Background: It is unclear as to whether the size ratio elastographic technique is useful for assessing ultrasound-detected ductal carcinoma-in-situ (DCIS) masses since they commonly lack a significant desmoplastic reaction. The objectives of this study were to determine the accuracy of this elastographic technique in DCIS and examine if there was any histopathological correlation with the grey-scale strain patterns. Materials and Methods: Female patients referred to the radiology department for image-guided breast biopsy were prospectively evaluated by ultrasound elastography prior to biopsy. Histological diagnosis was the gold standard. An elastographic size ratio of more than 1.1 was considered malignant. Elastographic strain patterns were assessed for correlation with the DCIS histological architectural patterns and nuclear grade. Results: There were 30 DCIS cases. Elastographic sensitivity for detection of malignancy was 86.7% (26/30). 10/30 (33.3%) DCIS masses demonstrated predominantly white elastographic strain patterns while 20/30 (66.7%) were predominantly black. There were 3 (10.0%) DCIS masses that showed had a co-existent bull's-eye sign and 7 (23.3%) other masses had a co-existent toothpaste sign, a strain pattern that has never been reported in the literature. Four out of 4/5 comedo DCIS showed a predominantly white strain pattern (p=0.031) while 6/7 cases with the toothpaste sign were papillary DCIS (p=0.031). There was no relationship between the strain pattern and the DCIS nuclear grade. Conclusions: The size ratio elastographic technique was found to be very sensitive for ultrasound-detected DCIS masses. While the elastographic grey-scale strain pattern should not be used for diagnostic purposes, it correlated well with the DCIS architecture.
Objective: Two control rooms of a petroleum complex were ergonomically evaluated for this study using diverse drawings of the rooms. The objective of the study is to show the evaluation process and its results, assessment method and essential elements for drawing-based ergonomic evaluation. Background: Ergonomics can enhance the working environment and safety for a control room when implemented effectively. A control room design was examined by ergonomists based on ergonomic principles and well-practiced standards. This study was conducted because a construction consortium wanted to perform an ergonomic evaluation of the control rooms before the construction. Method: A simple evaluation process, composed with a pre-evaluation, an evaluation and a post-evaluation, was employed to carry out the study. ISO 11064, NUREG-0700, NUREG/CR-6150, and CRIOP were reviewed to evaluate the control rooms. The assessment ranges over the control room layout, workstations, display and control, environmental requirements and safety. Three ergonomists participated in the study and performed the evaluation for two months. Results: An experienced consortium of construction companies designed the petroleum complex and control rooms. No significant ergonomic design problems were detected. Quantitative recommendations were provided for the layout and workstation dimensions. Specific design directions were also proposed for environmental requirements and safety. Conclusion and Application: Design advice and minor potential problems were reported as results of the drawing-based ergonomic evaluation. More practical suggestions could be additionally provided if an on-site assessment was conducted. However, the ergonomic evaluation used in this study could be helpful and applied to designing and evaluating other various control rooms prior to their construction.

Achieving consistent geometric design is an important goal in highway design to ensure obtaining safe, economical and smooth traffic operation. Most evaluation of consistency is based on 'speed change' in speed profile. According to literature, the speed depends on geometric elements, speed on tangent section prior to a curve, and background around roads. Especially, the radius is the most main element mentioned in various literature. Therefore, this paper shows two ways of calculating horizontal radius on real road, that is, three-dimensional road. First of all, the radius of horizontal curve is calculated based on physical method. The calculated radius contains not only superelevation but also longitudinal grade while the current minimum radius is calculated by considering superelevation and side friction according to the point-mass equation. Secondly, the problem of composed curves with distorted appearance by overlaying sag or crest vertical alignment has been known. To quantify the extent of distortion effects, the method of calculation of real seen so called 'Perspective Radius' is developed. The paper presents the perspective radius and recommended perspective radius.
As movies and dramas which show traditional clothes and fusion clothes are getting popular these days, lot of people are paying attention to these dress and ornaments. And as for the domestic collections which seem to reflect this trend, various fusion style clothes are getting widely adopted by people. Therefore, this study analyzed and considered the clothes designs when the country started to open itself to other countries, by selecting a movie titled "YMCA baseball team", a movie to ascertain vigorous historical verification, and by conducting the prior study and using the DVD capture program. This study is meaningful in that based on the comparison study on the costumes, it clarifies the degree of costumes(clothes) acceptance at the point of nation's opening to other countries, which is the source of fusion style, and further shed light on the role that the movie costumes will play. From this study, we can get the following conclusions. First, through the theoretical background it can be known that when the country opened itself to outside world the dress and ornaments of the time was adopted in a top-down way, namely, first adopted by the royal family and the upper class, which could have easy access to the western civilization and institutions, and then the low class later. Second, in the movie titled 'YMCA baseball team' the costumes were designed through the strict and thorough historic verification, and it can be confirmed that various dress and ornaments adopted at the time of nation's opening to the outside world centered around the clothes design for new women. Third, 'YMCA baseball team is evaluated as a movie that reproduces that time well and as a media that provides an effective clue to the dress and ornaments adopted by the public when the country opened itself to the outside world.
