AIDS-Related Non-Hodgkin Lymphoma: Imaging Feature Analysis of 27 Cases and Correlation with Pathologic Findings

  • Background: Some tumor types are related to HIV, including non-Hodgkin lymphoma (NHL). The morbidity and mortality of NHL has remained high, even after highly active antiretroviral therapy (HAART) was introduced. We collected cases of AIDS with NHL, and evaluated the imaging features and strategies for diagnosis. Materials and Methods: There were 27 patients with AIDS and tumors confirmed by pathology. There were 9 patients with Burkitt lymphoma, 16 with diffuse large B cell lymphomas (DLBCLs), and 2 with primary central nervous system (PCNS) lymphomas. All of the patients underwent a series of imaging studies. Three radiologists analyzed the images, and any disagreement was discussed until consensus was reached. Results: The radiologic manifestations of AIDS with NHL were mainly masses and lymphadenopathy, 3 patients having one mass and 12 two or more masses. 7 patients had lymphadenopathy in one site and 3patients had lymphadenopathy in two or more sites. Coarse mucosal folds, thickening of the gastrointestinal wall, and lumen narrowing were typical manifestations of NHL within the gastrointestinal tract. There were 4 patients with masses and 5 with lymphadenopathy inthe 9 with Burkitt lymphoma, and 11 patients with masses 5 with lymphadenopathy in the 16 with DLBCLs. Conclusion: NHL is a malignancy that usually occurs in patients with AIDS. Imaging is an important method by which to evaluate lesions, masses, and lymphadenopathy. Fine needle aspiration biopsy and stereotaxis biopsy are useful methods by which to diagnose NHL.

경부 임파선 질환 306예의 임상적 고찰 (Clincal Analysis of 306 Cases of Cervical Lymphadenopathy)

  • Clinical analysis was performed on 306 patients with cervical lymphadenopathy who were diagnosed histologically by fine needle aspiration biopsy cytology (FNABC) and/or excisional biopsy from Jan 1986 to Jan 1990 at Hanyang University hospital. The results obtained were as follows: 1) Of 306 patients with cervical lymphadenopathy, 216 (70.6%) were inflammatory lesions, and 90 (29.4%) malignant lesions. Tuberculous lymphadenitis of inflammatory lesions was most common (134 cass: 62%). Of malignant lesions, metastatic cancer was more frequent (75 cases: 83.3%). 2) The sex ratio were as follows: inflammatory lesion; M:F=1 : 1.8 (tuberculous lymphadenitis;M : F=1:2.3) malignant lesion; M : F=1.5 : 1 (metastatic cancer; M : F=2.6 : 1) 3) The peak age of inflammatory lymphadenopathy was 20-29 years old (38.9%), and that of malignant lesion 50-59 years old (46.7%). 4) In more than half of tuberculous lymphadenitis and metastatic cancer, the location of enlarged lymph nodes was one side of the neck and the number was more than one. 5) The common primary sites of metastatic cancer were lung and stomach. In 11 cases (14.7%), the primary site could not be found. 6) The sensitivity and the specificity of fine needle aspiration biopsy cytology (FNABC) was 0.83 & 1.0 in metastatic cancer respectively.

Improved DNA Extraction Method for Molecular Diagnosis from Smaller numbers of Cells

  • Isolating total DNA from small samples using traditional methods is difficult and inefficient mainly due to loss of DNA during filtration and precipitation. With advances in molecular pathology, DNA extraction from micro-dissected cells has become essential in handling clinical samples. Genomic DNA extraction using small numbers of cells can be very important to successfully PCR amplify DNA from small biopsy specimens. We compared our experimental genomic DNA extraction method (A) with two other commercially available methods: using spin columns (B), and conventional resins (C), and determined the efficacy of DNA extraction from small numbers of cells smeared on a glass slide. Approximately 50, 100, 200, 500 and 1000 cells were isolated from fine needle aspiration biopsy (FNAB) slides aspirated from histologically proven papillary thyroid carcinoma masses. DNA was extracted using the three techniques. After measuring DNA quantity, PCR amplification was performed to detect the ${\beta}$-globin and $BRAF^{V600E}$ gene mutations. DNA extracted by method (A) showed better yield than the other methods in all cell groups. With our method, a suitable amount of genomic DNA to produce amplification was extracted from as few as 50 cells, while more than 100 to 200 cells were required when methods (B) or (C) were applied. Our genomic DNA extraction method provides high quality and improved yields for molecular analysis. It will be especially useful for paucicellular clinical samples which molecular pathologists often confront when handling fine needle aspiration cytology, exfoliative cytology and small biopsy specimens.

