Background: Older adults use different ankle muscle activation patterns during difficult static balance conditions. It has been suggested that this is related to a decline in proprioception with age, resulting in reduced postural balance. However, the association between proprioception and ankle muscle activity during quiet standing has not been directly assessed. Objects: This study aimed to investigate the effects of age and sensory condition on ankle muscle activity and the association between ankle proprioception and ankle muscle activity. Methods: We recruited 10 young women and 9 older women. Ankle proprioception was evaluated using joint position sense (JPS) and force sense (FS) divided by dorsiflexion and plantarflexion. The electromyographic activity of the tibialis anterior (TA) and gastrocnemius (GCM) muscles was collected during quiet standing. Results: Older women activated GCM muscle more than young during quiet standing and when performing difficult tasks. Older women had more errors in JPS dorsiflexion and FS plantarflexion than did young. The GCM muscle activity is related to JPS dorsiflexion and FS plantarflexion. Conclusion: Lower proprioception of the GCM with age leads to increased muscle activity, resulting in reduced postural balance. There was no difference in TA proprioception or muscle activity among older women with frequent physical activity.
Background: Repetitive damage to the ankle joint causes chronic ankle instability, and studies comparing the effects of exercise in open and closed chains as a treatment method are very rare. Objectives: To investigate the effects of open and closed kinetic exercises on muscle activity and dynamic balance of ankle joint in adults with chronic ankle instability. Design: Single-blind randomized controlled trial. Methods: The selected 30 subjects are randomly divided into open kinetic chain exercise experimental group (EGI, n=10), closed kinetic chain exercise experimental group (EGII, n=10), and stretching control group (CG, n=10). Open and closed kinetic exercises lasted 30 minutes three times a week for six weeks and stretching exercises performed four actions for 20 seconds and five sets. The measurement tools using surface electromyography to measure muscle activity in the ankle joint. The dynamic balance of the ankle was evaluated using the Y-Balance test. Results: Following the intervention, closed and open kinetic chain exercise group showed significant difference in tibialis anterior and gastrocnemius muscle activity and dynamic balance (P<.05). However, no significant difference in tibialis anterior and gastrocnemius muscle activity and dynamic balance between closed and open kinetic chain exercise group (P<.05). Conclusion: This study provides evidence that closed and open kinetic chain exercise can be presented as an effective exercise for the muscle activity of ankle muscle and dynamic balance of the subject with chronic ankle instability.
Objective: This study is designed to investigate the effect of ankle joint position on hip extensor muscle activity when bridging exercise in sagittal plane. Design: Cross-sectional study Methods: The subjects were recruited from 20 healthy adult men. The subjects performed three types of bridging exercises (normal bridging, ankle dorsiflexion bridging, ankle plantar flexion bridging) three times for five seconds with a rest of 15 seconds between measurements and two minutes of rest between each motion. The target muscles were the gluteus maximus, biceps femoris, soleus, and tibialis anterior. A surface electromyography was used to measure the muscle activity of these muscles. Results: The results show there was no statistically significant difference between the three types of exercise in the gluteus maximus muscle activity. However, the biceps femoris showed a significant difference between the three types of exercises (p<0.05). Conclusions: In conclusion, when the three different bridging exercises were performed by adding ankle motion to normal bridging exercise, there was a significant difference in the muscle activity of the gluteus maximus relative to the biceps femoris muscle activity in the order of the ankle dorsiflexion bridging, normal bridging, and ankle plantar flexion bridging exercise. Therefore, this could be an effective option for a bridging exercise if applied to patients with a weak gluteal muscle and shortening of the hamstring muscle in further studies.
Purpose: This study compared the muscle activity of the lower limb according to the three types of fixed angles of the ankle joint during a lunge exercise. Methods: Twenty healthy subjects performed the lunge motion in a trial including the three types of fixed angle. The lunge motion with a neutral, 20° dorsiflexion, and 20° plantarflexion of the ankle joint were randomized and measured repeatedly. The muscle activity of the rectus femoris (RF), vastus medialis (VM), vastus lateralis (VL), biceps femoris (BF), and semitendinosus (ST) was measured by surface electromyography. Results: In the change in ankle joint angle, the RF, VL, BF, and ST muscle activity showed significant differences (p<0.05). In the 20° dorsiflexion position, the muscle activity of VL, BF, and ST showed a significant decrease compared to that in the neutral position (p<0.017). The muscle activity of RF and VL in the neutral position was greater than that in the 20° plantarflexion position (p<0.017). Only the muscle activity of the BF in the 20° plantarflexion position was significantly greater than the 20° dorsiflexion position (p<0.017). Conclusion: These results revealed a difference in the muscle activity of lower extremities in the proximal region according to the angle of the ankle joint during the lunge.