Background: As an attempt to develop a strategy to improve the protective immune response to GM-CSF-secreting CT26 (GM-CSF/CT26) tumor vaccine, we have investigated whether the apoptogenic treatment of GM-CSF/CT26 prior to vaccination enhances the induction of anti-tumor immune response in mouse model. Methods: A carcinogeninduced mouse colorectal tumor, CT26 was transfected with GM-CSF gene using a retroviral vector to generate GM-CSF-secreting CT26 (CT26/GM-CSF). The CT26/GM-CSF was treated with ${\gamma}$-irradiation or mitomycin C to induce apoptosis and vaccinated into BALB/c mice. After 7 days, the mice were injected with a lethal dose of challenge live CT26 cells to examine the protective effect of tumor vaccination in vivo. Results: Although both apoptotic and necrotic CT26/GM-CSF vaccines were able to enhance anti-tumor immune response, apoptotic CT26/GM-CSF induced by pretreatment with ${\gamma}$-irradiation (50,000 rads) was the most potent in generating the anti-tumor immunity, and thus 100% of mice vaccinated with the apoptotic cells remained tumor free for more than 60 days after tumor challenge. Conclusion: Apoptogenic pretreatment of GM-CSF-secreting CT26 tumor vaccine by ${\gamma}$-irradiation (50,000 rads) resulted in a significant enhancement in inducing the protective anti-tumor immunity. A rapid induction of apoptosis of CT26/GM-CSF tumor vaccine at the vaccine site might be critical for the enhancement in anti-tumor immune response to tumor vaccine.

Background: Fibroblast functions both as a structural element and as a vital immunoregulatory cell. Fibroblasts regulate inflammation through governing of chemokine expression. In order to elucidate the mechanisms by which the expressions of chemokines were regulated, the co-stimulatory effects of Th1 and proinflammatory cytokines were compared using nasal mucosal fibroblasts. Methods: Human nasal mucosa was obtained from surgery for septal deviation and the growth of fibroblasts was established. Fibroblasts from 4th to 6th passage were stimulated with various combinations of cytokines. To inhibit selected signaling pathways, fibroblasts were pretreated with cyclosporin A, wortmannin, staurosporine, and dexamethasone prior to the stimulation with cytokines. The supernatants were collected and chemokines were detected with a sandwich enzyme-linked immunosorbent assay. Results: $TNF-{\alpha}/IFN-{\gamma}$-induced production of RANTES was inhibited by all inhibitors used. MCP-1 was produced constitutively and $TNF-{\alpha}$-induced or $TNF-{\alpha}/IFN-{\gamma}$-induced production of MCP-1 was not inhibited by cyclosporin A or wortmannin, but by stauroporine or dexamethasone. All inhibitors used in this experiment inhibited $TNF-{\alpha}/IFN-{\gamma}$-induced or $IL-1{\beta}/IFN-{\gamma}$-induced production of MCP-2 in nasal mucosal fibroblasts. Although staurosporine or dexamethasone showed strong inhibitory effects, cyclosporin A or wortmannin did not inhibit the production of MCP-3 by $IL-1{\beta}/IFN-{\gamma}$ treatment. Conclusion: Chemokines were strongly induced by stimulation of cytokines in combination and showed different pattern of inhibition by the inhibitors. Therefore, it was assumed that cytokines acted on multiple pathways or on unknown pathways which converged to gene-specific transcription factors.
Background: This study aimed to investigate the distribution and prevalence of intraosseous loop (anastomosis between posterior superior alveolar artery and infraorbital artery) in Koreans detected on computed tomography (CT) images taken prior to sinus augmentation surgery. Methods: From the 177 patients who underwent sinus augmentation with lateral approach at Ewha Womans University Department of Implant Dentistry, 284 CT scans were evaluated. The canal height (CH), ridge height (RH), and canal height from the sinus floor (CHS) were measured on para-axial views at the first premolar, first molar, and second molar. The horizontal positions of the bony canals in the lateral wall were also classified. One-way analysis of variance (ANOVA) and t test were used to estimate the statistical differences (p < 0.05). Results: The intraosseous loops were detected in 92 CT scans (32 %). The mean vertical height of the bony canals from the alveolar crest (CH) was $23.45{\pm}2.81$, $15.92{\pm}2.65$, and $16.61{\pm}2.92mm$ at the second premolar, first molar, and second molar, respectively. In the horizontal positions of the bony canals, intraosseous type was the most predominant. The canal heights more than 15 mm and less than 17 mm were most prevalent (33.7 %) and those under 13 mm were 12.0 %. Conclusions: The radiographic findings in this study could be used to decide the lateral osteotomy line avoiding potential vascular complication. However, only one third of the canals could be detected in CT scans; a precaution should be taken for the possibility of severe bleeding during lateral osteotomy.
