  • Background and Purpose: The greater part of patients to visit Hospital of Oriental Medicine suffer from cerebrovascular accident(CVA). There is transcranial Doppler(TCD) in the diagnostic method to confirm cerebrovascular accident(CVA). Transcranial Doppler(TCD) is an accurate method of monitoring the blood flow velocities of the cerebral vessels and have been generally used to prevent symptomatic vasospasm and confirm cerebral infarction. So we studied, in the crebrovascular accident(CVA), to estimate whether transcranial Doppler(TCD) is useful to. Methods: Using transcranial doppler(Multigon Model 500M Transcranial Doppler System), we measured the mean and peak velocity and the direction of blood flow in 10 cerebrovascular accident(CVA)'s subjects who had been examined by Computed Tomography(CT) or Magnetic Resonance Imaging (MRI). Results : As an anticipation, in cerebrovascular accident(CVA)'s subject with Cb-infarction, the mean and peak velocity of blood flow fell down remarkably and the direction of blood flow was change or irritable. But didn't find out any signal in lacunar infarction. Also, in case with spontaneous hemorrhage, the velocity and direction of blood flow was change but this signal was short of diagnosis for Cb-hemorrhage. Besides, we found signals about embolism, stenosis, thrombosis and occlusion in cerebrovascular accident(CVA)'s subjects. Conclusion: In Cb-infarction, the result of TCD was equal to diagnosis with Computed Tomography(CT) or Magnetic Resonance Imaging (MRI). But about lacunar infarction or spontaneous hemorrhage, signals of TCD couldn't be found out or was insufficient more than Computed Tomography(CT) or Magnetic Resonance Imaging(MRI). In cerebrovascular accident(CVA)'s subject with embolism, stenosis, thrombosis or occlusion, signals of TCD were found out more than Computed Tomography(CT) or Magnetic Resonance Imaging(MRI). Therefore transcranial doppler(TCD) is necessary method which makes a diagnosis of cerebrovascular accident(CVA), with Computed Tomography(CT) or Magnetic Resonance Imaging(MRI).

Trend of Multidrug and Extensively Drug Resistant Tuberculosis in a Tuberculosis Referral Hospital, 2001~2005 (일개 결핵병원에서 다제내성결핵과 광범위내성결핵의 추이, 2001~2005)

  • Background: Multidrug-resistant tuberculosis (MDR-TB) and extensively drug resistant tuberculosis (XDR-TB) are serious threats to worldwide tuberculosis control, but the national burden and the trends of infectious spread are largely unknown. Methods: We retrospectively reviewed the results of drug sensitivity tests and medical records of patients that were diagnosed with culture-confirmed pulmonary tuberculosis and were admitted to the National Masan Tuberculosis Hospital between 2001 and 2005. Results: From 2001 to 2005, the proportion of MDR-TB among new cases was 9.2%, 13.8%, 16.9%, 23% and 27.0% in 2001, 2002, 2003, 2004 and 2005, respectively, and the proportion of MDR-TB among previously treated cases was 58.5%, 60.2%, 62.7%, 61.7% and 71.3% in 2001, 2002, 2003, 2004 and 2005, respectively. A significant increasing trend could be discerned for MDR-TB among both new and previously treated cases (p<0.001, p=0.002 for trend, respectively). The proportion of XDR-TB among new cases was 0%, 2.3%, 3.1%, 2.5% and 6.3% in 2001, 2002, 2003, 2004 and 2005, respectively, and the proportion of XDR-TB among previously treated cases was 9.1%, 15.7%, 17.3%, 19.9% and 19.1% in 2001, 2002, 2003, 2004 and 2005, respectively. A significant increasing trend could be discerned for XDR-TB among both new and previously treated cases (p=0.005, p<0.001 for trend, respectively). Conclusion: Both MDR-B and XDR-TB were gradually increased among both new and previously treated cases. Integrated national surveillance, including the public and private sectors, will be needed to estimate the exact status of antituberculous drug resistance.

Reduction of Outdoor and Indoor Ambient Dose Equivalent after Decontamination in the Fukushima Evacuation Zones

