According to Luhr's classification, a fracture in the mandible with a width of less than 15-20 mm is considered to be an atrophic mandibular fracture and its incidence is very rare. Because of the reduced cross-sectional area and smaller contact area of the fractured ends as well as the poorly vascularized bony structure and delayed bone healing, an atrophic mandibular fracture is a great challenge for oral and maxillofacial surgeons. Surgeons tend to perform closed reduction, because open reduction is considered a non-life-saving surgery among elderly patients. Thus, most of them have limited experience in surgical management. According to recent reports, open reduction yields a good result, and the Association for Osteosynthesis (AO) group has recommended open reduction. This is a case report of our two experiences of open reduction and rigid fixation of atrophic mandibular fractures by the AO principle. Articles were also reviewed here.

Fractures of the mandible occur with a greater frequency in the elderly. This study reports three cases of edentulous atrophic mandible fracture in elderly patients treated with open reduction technique. Three patients who presented with edentulous atrophic mandible fractures underwent surgical management using open reduction and internal fixation. After treatment, clinical evaluations and postoperative complications were examined with postoperative x-ray. Patients were followed with clinical and radiographic examinations. In the postoperative clinical evaluation, two male patients healed well, but one female patient complained of pain and swelling. In radiographic examinations, no union delay or lack of fusion was observed in the edentulous area. Open reduction technique is a viable treatment option for the edentulous atrophic mandible fractures in geriatric patients.
The fact that long-term use of proton pump inhibitors (PPIs) aggravates corpus atrophic gastritis in patients with Helicobacter pylori infection has been proven clinically and experimentally. Corpus atrophic gastritis is a known risk factor for gastric cancer. Therefore, gastric neoplasia might be associated with the long-term use of PPIs. One of the causes of worsening corpus atrophic gastritis, leading to the development of adenocarcinoma, might be bacterial overgrowth under conditions of hypochlorhydria. The production of potentially carcinogenic N-nitrosocompounds by nitrosating organisms under conditions of hypochlorhydria might be associated with carcinogenesis. Interactions between bile acids, pH, and H. pylori might also contribute to carcinogenicity, especially in patients with gastro-esophageal reflux disease (GERD). The concentration of soluble bile acids, which have bactericidal or chemorepellent properties toward H. pylori, in gastric contents is considerably higher in patients undergoing continuous PPI therapy than in healthy individuals with normal acid production. Under these circumstances, H. pylori might colonize the stomach body rather than the pyloric antrum. Hypergastrinemia induced by PPI administration might promote the development of gastric cancer. Because the main cause of corpus atrophic gastritis is H. pylori infection, and not PPI administration, H. pylori infection should be eradicated before starting long-term PPI therapy.

This study was investigated to diagnose pathogenic organisms of atrophic piglets in northern area of the Gyeongnam province, Korea. Samples such as feces, blood and necropsy specimens of 42 atrophic piglets (${\leq}10$ weeks old) were taken from May to December 2013 for this survey. Samples were examined by reverse transcriptase-polymerase chain reaction (RT-PCR) assay and bacteria isolation for detection of pathogenic agents. 93 pathogens were isolated from 42 samples can be classified into ll groups. We identified bacterial agents in 56 cases (60.2%) and viral agents in 31 cases (33.3%). However, 6 cases (6.5%) were undetected. Among these pathogens, the most prevalent disease were porcine reproductive and respiratory syndrome (PRRS) in 22 cases (23.7%). The major diseases were Colibacillosis in 15 cases (16.1%), Glasser's disease in 12 cases (12.9%), and porcine epidemic disease (PED) in 9 cases (9.7%). Mixed infections were accounted for 77.8% of atrophic piglets. In particular, the rate of mixed infections with PRRS virus showed the highest frequency (71.4%). In addition, there is a seasonal variation. Viral pathogens were dominantly detected in winter, but in the rest of the season bacterial agents were mainly detected. Gastrointestinal diseases occurred mainly in the pre-weaning piglets, the respiratory diseases and wasting diseases occurred mainly in the post-weaning piglets.

