대구.경북지역 20, 30대 남성 근로자의 연령 및 BMI에 따른 건강상태 (The Health Status according to the Age and BMI of Male Workers in Daegu.Gyeongbuk Region)

  • 본 연구에서는 대구 경북지역 20, 30대 남성근로자 224명을 대상으로 BMI에 따른 신체적 특성, 건강상태를 조사하였다. BMI는 20대 $23.67\;kg/m^{2}$, 30대 $23.95\;kg/m^{2}$로 조사대상자 모두 대한비만학회에서 제시한 정상 범위인 $18.5{\sim}23\;kg/m^{2}$에서 벗어난 과체중으로 조사되었다. BMI에 따른 신체적 특성은 체중, 이상체중비율, BMI 모두에서 유의적인 차이(p<0.001)를 나타냈다. 그리고 신체둘레 측정 결과 허리 둘레, 엉덩이 둘레, 허리/엉덩이 둘레비는 비만군, 과체중군, 정상군 순으로 유의적인 차이(p<0.001)를, 허리/엉덩이 둘레는 30대가 20대보다 크게(p<0.01)나타났다. 체구성성분 측정 결과 체지방률{body fat(%)}과 체지방량{body fat(kg)}은 비만군, 과체중군, 정상군 순으로 유의적인 차이(p<0.001)를 보였다. BMI가 증가함에 따라 관리요망군과 질환의심군의 비율이 유의적으로(p<0.001) 증가하였고, 평균 혈압은 비만군이 정상군 및 과체중군보다 높은 유의적인 차이(p<0.001)가 있었으며, 공복시 혈당은 30대가 20대보다 높은 유의적인 차이(p<0.05)를 보였다. 간질환의 유무를 알 수 있는 지표인 SGOT, SGPT 및 $\gamma-GTP$는 BMI별로 보면 비만군은 정상군 및 과체중군보다 높아 유의적인 차이(SGOT, SGPT; p<0.001, $\gamma-GTP$; p<0.01)를 보였으며 비만군의 SGOT는 43.58 U/L로 정상범위에서 벗어났다. $\gamma-GTP$를 연령별로 보면 30대가 34.74 U/L로 정상군에는 속하지만 20대의 28.06 U/L보다 높은 유의적인 차이(p<0.05)를 보였다. 건강상태와 음주 및 흡연 유무에 따른 생화학적 상태를 살펴보면, 건강상태에 따른 수축기혈압은 관리요망군, 질환의심군, 정상군 순으로 유의적인 차이(p<0.01)를 보였으며, 확장기혈압, 총콜레스테롤, 공복 시 혈당, SGOT, SGPT 및 ${\gamma}-GTP$는 정상군 및 관리요망군과 질환의심군 간에 유의적인 차이(p<0.001)를 보였다. $\gamma-GTP$는 음주 근로자가 33.79 U/L, 비음주 근로자가 26.64 U/L로 집단 간의 유의적인 차이(p<0.05)를 보였고, 수축기 혈압과 헤모글로빈은 각각 흡연 근로자가 124.65 mmHg, 15.82 g/dL이고, 비흡연근로자는 120.43 mmHg, 15.38 g/dL로 나타나 집단 간의 유의적인 차이(p<0.01)를 보였다. 체중이 많이 나갈수록 BMI, 이상체중비율(%), 허리둘레 및 엉덩이둘레, 허리/엉덩이 둘레비, 체지방량, 체지방률(%)이 높으며, 이에 따라 수축기혈압과 확장기혈압, 총콜레스테롤, 공복 시 혈당, SGOT, SGPT, $\gamma-GTP$ 모두 높아진다(p<0.001, p<0.01, p<0.05). 이상의 결과를 종합해보면, 조사대상자인 20, 30대 남성 근로자 대부분이 과체중에 속하며, 체중이 증가할수록 BMI, 허리둘레, 엉덩이둘레, 체지방률(%) 등이 증가하고 BMI가 높아질수록 수축기혈압과 이완기혈압, SGOT, SGPT, $\gamma-GTP$ 등의 생화학적 수치들이 높아져 건강상태가 좋지 않게 된다. 음주와 흡연 역시 혈압, $\gamma-GTP$ 등을 높여 건강상태 악화에 한 몫을 하는 것으로 나타났다. 따라서 20, 30대 남성근로자들의 연령 및 비만도의 증가에 따라 건강상태가 나빠지며 성인병 유병률의 가능성이 높아짐을 인식하고, 이러한 건강 위험요인들을 개선할 수 있는 노력이 필요할 것이라 사료된다. 정기적인 건강검진과 더불어 규칙적인 운동과 음주, 흡연, 업무 과중, 스트레스와 같은 생활습관 및 불규칙한 식습관을 개선할 수 있는 방안의 모색이 요구된다.

