Effect of Gagam-Danguieumja through Regulation of MAPK on LPS-Induced Inflammation in RAW 264.7 Cells (LPS로 유도된 RAW 264.7 cell의 염증반응에서 MAPK 조절에 의한 가감당귀음자(加減當歸飮子)의 항염증 효과)

  • Objectives : Danguieumja is a traditional medicinal prescription to treat skin disease. It was commonly used for the treatment of itching, chronic urticaria and atopic dermatitis in Korea by the addition or omission of several herbs. This study investigated the anti-inflammatory potential of Gagam-Danguieumja (GDE) water extract. Methods : We examined the effects of GDE on the lipopolysaccharide (LPS)-induced production of nitric oxide (NO) in a murine macrophage cell line, RAW 264.7 cells. Results : GDE inhibited production of NO in a dose dependent manner and also decreased the expression of inducible nitric oxide synthase (iNOS), cyclooxygenase-2 (COX-2). As a possible molecular mechanism of anti-inflammatory effect increased phosphorylation of mitogen-activating protein kinases (MAPK) by LPS were blocked by GDE treatment. Conclusions : These results suggest that GDE has an anti-inflammatory therapeutic potential through the inhibition of MAPK phosphorylation, thereby decreasing the expression of pro-inflammatory genes.

Three Cases Report of Molluscum Contagiosum Children treated by Gwakhyangjeonggisangamibang (곽향정기산가미방(藿香正氣散加味方)으로 치료한 소아 전염성 연속종 3례)

  • Objectives : Molluscum Contagiosum is a contagious disease most often seen in children. It is caused by molluscum contagiosum virus(MCV-1,2). The purpose of this study is to report three cases of Molluscum Contagiosum children treated by oriental medicine. Methods : The subjects are in regard to children diagnosed as Molluscum Contagiosum. We treated three children with herbal medicine(Gwakhyangjeonggisangamibang). Results : After treatment, the symptoms(papule on the whole body and pruritus) of Molluscum Contagiosum were improved. Two children of them were cured completely and the one child still appealed a little itching. But, the one child had atopic dermatitis, and suffered from itching primarily, so after treatment the itching sign decreased rather than before an attack of Molluscum Contagiosum. Conclusions : This study shows that oriental medicine can be an effective treatment for Molluscum Contagiosum. And the further study is needed with more cases and longer duration.

  • Lactobacillus rhamnosus GG(ATCC 53103) is one of the best researched probiotic strains in the world. Studies in children have shown that Lactobacillus rhamnosus GG effectively prevents early atopic disease in patients with high risk. The active molecules associated with the immunostimulatory sequence and anti-allergy effects of L. rhamnosus GG have not yet been identified. Unmethylated CpG motifs in bacterial DNA have a mitogenic effect in mouse immune cells, CpG-containing ISS oligodeoxynucleotides are potent Th1 adjuvants, effective in both preventing and reversing Th2-biased immune deviation in allergy models. The genomic DNA of L. rhamnosus GG is a potent inducer of murine B cell and dendritic cell immunoactivation. In L. rhamnosus GG genomic DNA, ID35 shows high activity in ISS assays in both mice and humans. The effects of ID35 result from a unique TTTCGTT motif located at its 5'-end, and its effects are comparable with murine prototype CpG 1826. L. rhamnosus GG is known to secrete proteinaceous pili encoded by the spaCBA gene cluster. The presence of pili structures may be essential for its adhesion to human intestinal mucus, explaining the prolonged duration of intestinal residence of this bacterium, compared to that of non-piliated lactobacilli.

A Concept Analysis of the Quality of Life for the Children (아동의 삶의 질에 대한 개념 분석)

  • Purpose: The purpose of this study was to attempt an accurate understanding of the quality of life for the children and provide basic materials for application to nursing research, intervention and further theoretical development. Method: Data obtained from such research methods as the lexical and literature review were analysed according to Walker and Avant's(1995) process of concept analysis. Results: As a result of analysis, it was found that the attributes of the concept of the quality of life for the children included value/acceptance, family relationship, safety, good relationship with friends, right of children, school life, parent relationship, leisure, economical stability, not child abuse, and health. There was the coexistence between antecedent element and the outcomes of subjective judgement - health, family, economic strength, education, friends, welfare, social-cultural environment which indicates the coexistence of positive and negative emotions. Conclusion: nursing intervention should be provided for children by concentrating focus on the coordination of internal and external situations in order to prompt them to express their life with positive emotions. Experiential reference was used to suit the purpose of this study based on the use of objective and subjective indicators, and the development of the instrument considering the children were proposed because of the absence of the unified measurement instrument.