  • Background: One of the most urgent issues following the accident at the Fukushima Daiichi nuclear power plant (FDNPP) was the remediation of the land, in particular, for residential area contaminated by the radioactive materials discharged. In this study, the effect of decontamination on reduction of ambient dose equivalent outdoors and indoors was evaluated. The latter is essential for residents as most individuals spend a large portion of their time indoors. Materials and Methods: From December 2012 to November 2014, thirty-seven Japanese single-family detached wooden houses were investigated before and after decontamination in evacuation zones. Outdoor and indoor dose measurements (n = 84 and 114, respectively) were collected based on in situ measurements using the NaI (Tl) scintillation surveymeter. Results and Discussion: The outdoor ambient dose equivalents [$H^*(10)_{out}$] ranged from 0.61 to $3.71{\mu}Sv\;h^{-1}$ and from 0.23 to $1.32{\mu}Sv\;h^{-1}$ before and after decontamination, respectively. The indoor ambient dose equivalents [$H^*(10)_{in}$] ranged from 0.29 to $2.53{\mu}Sv\;h^{-1}$ and from 0.16 to $1.22{\mu}Sv\;h^{-1}$ before and after decontamination, respectively. The values of reduction efficiency (RE), defined as the ratio by which the radiation dose has been reduced via decontamination, were evaluated as $0.47{\pm}0.13$, $0.51{\pm}0.13$, and $0.58{\pm}0.08$ ($average{\pm}{\sigma}$) when $H^*(10)_{out}$ < $1.0{\mu}Sv\;h^{-1}$, $1.0{\mu}Sv\;h^{-1}$ < $H^*(10)_{out}$ < $2.0{\mu}Sv\;h^{-1}$, and $2.0{\mu}Sv\;h^{-1}$ < $H^*(10)_{out}$, respectively, indicating the values of RE increased as $H^*(10)_{out}$ increased. It was found that the values of RE were $0.53{\pm}0.12$ outdoors and $0.41{\pm}0.09$ indoors, respectively, indicating RE was larger outdoors than indoors. Conclusion: Indoor dose is essential as most individuals spend a large portion of their time indoors. The difference between outdoors and indoors should be considered carefully in order to estimate residents' exposure dose before their returning home.

Edge effects confirmed at the clear-cut area of Korean red pine forest in Uljin, eastern Korea

  • Background: Forest edges create distinctive ecological space as adjacent constituents, which distinguish between different ecosystems or land use types. These edges are made by anthropogenic or natural disturbance and affects both abiotic and biotic factors gradually. This study was carried out to assess edge effects on disturbed landscape at the pine-dominated clear-cut area in a genetic resources reserve in Uljin-gun, eastern Korea. This study aims to estimate the distance of edge influence by analyzing changes of abiotic and biotic factors along the distance from forest edge. Further, we recommend forest management strategy for sustaining healthy forest landscapes by reducing effects of deforestation. Results: Distance of edge effect based on the abiotic factors varied from 8.2 to 33.0 m. The distances were the longest in $Mg^{2+}$ content and total nitrogen, $K^+$, $Ca^{2+}$ contents, canopy openness, light intensity, air humidity, $Na^+$ content, and soil temperature followed. The result based on biotic factors varied from 6.8 to 29.5 m, coverage of tree species in the herb layer showed the longest distance and coverage of shrub plant in the herb layer, evenness, species diversity, total coverage of herb layer, and species richness followed. As the result of calculation of edge effect by synthesizing 26 factors measured in this study, the effect was shown from 11.0 m of the forest interior to 22.4 m of the open space. In the result of stand ordination, Rhododendron mucronulatum, R. schlippenbachii, and Fraxinus sieboldiana dominated arrangement of forest interior sites and Quercus mongolica, Vitis amurensis, and Rubus crataegifolius dominated spatial distribution of the open area plots. Conclusions: Forest interior habitat lies within the influence of both abiotic and biotic edge effects. Therefore, we need a forest management strategy to sustain the stability of the plant and further animal communities that depend on its stable conditions. For protecting forest interior, we recommend selective logging as a harvesting method for minimizing edge effects by anthropogenic disturbance. In fact, it was known that selective logging contributes to control light availability and wind regime, which are key factors affecting microclimate. In addition, ecological restoration applying protective planting for the remaining forest in the clear-cut area could contribute to prevent continuous disturbance in forest interior.

  • Background: Conscious sedation is a minimally depressed level of consciousness that retains the patient's ability to maintain the airway continuously and to respond appropriately to physical stimulation and verbal command at any time, produced by a pharmacologic or nonpharmacologic method or a combination thereof. In dental anesthesia, a state of sedation in which the conscious patient is rendered free of fear, apprehension, and anxiety through the use of pharmacologic agents. Midazolam is a useful drug of benzodiazepine on minor operation of dentistry. The purpose of this study is to estimate the appropriate dosage protocol of midazolam to the patient receiving intravenous sedation at the department of oral and maxillofacial surgery. Methods: Total subject was fifty-one patients and fifteen patients of this subject were decided to control group (Group I) that they were not received intravenous sedation. The experimental group was randomly subdivided into two groups and each of those groups was administrated to 0.05 mg/kg (Group II) and 0.08 mg/kg (Group III) each other. All patients of the subject were surveyed with postoperative one day questionaire to evaluate the level of sedation. Results: In view of antegrade amnesia, 1. Group II and III were statistically different compared to Group 1, respectively. 2. Group II and III were not statistically different. Conclusions: Apparent antegrade amnesia was found in both groups 0.05 mg/kg and 0.08 mg/kg intravenously administered. So we present the protocol of the department of oval and maxillofacial surgery, Kangnung National University Dental Hospital. But further studies concerning patient's age, body weight, and general condition will be carried out.