입원 환아의 영양상태 평가 (Assessment of Nutritional Status in Hospitalized Pediatric Patients)

  • 목 적: 입원 환아의 영양결핍에 대한 통계가 국내에는 미비하여 영양평가의 지표를 삼기가 어렵다. 입원 환아의 영양상태를 평가하여 입원 환아 영양관리의 필요성을 강조하고 향후 영양평가의 기초자료로 삼고자 본 연구를 하였다. 대상 및 방법: 1994년 2월부터 7월까지(200명)와 1999년 2월부터 7월까지(233명) 조선대학교 부속병원 소아과에 입원한 환아를 대상으로 연령에 대한 체중비, 연령에 대한 신장비, 신장에 대한 체중비를 구하여 급성 및 만성 단백질-에너지 영양결핍의 유병률과 질환별 및 연령군에 따른 유병률의 차이 그리고 영양결핍의 질환별 분포를 비교 분석하였다. 영양상태의 평가는 Waterlow 분류기준에 의해 분석하였고, 생화학적 지표로는 혈색소, 혈청알부민, 총 임파구 수를 조사하여 영양결핍과의 관련성을 알아보았다. 결 과: 1) 급성 PEM은 신장에 대한 체중비로 산출하였는데 1994년에는 중증 0.5%, 중등도 7%, 경도 18%로서 입원 환아 200명 중 총 51명이었고, 1999년에는 중증 2.0%, 중등도 3.6%, 경도 20.0%로서 입원환아 223명 중 총 58명으로 나타나 두 연도에서 영양결핍에 대한 높은 빈도의 유병률을 보였다. 만성PEM에서도 1994년에는 중증 5%, 중등도 5.5%, 경도 25.5%로서 총 72명이었고, 1999년에는 중증2.24%, 중등도 4.04%, 경도 22.87%로서 총 62명으로 나타나 두 연도간의 유의한 차이는 없으나 각각 높은 빈도의 PEM을 보이고 있다(p=0.287). 급성과 만성 PEM 양쪽에 해당되는 환아는 1994년에 25명, 1999년에는 20명이어서 두 연도에서 PEM의 유병률은 1994년에 49.0%, 1999년에는 44.8%로 나타났다. 2) 연령군별로 영양결핍의 정도를 볼 때 급성 영양결핍에서는 경도의 PEM이 전반적으로 높게 나타났으며, 심한 만성 PEM의 유병률은 2세 이전의 영아에서 10.8%로서 높게 나타나고 있으나 이들은 통계학적으로 의미가 없었다(p=0.511). 3) 급.만성 PEM의 질환별 분류를 볼 때 1994년에는 급성 혹은 만성 PEM을 보였던 환아 98명 중 호흡기 질환이 43명으로 52%를 차지하여 가장 많았고, 소화기, 일반 내과, 신경계, 순환기 질환 순이었으며, 1999년에는 99명 중 호흡기 질환이 38명, 소화기, 일반내과, 신경계, 내분비, 순환기 질환순 이었다. 4) 각 질환에 대해 급.만성 PEM의 유병률을 볼때 1994년에는 혈액종양, 내분비, 신경계, 호흡기, 순환기, 소화기, 비뇨기계통의 질환 순으로 높았으며, 1999년에는 내분비, 순환기, 소화기, 호흡기, 혈액종양, 신경계, 비뇨기계통의 질환 순이었다. 5) 1999년에 측정한 신체계측에서 PEM을 보인 환아에서 생화학적 기준에 따라 혈청 알부민치, 혈색소 및 총 임파구 수를 분류해 볼 때 혈색소 및 혈청 알부민 수치가 총임파구의 수치보다 낮게 나타났다. 결 론: 입원 환아 단백질-에너지 영양결핍의 빈도는 1994년에 49%, 1999년에는 44.8%로 높게 나타나고 있음을 알 수 있었다. 따라서 질병의 예방과 효과적인 치료 및 환아에게 적합한 영양처방을 결정하기 위해서는 영양관리의 중요성을 인식하고 포괄적인 영양평가와 함께 올바른 식 습관 형성에 대한 지도를 비롯한 적극적인 영양관리가 이루어져야 한다.