Prospective Assessment of the Performance of a New Fine Needle Biopsy Device for EUS-Guided Sampling of Solid Lesions

  • Background/Aims: Endoscopic ultrasound-guided fine needle aspiration (EUS-FNA) remains the most common EUS-guided tissue acquisition technique. This study aimed to evaluate the performance of a new Franseen tip fine needle biopsy (FNB) device for EUSguided sampling of solid lesions and compare it with the historical FNA technique. Methods: $Acquire^{(R)}$ 22 G FNB needle (Boston Scientific Co., Natick, MA, USA) was used for solid tumor sampling (Study group). Tissue was collected for rapid on-site evaluation, and touch and crush preparations were made. Historical EUS-FNA samples obtained using $Expect^{(R)}$ 22 G FNA needle (Boston Scientific Co.) were used as controls (Control group). All specimens were independently evaluated by two cytopathologists blinded to the formal cytopathological diagnosis. Results: Mean cell block histology scores were significantly higher (p=0.046) in the FNB group (51 samples) despite a significantly lower (p<0.001) mean number of passes compared to the FNA group (50 specimens). The overall diagnostic yields for the FNB vs. FNA groups were 96% vs. 88%. The degree of tumor differentiation was adequately assessed in all cell block qualifying lesions in the FNB group. Two patients developed post-FNB abdominal pain. Conclusions: The new Franseen tip FNB device provides histologically superior and cytologically comparable specimens to those obtained by FNA, but with fewer passes.

Clinical utility of endoscopic ultrasound-guided tissue acquisition for comprehensive genomic profiling of pancreatic cancer

  • Background/Aims: Endoscopic ultrasound-guided tissue acquisition (EUS-TA) is essential for the diagnosis of pancreatic cancer. The feasibility of comprehensive genomic profiling (CGP) using samples obtained by EUS-TA has been under recent discussion. This study aimed to evaluate the utility of EUS-TA for CGP in a clinical setting. Methods: CGP was attempted in 178 samples obtained from 151 consecutive patients with pancreatic cancer at the Aichi Cancer Center between October 2019 and September 2021. We evaluated the adequacy of the samples for CGP and determined the factors associated with the adequacy of the samples obtained by EUS-TA retrospectively. Results: The overall adequacy for CGP was 65.2% (116/178), which was significantly different among the four sampling methods (EUS-TA vs. surgical specimen vs. percutaneous biopsy vs. duodenal biopsy, 56.0% [61/109] vs. 80.4% [41/51] vs. 76.5% [13/17] vs. 100.0% [1/1], respectively; p=0.022). In a univariate analysis, needle gauge/type was associated with adequacy (22 G fine-needle aspiration vs. 22 G fine-needle biopsy [FNB] vs. 19 G-FNB, 33.3% (5/15) vs. 53.5% (23/43) vs. 72.5% (29/40); p=0.022). The sample adequacy of 19 G-FNB for CGP was 72.5% (29/40), and there was no significant difference between 19 G-FNB and surgical specimens (p=0.375). Conclusions: To obtain adequate samples for CGP with EUS-TA, 19 G-FNB was shown to be the best in clinical practice. However, 19 G-FNB was not still sufficient, so further efforts are required to improve adequacy for CGP.

CT 유도하 국소적 폐질환의 경피적 흡입생검의 장점과 결과 분석 (Analysis of the Result and Merit of Computed Tomography Guided Percutaneous Needle Aspiration Biopsy of Focal Lung Lesion)

