During the last few years, nitric oxide(NO) as a potent macrophage-derived effector molecule against a variety of bacteria, parasites, and tumors has received increasing attention. More recent studies suggest that NO also has antiviral effects in both murine and human cells. The objective of the current study was to determine the effect of Yongdamsagantang(YST) on the production of NO. Stimulation of mouse peritoneal macrophages with YST after the treatment of recombinant $interferon-{\gammer}(rlFN-{\gammer})$ resulted in the increased NO synthesis. YST had no effect on NO synthesis by itself. When YST was used in combination with $rIFN-{\gammer}$, there was a marked cooperative induction of NO synthesis in a dose-dependent manner. The optimal effect of YST on NO synthesis was shown 6 hour after treatment with $rIFN-{\gammer}$. This increase in NO synthesis was reflected as increased amount of inducible NO synthase(iNOS) protein. NO production was inhibited by $N^G-monomethyl-L-arginine$. The increased production of NO from $rIFN-{\gammer}$ plus YST-stimulated cells was decreased by the treatment with staurosporin. In addition, synergy between $rIFN-{\gammer}$ and YST was mainly dependent on YST-induced tumor necrosis $factor-{\alpha}(TNF-{\alpha})$ secretion. These results suggest that the capacity of YST to increase NO production from $rIFN-{\alpha}-primed$ mouse peritoneal macrophages is the result of YST-induced $TNF-{\alpha}$ secretion.

Coronavirus disease 2019 (COVID-19) is currently a pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). COVID-19 are directly associated with hyper-activation of innate immune response that excessively produce pro-inflammatory cytokines and induce cytokine storm, leading to multi-organ-failure and significant morbidity/mortality. Currently, several antiviral drugs such as Paxlovid (nirmatrelvir and ritonavir) and molnupiravir are authorized to treat mild to moderate COVID-19, however, there are still no drugs that can specifically fight against challenges of SARS-CoV-2 variants. Panax ginseng, a medicinal plant widely used for treating various conditions, might be appropriate for this need due to its anti-inflammatory/cytokine/viral activities, fewer side effects, and cost efficiency. To review Panax ginseng and its pharmacologically active-ingredients as potential phytopharmaceuticals for treating cytokine storm of COVID-19, articles that reporting its positive effects on the cytokine production were searched from academic databases. Experimental/clinical evidences for the effectiveness of Panax ginseng and its active-ingredients in preventing or mitigating cytokine storm, especially for the cascade of cytokine storm, suggest that they might be beneficial as an adjunct treatment for cytokine storm of COVID-19. This review may provide a new approach to discover specific medications using Panax ginseng to control cytokine storm of COVID-19.
연구배경: HSVtk/GCV를 이용한 유전자치료에서 면역반응은 1) adenovirus 혹은 retrovirus와 같이 벡타로 사용된 virus의 단백질, 2) 치료목적으로 이입된 HSVtk 유전자의 생성물, 3) 암세포에 대해서 일어날 수 있다. 그리고 이러한 면역반응은 cytokines의 생성 혹은 cytotoxic tumor-specific T-cell의 생성을 초래하여 bystander effect에 의한 살상효과를 증가시키거나, anti-tumor immunity를 유도하여 tumor vaccine의 효과를 나타낼 수 있다. 한편 이와는 대조적으로 면역반응용 HSVtk 유전자를 발현하는 세포들을 파괴하여 이입된 HSVtk 유전자의 발현기간을 제한함으로서 유전자치료의 효과를 감소시킬 수도 있다. 본 연구는 retrovirus 벡타로 이입한 HSVtk 유전자치료에서 면역체계가 bystander effect에 의한 살상효과에 미치는 영향을 규명하고 면역체계가 이입한 유전자의 발현에 미치는 영향을 조사하고자 하였다. 방 법: Immunocompetent mice인 Balb/c mouse와 immunodeficient mouse인 Balb/c-nude 및 SCID mouse에서 retrovirus 벡타를 사용하여 HSVtk 유전자를 이입하고 치료효과를 조사하였다. 그리고 Balb/c mouse에 면역억제제인 cyclosporin을 투여하여 면역억제제가 bystander effect 및 유전자치료 효과와 유전자의 발현기간에 미치는 영향을 조사하였다. 결 과: Balb/c mouse에 HSVtk 유전자를 이입하고 GCV를 투여한 군은 GCV를 투여하지 않은 대조군에 비해 종양의 성장이 유의하게 억제되었으나 Balb/c-nude mouse와 SCID mouse의 경우 GCV를 투여한 군과 대조군 사이에 유의한 차이가 없었다. 면역억제제인 cyclosporin을 투여한 군에서 유전자 치료 효과가 cyclosporin을 투여하지 않은 정상 mouse에 비해 치료효과가 유의하게 작았다. Cyclosporin 투여에 따른 유전자의 발현기간에는 유의한 차이가 없었다. 결 론: Retrovirus 벡타를 사용한 HSVtk 유전자치료에는 면역증강이 치료효과를 증가시킬 것으로 생각된다.
