Estimation of cost by unnecessary readmission of the tertiary hospitals (불필요한 재입원 비용 추정에 관한 연구)

  • Unnecessary readmissions could be the result of the inadequate and unnecessary treatments. Adequate quality indicators for readmission are important because they can identify inadequate spending by inpatients as well as quality screening. This study attempted to estimate the cost incurred by unnecessary readmissions. The Health Insurance Claims Data of 18 years or older who were admitted in the tertiary hospitals in 2014 were analyzed. Admissions and readmissions were sorted and readmissions were classified into planned and unplanned readmissions. We adopted 28 days as a criteria for the classification of the readmission. Proportion of the patients were higher in readmissions among cancer, accompanied diseases, and special rehabilitation patients. Cost of the readmissions were 50% of the total cost of the admission among the patients of same diseases, same departments, and same hospitals. Almost 1,000billion Won were used by the unnecessary readmissions. We need to reduce the readmissions in regions, departments, and diseases studying the pattern of the readmissions. National level efforts are required to improve quality of care and reduce cost by the unnecessary readmissions.

A Study on the Characteristics of Residual Stress in the Manufacturing Process of AISI 1536V and AISI A387 (제조공정에 따른 강종별 잔류응력 특성에 관한 연구; AISI 1536V, AISI A387)

  • This study analyzes the residual stress of AISI 1536V for an engine shaft of the shipbuilding industry and AISI A387 for a reactor shell of the chemical refining industry by the hole drilling method with a strain gauge rosette, which transforms fine mechanical changes into electrical signals. Tensile residual stress is generated in the forging and heat treatment process because specimens are affected by thermal stress and metal transformation stress. In the heat treatment process, the residual stress of AISI A387 is almost 170% the yield strength at 402 MPa. Since during the machining process, variable physical loads are applied to the material, compressive residual stress is generated. Under the same condition, the mechanical properties greatly affect the residual stress during the machining process. After the stress-relieving heat treatment process, the residual stress of AISI A387 is reduced below the yield strength at 182 MPa. Therefore, it is necessary to control the temperature, avoid rapid heat change, and select machining conditions depending on the mechanical properties of materials during manufacturing processes. In addition, to sufficiently reduce the residual stress, it is necessary to study the optimum condition of the stress-relieving heat treatment process for each material.

Impact of Competition on Physician Behavior Clinics - Focused on Acute Otitis Media in Children - (의원급 의료기관 간 경쟁이 진료행태에 미치는 영향 - 유·소아 급성중이염 중심 융합연구 -)

  • This study analyzes the impact of competitions clinics on the total treatment cost and anti-biotics prescription rate. The result of implementing the basic statistics, correlations, and regression analysis by facilitating the evaluation data by Health Insurance Review & Assessment Service in 2015 for acute otitis media in children is shown as follows. First, there is a significant difference for each si-gun-gu for the competition index between total treatment cost and clinics, but there is almost no significant difference for the anti-biotics prescription rate. Second, competition in clinics has statistically important impact on the total examination cost And, third, competition in clinics has no statistically important impact on the anti-biotics prescription rate. There is a need for additional studies on re-examination rate, treatment cost per visit and so forth in order to clarify other factors of competition for medical institutions impacting on the physician behavior in the future studies.

A New Genetic Algorithm for Shortest Path Routing Problem (최단 경로 라우팅을 위한 새로운 유전자 알고리즘)

  • This paper presents a genetic algorithmic approach to shortest path (SP) routing problem. Variable-length chromosomes (strings) and their genes (parameters) have been used for encoding the problem. The crossover operation that exchanges partial chromosomes (partial-routes) at positionally independent crossing sites and the mutation operation maintain the genetic diversity of the population. The proposed algorithm can cure all the infeasible chromosomes with a simple repair function. Crossover and mutation together provide a search capability that results in improved quality of solution and enhanced rate of convergence. Computer simulations show that the proposed algorithm exhibits a much better quality of solution (route optimality) and a much higher rate of convergence than other algorithms. The results are relatively independent of problem types (network sizes and topologies) for almost all source-destination pairs.

  • The purpose of this study is to analyze the differences among short statured children with growth hormone deficiency, idiopathic, and normal children in order to find the effect of growth hormone deficiency on tooth developmental stage. We classified children diagnosed with short stature at the Pediatrics in Yeungnam University Hospital as subjects due to idiopathic and growth hormone deficiency. We analyzed the relationship between short stature and tooth development through the measuring of oral panorama and body index. Only the eruption of lateral incisors showed significant difference between short statured with growth hormone deficiency and idiopathic. Almost all tooth eruption was significantly delayed on short statured children with growth hormone deficiency compared to average group. In conclusion, short stature children with either growth hormone deficiency or idiopathic were affected not only in their somatic stature but also dental maturity. We look forward to this study presenting basic data for orthodontic therapy.

