개심수술후 혈량 증가를 위한 10% Pentastarch와 5% Albumin 용액의 비교연구 (10% Pentastarch Versus 5% Albumin Solution for Volume Expansion Following Cariopulmonary Bypass in Patients Undergoing Open Heart Surgery)

  • Pentastarch is a hydroxyethyl starch similar to hetastarch, but lower average molecular weight and fewer hydroxyethyl groups which result in enhanced enzymatic hydrolysis and faster renal elimination.This report was performed to compare the clinical efficacy and safety of 10 % pentastarch[Pentaspan , group I] for plasma volume expansion after open heart surgery with that of 5% albumin[Plasmanate, group II]. There were no statistically significant differences between the group I [n=18] and group II [n:19] in the preoperative parameters [age, sex, body weight] and operative parameters[bypass time, aorta cross clamping time]. During the first 24 hours after arrival of the patient in the surgical intensive care unit, colloid solution [500--1000 ml] was infused to maintain left atrial pressure of more than 8 mmHg, or cardiac index of 2.0 L/min/M2 of more. In results, there were 3 complications of hypotension immediately after infusion of 5 % albumin solution and 2 among the 3 patients were excluded for the study. However there was no complication after infusion of 10 % pentastarch solution. Hemodynamic responses to infusion was similar for both groups, although in group I a greater increase in both left atrial pressure[mean 1.8 versus 0.7 mmHg, p< 0.05] and right atrial pressure [mean 2.2 versus 1.7 mmHg, p < 0.05] was observed during infusion of the first 500 ml. There were no significant differences in any of the measured respiratory parameters[PaO2, intrapulmonary shunt, and effective lung compliance]. Homodilution with colloid significantly reduced hemoglobin [mean 1.2 versus 0.8 gm/dl], and serum protein and albumin level[total protein;4.8$\pm$ 0.5 versus 5.2 $\pm$0.5 gm/dl, p < 0.05: albumin: 3.2 $\pm$0.4 versus 3.6 $\pm$0.6 gm/dl, p < 0.05] by 6:00 AM on 1 day postoperatively, however there were no significant differences on 7 day postoperatively. The mean serum colloid osmotic pressure and osmolarity was similar in both group.There were no abnormal findings of liver function and kidney function in all the patients. There were no significant between-group differences in bleeding time, platelets, prothrombin time, activated partial thromboplastin time and amount of chest tube output measured on 1st and 7th postoperative day. These findings demonstrated that 10% pentastarch is more effective and safe for plasma volume expension than 5 % albumin solution with no adverse effects on coagulation. Also 10 % pentastarch is less expensive than 5 % albumin and it would appeare to be a reasonable first choice for plasma volume expansion.

실험적으로 유발한 개 신증후군의 진행 및 치유과정에서의 임상병리학적 변화 (Description of Clinicopathologic Changes during the Development and Clinical Resolution of Experimentally induced Canine Nephrotic Syndrome)

  • 이 연구의 목적은 실험적으로 유발한 면역매개성 사구체 질환이 신증후군으로 진행하는 과정과 치유과정에서 단백뇨 배설에 근거한 조기진단 및 치유과정의 감시에 대한 평가를 하는 것이다. 두당 endotoxin 1 $\mu$g과 native bovine serum albumin 5mg 을 9두의 건강한 개에 정맥주사하여 감작시키고, 그 후 1주일부터 양이온화한 bovine serum albumin 120mg을 주 5회 반복 정맥주사한 결과 5두에서 면역매개성 사구체신염과 신증후군이 발생하였다. 사구체신염이 발생한 실험동물에서는 요단백질/ 크레아티닌 비의 증가 (>1.0), 저알부민혈증 (< g/이), BUN 증가 (>40 mg/dl), 부종 등이 나타났다. 이것은 임상병리학적으로 평가 가능한 면역매개성 사구체시염 및 신증후군 모델견이 제작되었음을 제시하는 것이었다. 사구체신염의 유발과 신증후군의 진행과정에 요단백질/크레아티닌 비의 증가가 저알부민혈증, BUN 증가, 고콜레스테롤혈증보다 1-4주 먼저 출현하였다. 사구체신염에 의해 신증후군이 유발된 실험동물 모두에서 prednisolone 을 2.2 mg/kg씩 1일 2회 경구투여한 결과 혈중 알부민 농도의 증가와 콜레스테롤 농도의 감소가 먼저 일어나고, 그 후 1-4 주에 요단백질/크레아티닌 비가 감소하기 시작하였다. 종합적으로 볼때, 면역매개성 사구체신염성 신증후군의 조기진단에는 요 단백질/크레아티닌 비가 유리한 지표로, 그리고 초기 치유과정의 감시에는 혈청 알부민, 콜레스테롤이 더 유리한 임상 지표로 이용될 수 있음을 확인하였다.

