한의 치료로 호전된 이명 동반 돌발성 난청 환자 1례 (A Case Report of Korean Medicine Treatment for Sudden Hearing Loss Accompanied by Tinnitus)

  • Objectives : This study reports a 61-year-old man with sudden hearing loss and tinnitus whose symptoms improved remarkably after treatment with Korean medicine. Methods : The patient was treated with herbal medicine(gamisoyo-san, samul-tang gagambang, gongjindan, and yukmijihwang-hwan), acupuncture and moxibustion. Puretone audiometry(PTA), speech audiometry(SA), Korean tinnitus handicap inventory(K-THI), patient self-report, numeric rating scale(NRS) 11, and review of system(ROS) were performed. Results : After a month of treatment, PTA was improved from profound to mild and audiometric findings were improved in speech reception threshold and word recognition score. After 2 months of treatment, K-THI decreased from 92 to 28 and NRS 11 score decreased more than 6 in every item. Conclusions : These results demonstrate that Korean medicine could be effective sudden hearing loss and tinnitus.

소음성 난청 진전도 측정을 위한 청력측정법에 관한 연구 (A Study on a Hearing Test to Measure Progress of Noise Induced Hearing Loss)

  • 최근 노화현상 외에도 소음으로 인한 청력저하를 호소하는 사례가 늘어나고 있다. 특히 멀티미디어의 보급과 휴대용 정보기기의 발달로 지속적인 소음에 장시간 노출되어 청각 세포의 손상이 발생하여 청력 손실이 발생하는 소음성 난청 인구가 증가하고 있는 실정이다. 현재 병원에서 실시하고 있는 순음청력검사는 8000 Hz까지의 주파수대역을 수동으로 조절하며 검사하므로 8,000 Hz 이상의 확장 고주파수에서도 손실을 가져오는 소음성 난청의 경우 회화 영역에 직접적인 영향을 미치지 않아 조기에 청력저하를 파악하여 예방하기에는 부적합한 단점이 있다. 본 논문에서는 12,000 Hz∼20,000 Hz 고주파 대역을 이용하여 소음성 난청을 조기에 파악하여 예방하기 위한 청력측정법을 제안하였다. 휴대용 정보기기를 통해 이어폰을 많이 사용하는20대 남녀 50명을 대상으로 제안한 측정법으로 청력을 측정한 결과 36 %가 소음으로 인한 청력 저하를 보였다. 그 중 2 %는 청력의 손실정도가 심각함을 보였다.

휴대용 어음청력검사 시스템 구현 (Development of Ambulatory Speech Audiometric System)

  • In this study, we present an efficient ambulatory speech audiometric system to detect one's hearing problems at an earlier stage as possible without his or her visit to the audiometric testing facility such in a hospital or a clinic. To estimate a person's hearing threshold level in terms of speech sound response in his or her local environment, a digital assistant(PDA) device is used to generate the speech sound with implementing audiometric Graphic User Interface(GUI) system. Furthermore, a supra-aural earphone is used to measure a subject's hearing threshold level in terms of speech sound by the compensating the transducer's gain by adopting speech sound calibration system.

감각신경성 난청의 예후 인자에 대한 평가와 침구 치료에 대한 임상적 고찰 (The Assessments of Prognotic Factors on Sensorineural Hearing Loss And The Clinical Study with Acupuncture Treatment)

  • Objectives : The purpose of this study was clinical observation about sensorineural hearing loss compare with acupucture treatment and assessments of prognotic factors. Methods : This study was clinical observation about sensorineural hearing loss cases. for diagnosing in these cases, pure tone audiometry was used. The treatment, acupuncture was used. Results : Three patients(37.5%) remarkably improved, one patient(12.5%), slightly improved, four patients(50%), no change. Conclusions : These results suggest that the prognosis by age and initial hearing threshold was significant. The treatment on sensorineural hearing loss considered that bone condution improved.

  • The causes of occupational hearing loss are physical trauma. ototxic substances and noise. However, only noise induced hearing loss of workers has been reported in Korea as second cause of occupational disease. We report a case of progressive hearing loss in worker exposed to noise. carbon disulfide and hydrogen sulfide for about 13 years. Because of his hearing loss he had to move another position where no noise and no ototoxic substance is. Following audiometry showed that his hearing loss had worsen than before.

