Effect of Actinidia polygama on LPS-induced Inflammation in Mouse BV2 Microglial cells (목천료자(木天蓼子)가 LPS로 유되된 Mouse BV2 Microglial cells의 염증반응에 미치는 영향)

  • Actinidia polygama has long been used in traditional Korean medicine to treat rheumatoid arthritis and gout. Although numerous chemical compounds in the fruit extracts of A. polygama have been characterized and their role in inhibiting nitric oxide (NO) production has been reported, the anti-inflammatory properties of A. polygama extracts remain to be explored. In this study, we investigated the in-vivo effect of A. polygama extract on lipopolysaccharide (LPS)-induced inflammation in BV-2 microglial cell lines. We discovered that 100% ethyl alcohol extract of A. polygama effectively attenuates the release of NO and is superior to both water extract and 50% ethanol extract. Using MTT assay, western blot, and ELISA on LPS-induced BV-2 microglial cells lines, we established the ability of A. polygama extract to markedly suppress the expressions of inducible NO synthase (iNOS), cyclooxygenase-2 (COX-2), and pro-inflammatory cytokines, such as tumor necrosis factor alpha and interleukin-6. These results reveal that the anti-inflammatory property of A. polygama in BV-2 microglial cells is due to the downregulation of iNOS, COX-2, MAPK protein, and pro-inflammatory cytokines.

The Therapeutic Effect of Medical Patches Including Isoegomaketone on the Collagen Antibody-induced Arthritis in Mice (관절염 유발 동물 모델에서 이소에고마케톤 함유 패치의 관절염 치료 효과)

  • This study identified effects of Isoegomaketone for applicable patch on the arthritis in mice. Isoegomaketone (IK) was isolated from Perilla frutescens, which is annual herbal traditional medicinal, aromatic, functional food. IK has various physiological effects such as anti-inflammation, anti-oxidant, and anti-cancer. In the previous study, oral administration of IK to a mouse model of collagen antibody-induced arthritis(CAIA), which is similar to human rheumatoid arthritis(RA), alleviated symptoms. In this study, we attached a patch containing IK to mouse skin to demonstrate whether it had the same efficacy as oral administration in CAIA mouse. As a result of measuring the arthritis score, paw volume, and paw thickness, it was confirmed that arthritis symptoms were alleviated in the group to which the patch containing IK was attached. These results show that IK is effective in alleviating arthritis not only through oral administration but also through patches applied to skin, and that it has potential as a material for future patch development.

Anti-inflammatory Effects of Hydrogels Containing Piroxicam and Hydrolyzed Products of Scutellariae Radix on Collagen-induced Arthritis (콜라겐으로 유발된 관절염에 대한 피록시캄 및 황금 가수분해물 복합 히드로겔의 항염 효과)

  • In order to access the suppressive effects of piroxicam (PX) and hydrolyzed products of Scutellariae Radix (PSH) on arthritis, we investigated whether PSH gel could suppress the progression of collagen-induced arthritis. PX, one of nonsteroidal anti-inflammatory drugs has been used in the systemic and topical treatment in a variety of inflammatory conditions. Scutellariae Radix, one of the herbal medicines, was used for the purpose of anti-inflammatory and anti-bacterials. For the purpose of transdermal absorption of the hydrogel preparations, two classes of hydrogels (PX, PSH) were formulated with carbomer 940, diethylene glycol monoethyl ether, polyethylene glycol-8-glyceryl caprylate/caprate and triethanolamine. In carrageenan-induced edema in rat hind paws, inhibition of foot swelling was more increased in PSH than PX hydrogel. Rheumatoid factors including serum IgG, IgM and collagen specific antibody were present much lower in PSH gel treated mice than control. Histological examination revealed that PSH hydrogel inhibited infiltration of inflammatory cells into affected paw joint, compared with control. The PSH hydrogel would be a suitable preparation to increase transdermal treatment for anti-inflammatory effects on collagen-induced arthritis.