Purpose : Using a smartphone while walking districts attention and increases the risk of losing balance or falling. Ankle instability is caused by decreased muscle strength and decreased neuromuscular ability leading to postural control problems. Dual tasks increases the risk of falls by reducing postural control in adults with ankle instability. This study aimed to investigate the effect of performing a dual task on balance and muscle activity in adults with ankle instability using a smartphone. Methods : Forty-nine individuals with ankle instability participated in this study. A game of finding the wrong picture was performed using a smartphone in the dual task, and only looking at the blank screen of a smartphone was evaluated in the single task. The participants randomly performed single and dual task to evalutate balance and muscle atcitivy. Balance was evaluated using the Biodex balance system (BBS), and muscle activity was evaluated using surface EMG. Muscle activity of the gastrocnemius and tibialis anterior was measured at the same time as balance. Results : The results of this study showed that overall, anteior/posterior, and medial/lateral balance indices all showed significant differences when performing the dual task compared with those during the single task (p<.05). The muscle activity results showed a significant difference compared with that of the gastronemius muscle on the nondominant side during the dual task (p>.05). Conclusion : The results of this study showed that maintaining balance is more difficult when performing the dual task than during the single task, and only the muscle activity of the nondominant gastrocnemius muscle decreased. The dual task causes a decrease in concentration for postural control, which negatively affects postural stability. Individuals with ankle ankle instability should refrain from performing dual tasks, such as using smartphones, to prevent ankle damage.
The purpose of this study was to analyze the kinematic variables of ankle joints and EMG signal of the lower limbs muscle activity for the different walking speed. The subjects were 6 males of twenties. It was classified into three different walking speed-0.75m/s, 1.25m/s, 1.75m/s. The walking performances were filmed by high speed video camera and EMG signal was gained by ME3000P8 Measurement Unit. Tibialis anterior(TA), Gastrocnemius medial head(GM), Gastrocnemius lateral head(GL), Ssoleus(SO) were selected for the dorsiflexion and plantarflexion of the ankle joint. The result of this study were as follows: 1. In the gait cycle, The time parameters for the phases were showed significant difference without the terminal stance phase and terminal swing phase for the different walking speed. 2. The angle of ankle joint was no significant difference for each time point and MDF, MPF but increasing walking speed the angle had the increasing pattern slightly. 3. The angular velocity of ankle joint was showed the significant difference for LHC, RTO, RKC, LHU, MPF and MDF point along the walking speed. 4. TA was showed about 2-3 times muscle activity at the 1.75m/s than 1.25m/s in some phases. And it was showed the similar muscle activity between the 0.75m/s and 1.25m/s but, showed a little much muscle activity in the 0.75m/s. GM was showed about 2-3 times muscle activity in the 1.75m/s than 1.25m/s, and even much muscle activity at the 0.75m/s than 1.25m/s in some phases. GL was showed increasing pattern of muscle activity specially in the initial swing phase as the walking speed increased. SO was showed about 3 times muscle activity in the 1.75m/s than 1.25m/s during the plantarflexion of ankle joint. It was showed the similar muscle activity between the 0.75m/s and 1.25m/s but, showed a little much muscle activity in the 1.25m/s.
