Invasive Aspergillosis Involving the Lungs and Brain after Short Period of Steroid Injection: A Case Report

  • Invasive pulmonary aspergillosis (IPA) has emerged as a severe infection in patients with immunocompromised hosts. However, recently, several IPA cases, without an apparent predisposition to immunodeficiency, has been reported. A 72-year-old woman was admitted for evaluation of general weakness and poor oral intake. She reported no medical history, except for intraarticular injection of a corticosteroid for joint pain for the duration of two months. A chest radiography revealed multiple cavitary nodules in both lungs. Examination of specimens, obtained by percutaneous needle biopsy, led to a diagnosis of invasive aspergillosis. Brain magnetic resonance imagining revealed numerous peripheral thin enhancing cystic nodules in both cerebral hemispheres. We initiated intravenous administration of amphotercin B. However, the patient died after nine days. Here, we report an invasive aspergillosis case, which involves the lungs and brain after a short period of steroid injection.

  • The incidence of aspergillosis infections in the maxillary sinus has increased recently, because of overuse of antibiotics, steroids, anticancer agents, immunosuppressant, antimetabollites, and uncontrolled diabetes mellitus. The clinical features of maxillary sinus aspergillosis include pain, swelling and foul odor nasal excretion. This needs to be differentiated from bacterial maxillary sinusitis, and surgical treatment with antifungal agents are suggested. Recently, we treated two patients with maxillary sinus aspergillosis surgically (Caldwell Luc operation) and with antifungal agents(itraconazole). The results were satisfactory so we report these cases with literature review.

  • Primary infection of the larynx with aspergillus spp. with immunocompetent patient is extremely rare, few cases have been reported in the literature. It is more commonly seen as a part of a wider infection involving the respiratory system in immunocompromised patients. We present one case of primary laryngeal aspergillosis without any other airway tract extension and without any generalized immune deficit in a 69 year-old woman with history of hoarseness. Direct laryngoscopy and biopsy confirmed the diagnosis of aspergillosis.

  • Aspergillosis is not only the problem increasing the freguency for recent decades but also a serious one. Coexistent pulmonary diseases are frequent and the most common antecedent disease is Tuberculosis with healed cavitary lesion. Interest of the surgeons is aroused because of it`s predilection to sudden life-threatening, exsanguinating pulmonary hemorrhage. 11 patients with pulmonary aspergillosis evaluated at this hospital in a 7-year period, and the results were as follows: 1.male to female ratio was 1:1.75. 2.The most common symptom was gross or microscopic hemoptysis[72.7%]and other symptoms were chest pain[9.1%] and chronic productive coughing[9.1%]. 3.The right upper lobe was most common involving site[81.8%] 4.Surgical interventions were 8 cases of lobectomy, 1 case of lobectomy with wedge resection, 2 cases of wedge resection. 5.Post-operative complications developed in 5 patients and were 4 cases of wound infections, 2 cases of empyemas, 1 case of bleeding, 1 case of atelectasis.

Concurrent Nocardia Related Brain Abscess and Semi-Invasive Pulmonary Aspergillosis in an Immunocompetent Patient

  • We describe here the first case of a concurrent brain abscess caused by Norcardia spp. and semi-invasive pulmonary aspergillosis in an immunocompetent patient. After one year of appropriate antimicrobial therapy and surgical drainage of the brain abscess, the nocardia brain abscess and pulmonary aspergillosis have resolved.

  • This paper describes that invasive aspergillosis was found in oral cavity, especially in lower molar periodontium in a immunocompromised 34-year-old woman who had been admitted in hemato-oncology with the diagnosis of acute myeloid leukemia. Antifungal therapy and surgical excision of involved teeth, bone and gingiva were the treatment of choice. After treatment infected area was healed very well. We would like to report our case because we could not find any paper reporting on invasive aspergillosis occurring in lower molar periodontium.

  • The cutaneous aspergillosis is one of the most common dermatologic manifestations of disseminated infections associated with the Aspergillus organisms, but the isolated primary cutaneous disease itself can rarely occur in an immunocompetent host. We report a case of the primary cutaneous aspergillosis on the scalp and the neck in a 39-year-old immunocompetent male patient. There was a single purulent ulcer surrounded by the erythematous indurated plaque on the scalp with multiple satellite papules and a multi-lobulated granulomatous plaque with a crust on the neck. Skin biopsy demonstrated a fungus, the Aspergillus, in the deep dermis as the etiologic agent. No evidence of involvement in other organs was found. The patient exhibited no other apparent systemic diseases nor immunologic defects. An elliptical excision and a primary closure was performed, and the adjuvant antifungal treatment, oral itraconazole, was applied to prevent the recurrence by the satellite lesions.

  • Infection by aspergillus, which is a kind of mold, or a filamentous fungus, occurs rarely in larynx. Furthermore, primary laryngeal aspergillosis without any other airway tract extension and without any generalized immune deficit is extremely rare. We present a case of primary aspergillosis on vocal cord in a 72-year-old male who had no history of immune deficiency, voice abuse or steroid use.

병아리 Aspergillosis 1례 (Report of Aspergillosis of a chick in Korea)

  • A case of Aspergillosis is reported in a 12-day-old chick. The chick died with clinical signs such as dyspnea, gasping, and accelerated breathing. At necropsy, small white caseous nodules (approximately 1 mm in diameter) scattered throughout lung tissue. Upon H&I staining, multiple granulomas were observed in tertialy bronchi. Lesions consisted of central area of necrosis surrounded by macrophages, giant cells, lymphocytes, and some fibrous tissues. Hyphae of Aspergillus in the lesion branched into Y type, which was observed by PAS staining for fungus.

육용오리에서 aspergillosis 증례 (A case of aspergillosis in commercial domestic ducks)

  • A case of aspergillosis was observed in a flock of 5~10 days old commercial domestic ducks in Jeongeup. The clinical signs were depression and breathing by gasping or rapid opening of the bill. Grossly, lung was attached to pleura with white cheesy plaques. Numerous nodules was found in lung. Histopathologically, severe granulomatous pneumonia with intralesional fungal hypha were observed in the lung. Aspergillus spp. colonies was isolated from fungal culture and identified by polymer chain reaction.