LC-MS/MS Assay Validation for a New Immune Modulator, JHL45, and its Major Metabolite in Plasma: Application to Pharmacokinetic Studies in Rats

  • JHL45, a novel immune modulator for anti-atopic dermatitis and allergic airway disease, was synthesized from decursin isolated from Angelica gigas. In order to conduct a pharmacokinetic study of JHL45, an analytical method, ideally one that uses a minimal amount of biological sample must first be validated. In this study, a HPLC-MS/MS method was developed and validated for the quantification of JHL45 and its major metabolite, (+)-decursinol, from 10 ${\mu}L$ of rat plasma. JHL45 was stable under the analysis conditions, and intra- and inter-day accuracies exceeded 90.06%, with a precision variability ${\leq}$ 13.16% for each analyte. The mean values for Cmax, AUC8h, half-life of JHL45 in rats after intravenous administration of 5 mg/kg JHL45 were 24.59 μg/mL, 10.02 ${\mu}g{\cdot}h/mL$, and 1.88 h, respectively. The validated method herein will be useful for further pharmacokinetic studies of JHL45, as well as in preclinical and clinical phases.

A Study on the Variation of Hazardous Pollutant Emissions in Korea from 2006 to 2015 (2006~2015년 동안 국내 유해화학물질 배출량 변화에 관한 연구)

  • Objectives: The purpose of this study was to investigate the changes in hazardous pollutant emissions in Korea over the period of 2006-2015. Methods: A survey of hazardous pollutant emissions was conducted through the homepage for 2006-2015. The emission of hazardous chemicals and the health effect was examined using the Annual Report of Air Quality in Korea (2016) and National Health Statistics (2015) as references. Results: Hazardous pollutant emissions increased by 1.1 times over the past decade, and the circulation amount of chemicals was 15% compared to the previous year. $PM_{10}$ concentrations showed a tendency to decrease, but emissions have not declined. In addition, asthma and atopic dermatitis correlated with similar emissions, but allergic disease showed no connection. Conclusion: These results on hazardous pollutant emissions indicate that they have increased. However, no association between emissions and health effects was shown, so more research is needed.

  • Allergic rhinitis is a common disease of childhood characterized by nasal, throat, and ocular itching, rhinorrhea, sneezing, nasal congestion. Those affected with allergic rhinitis often suffer from associated inflammatory conditions of the mucosa, such as allergic conjunctivitis, rhinosinusitis, asthma, otitis media with effusion, and other atopic conditions, such as eczema and food allergies. Allergic rhinitis must be diagnosed and treated properly to prevent complications and impaired quality of life. Despite a high prevalence, allergic rhinitis isoften undiagnosed and inadequately treated, especially in the pediatric population. The first step in treatment is environmental control when appropriate. It may be difficult to eliminate all offending allergens effectively to reduce symptoms, so medications are often required. Many different classes of medications are now available, and they have been shown to be effective and safe in a large number of well-designed, clinical trials. Antihistamines are effective in treating immediate symptoms of sneezing, pruritus, watery eyes, and rhinorrhea. Second generation antihistamines are the preferred antihistamines because of their superior side effect profile. Thus, decongestants are commonly used with oral antihistamines. Intranasal corticosteroids are the most effective therapy for allergic rhinitis. Leukotriene modifier may be as effective as antihistamines in treating allergic rhinitis symptoms. Cromolyn sodium is an option for mild disease when used prophylactically, and ipratropium bromide is effective when rhinorrhea is the predominant symptom. When avoidance measures and medications are not effective, specific immunotherapy is an effective alternative. Only immunotherapy results in sustained changes in the immune system. Because of improved understanding of the pathogenesis, new and better therapies may be forthcoming. The effective treatment of allergic rhinitis in children will reduce symptoms and will improve overall health and quality of life, making a happier, healthier child.