This paper describes the slaughter check results of breeding pigs from the Korean Swine Testing Station for the control of endemic diseases. Gross lesions monitored in the present study included those conditions commonly associated with economically significant subclinical herd infections: enzootic pneumonia, pleuropneumonia, pleuritis, atrophic rhinitis, liver white spots, papular dermatitis and ileitis. A total of 128 slaughter pigs were investigated at 4 subsequent tests according to the slaughter check procedures established. The prevalence of enzootic pneumonia, pleuropneumonia and pleuritis in the initial test was 67.9%, 28.6% and 17.9%, respectively. However, these were decreased to 46.7%, 6.7% and 6.7%, respectively, in the last test after implementation of counter measures including clean-up protocols and medication programs (p > 0.05). The mean pneumonic score also significantly decreased from 6.8 in the initial test to 2.8 in the last test. The prevalence of atrophic rhinitis (${\geq}score\;2$) was 32.2% and mean atrophic rhinitis score of 1.1 were recorded. However, no significant improvement of conditions was achieved with the counter measures indicating that atrophic rhinitis was originated from the source herds and lesions developed early in the life. In the initial test, prevalence of liver white spots and papular dermatitis lesions was 21.4% and 25.0%, respectively. These conditions were cleaned by the implementation of parasite control measures with all-in all-out, strict clean-up protocols and proper medications adopted in the present study (liver white spots, p = 0.0124; papular dermatitis lesions, p = 0.0055). The prevalence of ileitis lesions in slaughter pigs from the initial test was 28.6%, it could be gradually reduced by the use of repeated treatments and control measures but the effect was not so significant (p > 0.05). In conclusion, slaughter check procedures were successfully established and applied for the control of endemic diseases in the Korean Swine Testing Station.
Pasteurella multocida Pasteurella multocida toxin (PMT) is a causal pathogenin atrophic rhinitis in pigs. To investigate the protective immunity and vaccination effect of recombinantPMT, the gene for PMT was isolated from the infective P. multocida D:4. The 2.3 kb XhoI/PstI fragment(PMT2.3) of PMT gene was expressed in E. coli BL21 (DE3) using the induced expression vector system. The recombinant protein of PMT2.3 having molecular weight of 84 kDa was purified by Ni-afinitycolumn chromatography. The PMT2.3 raised slightly less anti-PMT antibody titer than formalin-killedwhole cel, however, it showed more protective imunity against P. multocida D:4 infection in vaccinationand chalenge.
Onlay bone grafting, guided bone regeneration, and alveolar ridge split technique are considered reliable bone augmentation methods on the horizontally atrophic alveolar ridge. Among these techniques, alveolar ridge split procedures are technique-sensitive and difficult to perform in the posterior mandible. This case report describes successful implant placement with the use of piezoelectric hinge-assisted ridge split technique in an atrophic posterior mandible.

Scars can cause great psychological stress among patients. Currently, there are numerous topical agents, laser and surgical treatments available for skin rejuvenation and scar minimization. Laser-assisted drug delivery (LADD) is a treatment method that increases drug delivery by stimulating the skin physically and chemically to enhance the penetration of topical agents. This is one of the areas of great interest in the treatment of various skin diseases in addition to its use for cosmetic purposes. In particular, LADD is relatively non-invasive and has advantages in terms of accessibility and stability. Poly-L-lactic acid (PLLA) is a collagen stimulator known to gradually restore skin volume by inducing inflammation and fibroplasia. Herein, we report a case of treatment of an atrophic scar with fractional carbon dioxide laser-assisted PLLA delivery.
Objective: This is to report the efficacy of the sandwich technique for bone augmentation in a moderate atrophic posterior mandible through clinical and histological results in two cases. Subjects and Method: Two patients selected had moderate bone resorption in left lower edentulous area. Sandwich osteotomy using the piezosurgery was performed and the osteomized alveolar segments were elevated by 6mm in each two patients. The interpositional mineral allograft materials were inserted in the atrophic posterior mandibles. After four months healing period, bone biopsies in the grafted areas and placement of dental implants were performed. In both cases, panoramic views were taken preoperatively to measure the alveolar bone height for diagnosis, to monitor patient healing, and to evaluate bone healing and bone gain. Results: Sufficient vertical bone height was gained by using the sandwich technique and implants were placed successfully. In radiological evaluation, there was minimal resorption of bone height after the second operation and in histomorphometric evaluation, they showed favorable new bone formation without inflammation in the grafted areas. Conclusion: The sandwich technique can be an effective choice for augmenting vertical bone height in the atrophic mandible. More of cases and long term follow-up are needed to evaluate bone resorption and implant prognosis.
A native strain of Pasteurella multocida was isolated from pigs suffering from severe atrophic rhinitis at domestic farms in Gyeonggi Province, Korea, and was identified as capsular serogroup 'D' and somatic serotype '4' by disc diffusion decapsulation and gel diffusion precipitation tests, respectively. The P. multocida (D:4) induced atrophic rhinitis in healthy pigs by the secondary infection. The gene for outer membrane protein H (ompH) of P. multocida (D:4) was cloned in Escherichia coli DH5$\alpha$ by PCR. The open reading frame of the ompH was composed of 1,023 bp, possibly encoding a protein with 341 amino acid residues containing a signal peptide of 20 amino acids at N-terminus, and the gene product with molecular mass of ca. 38 kDa was identified by SDS-PAGE. Hydropathy profiles indicated that there are two variable domains in the OmpH. To express the ompH in E. coli, the gene was manipulated in various ways. Expression of the truncated as well as full-length forms of the recombinant OmpH was fatal to the host E. coli BL21 (DE3). However, the truncated OmpH fused with GST was consecutively expressed in E. coli DH5$\alpha$. A large quantity of the fused polypeptide was purified through GST-affinity chromatography.