A Study on Estimate of Sediment Yield Using Tank Model in Oship River Mouth of East Coast (Tank 모형을 이용한 동해안 오십천 하구의 유사량 평가에 관한 연구)

  • BACKGROUND: A large scale of sediment load delivered from watershed causes substantial waterway damages and water quality degradation. Controlling sediment loading requires the knowledge of the soil erosion and sedimentation. The various factors such as watershed size, slope, climate, land use may affect sediment delivery processes. Traditionally sediment delivery ratio prediction equations have been developed by relating watershed characteristics to measured sediment yield divided by predicted gross erosion. However, sediment prediction equations have been developed for only a few regions because of limited sediment data. Besides, little research has been done on the prediction of sediment delivery ratio for asia monsoon period in mountainous watershed. METHODS AND RESULTS: In this study Tank model was expanded and applied for estimating sediment yield to Oship River of east coast. The rainfall-runoff in 2006 was verified using the Tank model and we derived good result between observed and calculated discharge in 2009 at the same conditions. In relation to sediment yield, the sediment delivery rate of 2006 was very high than 2009 regardless of methods for estimating sediment load. It was thought to be affected by heavy rainfall due to the typhoon. CONCLUSION(s): For estimating sediment volume from watershed, long-term monitoring data on discharge and sediment is needed. This model will be able to apply to predict discharge and sediment yield simultaneously in ungauged area. This approach is more effective and less expensive method than the traditional method which needs a lot of data collection.

In situ ruminal degradation characteristics of dry matter and crude protein from dried corn, high-protein corn, and wheat distillers grains

  • Background: The continuing growth of the ethanol industry has generated large amounts of various distillers grains co-products. These are characterized by a wide variation in chemical composition and ruminal degradability. Therefore, their precise formulation in the ruminant diet requires the systematic evaluation of their degradation profiles in the rumen. Methods: Three distillers grains plus soluble co-products (DDGS) namely, corn DDGS, high-protein corn DDGS (HP-DDGS), and wheat DDGS, were subjected to an in situ trial to determine the degradation kinetics of the dry matter (DM) and crude protein (CP). Soybean meal (SBM), a feed with highly degradable protein in the rumen, was included as the fourth feed. The four feeds were incubated in duplicate at each time point in the rumen of three ruminally cannulated Hanwoo cattle for 1, 2, 4, 6, 8, 12, 24, and 48 h. Results: Wheat DDGS had the highest filterable and soluble A fraction of its DM (37.2 %), but the lowest degradable B (49.5 %; P < 0.001) and an undegradable C fraction (13.3 %; P < 0.001). The filterable and soluble A fraction of CP was greatest with wheat DDGS, intermediate with corn DDGS, and lowest with HP-DDGS and SBM; however, the undegradable C fraction of CP was the greatest with HP-DDGS (41.2 %), intermediate with corn DDGS (2.7 %), and lowest with wheat DDGS and SMB (average 4.3 %). The degradation rate of degradable B fraction ($%\;h^{-1}$) was ranked from highest to lowest as follows for 1) DM: SBM (13.3), wheat DDGS (9.1), and corn DDGS and HP-DDGS (average 5.2); 2) CP: SBM (17.6), wheat DDGS (11.6), and corn DDGS and HP-DDGS (average 4.4). The in situ effective degradability of CP, assuming a passage rate of $0.06h^{-1}$, was the highest (P < 0.001) for SBM (73.9 %) and wheat DDGS (71.2 %), intermediate for corn DDGS (42.5 %), and the lowest for HP-DDGS (28.6 %), which suggests that corn DDGS and HP-DDGS are a good source of undegraded intake protein for ruminants. Conclusions: This study provided a comparative estimate of ruminal DM and CP degradation characteristics for three DDGS co-products and SBM, which might be useful for their inclusion in the diet according to the ruminally undegraded to degraded intake protein ratio.