Evaluation of Pyunkang-tang® Administration on Hematological, Biochemical, and Protein and Lipid Profiles in Dogs

  • $Pyunkang-tang^{(R)}$ is a kind of herbal medicine and has been used for the treatment of atopy and allergic disease in humans over forty years. The purpose of this study was to evaluate the hematological, biochemical, and protein and lipid electrophoresis profiles after the oral administration of $Pyunkang-tang^{(R)}$ in healthy dogs. Fifteen clinically healthy beagle dogs were selected and orally administered either 33 ml of $Pyunkang-tang^{(R)}$ (group I, n = 5), 16.5 ml of $Pyunkang-tang^{(R)}$ (group II, n = 5), or 33 ml of distilled water (group III, n = 5) 3 times a day for 4 weeks. The results of the hematological, serum biochemical, and urine analysis did not differ significantly among the 3 groups. The oral administration of $Pyunkang-tang^{(R)}$ for 4 weeks was associated with significant changes in the serum globulin levels and an elevation in the high-density lipoprotein (HDL) concentrations in groups I and II (p < 0.05), compared to group III. The ${\alpha}1-globulin$ and ${\gamma}-globulin$ levels were significantly increased in group I, and the ${\alpha}1-globulin$ and ${\beta}-globulin$ levels were significantly increased in group II. The ${\alpha}2-globulin$ levels were significantly decreased in both groups. During the short-term evaluation of $Pyunkang-tang^{(R)}$ administration, we did not detect any specific adverse effects in the dogs. However, further evaluation of the safety and efficacy of $Pyunkang-tang^{(R)}$ for the treatment of atopic disease in dogs is needed.

Association with Genetic Polymorphism of rs117033348 and Allergic Disease in Korean Population

  • Allergy is an immune response that appears in certain people, and reactions such as coughing, shortness of breath, and hives occur. The immune system plays an important role in homeostasis and host defense, and allergies cause hypersensitivity reactions when an imbalance of immunity occurs. Mutations in the TLR genes are associated with autoimmune conditions such as allergies and asthma. It has been reported that a locus in the TLR1-TLR6-TLR10 region may be associated with atopic sensitization or allergy. Therefore, the purpose of this study was to select an allergy patient group and a healthy control group to determine how the genetic mutation of TLR1 affects the onset of disease. This study was conducted in 709 patients and 5,025 control groups out of 10,956 patients with data from KARE and HEXA cohorts. As a result of logistic regression analysis of 6 SNPs selected from the TLR1 gene, only rs117033348 showed a statistically significant correlation (P = 0.002356). The influence of rs117033348 was examined using PolyPhen-2, and a significant result was shown. Therefore, it can be predicted that the G base in rs117033348 will have an influence on the human body. In addition, Geography of Genetic Variants browser was used to confirm the geographical distribution of allele frequencies for the TLR1 gene. Although it was found that there was a large racial difference in the prevalence of TLR1 SNP, it could be confirmed that the polymorphism of rs117033348 conducted in this study was only specific in East Asia when compared with each race.

Eosinophilic Esophagitis and Eosinophilic Gastroenteritis: Similarities and Differences

  • Eosinophilic gastrointestinal disease (EGID), a chronic allergic condition characterized by dense infiltration of eosinophils in the digestive tract, is classified into two types, eosinophilic esophagitis (EoE), which features dense infiltration of eosinophils in the esophageal epithelial layer, and eosinophilic gastroenteritis (EGE), in which the entire digestive tract including the esophagus may be involved. Patients with EoE only have esophageal symptoms, since the other parts of the digestive tract are not involved. On the other hand, 80% of EGE patients have lesions in the small intestine. The esophageal epithelial layer in healthy individuals has no or negligible infiltration by eosinophils, while the small intestinal mucosal layer, especially the distal small intestinal mucosa, can show dense eosinophil infiltration even in the absence of disease. Therefore, histological changes observed in cases of EGE are not qualitative but rather quantitative, as compared to EoE, which has qualitative histopathological changes, indicating important pathogenetic differences between the types. Comparisons of clinical, laboratory, and morphological characteristics between EoE and EGE have revealed several interesting differences. Both EoE and EGE patients are frequently affected by atopic diseases, such as bronchial asthma and allergic rhinitis, and elevated plasma levels of Th2 type cytokines and chemokines are also similarly seen in both. On the other hand, age at diagnosis differs, as the former is generally found in individuals from 30 to 50 years old, while the latter appears in all age groups. Additionally, 80% of patients with EoE are male as compared to only 50% of those with EGE. Furthermore, approximately 60% of patients with EoE respond favorably to proton pump inhibitor (PPI) administration, whereas EGE patients rarely show a response to PPIs. Nevertheless, both diseases show a similarly favorable response to a six foods elimination diet and glucocorticoid administration. These similarities and differences of EoE and EGE provide important clues for understanding the pathogenesis of these EGID types.
