The impact of modern airport security protocols on patients with total shoulder replacements

  • Background: Advancements in airport screening measures in response to 9/11 have resulted in increased false alarm rates for patients with orthopedic and metal implants. With the implementation of millimeter-wave scanning technology, it is important to assess the changes in airport screening experiences of patients who underwent total shoulder arthroplasty (TSA). Methods: Here, 197 patients with prior anatomic and reverse TSA completed between 2013 and 2020 responded to a questionnaire regarding their experiences with airport travel screening after their operation. Of these patients, 86 (44%) stated that they had traveled by plane, while 111 (56%) had not. The questionnaire addressed several measures including the number of domestic and international flights following the operation, number of false alarm screenings by the millimeter-wave scanner, patient body habitus, and presence of additional metal implants. Results: A total of 53 patients (62%) responded "yes" to false screening alarms due to shoulder arthroplasty. The odds of a false screening alarm for patients with other metal implants was 5.87 times that of a false screening alarm for patients with no other metal implants (P<0.1). Of a reported 662 flights, 303 (45.8%) resulted in false screening alarms. Greater body mass index was not significantly lower in patients who experienced false screening alarms (P=0.30). Conclusions: Patients with anatomic and reverse TSA trigger false alarms with millimeter-wave scanners during airport screening at rates consistent with prior reports following 9/11. Patient education on the possibility of false alarms during airport screening is important until improvements in implant identification are made. Level of evidence: IV.

Relationship between chronotype and depressive symptoms among newly hired hospital nurses in the Republic of Korea

  • Background: This study was conducted to examine the relationship between chronotype and depressive symptoms to provide grounded knowledge in establishing nurses' health promotion strategies. Methods: The subjects of this study were 493 newly hired nurses working in 2 general hospitals within the university from September 2018 to September 2020. Sociodemographic and work-related characteristics were collected from a medical examination database and a self-reported questionnaire. These included sex, age, marital status, living situation, education level, alcohol consumption, physical activity, prior work experience before 3 months, workplace, and departments. To analyze the associations between the chronotype and depressive symptoms, multiple logistic regression analyses were performed to calculate odds ratios (ORs). Results: Among participants, 9.1% had depressive symptoms and 16.4% had insomnia. The subjects are divided into morningness (30.2%), intermediate (48.7%), and eveningness (21.1%). The multiple logistic regression analysis controlling for age, living status, education level, alcohol consumption, physical activity, workplace, prior work experience before 3 months, and insomnia, revealed that the OR of depressive symptoms in the eveningness group was 3.71 (95% confidence interval [CI]: 1.50-9.18) compared to the morningness group, and the R2 value was 0.151. It also can be confirmed that insomnia symptoms have a statistically significant effect on depressive symptoms (OR: 2.16, 95% CI: 1.03-4.52). Conclusions: Our findings suggest that evening-type nurses are more likely to have depression than morning-type nurses. We should consider interventions in a high-risk group such as the evening type nurses to reduce depressive symptoms in nurses.

Impact of Neoadjuvant Chemotherapy and Preoperative Irradiation on Early Complications in Direct-to-Implant Breast Reconstruction

  • Background Impact of previous radiation therapy and neoadjuvant chemotherapy (NACT) on early complications in direct-to-implant (DTI) breast reconstruction has not been elucidated. This study investigated whether DTI reconstruction is viable in patients with NACT or a history of preoperative chest wall irradiation. Methods Medical records of breast cancer patients who underwent nipple-sparing or skin-sparing mastectomy with DTI breast reconstruction from March 2018 to February 2021, with at least 1 year of follow-up in a single tertiary center, were reviewed. Demographic data, intraoperative details, and postoperative complications, including full-thickness necrosis, infection, and removal, were reviewed. Risk factors suggested by previous literature, including NACT and preoperative chest wall irradiation histories, were reviewed by multivariate analysis. Results A total of 206 breast cancer patients were included, of which, 9 were bilateral, 8 patients (3.9%) had a history of prior chest wall irradiation, and 17 (8.6%) received NACT. From 215 cases, 11 cases (5.1%) required surgical intervention for full-thickness necrosis, while intravenous antibiotics or hospitalization was needed in 11 cases (5.1%), with 14 cases of failure (6.5%) reported. Using multivariable analysis, preoperative irradiation was found to significantly increase the risk of full-thickness skin necrosis (OR = 12.14, p = 0.034), and reconstruction failure (OR = 13.14, p = 0.005). NACT was not a significant risk factor in any of the above complications. Conclusion DTI breast reconstruction is a viable option for patients who have received NACT, although reconstructive options should be carefully explored for patients with a history of breast irradiation.

