Impedance Audiometry in Children (학동기 아동의 Impedance Audiometry에 대한 연구)

  • Since Metz had employed the impedance audiometry in 1946, scholars have carried out many investigation. Brook, Jeger, Cooper reported and evaluated the clinical studies of impedance audiometry and its screening test. Recently, in Korea, a studies of impedance audiometry have been reported. We analysed 100 children aged between 7-16 years old who visited OPD of E.N.T. department with the complaints of the nasal obstruction and hard of hearing from Nov. 77-Feb. 79. Through the use of otoscope and impedance audiometry, we evaluated the types of tympanogram, static compliance and the acoustic reflex. This paper is dealing with the statistical study of impedance audiometric result and its literature.

Automated Audiometry: A Review of the Implementation and Evaluation Methods

  • Objectives: Automated audiometry provides an opportunity to do audiometry when there is no direct access to a clinical audiologist. This approach will help to use hearing services and resources efficiently. The purpose of this study was to review studies related to automated audiometry by focusing on the implementation of an audiometer, the use of transducers and evaluation methods. Methods: This review study was conducted in 2017. The papers related to the design and implementation of automated audiometry were searched in the following databases: Science Direct, Web of Science, PubMed, and Scopus. The time frame for the papers was between January 1, 2010 and August 31, 2017. Initially, 143 papers were found, and after screening, the number of papers was reduced to 16. Results: The findings showed that the implementation methods were categorized into the use of software (7 papers), hardware (3 papers) and smartphones/tablets (6 papers). The used transducers were a variety of earphones and bone vibrators. Different evaluation methods were used to evaluate the accuracy and the reliability of the diagnoses. However, in most studies, no significant difference was found between automated and traditional audiometry. Conclusions: It seems that automated audiometry produces the same results compared with traditional audiometry. However, the main advantages of this method; namely, saving costs and increased accessibility to hearing services, can lead to a faster diagnosis of hearing impairment, especially in poor areas.

  • Hearing-impaired people are increasing rapidly due to the global aging trend. Early detection of hearing loss requires an easy-to-use audiometry device for the public. Existing audiometry systems were developed as PC-based, PDA-based, or smartphone apps. These devices were often dependent on specific software platforms and hardware platforms. In this paper, we tried to improve software platform compatibility by using cross platform, and tried to implement IoT-based pure tone audiometry device which does not require sound pressure level correction due to hardware differences. Pure tone audiometry is available in a variety of ways depending on the type of hearing loss and age. Using the IoT-based audiometry device implemented in this paper, it will be possible for an app developer who lacks hardware knowledge to easily develop an app with various scenarios for hearing screening. The results of this study will contribute to overcoming the software and hardware dependency in the development of IoT-based healthcare device.

  • Since the clinical application of Impedance measurment by Metz in 1946. Many scholars carried out investigations. Brooks reported on the clinical studies of Impedance on 1053 children, Jerger evaluated on 398 children less than 6 years old, and Cooper studied on the abbreviated screening technic for school children. In Korea, Seoul National University Medical School reported on the experimental studies of Impedance with Madsen acoustic bridge (20~70) on 1976, and we reported on the clinical evaluation on 1023 school children aged between 7~14 with Teledyne Impedance bridge on 1976. This paper is dealt with the studies of Impedance bridge as 110 of children less than 6 years.

The Effect of Workplace Noise on the Hearing Threshold of Dental Technicians

  • This study was conducted to investigate hypoacusis due to workplace noise among dental technicians. Pure tone audiometry, impedance audiometry and heart rate variability were measured for 60 dental technicians and office workers in D city from July to November 2013, and a survey on the effects of noise was conducted. SPSS 19.0 was used for statistical analysis. In the result for threshold of pure tone audiometry, the average hearing threshold of left ears in the air conduction (AC) test showed a statistically significant decrease (P=0.019) among dental technicians, who are frequently exposed to workplace noise. The hearing threshold in the AC test for each frequency was significantly different between the two groups at 125 Hz (P=0.012) in right ears and at 1 kHz (P=0.022), 2 kHz (P=0.040), and 8 kHz (P=0.018) in left ears. Dental technicians who had worked for 16 years or longer had a significantly higher incidence of hypoacusis in the right ear (P=0.030) and in the left ear (P=0.010). In impedance audiometry showed a tympanometry result of type A in both the dental technician group and the office worker group.

