A Case of Acetaminophen-induced Acute Interstitial Nephritis Presenting with Acute Renal Failure (급성 신부전을 보인 아세트아미노펜에 의한 급성 간질성 신염 1례)

  • Acute interstital nephritis can occur by acetaminophen, but it is rarely presented as acute renal failure with azotemia. We report a case of acute interstitial nephritis induced by acetaminophen in a 14-year-old girl who developed non-oliguric acute renal failure. She has taken acetaminophen to control the persistent throat pain for the last two months. Renal biopsy revealed diffuse infiltration of mononuclear inflammatory cells admixed with eosinophils in the edematous interstitia. After the discontinuation of acetaminophen and the administration of corticosteroid, the serum creatinine level returned to normal.

  • A spayed female, 1-year-old, Himalayan, weighing 1.89 kg, was referred. The clinical signs included anorexia, depression, vomiting, urinary incontinence and vaginal discharge for 7 days after elective ovariohysterectomy. In laboratory examination, there was moderate azotemia. Abdominal radiographic findings included heterogeneous serosal detail loss and right renomegaly. Abdominal ultrasonography showed hyperechoic ascites, bilateral polycystic kidneys, right hydronephrosis, and right hydroureter. Excretory urography demonstrated an ureterovaginal fistula. Exploratory laparotomy confirmed this diagnosis and explained that the right ureter had been included in a ligature around the cranial vagina.

  • The effects of Soohwabunchungum on rats with nephrosis induced by a single tail-intravenous injection of PAN(puromycin aminonucleoside), 2.5mg/100g of body weight was evaluated in the present study. The effects of Soohwabunchungum on PAN nephrosis was evaluated by measuring the concentrations of albumin, total protein, total lipid, cholesterol, triglyceride, creatinine, BUN(blood urea nitrogen) and uric acid in the serum the amount of protein, creatinine, glucose, occult blood and volume of the 24 hours urine and the volume of intake water, To conclude, it can be inferred that Soohwabunchungum has the effects of improving proteinuria, hypoproteinemia, hyperlipidemia in nephrotic syndrome, and relieving azotemia when nephrotic syndrome is accompanied by the acute renal failure.

고양이에서 발생한 Perinephric pseudocysts와 선천성 Peritoneo-pericardial Kiaphragmatic Hernia in Cat

  • An 8-year old castrated domestic long-hair cat was presented with a two week history of abdominal distension. Physical examination revealed a non-painful, fluctuant, palpable mass in the right craniodorsal abdomen, and unilaterally muffled heart sounds on the right thorax. Routine clinico-pathological values were unremarkable apart from mild azotemia with a concurrent urine specific gravity of 1.031, which reflect a degree of renal dysfunction. Radiographic and ultrasound examinations of the thorax revealed the cardiac enlargement to be due to the congenital peritoneo-pericardial diaphragmatic hernia with liver occupying the right half of the pericardial sac. There was also a mild gypertrophy of the heart. Radiography and ultrasonography of the abdomen showed the mass to be composed of a large fluid filled cystic structures surrounding the right and left kidneys, and the kidneys themselves were of increased echogenecity. A diagnosis of perinephric pseudocysts was made. The patient responded well to the surgical procedures. Perinephric pseudocysts and peritoneo-pericardial diaphragmatic hernia in the cat are rare, and a case is described and the literature is reviewed in this report.

Primary renal fibrosarcoma with local invasion into the mesenteric membrane of a mongrel dog

  • A 14-year-old, 7.4 kg, neutered male mongrel dog presented with vomiting, anorexia, and hematuria starting 3 days prior to admission. Serum biochemical profiles indicated severe azotemia. Computed tomography revealed loss of normal left kidney structure. The organ was 1.5 to 2 times larger than the right kidney with mixed attenuation. Histopathologic examination was performed after nephrectomy. The renal mass and mesenteric membrane were positive for vimentin and stained blue with Masson's trichrome. In conclusion, this was a rare occurrence of primary renal fibrosarcoma, most likely originated from the renal capsule, with local invasion into the mesenteric membrane.