  • PTNB는 최근 CT 기술의 발전과 20 gauge 또는 22 gauge의 세침의 개발로 합병증을 줄이는 한편, 성공율이 높아 점차 임상적으로 널리 이용되는 추세에 있다. 이에 저자는 최근 44례의 PTNB를 분석하여 다음과 같은 결과를 얻었다. 환자의 분포는 50대 남자가 17례로 가장 많았고 종괴의 크기와 위치는 4cm 전후가 36례(84%)로 가장 많았다. 종괴의 국소적 위치는 변연부에 위치한것이 33례(75%)의 주로 폐의 변연부에 위치하였다. 조직 채취는 42례(95%)에서 가능했으며, PTNB의 민감도는 61%(27/44)였고 악성종괴와 양성종괴에 대한 민감도는 각각 79%(19/24), 44%(8/18)였다. 민감도가 비교적 낮은 이유는 단 1회 시술로 결과를 얻었고 민감도를 높이기 위한 반복 PTNB는 시행하지 않았기 때문으로 생각한다. 합병중인 기흉이나 각혈은 각각 1례에서 얻었고 모두 특별한 치료없이 호전되었다. 이상에서 PTNB는 다른 검사 방법 보다 간편하고 안전하며 기관지경을 통한 생검이 불가능하거나 생검후에도 병리소견을 못얻을 경우 PTNB를 통해 양성질환과 악성질환을 구분하므로서 환자의 치료 방침 결정에 도움을 줄 수 있을 것으로 사료되며, 국소적 폐 질환의 진단에 있어서 PTNB의 유용성은 더욱 증가하리라고 전망된다.

  • Extraskeletal mesenchymal chondrosarcoma is a relatively rare tumor and its cytologic findings have rarely been reported. We experienced a case of mesenchymal chondrosarcoma of soft tissue of the right lateral neck diagnosed by fine needle aspiration biopsy in a 59 year-old man. Cytologic findings showed two cell components. One was an undifferentiated, small cell component with moderate amount of cytoplasm and spindle nuclei. The second population was a chondroid component. These cytologic findings were diagnostic to mesenchymal chondrosarcoma.

굴곡성 기관지경을 이용한 성인의 기도내 이물 제거 (Fiberoptic Bronchoscopy for Removal of Endobronchial Foreign Bodies in Adults)

  • Foreign body aspiration occurs uncommonly in adults. A review of recent reports reveals only a few case reports on this topic. We have experienced 8 cases of endobronchial foreign bodies in adults from June 1988 to February 1991 which were removed successfully with fiberoptic bronchoscope and biopsy forceps. Only one of them had the primary disorder predisposing aspiration. Foreign bodies were located in right lower lobe (3 cases), right intermediate (2 cases), left lower lobe (2 cases) and left upper lobe bronchus (1 cases). Removal of endobronchial foreign boides in adult would be accomplished by fiberoptic bronchoscopy successfully.

  • Amyloid golfer is a rare disease entity that is defined as a symptomatic mass or clinically detectable thyroid enlargement because of amyloid deposition. We present a case of amyloid golfer diagnosed in the fine needle aspiration cytology(FNAC) in a 73-year-old Korean woman presented with nephrotic syndrome and thyroid enlargement. The thyroid function was in normal range. Thyroid scan showed a nodule, $4{\time}2cm$ in the right lobe with underlying diffuse golfer. Aspirates revealed benign looking follicular cells and scattered eosinophilic material. The sections of the cell block showed nodular deposit of eosinophilic hyalinized material in the interfollicular area. It showed apple-green birefringence under polarization with Congo red stain. The renal biopsy also exhibited deposition of eosinophilic materials in the glomeruli and interstitial vascular wall, which were confirmed as amyloidosis. This material was morphologically distinct from the colloid.

췌장암의 경피성 세침 흡인 세포학적 검색 (Percutaneous Fine Needle Aspiration Cytology of Malignancies of the Pancreas)

  • The fine needle aspiration (FNA) cytology findings in 19 cases of primary neoplasia of the pancreas are reported. The aspirates were obtained under ultrasound guidance in 16 cases and under direct vision intraoperatively in three cases. These cases represented 79% of 24 diagnoses in a series of 30 pancreatic FNAs. Of these 30 cases no cytologic diagnoses were made in six cases (20%) because of insufficient or inadequate samples The cytologic diagnoses were confirmed by histologic examination following resection or biopsy of the tumors. The diagnoses included 9 duct ceil adenocarcinomas, 1 mucinous adenocarcinoma, 2 mucinous cystadenocarcinomas, 1 acinar cell carcinoma, 1 papillary cystic tumor 3 islet ceil tumors, 1 neuroendocrine carcinoma and 1 leiomyosarcoma. The cytologic features of the neoplasia were detailed and the differential diagnosis was discussed. The important criteria for the cytodiagnosis of pancreatic tumors were reviewed. This review leads us to think that nonoperative (percutaneous) cytologic approaches to the diagnosis of pancreatic tumor are advantageous for the management of patients, and that correct cytologic diagnosis with pancreatic FNAs can easily be made, if adequate samples are obtained.