Background. Although there have been a great number of research studies based on the model of uncertainty in illness, few studies have considered the appraisal portion of model. Purpose. The purpose of this study was to test the mediating effect of appraisal in the model of uncertainty in illness. Additionally, this study aimed to examine the relationships among uncertainty, symptom severity, appraisal, and anxiety in patients newly diagnosed with atrial fibrillation. Methods. This study employed a descriptive correlational and cross-sectional survey design using a face-to-face interview method. Patients diagnosed with atrial fibrillation within the previous 6 months prior to data collection were interviewed by Mishel Uncertainty in Illness Scale-Community Form, appraisal scale, Symptom Checklist-Severity V.3, and State Anxiety Inventory. Results. A total of 81 patients with atrial fibrillation were recruited from two large urban medical centers in Cleveland, Ohio, U.S.A.. Symptom severity was the significant variable in explaining uncertainty ($\beta$=0.34). Individuals with greater symptom severity perceived more uncertainty. Uncertainty was appraised as a danger rather than opportunity, and those with greater uncertainty appraised a greater danger (p<.0l). While the appraisal of opportunity had the negative relationship with anxiety (r=-0.25), the appraisal of danger was positively associated with anxiety (r=0.78). The measure of goodness of fit (Q) of the model was .7863, and the significant test (X$^2$) for the Q was statistically significant (df =3, p<.00l). Accordingly, the overall mediating model of uncertainty in illness was proven not to be fit to the empirical data of patients with atrial fibrillation. Consequently, the mediating effect of appraisal was not supported by the empirical data of this study. Conclusion. The findings of this study were discussed in terms of their relevance compared with those of previous studies or theoretical framework and the plausible explanations on study findings. Lastly, in order to expand the present body of knowledge on uncertainty in illness model, recommendations for the future nursing studies were included.
Background: 6-year College of Pharmacy curriculum had started in Korea, and the students in college of pharmacy are supposed to have student practice in the hospital pharmacy, community pharmacy, pharmaceutical company and administration to experience the role of pharmacists in advance. However, despite Korean Association of Pharmacy Education provided its own teaching plan, most Pharmacy Schools and the hospital pharmacy have difficulty in performing the desirable student practice program because they seldom experienced it. So, we reported the student practice program in the hospital pharmacy conducted by Yonsei University College of Pharmacy prior to the other numerous universities and the evaluation of survey on the student pharmacy practice program. Method: Severance Hospital, Gangnam Severance Hospital and National Health Insurance Service Ilsan Hospital took part in the student pharmacy practice program of Yonsei University. Students took 8-week pharmacy practice in Severance Hospital or Gangnam Severance Hospital plus 4-week pharmacy practice in National Health Insurance Service Ilsan Hospital. Also, students had once-a-week presentation class at school. A survey was conducted to evaluate the student practice program. Results: The presentation class was considerably helpful to share their own experiences at different practice sites in different hospitals, but there were some disadvantages that students could not experience the specific pharmacy practice site on the day of once-a-week presentation at school and so on. The results of the survey on the student practice program reported that students were satisfied with the overall practice program and presentation class at school. Also, they answered that the student practice program in the hospital pharmacy was significantly helpful for planning of the future career. Conclusion: Through the performance of the student practice program in Yonsei University, the adjustment of the student practice program in the hospital is planned to provide better experience of practice to the students and the results will be helpful for the student practice in the hospital of the other colleges of pharmacy.
Background: Same-day sputum microcopy is recommended in areas where sputum smear microscopy external quality assessment (EQA) is effectively implemented and sturdy. In Addis Ababa, the status of EQA and drop-out of same-day sputum smear microcopy has not yet been assessed. The objective of this study was to assess the quality of same-day sputum smear microscopy and diagnostic drop-out of presumptive tuberculosis (TB) patients in health facilities (HFs) across Addis Ababa, Ethiopia. Methods: Amulti-analysis was conducted from September 2016 to July 2017 to determine the status of external quality assessment and diagnostic drop-out of presumptive TB patients registered for same-day sputum smear microscopy. Data was coded and entered in Microsoft Excel, and subsequently transferred and analyzed using SPSS version 20.0. Results: The drop-out of same-day sputum smear microscopy was 209 (6.2%). More than 33% of the specimens collected for purposes of same-day sputum smears were of poor quality. Among the selected HFs for the study: 13 (46.4%) used filter reagents prior to sputum smear staining while 75% of the selected HFs for the study used smear microscopy services interruption in a year. The sensitivity and specificity of the HFs participating in regional quality assessment scheme for the diagnosis of TB was 97.4% and 99.6%, respectively. Conclusion: The diagnostic drop-out of same-day sputum smear microscopy was high in Addis Ababa. Strengthening EQA, competency-based laboratory professionals training on sputum smear microscopy might reduce the reading errors in sputum smear. Awareness creation of the community on the benefits gained from completion of specimen provision for the same-day approach decreases diagnostic drop-out and enhances TB control program.