  • Rheumatoid arthritis (RA) is a chronic inflammatory autoimmune disease. characterized by leukocyte infiltration, a chronic inflammation of the joint, a pannus formation and the extensive destruction of the 3Iticular caJ1ilage and bone. Several proinflammatory cytokines such as tumor necrosis factor-${\alpa}$(TNF-${\alpa}$), interleukin-1${\beta}$ (IL-1${\beta}$) and interleukin 6 (IL-6) have been implicated in the pathological mechanisms of synovial tissue proliferation, joint destruction and programmed cell death in rheumatoid joint. In the Korean traditional medicine, Hominis placenta (HP) as an herbal solution of herb-acupuncture has been widely used to treat the inflammatory diseases including RA. In order to study the medicinal effect of HP herb-acupuncture on rheumatoid joint, an adjuvant-induced arthritis (AlA) was generated by the injection of 1.5 mg uf Mycobactelium tuberculusis. emulsified in squalene, 10 the base of the tail of Sprague-Dawley(SD) rats. After onset stage of polyarthritis, HP was daily injected to the Zusanti (ST36) acupuncture points in both of rat lags and the expression pattems of cytokines such as TNF-{\alpa}$, IL-1${\beta}$, and 1L-6 at the knee joint were analyzed using immunostaining and RT-PCR. The HP herb-acupuncture was found to be effective to alleviate the arthritic symptums in adjuvant-induced arthritic rats as regards the joint appearance and the expression profiles of inflammatory cytokines. In conclusion, therapeutic effects of HP herb-acupuncture on the rat with AlA might be related to anti inflammatory activities of the hurb-acupuncture.

  • Methotrexate is commonly used in rheumatoid arthritis as an anti-inflammatory agent, but treatment with methotrexate can lead to severe side effects, especially pulmonary complication. Interstitial pneumonitis is one of the most important pulmonary adverse effects of methotrexate and most patient present with a subacute febrile illness and peripheral eosinophilia is seen in about a half of patients. Almost all patients have abnormal chest roentgenograms and bibasilar interstitial infiltration with alveolar pulmonary consolidations is the most characteristic finding. Interstitial inflammation with mononuclear cell infiltration is a characteristic pathologic feature and findings that suggest acute hypersensitivity pneumonitis, such as bronchiolitis, granuloma formation with giant cells, and infiltration with eosinophils are often present. Methotrexate-induced pneumonitis is a potentially life threatening and unpredictable complication but it is difficult to make a definite diagnosis in the absence of high index of clinical suspicion. Early recognition and appropriate management may avoid the serious outcome. Herein we report a case of methotrexate-induced pneumonitis in a patient with rheumatoid arthritis.

A case of encephalitis in a juvenile rheumatoid arthritis patient treated with etanercept (소아기 류마티스모양 관절염 환자에서 etanercept 사용 후 발생한 뇌염 1예)

  • Tumor necrosis $factor-{\alpha}$ ($TNF-{\alpha}$) is a major proinflammatory cytokine involved in the pathophysiology of juvenile rheumatoid arthritis. Etanercept is an effective inhibitor of $TNF-{\alpha}$ and has shown a beneficial effect in patients with JRA. However, the most important cause of concern related to etanercept administration is infection. We report a case of encephalitis in a JRA patient receiving long-term treatment with etanercept. The patient was a 4-year-old boy with refractory JRA, and he received etanercept subcutaneously at a dose of $0.4\;mg\;kg^{-1}\;day^{-1}$ twice a week for 14 months, along with non-steroidal anti-inflammatory drugs, methotrexate, oral steroids, and sulfasalazine. The patient presented with sudden fever, headache, vomiting, a generalized tonic seizure, and changes in mental status. We suspected a central nervous system infection, and simultaneously administered antibiotics, an antiviral agent, and steroids. After 2 days of hospitalization, his mental function returned to normal, and he showed no further seizure-like movements. Brain magnetic resonance imaging scan of the patient showed a multifocal cortical lesion on both sides of the temporoparietooccipital lobe, which indicated encephalitis. Although we were unable to identify the causative organism of encephalitis, we think that the encephalitis may be attributed to infection, and the use of etanercept may have increased the risk of severe infection. Therefore, etanercept was discontinued and the patient recovered shortly after. To the best of our knowledge, this is the first case of encephalitis in a juvenile rheumatoid arthritis patient treated with etanercept.

