The Effects of Tongue Positions on the Cervical Range of Motion (혀의 자세가 경추의 관절가동범위에 미치는 영향)

  • Purpose : This study was to investigate that tongue positions have effect on the cervical range of motion (ROM). Methods : 18 subjects, 20 to 25 years of age, were participated in this study. The tongue positions were neutral position, anterior protrusion, posterior protrusion, superior protrusion, inferior protrusion, left side protrusion and right side protrusion. The neck movements were right side flexion, left side flexion, right rotation and left rotation. The cervical range of motion measured by cervical range of motion instrument (CROM, C9266-49, USA). The changes were analyzed using the paired t-test. SPSSWIN (ver. 20.0) was used for data analysis and the significance level was chosen as ${\alpha}$=0.05. Results : In the lateral flexion, the angle of left lateral flexion increased in anterior protrusion(p<0.05) and right side protrusion(p<0.05) significantly. In the rotation, the angle of right rotation increased in anterior protrusion(p<0.05), posterior protrusion (p<0.05), superior protrusion(p<0.05), inferior protrusion(p<0.05), and left side protrusion (p<0.05) significantly. Conclusions : In conclusion, we found that the tongue positions affected some cervical movements.

  • The author studied on the tongue position, tongue space and intermaxillary space at 4 groups; normal occlusion 30 cases, Angle's Class I bimaxillary protrusion 29 cases, Angle's Class II malocclusion 26 cases, Angle's Class III malocclusion 26 cases. This study used the data from cephalometric films of female subject aged from 16 to 23 years. Following results were obtained; 1. Dorsal height in Angle's Class III malocclusion was greater than normal occlusion. 2. Anterior Intermaxillary Height in Angle's Class I bimaxillary protrusion and Angle's Class III malocclusion was greater than normal occlusion. 3. Tongue space in Angle's Class I bimaxillary protrusion was greater than normal occlusion. 4. The ratio of the tongue space to the intermaxillary space (T.S./I.S.) in Angle's Class II malocclusion was greater and the ratio in Angle's Class III malocclusion is lesser than normal occlusion.

A STUDY ON THE MANDIBULAR MOVEMENT OF ANTERIOR OPENBITE PATIENTS (전치부 개교환자의 하악 운동에 관한 연구)

  • It is very important for the ideal restorations of anterior openbite patients to record the mandibular movement and to harmonize mandibular movement with other organs in stomatognathic systems. This study was designed to compare the mandibular movement of anterior openbite patients with that of normal bite(Angle Class I) patients, to ascertain which components of mandibular movement have differences between two groups, and to use for occlusal treatment of mandibular movement. Saphon Visi-trainer Model 3(Tokyo Shizaisha Co. Japan) and Denar Pantronic(Denar Corp.,U.S.A.) were used to record mandibular movement. Pantronic survey was peformed by using an arbitrary hinge axis according to manufacturer's direction. Twenty-eight adult who have physiologically normal occlusion(Angle Class I) and are free of TM dysfunction were selected as a control group(Group 1). Fifteen adult who are anterior openbite patient and have not anterior guidance function and have posterior interference at protrusion were selected as a experimental group(Group 2). The results are as follows : 1. There was no statistically significant difference between the average immediate and progressive side shift of anterior openbite patients(0.54mm, $7.57^{\circ}$) and those of normal group(0.49mm, $5.96^{\circ}$). 2. The average protrusive and orbiting condylar inclination of anterior openbite patient$(30.87^{\circ},\;32.27^{\circ})$ were significantly lower than those of normal group$(36.11^{\circ},\;39.04^{\circ})$ (P<0.05). 3. In the results of Visi-trainer recordings, the mean for the maximum protrusion, the maximum laterotrusion, the angle of laterotrusion and the angle of protrusion in the horizontal trajectory between group 1 and 2 did not differ significantly. 4. The mean for the angle of protrusion, the maximum opening in the frontal trajectory, the ICP-RCP(A-P) distance and the angle of protrusion in the sagittal trajectory differ significantly(P<0.05). 5. The significant correlation was found between orbiting condylar inclination and protrusive condylar inclination.