대학생의 골초음파 상태에 영향을 미치는 영양요인 분석 (Relation Between Nutritional Factors and Bone Status by Broadband Ultrasound Attenuation among College Students)

  • 본 연구는 최대 골질량에 도달하는 20대의 대학생 1039명(남학생 532명, 여학생 507명)을 대상으로 골초음파 상태와 영양소 섭취 및 생화학적 영양 상태를 나타내는 지표들과의 관계를 분석하였다. 대상자의 골밀도 수치에 따라 t-score$\underline{\geq}$-1인 정상 골밀도군과 t-score<-1 골감소군으로 분류하여 비교한 결과 남학생의 경우 정상 골밀도군이 90.4%, 골감소군이 9.6%이며 여학생의 경우 정상 골밀도군이 94.1%, 골감소군이 5.9%의 분포를 보였다. 체적지표인 BMI를 아시아-태평양 비만기준에 의거하여 분리하였을 때는 남학생은 저체중 3.4%, 정상+과체중 70.7%, 비만 25.9%, 여학생은 저체중 16.2%, 정상+과체중 76.5%, 비만 7.3%의 분포를 보였다. 정상 골밀도군과 골감소군의 골밀도와 생화학적 영양 상태는 남학생 여학생 모두 통계적으로 유의한 차이가 없었다. 전체대상자들의 영양소 섭취상태를 살펴 본 결과, 남학생의 경우 정상 골밀도군과 골감소군 간의 유의적인 차이가 나타나지 않았으나 여학생의 경우 동물성 단백질과 동물성 철분 섭취량이 정상 골밀도군에서 골감소군보다 유의하게 높았다. 평균 에너지 섭취량은 동일한 연령대의 한국인영양섭취기준(KDRIs)과 비교하였을 때, 각 남학생 정상골밀도군 약 74%, 골감소군 67%, 여학생 정상골밀도군 약 89%, 골감소군 81%의 섭취수준을 나타내었다. 평균 단백질섭취량은 남녀 두 군 모두 100%를 상회하는 섭취 양상을 나타냈으며, 강한 뼈를 유지하거나 각종 신진대사에 필수적인 영양소이기 때문에 골격건강에 가장 큰 영향을 미치는 칼슘의 경우 남학생과 여학생 모두 KDRIs와 비교하여 권장섭취기준의 약 63%정도로 부족한 것으로 나타났다. 골 건강 상태에 영향을 미치는 영양인자의 로지스틱 회귀분석 결과 연령, 성별 및 BMI를 교란변수로 통제하였을 경우 동물성 단백질, 동물성 철분 및 아연섭취는 골감소를 예방하는 보호 작용이 있는 것으로 나타났다. 동물성 단백질과 이들 미량 영양소의 섭취정도는 높은 식사의 질을 반영하므로 질적으로 우수한 식생활이 최대 골질량에 도달하는 시기인 20대의 골격형성과 건강을 위해 필수적인 것으로 사료된다.