Preconditioning Local Injection of Activated Platelet-Rich Plasma Increases Angiogenesis, VEGF Levels, and Viability of Modified McFarlane Flap in Diabetes-Induced Rats

  • Background The risk of flap necrosis in tissue reconstruction surgery is elevated in patients with vascular disorders, such as diabetes mellitus. Chronic hyperglycemia causes endothelial cell dysfunction and increases inflammatory process, causing vascular insufficiency. Platelet-rich plasma (PRP) contains high levels of platelets, growth factors, and fibrinogens. Its regenerative properties spark interest in supporting flap survival in relation to diabetic complications. Methods Thirty Wistar rats were divided into three groups. The first group included diabetic rats without PRP injection, which underwent flap procedure. The second group included diabetes-induced rats receiving PRP subcutaneous injection 1 day prior to flap procedure. The third group included nondiabetic rats receiving PRP injection 1 day prior to flap procedure. Flap tissue samples were taken on the seventh day to measure vascular endothelial growth factor (VEGF) levels using enzyme-linked immunosorbent assay method; angiogenesis and collagen density were measured from histopathology examination, and flap viability was analyzed using digital measurements. Results Analysis showed that flap viability, angiogenesis, and VEGF levels were significantly higher in the PRP-injected diabetic rats compared with diabetic rats that did not receive PRP. The levels of VEGF, angiogenesis, and viability of flaps in diabetic rats given PRP did not differ significantly compared with nondiabetic rats that received PRP. Conclusion Flap preconditioning through local injection of activated PRP enhances flap viability, VEGF levels and angiogenesis, in random skin flaps in diabetic rats, to the level where it does not differ significantly to nondiabetic rats that were given PRP.

A License Plate Recognition System Robust to Vehicle Location and Viewing Angle (영상 내 차량의 위치 및 촬영 각도에 강인한 차량 번호판 인식 시스템)

  • Recently, various attempts have been made to apply Intelligent Transportation System under various environments and conditions. Consequently, an accurate license plate recognition regardless of vehicle location and viewing angle is required. In this paper, we propose a novel license plate recognition system which exploits a) the format of license plates to remove false candidates of license plates and to extract characters in license plates and b) the characteristics of Hangul for accurate character recognition. In order to eliminate false candidates of license plates, the proposed method first aligns the candidates of license plates horizontally, and compares the position and the shape of objects in each candidate with the prior information of license plates provided by Korean Ministry of Construction & Transportation. The prior information such as aspect ratio, background color, projection image is also used to extract characters in license plates accurately applying an improved local binarization considering luminance variation of license plates. In case of recognizing Hangul in license plates, they are initially grouped according to their shape similarity. Then a super-class method, a hierarchical analysis based on key feature points is applied to recognize Hangul accurately. The proposed method was verified with high recognition rate regardless of background image, which eventually proves that the proposed LPR system has high performance regardless of the vehicle location or viewing angle.

Comparative Analysis of ICT Accessibility and Usability of Korean Students Based on PISA 2015 and 2018 Data

  • The OECD surveys the questionnaire on the background of ICT every three years since 2003. In this study, we compare and analyze the changes in ICT accessibility and ICT usability of Korean students in 2015 and 2018 using the ICT background data of OECD PISA. ICT accessibility refers to the degree of access to ICT equipment at school or at home. There are 10 items of accessibility surveys at schools used by PISA, and 11 items at home. ICT usability refers to how much ICT equipment is used at or outside of school for learning or non-learning purposes. There are ten items for surveying usability at school, and twenty-four items for surveying usability outside of school. In the analysis of this study, the arithmetic mean of the items is used. As a result, Korean students' accessibility at school improved from 25.48% in 2015 to 40.40% in 2018, and from the lowest group in 2015 to below-average group in 2018. In terms of students' usability at school, we analyzed the percentage of students who use the ICT equipment 'almost every day' and the students who use it 'everyday'. Both 2015 and 2018 are among the lowest. Accessibility at home is higher in 2018 than in 2015, and belongs to the average group of OECD countries in 2018. ICT Usability for learning purposes outside of school is the lowest group in both 2015 and 2018. ICT Usability outside of school for non-learning purposes is in the average group of OECD countries. As a result of this study, we can see that Korea's digital literacy education is weak compared to other OECD countries. Prior to education about the Fourth Industrial Revolution, investment in digital literacy education is needed.

  • The purpose of this study is to examine the theoretical background of art make-up and surrealism in depth and shed new light on creative thoughts coming through application of Depaysement and various kinds of expression realized through art make-up, where art and make-up are incorporated. The study method is to conduct prior research followed by a theoretical study of the concept and techniques of Surrealistic Depaysement and theoretical background of art make-up through specialized books and internet sources and then to create 4 artworks representing characteristics of Depaysement technique categorized in dual image, fusion of heterogeneous objects, modification and conversion, and change of space. This examination resulted in first, a foundational ground for fusion and use of art and make-up sectors by creating a realm of expression based on creativity through incorporation of art and makeup and second, a sense of freshness and unfamiliar-ness by creation of an image in a completely unfamiliar space. Therefore, the researcher sets a direction for ways to come up with new ideas and expand the realm of expression and hopes that the art-makeup does not just become an aesthetic tool but turn itself as an art genre.