  • Harford has pointed out that the application of impedance audiometry in the Eustachian tube function is of special value. As you know, with excessive negative pressure in the middle ear cavity with intact eardrum tympanograms reveal indirect evidence of Eustachian tube insufficiency. With normal Eustachian tube function eardrum should be pushed laterally by Valsalva maneuver, resulting in a temporary decrease in the compliance of the system, indicated by a swing of the balance meter needle. Therefore the authors measured the swing of the balance meter needle by the Valsalva maneuver in persons with intact eardrums, intact middle ear cavities, and intact Eustachian tube function. The results are as follows; The swing of the eardrum by Valsalva maneuver is 1.0 to 4.0 (mean $1.6{\pm}0.06$) and we conclude that the persons under 1.0 of the swing is suggestive of some disfunctions of the Eustachian tube.

Differential Diagnosis on the Types of Hearing Loss using Short Increment Sensitivity Index(SISI) Test and Bekesy Audiometry (미세증가감수지수검사와 자기청력검사를 이용한 난청 유형의 감별 진단)

  • As using recruitment phenomenon, SISI test and Bekesy audiometry are employed to differentially diagnose both the cochlear hearing loss and the retrocochlear hearing loss. The main purpose of this study is to find out the sensitivity and specificity of the SISI test within the scope of 1 KHz to 4 KHz in diagnosing various types of hearing losses. In this study, the Bekesy audiometry was conducted for Bekesy audiograms of certain groups according to Jerger's classification, which included persons with normal hearing and patients with hearing impairment. Subject persons of the SISI test were categorized into the several groups with normal hearing, mild hearing loss, moderate hearing loss, moderate-severe hearing loss and severe hearing loss, respectively. The sensitivity and specificity of certain groups were evaluated according to the SISI scores of 20 % below, 25 ${\sim}$ 65 %, and more than 70%. Also, the Bekesy audiometry is conducted on the several subject groups with normal hearing, mixed hearing loss, and sensori-neural hearing loss.

Effectiveness of Computer-Animated Pure Tone Audiometry for Screening (애니메이션을 이용한 순음청력선별검사 도구의 효용성에 대한 연구)

  • The purpose of this study was to develop a computer-animated pure tone audiometry for screening (CAPTAS) for toddler and to determine its validity and reliability. The CAPTAS utilizes an animated cartoon story producing visual and auditory stimuli. The intensities were 40 dB, 60 dB, 80 dB. The frequencies were 500 Hz, 1000 Hz, 2000 Hz, and 4000 Hz. The subjects were 20 (9 males and 11 females) severely hearing impaired children. As a result, The correlation coefficient between mean hearing threshold of children who were able to perform PTA and average hearing threshold of CAPTAS was performed and it revealed CAPTAS's high validity. And to verify the reliability of the re-test, all children had the CAPTAS and repeated it periodically. The result confirmed the reliability.

It is now widely used in clinical audiology and it plays an important role in otology. Recently there was the literature stated on normal value of various test c Impedance. This paper is dealing with the clinical evaluation of type c in comparison with following subjects as; 1. Comparison with the otoscopic finding. 2. Correlation with the pure tone audiometry. 3. Correlation with the stapedial reflex. 4. Correlation between pathologic negative pressure range and middle ear fluid.

Remote Patient Monitoring System for Diagnostic Pure-tone Audiometry (순음 청력검사를 위한 원격진단 모니터링 시스템)

  • This paper presents a remote patient monitoring system for diagnostic pure-tone audiometry. A pure-tone audiometer was developed for basic hearing screening; its performance was evaluated according to international standards in terms of linearity, accuracy, and total harmonic distortion. Pure-tone audiometry has a maximum hearing level of 104.9 dB HL that is comparable with other commercial products. The audiometer shows satisfactory linearity with a deviation of ${\pm}0.4dB$, an accuracy of ${\pm}0.025%$, and a total harmonic distortion (THD) of 0.21%. The remote patient monitoring systems include remote control devices based on wide area network (WAN) connections and an audiometer connected in series. Through experimentation, we successfully performed real-time diagnostic communication without delay in transferring audiometric data. This system is expected to supply domestic equipment in the audiometric market and to improve the quality of life of patients in non-clinical environments.