Effects of Samki-eum ($s\bar{a}nq\grave{i}-y\check{i}n$ ) on Freund's Complete Adjuvant-induced Arthritic Rats (Adjuvant 유발 관절염 랫트에 대한 삼기음의 효과)

  • Objectives The object of this study was to observe the favorable anti-arthritic effects of Samki-eum ($s\bar{a}nq\grave{i}-y\check{i}n$) on Freund's complete adjuvant (FCA)-induced arthritic Wistar rats. Methods Rheumatoid arthritis was induced by intradermal injection of FCA, and 300, 150 or 750 mg/kg of Samki-eum ($s\bar{a}nq\grave{i}-y\check{i}n$) were orally administered once a day for 14 days from 14 days after FCA treatments, and 15 mg/kg of dexamethasone was intraperitoneally administered as reference drug in this experiment. All rats were sacrificed at 14 days after continuous oral treatment of Samki-eum ($s\bar{a}nq\grave{i}-y\check{i}n$) or intraperitoneal administration of dexamethasone, and changes on the body weight, knee circumferences, gross arthritis score, inflammatory tissue prostaglandin (PG) $E_2$ levels were monitored with cartilage collagen components and glucosaminoglycans compositions - chondroitin sulphate, heparan sulphate and hyaluronic acid in the present study. Results As results of FCA treatment, classic rheumatoid arthritis featuring dramatical decreases on the body weights, cartilage collagen contents and bone glucosaminoglycans-chondroitin sulphate, heparan sulphate and hyaluronic acid contents, with increases on the knee circumferences, gross arthritis scores and inflammatory tissue $PGE_2$ levels. However, these changes from FCA-induced rheumatoid arthritis were clearly reduced by treatment of dexamethasone and both two different dosages of Samki-eum ($s\bar{a}nq\grave{i}-y\check{i}n$) 300 and 150 mg/kg in the present study. Although FCA-induced arthritis were more favorably inhibited by treatment of dexamethasone 15 mg/kg as compared with Samki-eum ($s\bar{a}nq\grave{i}-y\check{i}n$) 300 mg/kg, marked decreases of body weights were detected in dexamethasone 15 mg/kg treated rats, and Samki-eum ($s\bar{a}nq\grave{i}-y\check{i}n$) 300 mg/kg showed similar preserve effects on the cartilage glucosaminoglycan compositions in this study. Conclusions The results obtained in this study suggest that over 300 and 150 mg/kg of Samki-eum ($s\bar{a}nq\grave{i}-y\check{i}n$) showed favorable anti-arthritic effects on the FCA-induced arthritis mediated by suppression of $PGE_2$, a inflammatory mediator. However, detail mechanism studies should be conduced in future with the screening of the biological active compounds in this herb. Although overall anti-inflammatory effects Samki-eum ($s\bar{a}nq\grave{i}-y\check{i}n$) 300 mg/kg were lowered than those of dexamethasone 15 mg/kg treated rats, Samki-eum ($s\bar{a}nq\grave{i}-y\check{i}n$) 300 mg/kg treated rats showed similar preserve effects on the cartilage glucosaminoglycan compositions in this experiment.