가청범위압축방식 보청기의 청각학적 이득에 관한 연구 (Audiological Benefits in Dynamic Range Compression Hearing Aids)

  • Hearing aids play an very important role in aural rehabilitation for hearing impaired person who could not be medically treated. Especially, hearing aids with dynamic range compression (level dependent frequency response: LDFR) method concentrated dynamic output range of receiver into the narrowing dynamic range of sensorineural hearing impaired person. Thus, if aided hearing threshold level is improved and uncomfortable loudness level rises, then the users of hearing aids are favored with extended dynamic range. This study aims to evaluate audiological benefits in dynamic range compression hearing aids. To achieve this, pure tone and speech audiometry were examined to 15 children and 3 adults with narrow dynamic range moderate-to-severe binaural sensorineural hearing loss.

청신경종에서의 뇌간유발반응검사 소견에 대한 임상적 고찰 (CLINICAL STUDY ON BRAINSTEM EVOKED RESPONSE AUDIOMETRY IN ACOUSTIC NEUROMA)

  • 청신경종은 제8 뇌신경에서 기원하는 종양으로서 초기에 발견될수록 제거가 용이하기 때문에 조기진단이 중요한 의미를 갖는다. 뇌간유발반응검사는 청신경종의 초기에 객관적으로 청신경종의 유무를 확인할 수 있는 방법으로 사용되고 있다. 이에 저자들은 1982년부터 1986년까지 본원 이비인후과와 신경외과에 입원하여 수술한 청신경종 환자 27례에 대하여 뇌간유발반응검사를 검토하고 다음과 같은 결과를 얻었다. 27례중 9례에서 환측에서 제 1파만이 관찰되었고 이중 6례는 순음청력검사상 60 ㏈이하의 역치를 보였다. 13례에서는 환측의 반응이 전혀 없었고 5례에서는 양측에 이상이 있었다. 대개의 경우 진행된 상태에서 발견되어 수술을 시행했기 때문에 뇌간유발반응 검사상 특이 소견을 찾기 어려웠으나 청력이 남아 있는 감각신경성난청환자에 있어서는 진단적 의의가 있음을 알 수 있었다.

한양방 동시 치료를 통하여 회복되지 않은 양측성 돌발성 난청 환자 1례 (A Case of Bilateral Sudden Sensorineural Hearing Loss with Incomplete Recovery by Oriental and Western Medical therapy)

  • Back ground & Objectives : Bilateral sudden sensorineural hearing loss(SSNHL) is very rare disease. This is the first case report in the oriental medical literature. This study describes a case of bilateral SSNHL with incomplete recovery. Methods : We treated 1 patient who had SSNHL sequentially in both ears. He received oriental and western medical treatments for more than 2 weeks and measured the hearing recovery. For diagnosis on this case, pure tone audiometry was used. The degree of improvement was evaluated by Siegel's Criteria. Results & Conclusions : This patient discharged without hearing improvement. For bilateral SSNHL patients, the result of treatment is important for their qualities of life. But the recovery rate of hearing in bilateral SSNHL is lower than unilateral SSNHL. The oriental medical therapy had some theraputic effects on SSNHL and the theraputic effects on bilateral SSNHL requires further study.

COVID-19 백신 접종 후 발생한 라력(瘰癧)과 이롱(耳聾)에 대한 치험 1례 (A Clinical Case of Lymphadenopathy and Hearing Loss after Covid-19 Vaccination)

  • Objectives: The purpose of this study is to report a case of lymphadenopathy and hearing loss in a patient after coronavirus disease 2019(COVID-19) vaccination. Methods: The patient diagnosed as lymphadenopathy and hearing loss(sensorineural hearing loss) after the first and second rounds of the Pfizer COVID-19 vaccination. We applied Korean medical treatment including herbal medicine and acupuncture for lymphadenopathy and SSNHL. We used VAS(Visual Analog Scale), CT-scan and Pure Tone Audiometry to estimate they symptoms. Results: After the treatment, general symptoms of lymphadenopathy and hearing loss including aural fullness showed big improvement. Conclusions: This case report shows effect of Korean medicine on lymphadenopathy and SSNHL and suggests a possibility of Korean medicine to treat adverse events following COVID-19 vaccination.

Unusual Tumors Obstructing the External Auditory Canal: Report of Two Cases

  • Primary tumors arising from the external auditory canal (EAC) are rare. We describe two cases of mass lesions within the EAC causing slowly progressive hearing loss without otorrhea or otalgia. Otoendoscopic examination demonstrated total obstruction of the EAC, and pure tone audiometry revealed conductive hearing loss. Based on the findings of the histopathologic examination, one patient was diagnosed with venous hemangioma that was treated using surgical resection, and the other patient was diagnosed with diffuse large B-cell lymphoma (DLBCL) that was treated using external-beam radiation therapy. Although primary tumors in the EAC are rare, both benign tumors such as venous hemangiomas and malignant lesions such as DLBCL should be considered as possible differential diagnoses of mass lesions in the EAC.
