Purpose: Resorption of the alveolar bone is an unavoidable consequence of tooth extraction when appropriate alveolar ridge preservation (ARP) measures are not taken. The objective of this trial was to test the hypothesis that dimensional changes in the alveolar bone after tooth extraction would be reduced by inserting an equine collagen membrane and a collagen cone to fill and seal the alveolus (as ARP), in comparison to extraction with untreated alveoli. Methods: In this randomized clinical trial, 31 patients were directly treated with the collagen material after extraction of a tooth from the maxilla (the ARP group). Twenty-nine patients served as the control group. After extraction, no further treatment (i.e., no socket preservation measures) was performed in the control group. Changes in the alveolar process immediately after extraction and after an 8 (${\pm}1$)-week healing period were evaluated 3-dimensionally. Blinded analyses were performed after superimposing the data from the digitalized impressions and surfaces generated by cone-beam computed tomography. Results: Both the ARP and control groups showed a reduction of bone in the alveolar area after tooth extraction. However, significantly less bone resorption was detected in the clinically relevant buccal region in the ARP group. The median bone reduction was 1.18 mm in the ARP group and 5.06 mm in the control group (P=0.03). Conclusions: The proposed hypothesis that inserting a combination material comprising a collagen cone and membrane would lead to a difference in alveolar bone preservation can be accepted for the clinically relevant buccal distance. Background: Tooth extraction commonly leads to loss of residual alveolar ridge, thus compromising the room available for the implant placement. To combat the post-extraction alveolar loss, alveolar ridge preservation is practiced, with the advent of the biomaterial available. The purpose of this study was to assess the efficiency of calcium phosphosilicate biomaterial in alveolar ridge preservation. Twenty patients indicated for extraction were selected followed by socket grafting using calcium phosphosilicate. Implant placement was done 6 months postoperatively during which a core was harvested from the preserved sockets. Clinico-radiographic measurements of hard and soft tissues were taken at baseline and 6 months post-grafting. Results: There were no significant changes in the radiographic and soft tissue parameters while significant changes in hard tissue parameters with 1.9 mm (p = 0.013) gain in mid-buccal aspect and 1.1 mm (p = 0.019) loss in horizontal bone width were observed. The histomorphometric evaluation depicted the vital bone volume of 54.5 ± 16.76%, non-mineralized tissue 43.50 ± 15.80%, and residual material 2.00 ± 3.37%. Conclusion: The implants placed in these preserved ridges presented 100% success rate with acceptable stability after a 1-year follow-up, concluding calcium phosphosilicate is a predictable biomaterial in alveolar ridge preservation.
Purpose: This study was aimed to compare the survival and success rates, and long-term crestal bone loss according to the use of 2 connection types of dental implants (submerged-USII and non-submerged-SSII; Osstem $Implant^{(R)}$) by analyzing the change in alveolar bone height after 1 year under load and during final follow-up period. Materials and Methods: Between December 2004 and August 2008, patients with two types of Osstem implants (USII and SSII) were retrieved retrospectively. A total of 92 patients with 284 implants (USII=60, SSII=224) was finally selected. Their mean follow-up period was 7.5 years. The mesial and distal alveolar crestal bone changes were measured using radiographic images and the average was calculated at 1 year after loading and during final follow-up period. Result: Among the 284 implants, 4 USII and 7 SSII implants were removed, indicating 93.3% and 96.9% survival rates. Of the survived implants, mean crestal bone loss 1 year after loading was 0.39 mm for USII and 0.19 mm for SSII (P=0.018). During the final follow-up, mean crestal bone loss was 0.63 mm and 0.35 mm for USII and SSII, respectively, without statistical significance (P=0.092). According to the criteria for the success and failure of the implant by Albreksson and colleagues, final success rate was estimated as 86.7% for USII and 91.5% for SSII, respectively. Conclusion At 1 year after loading, the average crestal bone loss was significantly different between USII and SSII; however, both types met the criteria for implant success. During the final follow-up, both groups showed insignificant bone resorption patterns and did not show any pathological clinical symptoms. Therefore, both implants exhibited high long-term stability.
