유전자재조합 B형간염 백신의 기본접종 5년후 추적항체역가에 관한 연구 (5 Years Follow up Study of Anti HBs Titer After Basic Immunization by Recombinant HBV Vaccine)

  • 목 적 : 혈장백신의 항체의 지속성에 관한 보고는 다양하지만, 유전자재조합 백신의 추적관찰에 대한 연구는 많지 않아 저자들은 신생아시기에 유전자재조합 백신을 기본접종 후 접종방법에 따른 5년 후 항체의 지속성과 기본접종 후 각각의 항체역가에 따른 5년 후 항체음전율에 대해 알아보고자 본 연구를 시도하였다. 방 법 : 1993년 4월부터 12월까지 부산 문화병원에서 태어난 신생아 420례중 유전자재조합 백신을 기본접종 후 5년까지 추적관찰이 가능했던 114례를 대상으로 하였다. 기본접종 후와 5년 후 항체역가의 검사는 RIA로 시행하였다. 결 과 : 1) 2개월 접종을 하였을 경우 5년째 항체음성률은 5%이고, 6개월 접종을 하였을 경우 5년째 항체 음성률은 25.5%이었다(P value <0.05). 2) 2개월접종시 항체역가가 199.9 이하시 5%, 200~499.9시 0%, 500~999.9시 0%, 1,000 이상시 0%가 5년 추적검사시 항체음전이 되었다. 3) 6개월 접종시 항체역가가 199.9 이하시 66.7%, 200~499.9시 40%, 500~999.9시 23.9%, 1,000 이상시 22.5%가 5년 추적검사시 항체음전이 되었다. 결 론 : 이 연구에서는 유전자재조합 백신의 5년후 항체음전율은 5~25.5%였고, 항체가의 지속성은 접종방법과는 통계학저인 유의성을 보이지 않았고, 기본접종 후의 항체역가에 따른 5년 후 항체음전율과도 통계학적인 유의성은 보이지 않았다.

혈청과 뇌척수액에서 B형 간염 바이러스 표식자 존재의 비교 (Comparision of Hepatitis B Virus Markers in the Serum and the Cerebrospinal Fluid)

  • 1994년 4월 1일부터 8월 31일까지 영남대학 의과대학 부속병원 신경과에 내원한 환자중에서 뇌척수액 검사를 시행한 환자 50례를 대상으로 혈청과 뇌척수액의 HBV 검출률과 HBV의 감염과 신경계질환의 관련성을 알아보기 위해 혈청과 뇌척수액에서 HBV 표식자를 검출하여 다음과 같은 결과를 얻었다. 혈청 HBV 표식자 검출로 HBsAg 양성률은 12.0%, HBV 감염률은 74.0% 이었으며, HBV에 의한 감염을 표식자 검출 양상에 따라 A군은 12.0%, B군은 62.0%이었고, C군은 26.0%이었다. 뇌척수액 HBV 표식자 검출로 HBsAg 양성률은 6.0%, HBV 감염률은 36.0%이었으며, A군은 100.0%, B군은 38.7%이었고, C군에서는 검출되지 않았다. 신경계 질환별로 분류하면 VDA군은 26명, NVAD군은 24명이었다. 혈청 HBV 감염률은 VAD군은 88.5%, NVAD군은 58.3%로 두 군간의 유의한 상관관계가 있었다(p<0.05). 뇌척수액의 HBV 표식자 검출은 VAD군 53.8%, NVAD군 16.7% 검출되었다 (p<0.05). VAD군에서 혈청 HBV 감염이 있는 경우 60.9%, NVAD군은 28.6%에서 뇌척수액 HBV 표식자가 검출되었으며, 대상환자의 수가 적어 두 군간의 통계학적 유의수준은 없었다. 이상의 결과로 보아 혈청 HBV 표식자가 검출되는 경우 일부 뇌척수액에서 HBV 표식자가 검출되며, 혈청 HBsAg 양성일때 검출률이 증가하고, 바이러스와 연관된 신경계 질환에서 혈청과 뇌척수액의 HBV의 검출률이 높았다.

오가피(五加皮)의 항암효과에 관한 연구 (Study on the Anti-Cancer Effect of Acanthopanax Sessiliflorus)

  • Objectives: Hepatoma is a very serious disease in Korea and vvorldwiclc. Hepatitis B vims (HBV) has proved the most significant cause of hepatoma. We canied out this study to investigate the effect of Acanthopanax sessilifloms on inhibiting cell proliferation and DNA synthesis in HepG2.2.15 cell line and on inhibiting phosphorilation of oncogene (MAP kinase) in NIT/3T3-HBx ceIl. Methods: To investigate the anti-cancer effect of Acanthopanax sessiliflorus, we did the CellTiter 96 Aqueous Non-radioactive Cell Proliferation assay (Promega); MTS/PMS assay, [$^3H$]-thymicline incorporation assay, and we measured the gene expression through westem blotting. Results: Acanthopanax sessiliflorus showed an inhibiting effect on the increase of HepG2.2.15 in the NTS/PMS assay. It also showed an inhibiting effect on DNA synthesis of HepG2.2.15 in the [$^3H$]-thymidine incorporation assay. Acanthopanax sessiliflorus showed an inhibiting effect of phosphorilation of MAP kinase in HBV - X genes. too. Conclusions: The results suggested that this herb had an anti cancer effect. We may discover an effective anti-cancer herb medicine through further studies on this herb medicine.