Effect on the facet joint tropism and lumbar paraspinal muscles according to the type of lumbar disc herniation (허리 척추사이원반 탈출 정도가 척추 후관절의 비대칭과 허리 주변근육에 미치는 영향)

  • Background: The purpose of this study was to investigate the effects of the severity and direction of lumbar disc herniation (LDH) on the facet joints and paraspinal muscles. Design: Cross-sectional design. Methods: The subjects were divided according to the diagnosis for severity of unilateral herniation of L4-L5 disc. The groups consisted of disc protrusion group (n=15), disc extrusion group (n=15), and no disc herniation group (n=15). The asymmetry and angle of facet joints and the cross-sectional area of paraspinal muscles were analyzed and compared using magnetic resonance imaging (MRI). Results: The results showed that the angle of facet tropism was larger in disc extrusion group than the disc protrusion group and the difference was found to be significant difference (p<0.01). In addition, when both left and right angles of patients with unilateral disc herniation were measured, the results showed larger facet joint angle in the herniated area of the disc extrusion group than in the disc protrusion group. When paraspinal muscles were measured according to the severity of disc herniation and the degree of facet joint asymmetry, there was no difference in paraspinal muscles between the disc protrusion and disc extrusion groups. Meanwhile, the multifidus muscle was smaller in the group with facet tropism than the group without facet tropism (p<0.03), while there were no significant differences in the erector spinae and psoas muscles. Conclusion: Progression of disc herniation resulted in increased facet joint tropism, increased angle of the facet joints in the direction of disc herniation, and decreased size of the multifidus muscle.

A STUDY OF MANDIBULAR MOVEMENTS IN RELATION TO OCCLUSAL TYPE (교합유형에 따른 하악운동에 관한 연구)

  • Faces of normal occlusion, open bite and class III malocclusion were measured using cephalogram in 190 male and female subjects. Using M.K.G., types of mandibular movement, amount of horizontal and vertical movement were studied in relation to occlusal types, and were compared with each other. The following conclusions were obtained. 1. Movement of protrusion was most different in relation to occlusal type. 2. In normal occlusion, maximal protrusion were horizontal 7.66mm, vertical 3.01mm in male, 6.41, 2.92mm respectively in female and maximal inferior position were -24.32, 33.63mm in male,-23.48, 32.27mm respectively in female. 3. Angle between maximal inferior position and horizontal plane were $53.51^{\circ}$ in male, $53.84^{\circ}$ in female. 4. Generally, amount of mandibular movement was greater in male than female. 5. In open bite, path of protrusion was nearly straight without curve $69.2\%$ in male, $70.2\%$ in female. 6. In class III malocclusion, angle and amount of protrusion were smaller than normal occlusion and path was directed anterior-superiorly in $22.7\%$. 7. There was no correlation coefficient between measurement of face and mandibular movement.

Effects of a Prefabricated Functional Orthodontic Appliance on Children with Class II Division 1 Malocclusion

  • Background: This study aimed to evaluate the effect of a prefabricated functional appliance (Myobrace®) on skeletal, dental, and soft tissue components in children with Class II, division 1 malocclusion. Methods: Thirteen patients with Class II, division 1 malocclusion (9 girls and 4 boys; mean age, 8.2±0.9 years at the start and 9.3±1.0 years at the end of the treatment) were treated with Myobrace® for a mean period of 12.9±4.0 months. Patients were instructed to use the appliance daily for 1 hour and overnight while sleeping. A control group of 10 patients with untreated Class II, division 1 malocclusion (3 girls and 7 boys; mean age, 9.0±1.6 years at the start and 10.4±2.1 years at the end of the observation) was included to eliminate possible growth effects. The mean observation period for this group was 17.7±11.2 months. Lateral cephalograms were taken at the start and end of the treatment, and findings from 41 measurements were analyzed using the V-CephTM program. The mean and standard deviation of cephalometric measurements were analyzed using paired and independent sample t-tests. Results: The treatment group showed significant changes in SNB, ANB, maxillary protrusion, ramus height, proclination of upper anterior teeth, interincisal angle, overjet, and upper lip protrusion compared with the control group. However, only decrease in ANB, maxillary protrusion, overjet, upper lip protrusion, and increase in interincisal angle were significantly higher in the treatment group than in the control group. Conclusion: The prefabricated functional appliance induced skeletal, dentoalveolar, and soft tissue changes, resulting in a significant reduction in anteroposterior discrepancy.