  • Background and Purpose : This study shows the clinical effects on treatment with Rheumatoid arthritis using Aqua-acupuncture such as Ursi Fel Bovis Calculus and Cathami Flos that are known for which have anti-inflammatory and analgesic effects. Objective and Methods : This study has been carried out Aqua-acupuncture treatment such as Cathami Flos and Ursi Fel Bovis Calculus twice or three times a week with patients diagnosed as rheumatoid arthritis on 7 criteria of US Rheumatism Academy. Results : 1. If you see the sexual distribution, it shows the rates of male and female 1: 1.83. Age distribution shows as age ascend it shows high. 2. If you see the duration distribution, it shows chronic tendency in order of more than 6 months(63.3%), 2 to 5 months, and I month. 3. If you see the invaded distribution, it shows chronic tendency in order of hand(37 cases), wrist joint(34 cases), knee joint(32 cases), ankle joint(21 cases), elbow joint(16 cases), shoulder joint(14 cases), feet(14 cases), hip joint(9 cases), low back(6 cases), neck(4 cases). 4. Seeing the mean post-therapeutic index among groups on RA factor level distinction, it shows that 0-18(normal range) scored 89.0, 19-50 scored 94.8, 51-80 scored 101.2, 81-100 scored 97.3, 101-140 scored 102.4, above 141 scored 97.5. 5. Seeing the mean post-therapeutic index among groups on CRP level distinction, it shows that 0-0.4(normal range) scored 100, 0.5-1.0 scored 48.6, 1.1-2.0 scored 34.9, 2.1-3.0 scored 35.7, 3.1-4.0 scored 37.1, 4.1-5.0 scored 53.8, above 5.0 scored 70.4. 6. Seeing the mean post-therapeutic index among groups on ESR level distinction, it shows that 0-20(normal range) scored 48.6, 21-40 scored 57.5, 41-60 scored 59.2, 61-80 scored 59.2, above 81 scored 66.0. 7. If you see the satisfactory assesment after Aqua-acupuncture treatment such as Cathami Flos and Ursi Fel Bovis Calculus, Excellent is 11 cases, Good is 14 cases, and Moderate 5 cases. Conclusion : It will be better effects, if herbal medicine, moxibustion, and acupuncture are used together with Cathami Flos and Ursi Fel Bovis Calculus on rheumatoid arthritis as chronic inflammatory disease.

The Effects of Mulberry Extract Consumption on the Serum Levels of Oxidant and Inflammatory Factors in Middle-aged Women with Rheumatoid Factors (오디추출물 투여가 류머티즘 요인이 있는 일부 한국 중년여성의 혈중 항산화 및 항염증 관련 지표수준에 미치는 영향)

  • This study investigated the effect of mulberry fruit extract(MFE) on antioxidant and anti-inflammatory activities of middle aged women with rheumatoid factor (RF). Thirty two middle-aged subjects were divided into two groups which were normal middle-aged group (NMG) and abnormal middle-aged group whose serum RF level were > 10 u/mL (AMG). All groups had consumed MFE (100 mL/day) for 4 weeks. Anthropometric measurements, serum inflammatory factors, serum oxidative stress markers analyses were performed at baseline and then at 4 weeks following the study. There were no significant differences in anthropometric measurements, including BMI, WHR and body fat composition between two groups. But after 4 weeks MFE consumption, serum levels of C-reactive protein (CRP), ferric-reducing ability of plasma (FRAP), serum TNF-${\alpha}$, IL-2, IL-4 had significantly decreased (p<0.05) in AMG. These findings suggested that the MFE consumption as food may be protective against oxidation and inflammation like RA.

Effect of Citri Pericarpium ethanol extract on collagen-induced arthritis in mice (진피(陳皮) 에탄올추출물의 콜라겐 유도 관절염 마우스에서의 항관절염 효과 연구)

  • Objectives : The present study was undertaken to determine whether an ethanol extract of Citri Pericarpium (CP-E), which is the pericarp of Citrus unshiu Markovich is efficacious against collagen-induced arthritis (CIA) in mice. Methods : CIA was induced in male DBA/1J mice by intradermal injection of bovine collagen-II in Freund's incomplete adjuvant (IFA). The mice in the onset of arthritis were treated daily with oral administration of CP-E ethanol extract at different doses (50 and 100 mg/kg/bw) for 28 days. Arthritis index, histopathologic changes and the levels of TNF-${\alpha}$ as well as anti-CII IgG in blood were evaluated to confirm the anti-arthritic effect of CP-E on CIA in rats. Results : The results showed that comparing with untreated CIA mice, treated with CP-E extract significantly decreased the arthritic scores and the pathological changes of knee joint tissues, and also reduced the serum levels of TNF-${\alpha}$ and anti-CII IgG in CIA-mice. These results indicate that CP-E extract may effectively alleviate inflammatory response on CIA, and its anti-inflammation can be attributed, at least partially, to the inhibition of proinflamamtory cytokine, TNF-${\alpha}$ in CIA. Conclusions : This study suggest that CP-E has a therapeutic potential in inflammatory joint diseases such as rheumatoid arthritis.
