cystitis and to apply it to the small animal practice, ultrasonogram of urinary bladder with moderate distension(4ml/kg) and with complete distension(9ml/kg) to evaluate the irregularity and thickness of bladder, radiography and histopathological examination were performed after experimental cystitis induction. On double contrast cystography, mucosal membrane of the urinary bladder was smooth and thickening urinary bladder wall was not found before cystitis induction. At 3rd day post induction(PI), mucosal irregularity was noted at the cranioventral region of the urinary bladder. Thickening of the urinary bladder wall and filling defect was observed as well. Cystographic findings of 7, 10, 15, 21 day PI were similar to that of 3rd day PI. On ultrasonographic findings of the mural thickness in induction group, it was ascertain that the mural thickness with moderate distention was more thickened than with complete distention at transverse scan. Ultrasonographic findings at longitudinal scan were similar to those of transverse scan. On ultrasonographic findings of mucosal irregularity in induction group, from PI to 7 day PI, mucosal irregularity with moderate distention was more irregular than mucosal irregularity with complete distention. At 10 day PI, there was similarity between moderate distention and complete distention. On histopathologic examination of a section of urinary bladder taken 3 day PI, the mucosal and submucosa were infiltrated by a mixture of thick inflammatory exudate which was composed of neutrophil, plasma protein, bacterial colony and necrotic cells. Congestion, hemorrhage and edema were also observed in the submucosa. At 7th day PI, the mucosal change was similar to that of 3rd day PI, but neovascularization and fibroplasia were observed in the submucosa. At 15th and 21th day PI, mild hyperplasia of mucosal epithelium was observed in the mucosa. Deposition of collagen, neovascularization and severe diffuse infiltration of lymphocyte were observed. These results suggest that ultrasonographic examination with moderate distention is considered to be a more simple, rapid, noninvasive, sensitive and useful diagnostic method than other diagnostic methods for the diagnosis of the cystitis and the differential diagnosis of urinary tract infection.

Successful EDTA-Tris treatment of Pseudomonas aeruginosa infection of urinary bladder secondary to urolithiasis in a dog

  • About 8 year-old castrated male Yorkshire terrier was presented for evaluation of dysuria, stranguria, hemtauria, and pollakiuria. On history taking, dysuria first was observed three months ago and these signs were waxed and waned. Physical examination revealed mild left perineal swelling. On routine laboratory examination, no significant findings were identified. Positive contrast urogram identified peritoneal herniation of urinary bladder. Urinalysis showed proteinuria and hematuria. Urine sediment revealed epithelial cells, white blood cells and rod-shaped bacteria. Pseudomonas aeroginosa was isolated from urine obtained through cystocentesis, and had resistance against fourteen antibiotics. Cystitis caused by P. aeruginosa concurrent with cystolithiasis and perineal hernia was diagnosed. Cystotomy, herniorrhaphy and EDTA-Tris solution lavage of bladder were performed. The patient was recovered to normal condition 2 days after treatment. Two weeks later, bacterial culture of urine was negative and any abnormality in ultrasonogram and urinalysis was not observed except calcium oxalate dihydrate crystals.

New Insights for Febrile Urinary Tract Infection (Acute Pyelonephritis) in Children

  • Although asymptomatic bacteriuria, cystitis, and acute pyelonephritis (APN) have been categorized as urinary tract infections (UTIs), the immunopathogenesis of each disease is different. APN shows an age predilection; the majority of children (over 70-80%) with APN are under 1-2 years of age, with a male predominance. After 1-2 years of age, female predominance has been reported. This finding suggests that the immature immune state of infancy may be associated with the pathogenesis of APN. Escherichia coli is the most common etiologic agent; other uropathogens associated with UTIs originate from the host and comprise normal flora that are continuously altered by environmental factors. Therefore, uropathogens may have characteristics different from those of extraneous bacterial pathogens. Although antibiotic-resistant uropathogens, including extended-spectrum beta-lactamase-producing strains, are increasing in Korea and worldwide, treatment failure is rare in immune-competent children. The immunopathogenesis of APN remains unknown. Intact bacteria may not be the causative substances in renal cell injury; rather, smaller substances produced during bacterial replication may be responsible for renal cell injury and scarring. Moreover, substances from host cells such as proinflammatory cytokines may be involved in renal cell injury. A dimercaptosuccinic acid scan is used to detect the site of bacterial replication in the renal parenchyma, and may be influenced by the size of the focus and the stage of APN. Traditional aggressive studies used to identify vesicoureteral reflux after the first episode of APN have been modified because of rare cases of chronic kidney disease in patients with recurrent UTI.