The purpose of the current study was to investigate histologic changes in the alveolar bone of the lower molar region subsequent to the loss of their opposite molars, and to characterize chemical alterations by utilization of histochemical procedures. Twenty five rats(Sprague Dawley), approximately 150-200gm body weight, were used in this experiment. In the treated animals, upper molars were removed. The animals were decapitated by groups at the following intervals after teeth removals: 10th, 20th, 50th, 70th and 100th day. The normal, untreated rats were used as controls. The molar region of lower jaw, including the intact alvelar bone and teeth was dissected and specimens were decalcified in 3% formic acid. After the tissues were fully decalcified, the specimens were embedded in celloidin and sectioned in mesiodistal plane. These sections were stained in the following staining methods. Mallory azan stain and hematoxylin-eosin stain were utilized for structural evaluation. Polysaccharides were demonstrated by means of the PAS reaction. Acidmucopolysaccharides were studied by means of the colloidal iron stain. Alloxan-Schiff reaction was used for protein. The results were as follows: 1) In the control animals, bone resorption was noted in the distal alveolar bone proper and bone apposition was shown in the mesial alveolar bone proper. But in the treated animals, bone apposition was observed on the mesial and distal walls of the alveolus and osteoclastic activity was not noted in any walls. 2) Bone apposition was most prominent from the 10th to 20th day after treatment. 3) Appositional growth of cementum along the surface of root was prominent from the 50th to 70th day after treatment. 4) In the area where osteoblastic activity was apparent, osteoblasts were stained strongly in the PAS and alloxan-Schiff reaction. A plastic resorption line showed strong alloxan-Schiff reaction. 5) In the colloidal iron stain, the alveolar wall adjacent to the cementum apposition area was stained more strongly than the other areas.
The purpose of this study was to evaluate the mechanical effects when one implant fixture was connected to the natural teeth with reduced alveolar bone height. This study also examined the effects of increasing the number of abutment teeth and the effects of the intramobile connector and the titanium connector as they were inserted between the implant superstructure and the fixture. The distribution and concentration load was applied to the fixed partial denture(FPD) supported by implant and the natural teeth with reduced alveolar bone height. The stress and displacement of each element was observed and compared by the two-dimensional finite element method. The following results were obtained : 1. The greater the loss of alveolar bone in natural teeth area, the greater the displacement of FPD and the stress concentration in alveolar bone around implant, especially at the stress concentration in the mesial alveolar bone crest around implant fixture. 2. The displacement of FPD was increased more and that of implants fixture was decreased more when intramobile connector was used than titanium connector was used. Also the stress concentration in alveolar bone around implant fixture was greater when intramobile connector than titanium connector. One implication of this finding was that the difference in stiffness of implant and the natural teeth with reduced alveolar bone height could be partially compensated in case of the POM intramobile connector. 3. The amount and direction of displacement and the stress distribution of the 4-unit FPD was better than those of the 3-unit FPD. It implied that the difference of stiffness of implant and natural teeth with reduced alveolar bone height could be partially compensated in case of the 4 unit FPD.
어린이는 8-10세 경에 외상을 자주 받으며 구강 내에서는 상악 절치부가 호발하는 부위이다. 외상으로 인하여 상악 절치가 상실되면 어린이에게 합병증을 야기할 수 있으므로 가능한 한 상악 절치를 유지해야 하지만 조기 상실되는 경우가 있다. 상악절치의 조기 상실로 인한 합병증으로는 심미적 문제나 치조골의 수직적 높이와 수평적 너비의 감소, 인접치의 경사, 악궁 장경 감소 등이 있다. 그 중에서 치조골이 소실되면 보철적 수복 시에 기능성과 안정성, 심미적인 면에 영향을 준다. 이 증례는 외상으로 인하여 상악 중절치가 조기 상실된 9세 여아와 6세 남아에서 각각 5년 5개월, 3년 7개월 후의 치조골소실 정도를 cone beam computed tomography와 진단 모형 상에서 관찰하였기에 보고하는 바이다.

Purpose: The purpose of this study was to evaluate vertical bone loss and alveolar bone thickness in the maxillary and mandibular incisors of patients with skeletal class III malocclusion. This study also aimed to evaluate the periodontal condition of class III malocclusion patients who had not undergone orthodontic treatment. Materials and Methods: The sample included cone-beam computed tomography scans of 24 Korean subjects (3 male and 21 female). Alveolar bone thickness (ABT), alveolar bone area (ABA), alveolar bone loss (ABL), and fenestration of the maxillary and mandibular incisors were measured using 3-dimensional imaging software. Results: All incisors displayed an ABT of less than 1.0 mm from the labial surface to root level 7 (70% of the root length). A statistically significant difference was observed between the mandibular labial and lingual ABAs and between the maxillary labial and mandibular labial ABAs. The lingual ABA of the mandibular lateral incisors was larger than that of the mandibular central incisors. ABL was severe on the labial surface. A statistically significant difference was observed between the maxillary and mandibular labial ABL values(21.8% and 34.4%, respectively). Mandibular lingual ABL (27.6%) was significantly more severe than maxillary lingual ABL (18.3%) (P<0.05). Eighty-two fenestrations were found on the labial surfaces of the incisors, while only 2 fenestrations were observed on the lingual surfaces. Fenestrations were most commonly observed at root level 6. Conclusion: Careful evaluation is needed before orthodontic treatment to avoid iatrogenic damage of periodontal support when treating patients with class III malocclusion.
