중소 화학제품 제조업의 재해 특성 및 예방 (Accident Characteristics and Prevention in Small Manufacturing Industries of Chemical Products)

  • This study concerns with the accident characteristics and prevention in small manufacturing industries of chemical products. To investigate the accident characteristics, we used workers' compensation reports and employers' accident analysis reports. One hundred and seven injury accidents which results in more than 4 days absence were surveyed from the manufacturing companies with under 100 employees. These data were used to investigate the accident characteristics in terms of company size, injured person's age, work experience, accident time, activity at time of accident, accident type, injury type, injured body Part, and accident agency We propose the accident prevention policy based on the accident characteristics. These results can be used to develop more effective occupational safety management policies in small manufacturing industries of chemical products.

건설업 사고 발생원인 파악을 위한 사고 분석 모델 개발 (Development of Accident Cause Analysis Model for Construction Site)

  • Accident analysis models were developed to improve the construction site safety and case studies was conducted. In 2016, 86% of fatality accidents occurred due to simple unsafe acts. Structure related accidents are less frequent than the non structure related causes, but the number of casualties per accident is two times higher than non structure one. In the view of risk perception, efforts should be given to reduce accidents caused by low frequency - high consequence structure related causes. In case of structure related accident, structural safety inspection and management (including quality), ground condition management / inspection technology, and provision of risk information delivery system in case of non structure related accident were proposed as a solution. In analysis of relationship between safety related stakeholder, the main problem were the lack of knowledge of controller and player, loss of control due to duplicated controls, lack of communication system of risk information, and relative position error of controller and player.

  • The fatality of pedestrian accounts for about 21.2% of all fatality at 2007 year in Korea. In car to pedestrian accident it is very important to inspect the throw distance of pedestrian after collision for exact reconstructing of the accident. The variables that influence on the throw distance of pedestrian can be classified into the factors of vehicle and pedestrian, and road condition. It was simulated by PC-CRASH, a kinetic analysis program for a traffic accident in sedan type vehicle and SPSS program was used for regression analysis. From the results, the throw distance of pedestrian increased with the increasing of vehicle velocity, and decreased with the increasing of impact offset. Also it decreased with the increasing of velocity of pedestrian at accident, and throw distance at the road condition of wet was longer than that at dry condition. Finally, the regression model of sedan type vehicle on the throw distance of pedestrian was as follows; $$dist_i=2.39-0.11offset_i+0.59speed_i-545height_i-0.25walk_i+2.78wet_i+{\epsilon}_i$$.

  • The fatalities of pedestrian account for about 21.2% of all fatalities at 2007 year in Korea. To reconstruct exactly the accident, it is important to calculate the throw distance of pedestrian in car to pedestrian accident. The frontal shape of SUV vehicle is dissimilar to passenger car and bus, so the trajectory and throw distance of pedestrian by SUV vehicle is not the same of passenger car and bus. The influencing on it can be classified into the factors of vehicle and pedestrian, and road factor. It was analyzed by PC-CRASH for simulation, and SPSS s/w was used for regression analysis. From the simulation results, the maximum impact energy of multi-body of pedestrian was occurred to that of torso body at the same time. And the throw distance increased with the increasing of impact velocity, and decreased with the increasing of impact offset. Also it decreased with the increasing of velocity of pedestrian at accident, and the throw distance of wet road was longer than that of dry road. Finally, the regression analysis model of SUV(Nissan Pathfinder type)vehicle in car to pedestrian accident was as follows; $$disti_i=-0.87-0.11offseti_i+0.69speed_i-4.27height_i+0.004walk_i+0.63wet_i+{\epsilon}_i$$.

재해분석을 통한 배전선로 활선작업 공종별 위험지수 평가 (The Assessment of the Risk Index of Live-line Works on Distribution Line by the Accident Analysis)

  • The live-line works are very dangerous because of direct contacts with the distribution line or neighboring contacts. So the purpose of this study is to identify the risk factor by accident occurrence form and accident case analysis, and to suggest the quantified risk index by risk occurrence frequency and risk strength analysis. And the risk index assessment is researched by accident cases analysis on work type. Accident cases of transmission distribution line are researched based on data of the Ministry of Employment and Labor in the last ten-year period (2000~2009). In results of this paper, high risk isn't always a priority of safety measures. Risk occurrence frequency and risk strength have to be considered according to detail work types, work methods and conditions of field work. And safety management measures must be planned according to risk occurrence frequency and risk strength.