모유 경험이 미취학 아동의 신체발달, 손목 골밀도 및 영양섭취에 미치는 영향 (The Effect of Breast Feeding on Growth Development, Bone Mineral Density of Carpus, and Nutrient Intakes in Preschool Children)

  • The purpose of this study was to examine the long term effects of breast feeding on growth, bone development and nutrient intakes in preschool children. Subjects were 62 preschool children. Anthropometric characteristics and bone mineral density of carpus were measured using DEXA. The questionnaire was composed of health status, life style, dietary behaviors, and dietary intakes and was completed by the children's mothers. The average age of the boys was 62.4 months and that of the girls was 62.1 months. Average birth height and weight of the subjects was 50.9 cm and 3.4 kg for boys and 50.3 cm and 3.3 kg for girls, respectively. The average height, weight, $\%$ body fat, and obesity index were 111.7 cm, 19.6 kg, $15.0\%$, $-2.1\%$ in boys and 109.4 em, 18.7 kg, $17.5\%$, $0.2\%$ in girls, respectively. Forty children were fed colostrum, 21 were fed breast milk, 29 were fed formula, and 12 were fed mixed milk. There was no significant difference in growth status between children who were fed colostrum and those who were not. Children who had mixed feedings were significantly taller than those who did not (p <0.001), and children who had colostrum had significantly higher bone mineral density than those who did not ($0.25 \pm 0.04 g/ cm^2$ vs. $0.23 \pm 0.04 g/cm^2$, p < 0.05). However, the different feeding methods did not show any difference in bone mineral density. Except vitamin $B_6$, folic acid and vitamin E, vitamin, mineral and calorie intakes did not meet the Korean RDAs. Calcium intake was especially lower than recommendations by as much as $62\%$ and $70\%$ in boys and girls, respectively. There was no significant difference in nutrient intakes between children who had colostrum and those who did not. However, children who were formula consumed significantly more animal fat than those who did not (p < 0.05). Neonatal feeding and breast or formula feeding seems to associate with height, bone mineral density and animal fat intakes in preschool children, based on the results of this study. A longitudinal study is needed to clarify this relationship.

경기지역 일부 초등학생의 성별과 비만도에 따른 영양소 섭취상태와 혈청 Leptin 및 혈청 지질에 관한 연구 (A Study of Nutrient Intake, Serum Lipid and Leptin Levels of Elementary School Students with Different Obesity Index in Kyunggi Area)

  • The purpose of this study was to investigate the relationship among nutritional status, serum lipids and leptin of 134 elementary school students with different obesity index residing in Kyunggi-do, Korea. Subjects were assigned to under weight, normal weight or over weight group according to the their obesity index. Their nutritional status, serum lipid and leptin levels were evaluated based on 24-hr dietary recalls, anthropometric measurements and blood analysis. The mean weight, height and obesity index of total subjects was 150.8cm, 45.6kg and 5.4%. The mean % of body fat was higher while LBM and TBW (total body water) were lower in over weight subjects than those of other two groups. Total food, plant protein, carbohydrate, and vitamin C intakes of underweight male students were lower than those of overweight male students. The consumption of total fat was the highest in the female normal weight subjects and the lowest in female overweight subjects. Male students consumed more plant protein and vitamin C than female students. The mean daily intake of vitamin B$_2$, calcium and zinc of all subjects did not meet the Korean RDA. Also, iron intake of female students was lower than the Korean RDA. The mean level of serum leptin was the highest in male and female overweight subjects (p<0.001). The HDL-cholesterol level was the highest in the underweight group of male subjects (p<0.05) and in the normal weight group of female subjects (p<0.001) The serum level of leptin and atherogenic index (AI) of females were higher than those of males. Serum leptin of total subjects had significantly positive correlation with obesity index, weight, SBP, body fat, cholesterol intake, serum triglyceride, and AI. However, serum leptin was negatively correlated with LBM, TBW, intake of plant protein, vitamin B$_1$, vitamin C and serum HDL-cholesterol. Obesity index was also positively correlated with weight, SBP, body fat, serum triglyceride and AI. To summarize the results, the vitamin B$_2$, calcium, zinc, iron and fiber intakes should be supplied to elementary school students, especially those who were overweight. The average serum leptin level of students showed remarkably positive correlation with obesity index, body fat, serum lipids and AI. Also, it was suggested that the serum leptin might be related to the dietary factors like cholesterol, plant protein, vitamin B$_1$ and vitamin C of elementary school students.