Non-invasive Fdlow-up of Pulmonary artey by EBT Other Palliatrve Shunt Operatin (청색증형 선천성 심질환아의 고식적 수술 이후 EBT에 의한 폐혈관계이 비침습적 추적 검사)

  • Background :To assess the accuracy of Electron-Beam Tomography(EBT) in following evaluation of the pulmonary vascular system after a shunt operation in the cyanotic con-genital heart disease with pulmonary stenosis or pulmonary atresia. Material and Method : Sixteen patients(M:F=11:5) who received Blalock-Taussig(n=8) bidirectional cavo-pulmonary shunt(n=10) and unifocalization (n=2) were ncluded in the study. We evaluated the patency of the shunt the morphology of intrapericardial and hilar pulmonary arteries(PA) peripheral pulmonary vascularity by background lung attenuation and the abundance of arterial & venous collateral. Angiography(n=12) and echocardiography(n=20) were used as the gold standard for the comparison of EBT results. Result: EBT was consistent with angiogram/ echo in 100% of the evaluation for the patency of the shunt and in 12(by angiogram 100%) and 19(by echo 95%) for the detection the hypoplasia stenosis or interruption of central PA In measuring of PA EBT and angiogram corrlated(r=0.91) better than EBT-echo(r=0.88) or echo-angiogram(r=0.72) Abundant systemic arterial collateral were noted in 4 and venous collateral in 3 cases. In evaluating the peripheral pulmonary vascularity the homogenous and normal-ranged lung attenuation(m=6) decreased but homo-genous attenuation(n=1) segment-by-sgment heterogeneous attenuation(n=3) homogenous but asymmetrical attenuation(n=3) segment-by-segment heterogeneous attenuation(n=3) homogenous but asymmetrical attenuation(n=3) and venous congestion(n=2) were observed nd 12 of them were compatible with the blood flow pattern revealed by cardiac catheterization. Conclusion: EBT was accurate in the integrated evaluation of the pulmonary vascular system after the shunt including the patency of the shunt operaion the morphology and dimension of the central and hilar PAs and the loco-regional pulmonary flow in the lung parenchyma. It suggests the useful information about the need of secondary shunt operation the proper timing time for total repair and the need of interventional procedure prior to total repair.

  • This study is to analyze works of Memphis design in the 1980s, which captured the appearance of a diversified society since the 20th century, and to present practical designs that can communicate with the public through the formulation of indoor lighting. As a research method, the background of the occurrence of Memphis groups, the theoretical background and the formative features of Memphis designs are considered around literature and prior research. The research applied Memphis's formative characteristics, colors and patterns to incorporate newly formulated designs into lighting. Lighting lamps in the forms that have made small changes to basic shapes show not only its functionality and decorability, but also its scalability as a formative language that conveys the emotions contained within humans. In addition, the choice of materials according to the changes in sculpture in the production of the work will suggest the possibility of creating a new formative art.

Comparison of Clinical Significance Between Chest CT Scan and Bronchoscopy Prior to Bronchial Artery and Outcome of Embolization in Patients with Hemoptysis (객혈환자의 기관지동맥 조영술 전 흉부 전산화 단층촬영과 기관지내시경 검사의 유용성 비교 및 색전술 성적)

  • Background : Emergency management in hemoptysis is bronchial artery angiography and embolization. This study was designed to investigate the accuracy of localization of bleeding site by simple roentgenogram, computed tomography(CT) and bronchoscopy prior to embolization and to evaluate the outcome of embolotherapy. Method : We retrospectively evaluated 50 patients performed bronchial artery embolization(BAE), admitted to tertiary university hospital due to hemoptysis. Results : The most common causes were pulmonary tuberculosis, old tuberculous related parenchymal damage, aspergilloma, and bronchiectasis. The success rate of BAE within one month was 90%; within 3 months was 88%; during follow up period of mean 11.6 months was 76%. The concordant rate of simple roentgenogram with angiographic outcome in terms of bleeding site is 70%; in chest CT 80%; in bronchoscopy 81%; in combined information of simple roentgenogram and CT 83%; in combined information of simple roentgenogram and bronchoscopy 78%. Conclusion : The diagnostic accuracy for the bleeding site was similar between chest CT and bronchoscopy, showing high diagnostic yield. The success rate of BAE was comparative to prior studies. Further study will be needed in a large scale in near future.