Objective: This study was performed to investigate the changes in alveolar bone after maxillary incisor intrusion and to determine the related factors in deep-bite patients. Methods: Fifty maxillary central incisors of 25 patients were evaluated retrospectively. The maxillary incisors in Group I (12 patients; mean age, $16.51{\pm}1.32years$) were intruded with a base-arch, while those in Group II (13 patients; mean age, $17.47{\pm}2.71years$) were intruded with miniscrews. Changes in the alveolar envelope were assessed using pre-intrusion and post-intrusion cone-beam computed tomography images. Labial, palatal, and total bone thicknesses were evaluated at the crestal (3 mm), midroot (6 mm), and apical (9 mm) levels. Buccal and palatal alveolar crestal height, buccal bone height, and the prevalence of dehiscence were evaluated. Two-way repeated measure ANOVA was used to determine the significance of the changes. Pearson's correlation coefficient analysis was performed to assess the relationship between dental and alveolar bone measurement changes. Results: Upper incisor inclination and intrusion changes were significantly greater in Group II than in Group I. With treatment, the alveolar bone thickness at the labial bone thickness (LBT, 3 and 6 mm) decreased significantly in Group II (p < 0.001) as compared to Group I. The LBT change at 3 mm was strongly and positively correlated with the amount of upper incisor intrusion (r = 0.539; p = 0.005). Conclusions: Change in the labial inclination and the amount of intrusion should be considered during upper incisor intrusion, as these factors increase the risk of alveolar bone loss.

Purpose: The aim of this study was to evaluate 3-dimensional cone-beam computed tomography (CBCT) images of alveolar bone changes in patients who underwent minimally invasive periodontal surgery-namely, the pinhole surgical technique (PST). Materials and Methods: Alveolar bone height was measured and compared on CBCT images of 254 teeth from 23 consecutive patients with Miller class I, II, or III recession who had undergone PST. No patient with active periodontal disease was selected for surgery. Two different methods were used to assess the alveolar bone changes postoperatively. In both methods, the distance between the apex of the tooth and the mid-buccal alveolar crestal bone on pre- and post-surgical CBCT studies was measured. Results: An average alveolar bone gain >0.5 mm following PST was identified using CBCT(P=0.05). None of the demographic variables, including sex, age, and time since surgery, had any significant effect on bone gain during follow-up, which ranged from 8 months to 3 years. Conclusion: PST appears to be a promising treatment modality for recession that results in stable clinical outcomes and may lead to some level of resolution on the bone level. More long-term studies must be done to evaluate the impact of this novel technique on bone remodeling and to assess sustained bone levels within a larger study population.
Objectives: Periodontitis is an inflammatory disease with the destruction of periodontal ligament, alveolar bone loss and inflammation of gingva, leading to teeth loss. Eclipta prostrata L. (Ecliptae Herba) has been used to treat the inflammatory disease as a Korean traditional medicine. The aim of this study is to investigate the effects of E. prostrata L. on periodontitis. Methods: E. prostrata L. was extracted with water and lyophilized. The aqueous extract of E. prostrata L. (EP) was topically applied to the periodontal lesion for 2 weeks. To induce the periodontitis, a 3-0 nylon ligature was placed around the cervix of the lower first molar in rat. Rats were divided into 3 groups (n = 7); NL group (non-ligatured and non-treated), L group (ligatured and vehicle-treated) and EP group (ligatured and EP-treated). After sacrifice, the mandibles was dissected and stained with methylene blue solution to analyze the alveolar bone loss. The expression of MMP-9 was determined in gingival tissues. To confirm the effect of EP on recovery of gingiva, mRNA expressions of type I pro-collagen and MMP-9 levels were investigated in LPS-treated HS68 fibroblast cells. In addition, inflammatory mediators were evaluated in LPS-treated RAW264.7 cells. Results: Alveolar bone loss was significantly inhibited by EP treatment. The mRNA expression of MMP-9 was attenuated in rats treated with EP. In addition, treatment with EP increased the expression of type I pro-collagen, while the expression of MMP-9 was decreased in LPS-stimulated HS68 fibroblast cells. Furthermore, EP down-regulated the LPS-induced IL-6, $TNF-{\alpha}$, COX-2 and iNOS production in RAW264.7 cells. Conclusions: These results suggest that EP have ameliorative effects on periodontitis through inhibiting alveolar bone loss and modulating the inflammatory mediators. Therefore, E. prostrata L. may be an alternative on patients with periodontitis.