Numerical Study on the Impact of Power Plants on Primary PM10 Concentrations in South Korea

  • To develop effective emission abatement strategies for eighteen coal-fired power plants located throughout Korea, power plant emission data and TAPM (The Air Pollution Model) were used to quantify the impact of emission reductions on primary $PM_{10}$ concentrations. TAPM was validated for two separate time periods: a high $PM_{10}$ concentration period from April 7 to 12, 2016, and a low $PM_{10}$ concentration period from June 1 to June 6 2016. The validated model was then used to analyze the impacts of five applicable power plant shut-down scenarios. The results showed that shut-down of four power plants located within the Seoul metropolitan area (SMA) would result in up to 18.9% reduction in maximum $PM_{10}$ concentrations, depending on synoptic conditions. A scenario for the shutdown of a single low stack height with highest-emission power plant located nearest to Seoul showed a small impact on averaged $PM_{10}$ concentrations (~1%) and 4.4% ($0.54{\mu}g/m^3$) decrease in maximum concentration. The scenario for four shutdowns for power plants aged more than 30 years within SMA also showed a highest improvement of 6.4% ($0.26{\mu}g/m^3$ in April) in averaged $PM_{10}$ concentrations, and of 18.9% ($2.33{\mu}g/m^3$ in June) in maximum concentration, showing almost linear relationship in and around SMA. Reducing gaseous air pollutant emissions was also found to be significant in controlling high $PM_{10}$ concentrations, indicating the effectiveness of coreduction of power plant emissions together with diesel vehicle emissions in the SMA. In addition, this study is implying that secondary production process generating $PM_{10}$ pollution may be a significant process throughout most regions in Korea, and therefore concurrent abatement of both gas and particle emissions will result in more pronounced improvements in air quality over the urban cities in South Korea.

  • Almost all network systems provide an authentication mechanism based on user ID and password. In such system, it is easy to obtain the user password using a sniffer program with illegal eavesdropping. The one-time password and challenge-response method are useful authentication schemes that protect the user passwords against eavesdropping. In client/server environments, the one-time password scheme using time is especially useful because it solves the synchronization problem. In this paper, we present a new identification scheme: OPI(One Pass Identification). The security of OPI is based on the square root problem, and OPI is secure: against the well known attacks including pre-play attack, off-line dictionary attack and server comprise. A number of pass of OPI is one, and OPI processes the password and does not need the key. We think that OPI is excellent for the consuming time to verify the prover.

  • The automotive door has a large finite element model in analysis and many design requirements such as stiffness, natural frequency, side intrusion, etc. Thus, various related governing equations should be solved for systematic analysis and design. Because each governing equation has different characteristics, it is almost impossible to solve them simultaneously. Instead, they are separately handled and the analysis results are incorporated into the design separately. Currently, the design is usually conducted by trials and errors with engineering intuition in design practice. In this research, MDO methods are proposed to solve the problems that share design variables in disciplines. The idea is from the Gauss-Seidel type method for multi-discipline analysis. The developed methods show stable convergence and the weight of the door is reduced by fifteen percent.

Joint Compensation of Transmitter and Receiver IQ Imbalance in OFDM Systems Based on Selective Coefficient Updating

  • In this paper, a selective coefficient updating (SCU) approach at each branch of the per-tone equalization (PTEQ) structure has been applied for insufficient cyclic prefix (CP) length. Because of the high number of adaptive filters and their complex adaption process in the PTEQ structure, SCU has been proposed. Using this method leads to a reduction in the computational complexity, while the performance remains almost unchanged. Moreover, the use of set-membership filtering with variable step size is proposed for a sufficient CP case to increase convergence speed and decrease the average number of calculations. Simulation results show that despite the aforementioned algorithms having similar performance in comparison with conventional algorithms, they are able to reduce the number of calculations necessary. In addition, compensation of both the channel effect and the transmitter/receiver in-phase/quadrature-phase imbalances are achievable by these algorithms.

  • An approximate line search is presented for dynamic response optimization with Augmented Lagrange Multiplier(ALM) method. This study empolys the approximate a augmented Lagrangian, which can improve the efficiency of the ALM method, while maintaining the global convergence of the ALM method. Although the approximate augmented Lagragian is composed of only the linearized cost and constraint functions, the quality of this approximation should be good since an approximate penalty term is found to have almost second-order accuracy near the optimum. Typical unconstrained optimization algorithms such as quasi-Newton and conjugate gradient methods are directly used to find exact search directions and a golden section method followed by a cubic polynomial approximation is empolyed for approximate line search since the approximate augmented Lagrangian is a nonlinear function of design variable vector. The numberical performance of the proposed approach is investigated by solving three typical dynamic response optimization problems and comparing the results with those in the literature. This comparison shows that the suggested approach is robust and efficient.