농촌보건사업지역(農村保健事業地域)의 아동영양(兒童營養) 실태조사(實態調査) (Child Nutrition Survey in Rural Health Project Areas)

  • The main purposes of the nutrition and clinical surveys were to provide baseline information on the nutritional status of pre-school children in rural health demonstration project areas of the Korea Health Development Institute (KHDI) for nutrition guidance services for the MCH target group. The survey covered a total of 222 pre-school children and 135 mothers in Okgu Gun, Cholla Pukto Province from August 10 to August 17, 1979. The survey results are summarized as follows: 1) Family Environment Seventy percent of the households had more than three children, and the mean family sire was 6. Sixty-nine percent of the mothers and 47% of the fathers of the surveyed households were educated at or below the primary school level. The majority, 70% of the mothers, were aged between 20 years and 35 years. 2) Anthropometric Measurements and Hemoglobin Value 4.5% of the children were lower than 80% weight for age of the Korean standard, and 5.4% were lower than 85% arm circumference for age of the Jelliffe's standard resectively, and those were suffering from protein-energy malnutrition. Angular stomatitis were observed on 66.2% of the subjects. Mean hemoglobin value was 11.1g/100m1, and 44.2% of the subjects were categorized as anaemia. 3) Food and Nutrient Intake of animal foods was very low, ranging from 2.9 to 17%. Consumption of eggs was less than 2% of total food intake, and intake of legumes was also very scanty, between 0.8 to 3.7%. These data present evidence of very poor protein intake, quality as well as quantity. Energy intake of children was 60.0 to 64.4% of the recommended allowance, and mean protein intake only met 47.4% of the recommendation. Low intake of vitamins except thiamin were also found. 4) Mother's Nutrition Knowledge Eighty-five percent of the mothers were entirely ignorant regarding the 'five basic food group' which is most important fact on food and nutrition guidance. Mean knowledge score from 14 basic questions about food and nutrition was as low as 5.1. There was a significant positive correlation between mother's educational level and nutrition knowledge score. 5) Family Planning Variable There were significant correlation among maternal, family planning variables, and some of the nutritional and physical measurements. The study revealed that the mother's educational level and nutrition knowledge score are more crucial factors than the family planning variables on effecting food intakes on children. Recommendation : According to the results of the surveys, there were high incident rates of nutritional anaemia and angular stomatitis among pre-school children, and most of rural women had very limited knowledge about food and nutrition. As a main part of the health education activities, the community health workers should provide nutrition education to the village mothers to improve the nutrional status of young children in rural areas. Nutrional promotion at the primary health care level should be mainly based on appropriate nutrition education.

서울지역 거주 노인들의 만성질환에 따른 식품 및 영양소 섭취 실태 조사 (The intake of food and nutrient by the elderly with chronic disease in the Seoul area)

