The incidence of septic arthritis after knee arthroscopy surgery has rarely been reported in approximately 0.04% to 3.4%. Moreover, septic arthritis after arthroscopic meniscal repair is more rare. There have been some reported cases, but in Korea, it has not been reported thus far. Herein, we report two cases of septic arthritis after arthroscopic meniscal repair with review of literature.

A 25-year-old man presented with a history of pain and crepitus in the right shoulder; he had been previously treated with arthroscopic anterior stabilization using four metallic suture anchors for recurrent traumatic anterior instability 1 year earlier. In this report, we present a patient with recurrent glenohumeral instability combined with anchor-induced arthropathy who was managed with modified arthroscopic transglenoid reconstruction following arthroscopic suture anchor retrieval.

Recently, the interest on anatomical ACL reconstruction with double bundle technique is increased to reproduce the original load distribution, and kinematics of the knee. We developed an arthroscopic double bundle ACL reconstruction technique using autogenous quadriceps tendon with 2 splitted graft and patellar bone plug. The anteromedial bundle and posteolateral bundle of the ACL is replicated with each splitted graft of quadriceps tendon and fixed with biodegradable interference screw on the 2 femoral tunnels. The patellar bone plug of quadriceps tendon is fixed with biodegradable interference screw within the 1 tibial tunnel. We suggest that our technique using quadriceps tendon may be an alternative in arthroscopic double bundle ACL reconstruction.

Retear patterns after arthroscopic rotator cuff repair are classified into two patterns according to retear location. Type 1 is when the retear pattern occurs directly on the tendon at the bone repair site using the suture anchor repair method. Type 2 is when the retear pattern occurs at the musculocutaneous junction with a healed footprint in patients who undergo the suture bridge method. Here, the authors report another retear pattern, which was identified as a type 2 retear on magnetic resonance imaging in patients who had undergone arthroscopic rotator cuff repair by the suture-bridge technique. This pattern was different from the type 2 retear and occurred at the portion of the cuff away from the healed rotator cuff under the view of the arthroscope.

The giant cell tumor of tendon sheath is very rarely present inside the knee joint. The authors report a case of intraarticular giant cell tumor of tendon sheath arising from posterior cruciate ligament which was successfully excised arthroscopically using posterior trans-septal portal at the time of arthroscopic reconstruction of anterior cruciate ligament.

This article describes a modified arthroscopic technique of anterior cruciate ligament (ACL) reconstruction using quadrupled hamstring tendon graft. The autogenous semitendinosus and gracilis grafts are harvested without detachment of the tibial insertion. To obtain longer graft, the accessory tibial insertions of the hamstring tendons are dissected. The EndoButton(Acupex Microsurgical, Andover, MA) is used for femoral fixation and two spiked staples are used for tibial fixation in a belt buckle fashion. Then the residual anterior laxity is restored by additional absorbable interference screw fixations. In this technique. more viable graft is obtained and firmer distal fixation is achieved by preservation of the tibial insertion of hamstring tendons.

Isolated posterior cruciate ligament injuries are rare and their management is controversial. But, there is general concept that a bony avulsion of posterior cruciate ligament should be repaired. The treatments for the bony avulsion of posterior cruciate ligament were conservative treatment, open reduction and internal fixation and arthroscopic fixation. We report 2 cases of posterior cruciate ligament avulsion fractures, which were arthroscopically reduced and stabilized with cannulated screws and Kirschner wires, and introduce the arthroscopic fixation technique.

A making appropriate portals is most important start in the arthroscopic surgery, and a step which determines the easiness of the operation procedures. It affects the precision of the examination of the target joints, making the next portals, and handling the instrument in the joint space. The knowledge about the normal and pathologic structures and understanding the portals should be needed to make an arthroscopic surgery ease.

An isolated tear of the subscapularis is uncommon, and there are a few literatures regarding the treatment of this problem. But, the incidence has increased with development of the arthroscopic techniques. An all-arthroscopic rotator cuff repair is a challenging procedure that can be effectively performed for treatment of subscapularis tendon tears. Often, tears of the subscapularis tendon do not involve entire tendon, and retraction of the torn edge is within to 2 cm of its attachment site. Occasionally, the entire tendon is torn and retracted medially to the glenoid. This article outlines the examination, preoperative planning and details the steps necessary to perform this procedure on upper third of subscapularis tears.
