It is very important for the ideal restorations of anterior openbite patients to record the mandibular movement and to harmonize mandibular movement with other organs in stomatognathic systems. This study was designed to compare the mandibular movement of anterior openbite patients with that of normal bite(Angle Class I) patients, to ascertain which components of mandibular movement have differences between two groups, and to use for occlusal treatment of mandibular movement. Saphon Visi-trainer Model 3(Tokyo Shizaisha Co. Japan) and Denar Pantronic(Denar Corp.,U.S.A.) were used to record mandibular movement. Pantronic survey was peformed by using an arbitrary hinge axis according to manufacturer's direction. Twenty-eight adult who have physiologically normal occlusion(Angle Class I) and are free of TM dysfunction were selected as a control group(Group 1). Fifteen adult who are anterior openbite patient and have not anterior guidance function and have posterior interference at protrusion were selected as a experimental group(Group 2). The results are as follows : 1. There was no statistically significant difference between the average immediate and progressive side shift of anterior openbite patients(0.54mm, $7.57^{\circ}$) and those of normal group(0.49mm, $5.96^{\circ}$). 2. The average protrusive and orbiting condylar inclination of anterior openbite patient$(30.87^{\circ},\;32.27^{\circ})$ were significantly lower than those of normal group$(36.11^{\circ},\;39.04^{\circ})$ (P<0.05). 3. In the results of Visi-trainer recordings, the mean for the maximum protrusion, the maximum laterotrusion, the angle of laterotrusion and the angle of protrusion in the horizontal trajectory between group 1 and 2 did not differ significantly. 4. The mean for the angle of protrusion, the maximum opening in the frontal trajectory, the ICP-RCP(A-P) distance and the angle of protrusion in the sagittal trajectory differ significantly(P<0.05). 5. The significant correlation was found between orbiting condylar inclination and protrusive condylar inclination.

Objective: The aim of this study was to investigate the three-dimensional (3D) position of the center of resistance of 4 mandibular anterior teeth, 6 mandibular anterior teeth, and the complete mandibular dentition by using 3D finite-element analysis. Methods: Finite-element models included the complete mandibular dentition, periodontal ligament, and alveolar bone. The crowns of teeth in each group were fixed with buccal and lingual arch wires and lingual splint wires to minimize individual tooth movement and to evenly disperse the forces onto the teeth. Each group of teeth was subdivided into 0.5-mm intervals horizontally and vertically, and a force of 200 g was applied on each group. The center of resistance was defined as the point where the applied force induced parallel movement. Results: The center of resistance of the 4 mandibular anterior teeth group was 13.0 mm apical and 6.0 mm posterior, that of the 6 mandibular anterior teeth group was 13.5 mm apical and 8.5 mm posterior, and that of the complete mandibular dentition group was 13.5 mm apical and 25.0 mm posterior to the incisal edge of the mandibular central incisors. Conclusions: Finite-element analysis was useful in determining the 3D position of the center of resistance of the 4 mandibular anterior teeth group, 6 mandibular anterior teeth group, and complete mandibular dentition group.
The aims of occlusal adjustment are as follows: to eliminate occlusal interference, to redirect force generated during function to which is favorable for teeth, to improve mastication efficiency and simultaneously establish stable maximal intercuspation or centric occlusion. Also, it should permit mandible to move freely from all positions. The sequence of occlusal adjustment in natural teeth and fixed prosthesis shall be as follows: 1) Eliminate interference that prevent optimal intercuspation and recontouring adjustment 2) Establish maximal intercuspation 3) Eliminate interference in lateral mandibular movement 4) Eliminate interference in anterior mandibular movement 5) Refine occlusal relationships.
Prediction of the soft tissue changes following hard tissue movement is very important from the esthetic view point for patients who have orthognathic surgery. There are many cephalometric analysis of facial bone and soft tissue on the lower lip and chin region but few soft tissue analysis on the midface after mandibular setback surgery. This study was performed to obtain whether the mandibular posterior movement has influence on the midface and the predictable ratio of post-operative measurement values of the soft tissue changes following mandibular setback surgery. Fifteen patients (8 males and 7 females) who had undergone mandibular setback surgery were selected and analyzed the soft tissue movement on the upper lip and the cheek region. Post-operative changes of the soft tissue measurements after mandibular surgery were examined on pre- and post-operative cephalometrics and the ratio of changes were analyzed after drawing the reference line on the face with the barium sulfate solution. The reference lines were perpendicular to the intercanthal line from infraorbital foramen and lateral canthus. The results obtained were as follows : 1. There were tendancy of anterior movement of soft tissue adjacent the nose after mandibular surgery 2. There were incerased tendancy of the amount of anterior movement from the nasal crease to the cheek region. 3. The amount of anterior movement of the soft tissue was larger below the palatal plane compared with above the palatal plane in the cheek region. 4. The upper lip length was increased and moved posterior direction after mandibular setback surgery 5. The lower lip was moved posterior direction by posterior movement of the mandibular structure 6. Soft tissue of the midface around the nose moved anterior direction after mandibular setback surgery but there was no correlation between the amount of mandibular setback and the amount of the soft tissue changes
The author observed the eruption pattern of mandibular anterior teeth by orthopantonography taken by children who came to pedodontic department, school of dentistry, S.N.U. and then got the following results.
