소아환자 마약성 진통제 금단증상관리에 대한 간호사의 지식과 태도 (Pediatric Nurses' Knowledge and Attitude on Iatrogenic Narcotic Analgesic Withdrawal Symptoms Management)

  • Purpose : This study investigates pediatric nurses' knowledge and attitudes regarding iatrogenic narcotic analgesic withdrawal symptoms management (INAWSM). Methods : In this cross-sectional study, 253 pediatric nurses working at a tertiary hospital in Seoul participated. Instruments were the inventory consisting questionnaires on 35 items knowledge and 12 items attitude for INAWSM. Further, descriptive statistics, t-test, and analysis of variance were used to evaluate the nurses' knowledge and attitudes. Results : The correct answers rate of knowledge toward INAWSM was 44.9%. With respect to the pediatric nurses' attitudes, the participants scored an average value of $2.83{\pm}0.25$ out of 4 points. The factor related to the knowledge and attitude level was age, pediatric nursing experience, educational experience, and educational needs. Conclusions : Results revealed that pediatric nurses had inadequate knowledge and attitude on INAWSM despite a compulsory education. Therefore, it is necessary to develop an efficient INAWSM educational program.

Curcumin의 신경병증성 통증 억제효과 (Curcumin Attenuates Chronic Constriction Nerve Injury-Induced Neuropathic Pain in Rats)

  • Nerve injury can lead to neuropathic pain, which is often resistant to current analgesics and interventional therapeutic methods. Extracellular signal-regulated kinase (ERK) plays important role in the induction of neuropathic pain. We explored the antinociceptive effect of curcumin and its effect on ERK in the spinal cord in the neuropathic pain model of rats induced by chronic constriction injury (CCI) of the sciatic nerve. In injured rats, mechanical allodynia, which is one of characteristics of neuropathic pain developed and the activation of ERK in spinal cord significantly increased compared with control group. However, administration of curcumin (50 mg/kg/day p.o) for 7 days started from one day before the injury prevented the development of mechanical allodynia and increase of ERK phosphorylation. These results indicate that curcumin can be a new therapeutic agent in the treatment of neuropathic pain.

  • Hypnosis is an altered state of consciousness that comprises of heightened absorption in focal attention, dissociation of peripheral awareness, and enhanced responsiveness to social cues. Hypnosis has a long tradition of effectiveness in controlling somatic symptoms, such as pain. Pain, the most common symptom in clinical practice, is a multi-dimensional experience, which includes sensory-discriminative, affective-emotional, cognitive and behavioral components. There is a growing recognition for hypnosis and related techniques in pain management. Psychological approaches to pain control, such as hypnosis, can be highly effective analgesics, but are underused in Korea. In this article, we would like to review the basic concepts of hypnosis, the mechanism, and the outcome data of the analgesic effects of hypnosis, and also, its limitations.

일부 인문계 고등학교 남학생들의 건강행위에 관한 연구 (A Study on the Health Behavior of High School Students)

  • The Purpose of this study is to analyze the health behavior of high school students and to find factors influencing it. A questionnaire survey was used for collecting data from 201 male students between May 1 and June 30, 1993. The data were analyzed by the multiple regression technique in an SPSS package. The major results are as follows: (1) 84.5% of the students responded to the survey are regularly exercising to promote their health and 44.8% of them are doing something to manage stress. (2) 28.9% of the students are smoking, 43.3% of them are drinking, 5% of them are taking adhesives, 6.5% are taking stimulants, and 4.5% of them are taking analgesics. (3) while self-efficacy of the students and health interests of their parents are positively related to health promoting behaviors, they are negatively related to health risk behaviors.

  • A series of 4-amino Capsaincin anallogs 15, 17 and 19 were prepared to investigate the bioisosteric effect of 4-amino group, and all these compounds exhibited moderate or weak potency from their analgesic test. From our previous results and others, 4-hydroxyl group as well as 3-methoxy substituent could be crucial for high analgesic activity. This biological results also shows that the activity is sensitive to alkyl chain length in hydrophobic region and the phenylacetic amides 19 are more active than the corresponding urea derivatives 17.

  • In order to improve physico-chemical properties and to enhance stability of drugs, amino acid salt has been widely adoptd in pharmaceutical synthesis. Acetylsalicylic acid lysinate is one of the widely used analgesics and it is a good example of t5his synthesis. In the case of bacetylsalicylic acid lysinate synthesis, racemization of natrually occurred lysine is esential because the racemic lysine salt of the drug shows better yield, crystallinity and dryness than that of the L-lysine salt. To esatablish a simple, practical and economical process for L-lysine racemization, L-lysine treatments with phosphoric acid and with acetic acid were compared and the optimum conditions for its process and derivatization were investigated by chiral separation methods using GC_MS spectroscopy.

  • Houttuynia cordata Thunb (HCT) is a herbal plant growing in China, Japan and Korea. According to the records of the Chinese medicine, this plant has been used as folk medicine for analgesics, beriberi, edema, hepatitis, icterus, etc. TCDD (2, 3, 7, 8 -tetrachlorodibenzo-p-dioxin), one of the notorious toxic environmental pollutants, damages various organs including liver and is regarded as an endocrine disrupter. After 7 days from TCDD (1$\mu$g/kg) injection, HCT (200 mg/kg) was administered into rats intraperitoneally for 4 weeks. (omitted)

Effects of propofol-remifentanil versus sevoflurane-remifentanil on acute postoperative pain after total shoulder arthroplasty: a randomized trial

  • Background: While some evidence indicates that propofol-based anesthesia has less postoperative pain than sevoflurane-based anesthesia, these results are controversial. We compared acute postoperative pain intensity and opioid consumption after total shoulder arthroplasty between propofol-remifentanil (PR) and sevoflurane-remifentanil (SR) anesthesia. Methods: Among 48 patients undergoing shoulder arthroscopic surgery anesthetized with PR or SR, postoperative pain intensity was assessed at 30 minutes and at 2, 6, 12, and 24 hours. The total patient-controlled analgesia volume and number of patients requiring rescue analgesics were assessed. Results: No significant difference in postoperative pain intensity was observed between the two groups. Postoperative opioid consumption and analgesic requirements were also comparable in the first 24 hours after surgery. Conclusion: PR and SR anesthesia for shoulder arthroscopic surgery provide comparable postoperative analgesia results.