  • This study was carried out in order to investigate the status of food and nutrient intakes depending on the types of chronic disease of the elderly at welfare centers in Seoul. The total number of subjects was 299 elderly over the age of 60 (82 men and 217 women); they were grouped by the disease status: normal, cardiovascular disease (CVD), diabetes mellitus (DM) and arthritis patients groups. The dietary intakes were obtained by a 24-hour recall and analyzed by CAN-Pro 4.0; they were then compared with the dietary reference intakes for the Korean (KDRIs). In the results of anthropometric data according to chronic disease, the waist circumference of the normal group in males and females was lower than the other groups; in particular, the normal female group showed a lower level of waist-hip ratio, body mass index (BMI) and body fat % than those of the other groups. The arthritis group showed a higher intake of milk products than the other groups (p < 0.05); consequently, calcium intake was significantly higher than that of the other groups (p < 0.001). Vitamin A intake in the arthritis group was significantly higher than the intake in the normal and DM groups (p < 0.01). Normal and arthritis groups showed significantly higher intake of zinc and copper than the CVD group (p < 0.01). In general, about 80% of the subjects in this study revealed insufficient intakes of riboflavin, vitamin C, calcium, dietary fiber, vitamin D, biotin and potassium, compared with the estimated average requirement (EAR) or adequate intake (AI). However, sodium intake of all groups was higher than the AI of KDRIs. In conclusion, nutrient intake, according to the chronic disease, showed significant difference in some of the micronutrients, vitamin A, Ca, Zn and Cu. Therefore, we suggest that dietary guidelines, such as reducing the intake of salty foods and sweet drinks and increasing the intake of foods with sufficient calcium, vitamin D and vitamin C, are necessary to the improvement of eating habits for the elderly.

비만관련 습관 및 비만도 변화에 영향을 미치는 관련요인 분석 (The Associated Factors with Obesity-related Habits and Obesity index Change)

  • Obesity in children is a major concern of public health. This study was performed to illuminate its effect on weight control program and the associated factors of obesity-related habits and obesity index in primary school obese children. Weight control program consisted of behavioral modification, nutrition education and exercise during 17 weeks. The sample consisted of treatment group(n=42) and control group(n=41). There was no statistical difference between the two groups in obesity index, socioeconomic status and grade. To assess the effects of weight control program, the subjects were given pre-test and post-test such as the measurement of anthropometric values and self-reporting questionnaire. This result of this study were as follows; 1. After weight control program was applied, there was a significant decrease in obesity index among the treatment group. Obesity-related habits score of the treatment group increased significantly, While there was not much difference between the pre-test and the post-test among the control group. But exercise habit didn't increase significantly in the both groups. 2. Correlation between obesity-related habits and obesity index were not evident. 3. After application of weight control program, the factors associated with change of eating habit were children's past experience of weight control, motivational change toward weight control program and friends' support for treatment group. The factors associated with change of exercise habit were post-test motivation score and friends' support. Motivational change toward weight control and pre-test self-efficacy of exercise behavior were counter-related to exercise habit. For change of other obesity-related habits, initial obesity index, motivational change, post-test self-efficacy score of exercise behavior and paternal educational status were closely associated. But post-test self-efficacy score of eating behavior was unrelated. 4. Only the factor of experience of weight control was associated with change of obesity index. 5. For the both groups, the factors associated with change of eating habits were post-test self-efficacy of eating behavior and family's support. The factors associated with change of exercise behavior were self-efficacy changes of exercise behavior and friends' support. The factors associated with change of other obesity-related habits were self-efficacy change of eating behavior. Initial obesity index was associated with change of obesity index. 6. The rate of dropouts from weight control program was 28.6% (12/42) in treatment group. Initial obesity index, other obesity-related habits except eating exercise habits, friend's support were associated with dropout. In conclusion, these results indicated that weight control program in primary school settings was effective. Direct exercise regimen and practice was demanded. In addition to the program itself, much of the success is dependent on the degree of motivation of the children involved and support provided by their parents and friends. Further study need to be performed under the condition that the weight control progrom is applied for a longer period.

미숙아의 초기 영양섭취 및 성장상태에 관한 연구 (Nutritional and Growth Status of Premature Infants During Neonatal Period)