WEB 기반 교통사고 분석 (Analysis System for Traffic Accident based on WEB)

  • 겨울철 도로에서 발생하는 안개 및 결빙구간 교통사고 사망률의 경우는 도로조건 및 기상조건이 매우 중요한 요소 이다. 본 논문에서는 교통사고 예측 데이터를 가정하고 교통사고 위험율을 에측 하는 모의실험을 수행하였다. 그뿐만 아니라, 본 논문에서는 교통사고를 줄이고 교통사고를 예방하기 위해서, 교통공단에서 제공하는 교통사고 사망자 데이터를 WEKA 데이터 마이닝 기법 및 TENSOR FLOW 공개 소스를 이용해서 요인 분석 및 교통사고 치사율 사망을 예측하였다. 추가적인 기능으로는 지도 표시 기능을 이용해서, 운전자가 WEB 기반에서, 안개 및 결빙구간 정보를 운전자에게 제공하는 모의실험 및 교통사고 사진을 실시간으로 전송하는 모의실험 결과를 설명하였다.

Development of mechanistic cladding rupture model for severe accident analysis and application in PHEBUS FPT3 experiment

  • Cladding ballooning and rupture are the important phenomena at the early stage of a severe accident. Most severe accident analysis codes determine the cladding rupture based on simple parameter models. In this paper, a FRTMB module was developed using the thermal-mechanical model to analyze the fuel mechanical behavior. The purpose is to judge the cladding rupture with the severe accident analysis code. The FRTMB module was integrated into the self-developed severe accident analysis code ISAA to simulate the PHEBUS FPT3 experiment. The predicted rupture time and temperature of the cladding were basically consistent with the measured values, which verified the correctness and effectiveness of the FRTMB module. The results showed that the rising of gas pressure in the fuel rod and high temperature led to cladding ballooning. Consequently, the cladding hoop strain exceeded the strain limit, and the cladding burst. The developed FRTMB module can be applied not only to rod-type fuel, but also to plate-type fuel and other types of reactor fuel rods. Moreover, the FRTMB module can improve the channel blockage model of ISAA code and make contributions to analyzing the effect of clad ballooning on transient and subsequent parts of core degradation.

Risk Assessment for Marine Pilot Occupational Accidents using Fault Tree and Event Tree Analysis

  • Maritime transportation is one of the most complicated and hazardous business fileds. Maritime accidents still occur despite several precautions since maritime is exposed to natural factors more than any other industries. In this harsh environment as a part of their job, marine pilots often embark/disembark to/ from vessels and confront life-endangering personal accidents. In the maritime field, several risk assessments are applied. However, all of them could not evaluate occupational accident risk for maritime pilot specifically. This paper performs specific risk analysis using the bow-tie method based on past accident records. This paper aims to qualify root causes and quantify root causes by importance level according to occurrence probability. As a result of analysis, occupational accident occurrence probability is found to be 14%, indicating that accident occurrence rate is significantly high. Hence, the probability of root causes triggering accidents and accident occurrence probability can be ascertained so that preventive measurements can be implemented. Besides theoretical achievement, this paper provides safety awareness to marine pilots, Marine Pilot Organizations, and ship crew who play a key role during marine pilots' transfer.

  • The accidnet of breaking insulator rod leads to inturruption of moving the subway. We investigate the analysis of analysis of breaking accident of FRP insulator rod installed in dead section for catenary feeding system. To analysis of accident reason, SEM is used to analysis microscopic struture on surface of cross section of broken FRP insulator rod. At the same time, we examine the chage of atomic amount on solace of accident insulator through EDX analysis. Also, the test for tensional breaking load is condoled to check the mechanical strength.

통합적 휴먼에러 분석 모델을 이용한 자동차 유리공장의 사고 원인 분석 (Analysis of Accidents Causes in an Auto-Glass Manufacturing Company using the Comprehensive Human Error Analysis Model)

  • To prevent similar accidents with the basis of industrial accidents already occurred in industrial plants, it would be possible only after true causes are grasped. Unfortunately, however, most accident investigation carried out with the basis of legal regulation failed to grasp them so that similar accidents have been repeated without cease. This research aimed to find out differences between results from conventional accident investigation and those from human error analysis, and to draw out effective and practical counter-plans against industrial accidents occurred repeatedly in an autoglass manufacturing company. As for analysis, about 110 accident cases that occurred for last 7 years were collected, and by adopting the Comprehensive Human Error Analysis Technique developed by the previous researchers, not direct causes but basic fundamental causes that might induce workers to human errors were sought. In consequence, the result showed that facility factors or environmental factors such as improper layout, mistakes in engineering design, and malfunction of interlock system were authentic major accident causes as opposed to managerial factors such as personal carelessness or failure to wearing personal protective equipments, and/or improper work methods.