  • The nucleoside analogue, L-FMAUS was synthesized from L-FMAU which has been shown to have significant antiviral acitivity against hepatitis B virus (HBV). The anti-HBV activity and toxicity of the L-FMAUS were examined by a cell culture system using a hepatitis B virus (HBV) producing cell line, HepG2 2.2.15. L-FMAUS was assayed for the inhibition of HBV multiplication by measurement of HBV DNA and surface antigen (HBsAg) levels in the extracellular medium of HepG2 2.2.15 cells after an 8-day treatment. IFN-γ: A Crucial Player in the Fight Against HBV Infection?

  • About 0.8 million people die because of hepatitis B virus (HBV) infection each year. In around 5% of infected adults, the immune system is ineffective in countering HBV infection, leading to chronic hepatitis B (CHB). CHB is associated with hepatocellular carcinoma, which can lead to patient death. Unfortunately, although current treatments against CHB allow control of HBV infection, they are unable to achieve complete eradication of the virus. Cytokines of the IFN family represent part of the innate immune system and are key players in virus elimination. IFN secretion induces the expression of interferon stimulated genes, producing proteins that have antiviral properties and that are essential to cell-autonomous immunity. IFN-α is commonly used as a therapeutic approach for CHB. In addition, IFN-γ has been identified as the main IFN family member responsible for HBV eradication during acute infection. In this review, we summarize the key evidence gained from cellular or animal models of HBV replication or infection concerning the potential anti-HBV roles of IFN-γ with a particular focus on some IFN-γ-inducible genes.

인진청간탕(茵蔯淸肝湯)의 항암효과(抗癌效果)에 관(關)한 연구(硏究) (Study on the Anti-Cancer Effect of Injinchunggan-tang(Yinchenqinggan-tang))

  • Objectives: Hepatoma is a very serious disease in Korea and worldwide. Hepatitis B virus (HBV) has proved the most significant cause of hepatoma. We carried out this study to investigate the effect of Injinchunggan-tang (Yinchenqinggan -tang) on inhibiting cell proliferation and DNA synthesis in HepG2.2.15 cell lines and on inhibiting phosphorilation of oncogene (MAP kinase) in NIT /3T3-HEx cells. Methods: First we confinned the Hepatitis B virus producing ability of HepG2.2.15 cells. To investigate the anti-cancer effect of Injinchunggan-tang (Yinchenqinggan-tang), we did the NTS/PMS assay, [3H]-thymidine incorporation assay and transfection of pcDNA-X. We also measured the gene expression through western blotting. Results: Injinchunggan-tang (Yinchenqing gan tang) showed the suppressing effect of HepG2.2.l5 increase in the MTS/PMS assay and the inhibiting effect of DNA synthesis of HepG2.2.15 in the [3H] thymidine incorporation assay. Injinchunggan-tang (Yinchenqinggan-tang) also showed the inhibiting phosphorilation effect of MAP kinase in HBV -X genes. Conclusions: From the above Injinchunggan-tang (Yinchenqinggan-tang) is thought to have an anti-cancer effect on the hepatoma from HBV. It is suggested that further studies on this prescription would give us a better medicine with an anti-cancer effect.

B형 간염 전파관련요인에 관한 역학적 연구 (An Epidemiological Study on the Selected Risk Factors of Hepatitis B Virus Infection)

  • An attempt to confirm the associations of some selected risk factors of HBV infection and measure their risks, a cross-sectional study with 1,209 urban office workers was carried out. For the study, a simple questionnaire which contained several questions on personal experience and behaviors on several known selected risk factors of HBV infection was applied to each subject, and the Hepatitis B virus surface antigen and its antibody were checked by RPHA and PHA method, respectively. Risk factors chosen for this study were experience of blood transfusion and personal contact variables, such as frequencies of eating-out, drinking after office hours, going to tea room, sharing cigarettes, etc. The results obtained were as follows: 1. The proportion of HBsAg positive was 10.6%, and total HVB infected including the Anti-HBs positive cases without vaccination was 44.2%. Both were higher in male than in female. 2. Frequent personal contact through glasses and dishes in eating-outs and drinkings turned out not to be a significant risk factor of Hepatitis B surface antigenecity. 3. Frequent visits to tea room was a significant risk factor of HBV infection which combined HBsAg positive cases and Anti-HBs cases who had not received HBV vaccination. The odds ratio was 1.56 4. Blood transfusion was not a significant risk factor of both HBsAg positive and total HBV infection. In summary, indirect oral contacts through eating-outs and drinkings was not significant risk factor in Korea at least between adults. Blood transfusion is no more major source of HBV infection in Korea probably because the adquate screening test of HBsAg for the blood donors is being made.

