Functional Evaluation of TSH Secretory Reserve Capacity in Hypothalamopituitary Disorders (시상하부-뇌하수체 질환에 있어서 뇌하수체의 TSH 분비능에 관한 연구)

  • The TRH stimulation test was known as a highly diagnostic method in hypothalamopituitary disorders. To evaluate the location and the extension of the lesion, we estimated TSH response to TRH test in 27 patients. Correlation between volume of sella and TSH response was also studied. The results obtained were as follows: 1. In Sheehan's syndrome, TSH response after TRH test were not observed in all of 12 patients. 2. All 2 acromegaly patients showed normal TSH response. 3. In 4 cases of chromophobe adenoma, 2 cases showed no TSH response. In 2 responded cases, one patient whose tumor mass extended to suprasella region was hypothyroid state. 4. In craniopharyingioma 3 cases, the tumor which extended to intrasella showed hypothyroid and no TSH response. 5. Correlation between volume of sella and TSH response were valuable in 2 cases, but no diagnostic significance. 6. In diabetes inspidus, TSH response were all absent. 7. In primary amenorrhea, TSH response observed in 1 case, which conformed with isolated FSH deficiency.

  • Radix Angelicae Gigantis is sweet and pungent in flavor, warm in property. Its effects are tonifying the blood, promoting blood circulation, relieving pain and moistening the bowels. Its indications are blood deficiency syndrome characterized by sallow complexion, dizziness, irregular menstruation, amenorrhea, pains due to blood stasis, and rheumatic arthralgia. Using genes of A. gigas, A. acutiloba, and A. sinensis, the origin of which is identified, as criteria, we analysed many kinds of Angelica with RAPD and RFLP on ITS region, in order to compare and discriminate genes extracted from crude drugs ‘Dang-gui’, that are produced in Korea on the one hand and imported on the other hand. We reached the following conclusion. 1. We could extract DNA from both original plant and dried plant. 2. Especially Uniprimer #1, Uniprimer #2, Uniprimer #4 and Uniprimer #9 were useful. 3. Among the restriction enzymes Sma I, Msp I, Hae III, and Hinf I, used in this experiment, four restriction enzymes except Hinf I could be used properly in discriminating all samples used as A. gigas. We think that this result can be used as a method of discriminating crude drug of Angelica L. related drugs, and used in controlling quality and circulation.

Hyperprolactinemia after taking Levosulpiride and its Causality Assessment: An Adverse Event Reported by a Community Pharmacy (Levosulpiride 복용 이후 발생한 고프로락틴혈증 및 그 인과성 분석: 지역약국에서 보고된 부작용 증례)

  • Levosulpiride is one of the most frequently prescribed medicines in Korea. An adverse drug reaction (ADR) after taking levosulpiride was reported at a community pharmacy in Korea. A 31-year-old woman reported the symptoms of lactation and amenorrhea after taking levosulpiride; an evaluation of whether these symptoms were caused by the medication was therefore necessary. Several tools can be used to determine if the ADR resulted from the administered drug or other factors, including the World Health Organization-Uppsala Monitoring Centre (WHO-UMC) criteria, the Naranjo scale, and the Korean causality assessment algorithm (Ver. 2). The causality was evaluated as "possible" by the WHO-UMC and Naranjo scales, but as "probable" by the Korean causality assessment algorithm (Ver. 2). In conclusion, the information provided did not indicate definite causality and there were slight differences in the results obtained from each assessment method.

  • Objective: To report the pedigree of Kallmann syndrome inherited in autosomal dominant mode with variable expressivity. Material and Method: Case report. Results: The patient had amenorrhea and anosmia but did not have a sign of absolute hypo gonadotropic hypogonadism. Her father had an anosmia and her two elderly sisters also had an anosmia but delivered babies uneventfully. Her two male siblings did not show any signs of hypogonadotropic hypogonadism. Conclusion: Kallmann syndrome has many different modes of inheritance such as autosomal dominant, autosomal recessive, and X-linked form. So the careful investigation of family pedigree is required.

Study on Cytotoxic Activities of the Essential Oil Compounds from Ligusticum chuanxiong against Some Human Cancer Strains (토천궁 정유 성분의 수종 사람 암 세포주에 대한 세포 독성)

  • Ligusticum chuanxiong (Umbelliferae) is a perennial herb that has been used for invigoration of blood in Korean traditional medicine. It is especially important in gynecological therapy of amenorrhea and dysmenorrhea. In this study, the essential oil of L. chuanxiong was obtained by steam distillation and its main components of L. chuanxiong, Z-ligustilide and butylidene phthalide, were isolated by silica gel column chromatography. We investigated the cytotoxic effects of the essential oil fraction of L. chuanxiong and its main components on MCF-7, HeLa and SK-Hep-1 cell lines by measuring the number of surviving cancer cells after treatment through direct cell counting and MTT analysis, and by examining the morphological changes under the microscope. The essential oil from the rhizomes of L. chuanxiong and its main components showed significant cytotoxic activities for all three tested cell lines. We also observed morphological changes of shrinking and blebbing in the membranes of the three cell lines, depending on the concentration of L. chaunxiong oil or its main components.