  • 본 연구는 미숙아들의 영양공급 및 성장상태를 조사하기 위하여 출생시 체중이 2500g이하이며 在胎期間이 37주 미만인 신생아중 20일이상 입원한 미숙아를 대상으로 그들의 역학적 조사, 영양 섭취량과 공급 방식, 체위의 변화, 혈액학적 검사등을 비교 분석한 그 결과는 다음과 같다. 1) 출생체중별 在胎期間과 apgar score를 보면 1499g이하 미숙아군은 31주, 4/7(1min/5min)이며 1500~1999g, 2000~2500g 미숙아군은 각각 33주, 8/9(1min/5min)이었다. 2) 미숙아들의 체중별 1일 kg당 열량과 수분 섭취량을 평균적으로 보면 1499g 이하 미숙아군은 102.0Kcal, 150.2ml이며 1500~1999g 미숙아군은 119.3Kcal, 136.4ml이며 2000~2500g 미숙아군은 101.7Kcal, 141.5ml이었다. 3) 1999g이하 미숙아군의 영양 공급 빈도 중 dextrose와 dextrose/hyperal 공급시일 즉 관주입법으로 공급한 시일을 평균적으로 보면 1499g이하 미숙아군은 7.7일이여 1500~1999g 미숙아군은 3.8일이었다. dextrose/formula와 dextrose/hyperal/formula공급시일은 1499g이하 미숙아군은 평균 14.2일, 1500~1999g 미숙아군은 10.9일이었다. 4) 미숙아들의 초기 체중 감소율을 보면 1499g이하 미숙아군은 5.4%, 1500~1999g 미숙아군은 6.4%, 2000~2500g 미숙아군은 11.4%로 출생체중이 높은 미숙아군의 감소율이 높게 나타났다. 5) 혈액학적 검사시 1499g 이하 미숙아군의 10일째 Hb은 12.1g/dl, Hct은 36.7%, MCHC는 33.2%이며 1500~1999g 미숙아군의 수치는 15.4g/dl, 46.7%, 33.8%이며 2000~2500g 미숙아군은 14.3g/dl, 42.5%, 351.1%로 조사기간중 20일째까지 Hb과 Hct수치는 점차 감소하였다. 6) Amino acid 투여의 영향을 보면 amino acid투여군의 초기 체중 감소율이 9.0%로 amino acid투여 안한군(9.4%)보다 단백질과 열량을 적게 섭취하였음에도 초기 체중 감소율의 유의적 차이는 없었다.

만성 질환 노인에서의 면역 성분 양상과 식이예방인자 (Inflammatory Cytokines and Dietary Factors in Korean Elderly with Chronic Disease)

  • The purpose of this study was to investigate the association between inflammatory cytokines and chronic disease status in Korean elderly. The subjects were 248 elderly people aged over 65 years recruited from Health Center in Seoul. The subjects were classified into 3 groups based on their disease (diabetes, hypertension, and hyperlipidemia) status: subjects with one diagnosed disease of diabetes, hypertension, and hyperlipidemia fall into singular group (n=89), subjects with more than 2 disease into multiple group (n=39), and those with free of the diseases into normal (n=122). Anthropometric and biochemical characteristics, and dietary intakes were assessed. Dietary intakes were surveyed by 24-recall method. The means of IL-2, IL-6, MCP-1 and C3 were not differ among 3 groups. However, when subjects classified into tertiles of IL-6, MCP-1, TNF-$\alpha$ and C3 and frequencies of each fertile were compared, the multiple group showed significantly lower frequencies in lowest fertile than normal group (p<0.05), suggesting higher tendency of inflammatory responses. For hematological values, blood pressure, triglycerides, fasting blood glucose levels were highest in multiple group (p<0.05) compared to other 2 groups. BMI, body fat(kg), and triceps skinfold thickness were also significantly higher in multiple group than in 2 other groups(p<0.05). Moreover, the concentrations of IL-2, IL-6 and C3 were significantly correlated with hematologic values of fasting blood glucose, total cholesterol, LDL-cholesterol, and triglycerides or obesity factors such as triceps skinfold thickness, BMI, and body fat(%). Among singular and multiple group, the subjects with higher intakes for vitamins A, C, and E showed the higher level of IL-2 and the lower level of MCP-1, and C3. In conclusion, blood concentrations of triglycerides and proinflammatory cytokines, blood pressure, obesity parameters (BMI, body fat, triceps skinfold thickness) were higher in multiple group than in normal, but this result strongly suggest that the increasement of the vitamin A, C, and E intakes would modify the cytokine levels to reduce the inflammatory response in the elderly people with chronic diseases.