  • This paper analyzes the running mechanism of flexible and thin foil above rotating protrusion through a numerical simulation. The scope of analysis is confined to the phenomena of elastohy-drodynamic lubrication between the stationary and rotary drums with a running protrusion and thin foil. This mathematical model is based on the modified Reynolds equation and the equation of plate, considering the geometry of protrusion, running direction of protrusion, and the effect of geometric nonlinearity. Finite element method is adopted as a numerical simulation technique to solve the avobe coupled nonlinear equations. In numerical analysis, the effects of the scanning angle in Reynolds equation and the nonlinear term in plate equation are evaluated. Furthermore, the simulation is applied to the situation that thin foil is located in the entire drums (stationary and rotary drums).

Classification of Torso Shape According to Abdominal Protrusion of Middle-Aged Women (중년 여성 복부 돌출 정도에 따른 토르소 형태 분류)

  • The purpose of this study was to classify the torso shape based on abdominal protrusion caused by changes in the physical characteristics of middle-aged women. This study analyzed 3D shape data of 401 females ranging in age from 40 to 59 years who participated in the 6th Size Korea project. Based on the Size Korea 3D measurement standard, 27 additional items such as height, protrusion, and angle were measured in the 3D scan data. Nine factors were extracted from the analysis of constituent factors of the torso: "vertical size of torso," "flatness and protrusion of abdomen," "torso front extrusion," "upper body height," "bust size and flatness," "size of belly and angle of lower abdomen," "hip length," "hip flatness," and "horizontal size of bust." As a result of the cluster analysis using these nine factors, the torsos of middle-aged women were classified into three types. Type 1 has upper abdominal deposition with a small and long upper body and an advanced abdomen; type 2 has lower abdominal deposition with a small and short torso and a small belly and hip flexion; and type 3 has central abdominal deposition with a big and long torso, large breasts, and protruding abdo¬men front. The middle-aged women were mostly distributed in Type 2. The above results will be useful as basic data for the development of clothing with improved fit to accommodate the changed physical characteristics of middle-aged women.

A STUDY ON THE MOVEMENTS OF THE MANDIBLE IN CLEFT LIP AND PALATE INDIVIDUALS (순, 구개열자의 하악운동에 관한 연구)

  • In order to obtain the basic data of the movements of the mandible for orthodontic treatment and gnathosurgery of cleft individuals, the ranges and shapes of the movements of the mandible were measured in frontal, sagittal, and horizontal view with Saphon Visi-Trainer CII, in 19 adult cleft lip and palate individuals. The subjects included 5 BCLP, 9 UCLP, and 5 CLA patients. The measurements were compared with the values of normal individuals reported by Kang 1. Frontal view. The mean values for maximal laterotrusion were almost the same as those previously reported for the normal individuals. Mandibular deviation in maximal opening was usually toward the right, while it was mainly toward the left in normal individuals. Typical shield was formed only in the CLA group. 2. Sagittal view; The mean value for maximal protrusion was not different from that of normal group and antero-posterior deviation showed a significant difference. The angle of maximal protrusion and horizontal plane was less than that of normal group. 3. Horizontal view; The mean values for maximal laterotrusion and protrusion were not different from those of the normal group. The angle of the laterotrusion and horizontal plane was larger in the left and smaller in the right.

AIM ORTHODONTIC CASE REPORT OF MILWAUKEE BRACE WEARER (Milwaukee brace 장착자(裝着者)의 교정치험례(橋正治驗例))

  • 10years and 8 months old patient, who had been wearing Milwaukee brace for 5 months, was treated by multibanded system without extract on. She complianed severe protrusion and interdental spacing of upper anterior teeth. Cephalometric analysis revealed short anterior facial height, infraclusion of the lower first molars and severe profrusion of upper and lower anterior teeth. During orthodontic treatment Milwaukee brace was replaced by TLSO, so the orthopedic force on the dentofacial region was eliminated. After 2 years and 3 months, she gained raised bite, increased interincisal angle, salient reduction in the protrusion of upper central incisals with agreeable overjet, complete obliteration of interdental spacing and reduction of protrusion of upper and lower lips.