목 적: 소아 만성 B형 간염 환아들에서 라미부딘 치료의 효과와 지속성에 대해 알아보고자 하였다. 방 법: 1998년 1월부터 2008년 5월까지 서울아산병원 소아과에서 만성 B형 간염으로 진단받고 라미부딘치료를 받은 44명의 환아들의 의무기록을 후향적으로 분석하였다. 12개월 이상 추적 관찰했던 환아들을 대상으로 발병시 성별, 연령, 치료 전후의 ALT, 혈청 HBVDNA, HBeAg, anti HBe, HBsAg, anti HBs의 변화를 조사하였고, 치료의 지속성과 내성 발현에 대해 조사하였다. 결 과: 라미부딘 치료 후 3년 이내에 총 44명의 환아중 21명(48%)에서 혈청 전환이 이루어졌고, 34명(77%)에서 HBV DNA가 음전되었으며, 41명(93%)에서 혈청ALT가 정상화되었다. Kaplan-Meier법에 의한 HBeAg 누적 혈청 전환율은 3년째에 60%로 나타났다. 치료를 마친 환아 25명 중 혈청 전환된 18명의 환아 모두 약물중단 이후 최대 3년 추적 관찰시 재발 없이 지내고 있어 라미부딘 치료 반응의 지속성을 보여 주었다. 치료 중 12명(27%)에서 돌파 현상이 나타났고, 11명(25%)에서 YMDD 돌연변이가 발견되었다. 치료 전 혈청 ALT 수치는 혈청 전환을 이룬 군에서 더 높았지만, 통계적 유의성은 없었다(338${\pm}$542 IU/L vs. 144338${\pm}$163 IU/L, p> 0.05). 결 론: 만성 B형 간염 환아에서 라미부딘 치료시 절반 정도에서 치료 효과를 기대할 수 있었다. 라미부딘의 치료 반응은 장기적으로 지속되었으며, 향후 약제 내성 돌연변이의 증가에 따른 이차 약제에 대한 추가적인 연구가 필요하다고 생각된다.
최근 수십 년 간 종양 용해성 바이러스(Oncolytic viruses; OV)는 암 치료제로서의 잠재성에 의해 광범위하게 연구되어왔다. 종양 용해성 바이러스는 두 가지의 독특한 장점을 가지고 있는데, 첫째로 암세포만을 특이적으로 감염시키고 사멸시킬 수 있다는 것이고, 두 번째로는 암이 진행되는 초기 단계에 숨어서 인식되지 않는 상태인 종양 관련 항원들을 인식하는 특정한 적응 면역을 활성화 시키는 것이다. 2015년에는 유전자 변형 종양 용해성 바이러스인 Talminogene laherparepvec (T-VEC)이 미국 식약청(FDA)의 승인을 받았으며, 현재는 다양한 종양 용해성 바이러스들이 단일로 사용되거나 기존의 암 치료 방법인 면역 치료법, 방사선 치료법, 화학 치료법과 함께 사용되어 임상 시험에서 활성이 연구되고 있다. 종양 용해성 바이러스 치료법의 효능은 항 종양 면역 활성과 항바이러스 반응의 균형이 어느 정도인가에 의해 조절되기 때문에, 획기적인 성과에도 불구하고 암 치료를 위한 종양 용해성 바이러스의 개발은 전달 방법, 바이러스를 인식하는 신체 내 항체 및 종양의 복잡성, 가변성, 반응성에 따른 항바이러스의 면역 유도와 같은 다양한 장애물을 극복하여야 하는 문제가 있다. 종양 내에 직접 종양 용해성 바이러스를 투여하는 방법은 눈에 띄는 부작용이 없이 고형 종양을 줄이는 것에 성공하였으나, 아쉽게도 뇌종양 같은 일부 종양에는 사용할 수 없고 전신 투여가 필요한 단점이 존재한다. 이러한 장애물들을 극복하기 위해서 종양 용해성 바이러스의 효능을 높이기 위한 형질 전환 유전자의 삽입 혹은 면역 조절 물질과 바이러스를 조합하는 등의 다양한 전략들이 개발되고 있다.