수종(數種)의 한약재(韓藥材)가 B형 간염(肝炎)바이러스 증식억제(增殖抑制)에 미치는 효과(效果) (Effects of Several Herbal Medicines on the Replication of Hepatitis B Virus)

  • This study was performed to investigate an anti-HBV activities of the aqueous extracts from 10 Korean herbal medicines in the HepG2 2.2.15 cell culture system and the results were as follows: 1. The extracts of 6 plants (Herba Artemisiae Capillaris, Radix et Rhizoma Rhei, Cortex Cinnamomi, Fructus Chebulae, Fructus Rubi and Radix Rubi) decreased, significantly and dose-dependently, the levels of extracellular HBV virion in the concentrations (10, 100, 500 and $1,000\;{\mu}g/m{\ell}$) tested. 2. However, others (Radix lsatidis, Lignum Sappan, Herba Lysimachiae and Fructus Lycii) did not show any effect either on the replication of HBV or on the levels of virion DNA in the culture media of HepG2 2.2.15 cell. 3. Among the 6 plants which showed the inhibitory potency on the production of extracellular HBV virion, Radix et Rhizoma Rhei, Cortex Cinnamomi, Fructus Chebulae, Fructus Rubi and Radix Rubi except Herba Artemisiae Capillaris also showed the inhibition of the replication of intracellular HEV DNA in the range of $100{\sim}500\;{\mu}g/m{\ell}$. Considering the above results, it is thought that 6 plants(Herba Artemisiae Capillaris, Radix et Rhizoma Rhei, Cortex Cinnamomi, Fructus Chebulae, Fructus Rubi and Radix Rubi) possess the anti-HBV activities in the HepG2 2.2.15 cell culture system. We thus suggest that these plants possess a potential as a therapeutic agent for the chronic viral hepatitis. These results might be useful as a basic data for the development of the new preventive drugs for HBV diseases.

Construction and Characterization of an Anti-Hepatitis B Virus preS1 Humanized Antibody that Binds to the Essential Receptor Binding Site

  • Hepatitis B virus (HBV) is a major cause of liver cirrhosis and hepatocellular carcinoma. With recent identification of HBV receptor, inhibition of virus entry has become a promising concept in the development of new antiviral drugs. To date, 10 HBV genotypes (A-J) have been defined. We previously generated two murine anti-preS1 monoclonal antibodies (mAbs), KR359 and KR127, that recognize amino acids (aa) 19-26 and 37-45, respectively, in the receptor binding site (aa 13-58, genotype C). Each mAb exhibited virus neutralizing activity in vitro, and a humanized version of KR127 effectively neutralized HBV infection in chimpanzees. In the present study, we constructed a humanized version (HzKR359-1) of KR359 whose antigen binding activity is 4.4-fold higher than that of KR359, as assessed by competitive ELISA, and produced recombinant preS1 antigens (aa 1-60) of different genotypes to investigate the binding capacities of HzKR359-1 and a humanized version (HzKR127-3.2) of KR127 to the 10 HBV genotypes. The results indicate that HzKR359-1 can bind to five genotypes (A, B, C, H, and J), and HzKR127-3.2 can also bind to five genotypes (A, C, D, G, and I). The combination of these two antibodies can bind to eight genotypes (A-D, G-J), and to genotype C additively. Considering that genotypes A-D are common, whereas genotypes E and F are occasionally represented in small patient population, the combination of these two antibodies might block the entry of most virus genotypes and thus broadly neutralize HBV infection.

ALT (Alanine Aminotransferase) 검사성적과 E형 간염항체 양성률간의 관련성 (The Correlation between ALT (Alanine Aminotransferase) Value and Prevalence of Anti-HEV)

  • In order to elucidate the prevalence of anti-HEV, anti-HCV and HBsAg in ALT (Alanine Aminotransferase)-elevated patient group and healthy person control group, we examined anti-HEV, anti-HCV and HBsAg in 92 persons, respectively. The prevalence of anti-HEV in ALT-elevated patient group (4.3%) was not significantly higher than that (2.2%) of healthy person control group(P>0.05). While in healthy person control group the prevalence of anti-HEV in female (2.4%) was higher than that (2.0%) in male, and in ALT-elevated group male (4.8%) was higher than female (3.3%), there were no significances. The positive rate of anti-HEV was not significantly increased with age between healthy person control and ALT-elevated patient group. The odds ratio's of HEV, HCV and HBV with ALT-elevated patient group were 2.05 (95% CI: 0.31-16.54), 8.67 (95% CI: 1.06-188.63) and 7.23 (95% CI: 2.47-22.71), respectively, but had no significance. It was turned out that HCV and HBV were significantly correlated with ALT-elevated patient group, but HEV had no significance with it in this paper.
