Herpes zoster as a result of reactivated varicella-zoster virus is characterized by vesicular eruptions on skin and painful neuralgia in the dermatome distribution. Pain during an acute phase of herpes zoster has been associated with a higher risk of developing postherpetic neuralgia. The current therapies for herpes zoster including analgesics and sympathetic nerve block as well as antiviral agents are important to alleviate pain and prevent postherpetic neuralgia. However, in some cases, the pain does not respond well to these treatments. We had a case in which a patient with herpes zoster did not respond to conventional therapy so we attempted to administer intravenous infusion of vitamin C which resulted in an immediate reduction in the pain.

Objective: We report a case of non-specific Aconiti Tuber poison complaining only of severe peripheral neurotoxicity without cardiac dysfunction. Methods: The authors evaluated the symptom changes of a patient who was hospitalized in an Oriental hospital for fourteen days. The patient received acupuncture, herbal medicine, moxibustion and analgesics. Result: No abnormality in examination for cardiac function or biochemical parameters was present. The severity of pain and dysesthesia in lower extremities gradually receded during the period of treatment with herbal and western medicines. Conclusion: This study provides helpful information for treatment of Aconiti Tuber toxicity.

Postoperative pain, poor oral intake and various complaints of adenotonsillectomized patients are the main problems for the otolaryngologist. Steroids have been advocated to reduce morbidity after adenotonsillectomy, but the results are conflicting. This study is to determine the effect of preoperative and postoperative oral steroid. Material and Methods : A prospective, randomized study was performed on 40 patients from 4 to 13 years of age. 20 patients undergoing adenotonsillectomy received steroid(experimental group), others undergoing adenotonsillectomy did not receive steroid(control group). An oral prednisolone was administered preoperatively for 3 days and postoperatively 7 days. A dosage was determined by patient's weight. Postoperatively each patients was examined for weight loss as well as for subjective signs of pain, oral intake, activity, mouth odor and analgesic usage. Results： experimental group showed decreased morbidity in view of postoperative pain, oral intake with statistical significance (p<0.05). The usage of analgesics was decreased in steroid group. but, activity, mouth odor and body weight showed no statistical difference. Conclusion： The results showed that preoperative and postoperative oral steroid affect the postoperative morbidity in adenotonsillectomy.

Diabetic Peripheral Polyneuropathy is one of the most distressing complications of diabetes, Drugs such as narcotic analgesics, tricyclic antidepressants, anticonvulsants and phenothiazines have been used to treat diabetic peripheral polyneuropathy, but these drugs are not very effective for the many patients and the side effects may become intolerable. In oriental medicine, usually the main etiology of diabetic Peripheral Polyneuropathy is insufficiency of Blood due to the state of dry-heat in the human body. Therefore, we use Bogan-tang to treat numbness of Diabetic Peripheral Polyneuropathy. Bogan-tang was administrated twice a day for 15 days, and VAS Scale was performed every day to evaluate numbness and insufficiency of blood. In both cases, numbness and insufficiency of blood were improved without any side effects. This study suggests that Bogan-tang is an effective drug in the treatment of Diabetic Peripheral Polyneuropathy.

Celiac Plexus neurolysis (C. P. N) has been commonly used to treat malignant pain of the abdominal area that is intractable. It relieves the pain effectively instead of using massive systemic narcotic analgesics. C. P. N. with modified transaortic technique was performed under C-T guidance, in which a single needle was advanced from a left posterior paramedian approach through the aorta in order to inject anesthetic agents directly into the celiac plexus. There was marked pain relief without any hemorrhagic and neurological or other complications. We found this modified transaortic method of C. P. N. to be very effective, safe and easy to perform compared with the classic two needle technique.

The Tolosa-Hunt syndrome(THS) consists of a painful ophthalmoplegia related to a granulomatous inflammatory process in the cavernous sinus, which may be documented by cerebral magnetic resonance imaging with gadolinium enhancement. Generally, it has been reported that 2 weeks steroid therapy relieves the symptoms(periorbital pain, ophthalmoplegia) of THS. In this case, there had been no improvement of symptoms by western medical treatments(steroid & analgesics). but, the symptoms were resolved clearly by acupuncture therapy during 6 weeks(5 times/week). If more clinical trials like this are proved to be effective in THS, we can expect that acupuncture will be another therapeutical modality in THS.

Purpose : This study investigates pediatric nurses' knowledge and attitudes regarding iatrogenic narcotic analgesic withdrawal symptoms management (INAWSM). Methods : In this cross-sectional study, 253 pediatric nurses working at a tertiary hospital in Seoul participated. Instruments were the inventory consisting questionnaires on 35 items knowledge and 12 items attitude for INAWSM. Further, descriptive statistics, t-test, and analysis of variance were used to evaluate the nurses' knowledge and attitudes. Results : The correct answers rate of knowledge toward INAWSM was 44.9%. With respect to the pediatric nurses' attitudes, the participants scored an average value of $2.83{\pm}0.25$ out of 4 points. The factor related to the knowledge and attitude level was age, pediatric nursing experience, educational experience, and educational needs. Conclusions : Results revealed that pediatric nurses had inadequate knowledge and attitude on INAWSM despite a compulsory education. Therefore, it is necessary to develop an efficient INAWSM educational program.

Nerve injury can lead to neuropathic pain, which is often resistant to current analgesics and interventional therapeutic methods. Extracellular signal-regulated kinase (ERK) plays important role in the induction of neuropathic pain. We explored the antinociceptive effect of curcumin and its effect on ERK in the spinal cord in the neuropathic pain model of rats induced by chronic constriction injury (CCI) of the sciatic nerve. In injured rats, mechanical allodynia, which is one of characteristics of neuropathic pain developed and the activation of ERK in spinal cord significantly increased compared with control group. However, administration of curcumin (50 mg/kg/day p.o) for 7 days started from one day before the injury prevented the development of mechanical allodynia and increase of ERK phosphorylation. These results indicate that curcumin can be a new therapeutic agent in the treatment of neuropathic pain.