1) In the degree of eruption pattern of anterior teeth, female is slightly faster than male. 2) Eruption pattern of central incisors was similar to lateral incisor, but the degree of lateral incisors movement was more severe. 3) Eruption pattern of canine was out of normal process when the adjacent teeth were abscence and there was not distal movement such as in maxillary canine.
The purpose of this study was to improve limitations and disadvantages of the mechanical pantograph and the Visi-Trainer, and to design the reliable and reproducible device mandibular movement tracking device (MMTD) that is more simple, convenient and save the chair time than the mechanical pantograph and Visi-Trainer. MMTD was consist of head frame, horizontal bar, condylar path tracking stylus holders, anterior path tracking stylus holder, two condylar path plastic recording plates, one anterior path plastic recording plate, toggles and open occlusal clutch. To prove the reliability and reproducibility of MMTD, a five adults were selected and mandibular condylar movement was recorded one time by the mechanical pantographic tracing and MMTD. The border movement recording of the mandibular incisor (frontal, sagittal and horizontal) was also recorded by Visi-Trainer and the MMTD. The obtained results were as follows; 1. The condylar movement path (sagittal, horizontal) of the MMTD was not coincidence with that of mechanical pantograph. 2. Measurements of the angulation which established between working and balancing path records by use of the mechanical pantograph and MMTD, there was no significance. 3. In a view of MMTD's reproducibility, there was revealed almost similar recording pattern. 4. The border movement recording of the mandibular incisor by the MMTD and the Visi-Trainer showed almost same reliability and reproducibility. 5. The subjects were able to his original mandibular movements by use of open occlusal clutch in the MMTD.
It is important to harmonize the occlusion with TMJ and neuromuscular system. However, clinically, how to harmonize is very difficult. The mandibular movement is the motion in which all component part of stomatognathic system participate. This study was performed to compare mandibular movement of mandibular prognathic patients group with that of normal group, to ascertain which components of mandibular movement have differences between two groups, and to use for occlusal treatment of mandibular movement. Thirteen adult who have physiologically normal occlusion and are free of TMJ dysfunction were selected as a control group(Group 1). Eight adult who are mandibular prognathic patient and have more than four anterior teeth crossbite, therefore have not anterior guidance function and have posterior interference at protrusion were selected as a experimental group(Group 2). Electronic pantograph, Denar Pantronic (Denar Corp., U.SA.), was used to record mandibular movement. Pantronic survey was performed by using an arbitrary hinge axis according to manufacturer's direction. Of the Pantronic recordings, immediate side shift (ISS), progressive side shift (PSS), orbiting condylar path (ORB), protrusive condylar path (PRO) between two groups were compared and analysed. The results were as follows: 1. The average protrusive and orbiting condylar inclination of mandibular prognathic patient$(28.44^{\circ},\;36.94^{\circ})$ was significantly lower than those of normal group$(40.15^{\circ},\;48.00^{\circ})$ (P<0.01). 2. There was no statistically significant difference between .the average immediate and progressive side shift of mandibular prognathic patient $(0.37mm,\;6.19^{\circ})$ and those of normal group$(0.52mm.\;5.96^{\circ})$ (P>0.01). 3. The significant correlation was found between orbiting condylar inclination and protrusive condylar inclination.