A literature study of acupuncture and moxibustion therapy in the the uterine section (in the Naegyeong Chapter) of Dong-Ui-Bo-Gam (동의보감(東醫寶鑑) 포문(胞門)의 침구법(鍼灸法)에 관한 소고(小考))

  • Objectives : The aim of this study was to show the rationale of point-selection for acupuncture and moxibustion in the Uterine section(in the Naegyeong Chapter) of the Dong-Ui-BO-Gam. Methods : We reviewed the causes of each disease in the Uterine section of the Dong-Ui-BO-Gam, and then explained the rationale of acupoint-selection for the treatment of those diseases referring to etiology and physiology of Oriental medicine, other applications of each acupoints in the Dong-Ui-BO-Gam, characteristic of each acupoints, flow of Gi (Qi) through meridian pathways and specific acupoints etc. Results : There are comments on acupuncture and moxibustion for abnormal menstruation, amenorrhea, metrorrhagia, leukorrhea, bleeding from uterus after menopause in the Uterine section of the Dong-Ui-BO-Gam. Conclusions : Acupoints of conception vessel, and three foot Yin meridians are preferably used for acupuncture and moxibustion in the Uterine section of the Dong-Ui-BO-Gam. Out of them, CV3 is most frequently used and SP6, CV6, BL23, LR2 are also used often.

A Case Report of Primary Ovarian Insufficiency Treated with Korean Medicine Treatment (한의치료로 호전된 조기난소부전 환자 치험 1례 보고)

  • Objectives: The purpose of this study is to report the case of Korean medicine treatment on primary ovarian insufficiency. Methods: The patient in this case was 29-year-old female who was diagnosed with primary ovarian insufficiency. She had irregular menstruation and hypomenorrhea more than 6 months. She also suffered from hot flash and vaginal dryness. She was treated over 1 year with Korean medicine treatment, such as herbal medicine, acupuncture, and pharmacopuncture. We assessed the clinical symptoms, menstrual status and serum hormone level during the treatment. Results: After treatment, symptoms of primary ovarian insufficiency were relieved, level of serum FSH decreased and level of serum E2 increased. We maintained the treatment over 1 year and kept follow-up measurements of serum hormone level. Conclusions: This study shows that Korean medicine treatment can be effective in treating primary ovarian insufficiency. The report suggests the long treatment procedure for primary ovarian insufficiency.

  • Sesame (Sesamum indicum L.) is a rich source of edible oil most commonly it is used as a food product mainly in bakeries and also use as a common source of oil in daily kitchen needs. Due to the presence of some special phytochemicals like proteins, fibers, oil, minerals and antioxidants it is highly used for medicinal and therapeutic purposes. It is a good source of energy and act as an antiaging agent. Its seeds are used as Anti-helmintic, antihypertensive, antimicrobial, cytotoxic and Hepatoprotective but its seed coat which is a byproduct of sesame and a cherished source of fibers is normally use for animal feedstuff. In Unani system of medicine it is used both as drug & diet (dawa wa ghida). In classical Unani literature it is indicated in various disorders like Asthma, Dry Cough, Gastritis (due to any drug, excessive use of alcohol), Dryness of Intestine, Dryness in throat, Renal Stone, Bleeding Piles, Amenorrhea, Retention of urine, Dysuria, Orchitis, Sexual Debility, Anorexia. The present review article, an attempt have been made to compile all the pharmacological and Pharmacognostical characters of Sesamum indicum with special reference to Unani literature.

A Review of Clinical and Preclinical Studies on the Therapeutic Potential of Black Seeds (Nigella sativa) in the Management of Polycystic Ovarian Syndrome (PCOS)

  • Objectives: Polycystic ovary syndrome (PCOS) is a condition that occurs frequently among women of reproductive age and is a polygenic, multifactorial, endocrine, and metabolic disorder. PCOS is becoming more common as a result of risk factors such as current lifestyle, overnutrition, and stress. The use of traditional herbal medicine is higher among the global population. Hence, this review article focuses on the potential of Nigella sativa to manage women with PCOS. Methods: A literature search was carried out using databases including Medline, Google Scholar, EBSCO, Embase, and Science Direct, as well as reference lists, to identify relevant publications that support the use of N. sativa in the management of women with PCOS. Results: Several clinical and preclinical studies have demonstrated that the major bioactive constituent of black seed (N. sativa), thymoquinone, has potential for managing women with PCOS. Moreover, N. sativa may help to manage oligomenorrhea and amenorrhea in women with PCOS through its anti-inflammatory and antioxidant properties. Conclusion: N. sativa has potential for use as a herbal medicine for managing women with PCOS as an integrative medicine along with traditional and modern medicine in conjunction with calorie restriction and regular exercise.

Bone Mineral Density in Premenopausal Amenorrheic Women with Hypogonadism (폐경전 무월경 여성에 있어서 골밀도 측정에 관한 연구)

  • Bone mineral density was measured to assess whether the patients with premonopausal hypogonadism, comprised of 19 patients with hypogonadotropic hypogonadism(HH), 55 with premature ovarian failure(POF), 23 with hyperprolactinemia(HPLN), and 8 with Sheehan's syndrome. All aged from 20 to 39, were associated with some decrease in regional bone mass compared with that of 63 normal-associated with some decrease in regional bone mass compared with that of 63 normal-cycling control women matched with age and sex. Measurement of bone mineral density was carried out using Dual photon absorptiometry at four sites; femur neck, ward's triangle, trochanter and spine(L2-L4). Bone mineral density at all four sites were significantly decreased in patients with HH(p<0.01), POF(p<0.01). In hyperprolactinemic patients, the decrease in bone mass was significant at femur neck and Ward's triangle(p<0.05). The patients with POF were noted to be associated with significant bone loss when their duration of amenorrhea exceeded one year. In this study, the degree of loss of bone mass and the affected sites seemed to be different depending on the various types of hypogonadism and POF was noted have caused the appearance of loss of bone mass earlier than other thpes of hypogondism we experienced.