  • A 3-year-old female Cocker Spaniel dog acutely developed hepatopathy following ingestion of root of Cycad (Cycas revoluta). Seven hours after the ingestion, she showed acute continuous nausea, vomiting, and marked depression and was presented to the local veterinary clinic. At physical evaluation, the dog showed moderate weakness, pale mucous membrane, jaundice, and dehydration. Serum biochemical analysis revealed elevated alanine aminotransferase and aspartate aminotransferase, and mild azotemia. The dog was treated with intravenous fluid, antibiotics, and hepatic protectants for 10 days and recovered without any sequela. Although the ingestion of Cycad in dogs is rare, it can be treated successfully with appropriate supportive therapy.

  • The hemolytic uremic syndrome(HUS), a heterogenous group of disorders characterized by micorangiopathic hemolytic anemia, thrombocytopenia, and azotemia, is the most frequent cause of acute renal failure in children. The association of the HUS with E. coli O157 : H7 has been well-described, but the other intestinal infection have been relatively less reported to date. We report a 18-month-old boy presenting with typical clinical characteristics of HUS associated with amoebic dysentery with a brief review of literatures.

Acute kidney injury caused by administration of zaltoprofen in a cat

  • A 5-year-old, 2.7 kg, spayed female Scottish Fold cat presented with hematemesis after administration of oral zaltoprofen, a non-steroidal anti-inflammatory drug, by the owner. Diagnostic imaging and blood analyses indicated development of acute kidney injury (AKI) resulting from zaltoprofen ingestion. To correct dehydration and anemic conditions, the cat received intravenous fluid therapy with whole blood transfusion and peroral N-acetylcysteine. Clinical signs resolved, but persistent azotemia was unresolved indicating that AKI could progress to chronic kidney disease. This case suggests that although zaltoprofen may have low adverse effects on humans, administration of zaltoprofen in cats can have serious adverse effects.

  • To clarify the clinical spectrum of non-oliguric acute renal failure, we undertook a retrospective study of 16 cases of acute renal failure following trauma and surgery, 6 of whom were non-oliguric throughout their periods of azotemia. With the clinical symptoms, BUN, creatinine and some urinary diagnostic indexes, we analyzed the differences between non-oliguric and oliguric renal failure. And the results were: 1. The nonoliguric form [10 cases] was more common than oliguric [6 cases], especially post-traumatic failure. 2. The both types belonged to parenchymal renal failure by the U/P UN ratio and urinary Na concentration [Vertal, 1967]. 3. The non-oliguric form was lesser severe in urinary diagnostic indexes and clinically. The non-oliguric renal failure has better clinical course and prognosis, the failure to diagnose it is as dangerous as the failure oliguric renal failure.

Evaluation of Therapeutic Effect of the Extract from Rhubarb (Rheum officinalis) in Dogs with Chronic Renal Failure (개의 만성 신부전에서 대황 추출물의 치료학적 효과의 평가)

  • This study was designed to evaluate the clinical efficacy and safety of Rhubarb extracts ($Rubenal^{(R)}$) in dogs with chronic renal failure (CRF). Client-owned 40 dogs with CRF graded International renal interest Society (IRIS) II-III were enrolled in this study. The dogs were equally allocated and blindly administered with $Rubenal^{(R)}$ or placebo. The following items were evaluated at day 0, 30, 90 and 180: body condition score (BCS), clinical score (appetite, polydipsia/polyuria, quality of life score), hemogram (WBC, RBC, PCV), serum biochemistry (ALT/AST, ALP, Creatinine/BUN, total protein, albumin), serum electrolyte (Na, K, Cl, Ca, P), systolic blood pressure, urinalysis (UPC, USG) and IRIS stage. In this study, we found that the $Rubenal^{(R)}$ preparation was well tolerated by dogs and induced no adverse effects. Statistically significant improvements were observed in clinical score (quality of life score by vet and clients), serum BUN and creatinine levels, serum phosphorus concentration, level of proteinuria, and the IRIS score of CRF in dogs after 6 month of treatment of $Rubenal^{(R)}$. Those findings suggested that the Rhubarb extracts can improve the clinical signs of CRF (i.e. azotemia, hypertension, proteinuria, hyperphosphoremia) and the quality of life (i.e. BCS, clinical score) and can retard the progression of CRF in dogs. Therefore the Rhubarb extracts can be a good supplementary drug for treating dogs with subclinical and clinical renal diseases. However, care should be taken for interpreting our result, because this study is not double-blinded controlled study but pilot study.