The purpose of this study was analyse the mandibular movements in patients with internal derangement of the temporomandibular joint according to diagnostic subgroups. The author classified patients with internal derangement of the temporomandibular joint into 4 diagnostic subgroups by means of the magnet resonance imagings, and evaluated the clinical signs and the mandibular movements with Mandibular Kinesiograph(MKG) in each subgroups. The mandibular movements, measured in this study, were the types of movement in frontal and sagittal plane, velocities in opening and closing movement, and the opening and closing movement, and the opening and closing velocity pattern. The data were compared between the 5 groups including the normal group. The results were as follows : 1. Pain was more frequently observed in the anterior disc displacement without reduction group than in the anterior disc displacement with reduction group. Sound of joint was more frequently observed in the anterior disc displacement with reduction group, and limitation of mandibular opening movement was more frequently observed in the anterior disc displacement without reduction group. Duration of the anterior disc displacement without reduction group was significantly short compared to that of the anterior disc displacement with reduction group, and duration of the unilateral anterior disc displacement without reduction group was shortest in the experimental group. The frequency of Angle's classifications had not significant correlations between the experimental groups. 2. Active and passive range of the opening movement, maximum protrusive movement, maximum lateral movement toward left side were significantly decreased in the experimental groups compared to the control group, but there was no significant difference in the range of the maximum lateral movement toward right side between the control and experiment groups. In unilateral anterior disc displacement without reduction group, the range of maximum lateral movement toward unaffected side was no significant difference in the range of the maximum lateral movement between toward affected side and toward unaffected side. 3. Maximum opening velocity, maximum closing velocity, average opening velocity, average closing velocity and maximum velocity of terminal tooth contact were significantly decreased in the experimental groups compared to control group. There was no significant difference in maximum opening velocity and maximum velocity of Terminal tooth contact between the subgroups of the experimental group each other, but there was significant difference in maximum closing velocity, average opening velocity and average closing velocity between the subgroups each other. 4. In the frontal plane of the MKG, the frequency of complex deviation type(F-2)pattern was significantly increased in the anterior disc displacement with out reduction group compared to the anterior disc displacement with reduction group and the control group. In the sagittal plane, the frequency of coincident type(S-1)was decreased in the same group. 5. In the maximum opening velocity pattern, the frequency of no-peak type (OV-3)in the unilateral anterior disc displacement with reduction group was significantly increased compared to the control group. The frequency of 1-peak type (OV-1) and 2-peak type (OV-2) was decreased in the anterior disc displacement with out reduction group, but the frequency of no-peak type (OV-3)was increased in the same group. In the maximum closing velocity pattern, the frequency of no-peak type (CV-3) was significantly increased in the anterior disc displacement without reduction group. Compared to the anterior disc displacement with reduction group and the control group. The frequency of 1-peak type (CV-1) and 2-peak type (CV-2) in the anterior disc displacement with reduction group was decreased than that in the control group.
This study was performed to measure the mandibular movement and the changes of masicatory and sternocleidomastoid muscle activity reflected by occlusal disturbance during habitual chewing. For this study, 18 subjects(14 males and 4 females with an average age of 24.0) were selected. The impression of each subject were taken for measuring intermolar distance on lower dentition. The activities of masticatory and sternocleidomastoid muscle and the mandibular movement were recorded and analyzed during habitual chewing by means of E.M.G.(electromyograph), E.G.N.(electrognathograph), rotate program in BioPak analyzing system(BioResearch Inc.). This study was performed to measure the changes of the mandibular movement and the masticatory muscular activities - anterior temporal and masseter muscle of both side - reflected by intentional increase of anterior guidance angie. For this study, 5 volunteers (3 males and 2 females with average age of 24.0) were selected. Each volunteer had Angle's classification I and did not have any missing tooth except third molar and any extensive restorations. Metallic guide plate was made at volunteer's working model fabricated by improved dental stone and cemented to the palatal surface of maxillary central incisor using resin cement(Panavia $21^{(R)}$) and then adjusted not to give any occlusal interferences at intercuspal position. The activity of masticatory muscles and the changes of mandibular movement were recorded by EMG and Sirognathograph in Biopak analysing system(Bioresearch Inc., Milwaukee, Wisconsin, USA). Measurement was done at before experiment, immediatley after placement, 1 week after placement, immediately after removal, and 1 week after removal. The results were as follows: 1. Moderate phonetic disturbance and mild headache were occured to 3 volunteers for 2 days after setting and 1 volunteer had positive reaction to percussion and slight midline diastema. But all of these clinical signs were diappeared 1 week after removal and the other volunteer did not have any special clinical sign. 2. In the EMG of the mandibular rest position, the mean value of anterior tempotal muscle was increased immediately after placement(p<0.01) and then decreased 1 week after placement(p<0.05) and increased 1 week after removal(p<0.05) but not recovered as before experiment. The mean value of masseter muscle was decreased during the experiment period. 3. In the EMG during mandibular protrusive movement, all muscular activity was decreased during the experiment period. Reduced activity was not recovered 1 week after removal(p<0.03). 4. During the habitual opening, anteroposterior movement of mandible was decreased immediately after placement(p<0.05) and then increased 1 week after placement but not statistically significant(p>0.1). Vertical movement was not shown significant difference during the experiment period(p>0.1). Lateral movement was decreased immediately after placement(p<0.05) and then increased 1 week after placement but not recovered as before experiment. The opening and closing velocity of mandible was shown minor changes but not statistically significant. 5. During the habitual opening, anteroposterior movement of mandible was decreased 1 week after placement(p<0.05) and then increased immediately after removal and recovered 1 week after removal as before experiment. Vertical movement was not shown significant changes. Lateral displacement of mandible was increased continuously and recovered 1 week after removal. Opening velocity was temporarily increased immediately after removal but recovered and closing velocity was not shown significant changes. 6. During the right side chewing, anteroposterior movement of mandible was increased immediately after removal but recovered and vertical movement was not shown statistically significant results. Lateral displacement and velocity of mandible were not shown significant results. 7. During the left side chewing, the changes of mandibular movement pattern were not shown statistically significant results.