  • Realizing the scope of utilizing by-products of silk cocoons by applying appropriate methods is the immediate crave to optimize returns. The nutritive value of pupae suits for human diet, feed for poultry, carps, fish, rabbits, piggery and dogs. The pupal skin, fat, oil, cocoon palade have applications in oleo chemical, soap, glycerin, cosmetic, artificial fibres, membranes and n-triacontanol isolation. The pupal proteins Chitin, Shinki fibroin, Serrapeptidase, glucosamine are latent precursors of post surgical, anti-carcinogenic, anti-inflammative, anti-bacterial, anti-histaminic, gastric, hepatitis, pancreatitis, leukocytopenia, neurological, ophthalmic, blood pressure, cardiac and diabetic medicines and for preparation of vitamins A, E and K. The silk and its proteins sericin and fibroin are potentially used for wound healing, diabetes, impotence, sinusitis, arthritis, edema, cystitis, epididymitis, tissue regeneration, cancer, post-surgical trauma and used as anti-oxidatives, bio-adhesives, ultra violet screens and bio-active textiles. The waste cocoons can be used in making art crafts like garlands, carpets, overcoats, decoratives and greeting cards. The in-depth research towards utility optimization and make aware this reality to sericulturists, reelers, weavers, traders, entrepreneurs, policy makers etc., is the upright want of the today's Sericulture industry.

소아 요로감염의 원인 Escherichia coli 균의 계통 분류와 독성인자 분석 (Phylogenetic Groups and Virulence Factors of Escherichia coli Causing Urinary Tract Infection in Children)

  • 목적: 요로감염은 소아에서 흔한 세균 감염이며, Escherichia coli가 주요 원인균이다. 본 연구는 우리나라에서 소아 요로감염을 일으키는 E. coli의 계통 분류와 독성인자를 분석하고자 하였다. 방법: 2010년 10월부터 2013년 4월까지 요로감염으로 입원한 33명의 소아 환자로부터 검출된 E. coli균주를 대상으로 하였다. 중합효소연쇄반응을 통해 E. coli의 계통 분류 및 5가지 독성인자(fimH, sfa, papA, hylA, and cnf1)를 조사하였다. E. coli의 분자유전학적 특징을 환자의 임상적 진단과 동반된 방광요관 역류에 따라 분석하였다. 결과: 대부분의 요로병원성 E. coli 는 계통 분류에서 B2군(84.8%)에 속했으며, 나머지는 모두 D군(15.2%)에 해당되었다. 독성인자는 fimH (100%), sfa (100%), hylA (63.6%), cnfI (63.6%), 그리고 papA (36.4%)의 분포를 보였다. 임상 진단에 따른 계통 분류에서 급성 신우신염의 경우 B2군이 92.3%, D군이 7.7%를 나타냈으며, 방광염에서는 B2군에서 57.1%, D2군은 42.9%였다. 독성인자는 양 군에서 비슷하게 분포하였다. 급성 신우신염에서 방광요관 역류의 유무에 따른 계통 분류의 분포에는 차이가 없었으나, 독성인자의 경우 papA 유전자가 방광요관 역류가 동반되지 않은 군에서보다 방광요관 역류 군에서 적게 나타났다(43.8% vs. 20.0%, P=0.399). 결론: 본 연구는 국내 소아 요로감염의 원인 E. coli 균주의 분자유전학적 역학 자료를 제시하였으며, 이 결과는 향후 소아 요로감염의 발생 기전을 이해하는 데 기초가 될 것으로 생각된다.