Objective: This study attempted to compare the effects of bridge exercise using a sling according to the angle of the ankle to confirm the effective lower extremity muscle activation posture of patients with patellofemoral pain syndrome(PFPS). Design: Cross-sectional study Methods: Seventeen patients with PFPS were recruited and the muscle activities of the vastus medialis, vastus lateralis, rectus femoris, and gluteus maximus were measured according to the ankle position (dorsiflexion, neutral, plantar flexion). After measuring the maximum number of isometric contractions of vastus medialis, vastus lateralis, rectus femoris, and gluteus maximus, bridging exercise using a sling according to each ankle posture was applied to measure lower extremity muscle activity. The evaluation was performed 3 times for 10 seconds. The three ankle postures were randomly performed and the average values were compared. Results: As a result of this study, the vastus medialis muscle showed high muscle activity in the order of dorsiflexion, plantar flexion, and neutral position bridge exercise (p<0.05). And the vastus lateralis showed high muscle activity in the order of dorsiflexion, neutral, and plantar flexion (p<0.05). However, rectus femoris and gluteus maximus did not show significant muscle activity according to the ankle posture, but muscle activity was highest in the dorsiflexion posture. Conclusions: As a result of this study, muscle activity was high in the order of vastus medialis and vastus lateralis during ankle dorsiflexion. This is thought to be a major factor that can be applied in various ways in clinical practice according to the ankle angle when treating PFPS patients.
Objective: The purpose of this study was to investigate the effect of thoracic spine and ankle joint alignment on trunk and upper limb muscle activity during trunk forward lean exercise using a sling. Methods: 25 subjects participated in this study. All subjects performed trunk forward lean exercise using a sling under four conditions according to the alignment of the thoracic spine and ankle joints. Trials were performed 3 times in each condition. Muscle activity of the trunk and upper extremity was measured using electromyography. Results: In the dorsiflexion, the thoracic kyphosis condition showed significantly higher muscle activity in the pectoralis major, rectus abdominis, latissimus dorsi, transverse abdominis than dorsiflexion(p<0.05). In the plantar flexion, thoracic kyphosis condition showed significantly higher muscle activity in pectoralis major, transverse abdominis, latissimus dorsi muscle activity than dorsiflexion(p<0.05). Conclusions: Regardless of ankle alignment, thoracic kyphosis condition increased the activity of the pectoralis major, transverse abdominis, latissimus dorsi. Therefore, regardless of the alignment of the ankle, it is recommended to perform the trunk forward lean exercise using a sling in thoracic kyphosis.
Background: Task-oriented training on an unstable support surface is an effective intervention for improving the ankle joint stability and muscle strength in patients with ankle instability. This study examined the effects of balance training on an unstable support surface in patients with ankle sprains with ankle instability. Methods: Forty-four patients with ankle sprains participated in this study. Screening tests were performed and assigned to an experimental group, who performed task-oriented training on an unstable support surface (n=22), and a control group, who performed task-oriented training on a support surface (n=22) using a randomization program. All interventions were applied 3 times per week for 4 weeks. The numeric rating scale (NRS), cumberland ankle instability tool (CAIT), balance ability, muscle activity, and muscle thickness were compared to evaluate the effects of the intervention. Results: Both groups showed significant differences in the NRS, CAIT, balance ability, and muscle activity between before and after the intervention (p<.05). In addition, there were significant differences in balance ability, muscle activity, and muscle thickness between the experimental and control groups (p<.05). Conclusion: Task-oriented training on an unstable support surface is an effective intervention for improving the balance ability, muscle activity, and muscle thickness during contraction.
Objective: The purpose of this study was to investigate the effects of combining Mulligan taping and flossing bands on lower limb muscle activity and static and dynamic balance. Design: A randomized controlled trial. Methods: Sixty-eight patients with chronic ankle instability were randomized into three groups that were treated with Mulligan taping (MT, n=22), flossing band (FB, n=23) and Mulligan taping combined with flossing band (MT+FB, n=23), and various parameters were compared before and after the intervention. The muscle activity of the lower extremities, including the tibialis anterior, peroneus longus and medial of gastrocnemius muscles was measured using BTS FREE EMG 1000, while the static and dynamic balance were measured using the Biorscuue balance measuring equipment. Results: There was a significant difference in muscle activity of the tibialis anterior muscle, before and after the intervention, in the MT group (p<0.01), FB (p<0.001) and MT+FB (p<0.001). There was also a significant difference in the muscle activity of the tibialis anterior muscle in the MT+FB group when compared with that in MT and FB groups (p<0.05). We also observed a significant difference in the dynamic balance all the groups (p<0.001). Conclusions: Therefore, combining Mulligan taping and flossing bands for patients with chronic ankle instability may improve dynamic balance and tibialis anterior muscle activity.