The Correlation between Radiologic Findings and Clinicopathological Prognostic Factors in Small Peripheral Adenocarcinoma of Lung (말초 폐 발생 소형 선암에서 화상적 소견과 병리적, 임상적 예후와의 관계)

  • Background : Tumor size in lung cancer is not as good a prognostic factor for adenocarcinoma as it is for other types of lung cancer; therefore it is difficult to estimate the prognosis preoperative. However, there have recently been some reports on the radiologic findings correlating to the clinicopathologic prognostic factors in peripheral small adenocarcinoma of lung. We tried to evaluate the prognostic importance of High-Resolution CT (HRCT) findings of such adenoearcinoma, Material and Method: One houndred and seventy-six surgically resected small peripheral adenocarcinoma measuring 3 cm or less in greatest dimension were reviewed radiologically and clinicopathologically. Result: The patients with greater extent of ground-glass attenuation (GGA) had better clinico-pathological factors. The tumors with gross appearance of GGA or bubble-like shape showed better clinicopathological prognostic factors than scar-like or solid shape. Conclusion: HRCT findings of small peripheral adenocarcinomas of the lung correlated well with the histologic and clinical prognostic factors. We can predict the post-operative prognosis with the radiologic findings.

Korean Herbal Medicine on Liver Function : A Systematic Review in Korean Literature (한약 복용이 간기능에 미치는 영향 : 국내 문헌에 대한 체계적 고찰)

  • Background : The safety of Korean herbal Medicine (KHM: prescribed herbal medicine by doctors of traditional Korean medicine) is an important issue in Korea. Although both fields. western medicine and traditional Korean medicine. have been studied on the safety of herbs and KHM, their results were not concordance with each other. Objectives : This study aims to review the influence of KHM on liver function in Korean literature systematically. Additionally, we tried to estimate the change of data of liver function test (LFT) and the incidence of liver injury (LI) after the use of KHM. Methods : Systematic literature searches were performed on 4 major databases of Korea from their inception to May 2008. Screening and selection of the studies and the extraction of data were performed independently by two authors. There were no restrictions on the types of publication including grey literatures. Results : Forty studies were included. Only sixteen studies were performed prospectively and fifteen studies collected data from outpatients. Only 8 studies reported the occurrence of LI after the use of KHM. Nineteen cases of LI showed no or mild symptoms and the elevation of LFT was not high. All of LI patients used conventional western drug and KHM concomitantly. and causality of LI was not assessed properly. The incidence of LI related to the use of KHM was estimated as 0.59%-0.76% from all data of these studies. The conflicting results were shown on the change of alanine aminotransferase (ALT) and total bilirubin (TB) after the use of KHM. Conclusions : KHM might be a minor cause of LI in Korea. However the results are not strongly supported as enough to make the safety issue clear because of the limitations of original studies. More rigorous studies are required for answering the safety issue of KHM with the cooperative investigation of both fields of Korean traditional medicine and western medicine.

Anti-fibrotic effects of Orostachys japonicus A. Berger (Crassulaceae) on hepatic stellate cells and thioacetamide-induced fibrosis in rats

  • BACKGROUND/OBJECTIVE: Orostachys japonicus A. Berger (Crassulaceae) has been used in traditional herbal medicines in Korea and other Asian countries to treat various diseases, including liver disorders. In the present study, the anti-fibrotic effects of O. japonicus extract (OJE) in cellular and experimental hepatofibrotic rat models were investigated. MATERIALS/METHODS: An in vitro hepatic stellate cells (HSCs) system was used to estimate cell viability, cell cycle and apoptosis by MTT assay, flow cytometry, and Annexin V-FITC/PI staining techniques, respectively. In addition, thioacetamide (TAA)-induced liver fibrosis was established in Sprague Dawley rats. Briefly, animals were divided into five groups (n = 8): Control, TAA, OJE 10 (TAA with OJE 10 mg/kg), OJE 100 (TAA with OJE 100 mg/kg) and silymarin (TAA with Silymarin 50 mg/kg). Fibrosis was induced by treatment with TAA (200 mg/kg, i.p.) twice per week for 13 weeks, while OJE and silymarin were administered orally two times per week from week 7 to 13. The fibrotic related gene expression serum biomarkers glutathione and hydroxyproline were estimated by RT-PCR and spectrophotometry, respectively, using commercial kits. RESULTS: OJE (0.5 and 0.1 mg/ mL) and silymarin (0.05 mg/mL) treatment significantly (P < 0.01 and P < 0.001) induced apoptosis (16.95% and 27.48% for OJE and 25.87% for silymarin, respectively) in HSC-T6 cells when compared with the control group (9.09%). Further, rat primary HSCs showed changes in morphology in response to OJE 0.1 mg/mL treatment. In in vivo studies, OJE (10 and 100 mg/kg) treatment significantly ameliorated TAA-induced alterations in levels of serum biomarkers, fibrotic related gene expression, glutathione, and hydroxyproline (P < 0.05-P < 0.001) and rescued the histopathological changes. CONCLUSIONS: OJE can be developed as a potential agent for the treatment of hepatofibrosis.