목 적 : B형 간염 바이러스(hepatitis B virus; HBV) 보유 산모로부터 출생한 신생아의 수직감염 양상을 관찰하고 HBV 백신의 면역반응 및 방어효과를 평가하기 위하여 본 연구를 시행하였다. 대상 및 방법 : 1995년 7월부터 1996년 12월까지 18개월간 가톨릭대학교 성빈센트병원을 방문한 HBV 보유 산모와 이들로부터 분만된 신생아중 12개월간 추적관찰이 가능하였던 78례를 연구대상으로 하였다. 이들 HBV 보유 산모의 신생아에게 출생 직후부터 HBV 면역 글로불린(HBIG)과 가열 불활성화 혈장 백신을 접종한 후 4, 8, 12개월시에 검체를 채취하여 일반표지자의 양상을 면역효소법 및 방사면역법으로 측정한 후 HBV 보유 산모와 그 신생아의 각 표지자간의 관계 및 변화를 분석하였다. 결 과 : 1) 연구 대상 산모 중 HBV 보유자는 5.0%(106/2,117), HBeAg 양성 및 음성의 비율은 38.5%(37/96)와 61.5%(59/96)이었다. 2) HBV 보유 산모로부터 출생한 영아에 있어서 백신에 대한 anti-HBs 양전율은 4, 8, 12개월 시 각각 85.9%(67/78), 75.6%(59/78), 73.1%(57/78)로 유의한 차이가 있었으나(P<0.05), 산모의 HBeAg 유무와는 관련이 없었다. 형성된 anti-HBs의 기하 항체가는 4개월에 비하여 8개월, 12개월에 유의하게 증가하였다(P<0.05). HBV 백신과 HBIG 투여의 HBV 보유자 이행에 대한 12개월시의 방어 효과는 HBeAg 양성 및 음성 산모로부터 분만된 영아에서 각각 89.8%와 100%이었다. 3) 12개월의 관찰 기간동안 산모로부터 감염이 있었던 경우는 78명 중 5명으로 모두 HBeAg 양성 산모에서 분만된 영아이었고 이 중 3명에서 HBV 보유자로 이행되었다. 출생 직후 HBeAg 양성 및 음성 산모의 신생아에서 HBsAg 양성이었던 경우는 각각 4명으로 HBeAg 양성 산모의 신생아 4명 중 3명에서 감염이 발생하였다. 결 론 : 국내에서 개발된 백신의 하나인 가열 불활성화 B형 간염 혈장 백신의 항체 양전률은 85.9%이었으며, HBIG과 동시 투여하였을 때 12개월간의 HBV 보유자 이행에 대한 방어효과는 HBeAg 양성 산모에서 출생한 경우 89.8%인 반면 HBeAg 음성 산모에서 출생한 경우는 100%이었다.
Cervical cancer is one of the leading causes of female death from cancer worldwide with about 500,000 deaths per year. A strong association between certain human papilloma viruses (HPV types 16 and 18) and cervical cancer has been well known. An extract of natural products, named as Somatid, has been used to investigate whether this agent has the ability of inhibiting the oncogenes E6 and E7 of HPV type 16. This Somatid inhibited the proliferation of human cervical cancer cell lines (C-33A, SiHa, CaSki) and HaCaT keratinocytes in a dose response manner, In vitro binding assay and ELISA showed that Somatid inhibited the in vitro biding of E6 and E6AP which are essential for the binding and degradation of the tumor suppressor p53. In addition, Somatid inhibited the in vitro binding of E7 and Rb which is essential tumor suppressor for the control of cell cycle. The levels of mRNA for E6 and E7 were also decreased by Somatid. Our data suggested that Somatid inhibited the oncogenecity of E6 and E7 of HPV 16 type, thus can be used as a putative anti-HPV agent for the treatment of cervical carcinomas caused by HPV.