Utility of Serum Albumin for Predicting Incident Metabolic Syndrome According to Hyperuricemia

  • Background: Serum albumin and uric acid have been positively linked to metabolic syndrome (MetS). However, the association of MetS incidence with the combination of uric acid and albumin levels has not been investigated. We explored the association of albumin and uric acid with the risk of incident MetS in populations divided according to the levels of these two parameters. Methods: In this retrospective longitudinal study, 11,613 non-MetS participants were enrolled among 24,185 individuals who had undergone at least four annual check-ups between 2006 and 2012. The risk of incident MetS was analyzed according to four groups categorized by the sex-specific medians of serum albumin and uric acid. Results: During 55,407 person-years of follow-up, 2,439 cases of MetS developed. The risk of incident MetS increased as the uric acid category advanced in individuals with lower or higher serum albumin categories with hazard ratios (HRs) of 1.386 (95% confidence interval [CI], 1.236 to 1.554) or 1.314 (95% CI, 1.167 to 1.480). However, the incidence of MetS increased with higher albumin levels only in participants in the lower uric acid category with a HR of 1.143 (95% CI, 1.010 to 1.294). Conclusion: Higher levels of albumin were associated with an increased risk of incident MetS only in individuals with lower uric acid whereas higher levels of uric acid were positively linked to risk of incident MetS regardless of albumin level.

Cold Ethanol Fractionation and Heat Inactivation of Hepatitis A Virus During Manufacture of Albumin from Human Plasma

  • The purpose of the present study was to examine the efficacy and mechanism of fraction IV cold ethanol fractionation and pasteurization (60$^{\circ}C$ heat treatment for 10 h), involved in the manufacture of albumin from human plasma, in the removal and/or inactivation of the hepatitis A virus (HAV). Samples from the relevant stages of the production process were spiked with HAV and the amount of virus in each fraction then quantified using a 50% tissue culture infectious dose (TCID$\_$50/). HAV was effectively partitioned from albumin during the fraction IV cold ethanol fractionation with a log reduction factor of 3.43. Pasteurization was also found to be a robust and effective step in inactivating HAV, where the titers were reduced from an initial titer of 7.60 log TCID$\_$50/ to undetectable levels within 5 h of treatment. The log reduction factor achieved during pasteurization was $\geq$4.76. Therefore, the current results indicate that the production process for albumin has sufficient HAV reducing capacity to achieve a high margin of virus safety.

  • Colloid solution is commonly used to increase the oncotic pressures of priming solutions used in the cardiopulmonary bypass circuit. To study the effectiveness of this practice, we retrospectively evaluated 24 patients undergoing cardiac operations to receive isoelectric solution plus 50 gm of albumin[group A], isoelectric solution plus 25 gm of albumin[group B] and isoelectric solution without albumin[group C] as the prime solution for the bypass circuit. Various clinical parameters related to the perioperative fluid balance, cardiopulmonary function, and renal function were studied. There were no differences between the three groups with regard to postoperative clinical parameter of cardiopulmonary, liver function and renal function. But at the previous report by Cho et al. [38], there was a significant difference between groups with regard to hematologic property, especially in crenated red blood cells. These hematologic events did not affect the clinical outcome but thorough evaluations are needed in order to prove the clinical effect of crenated red blood cell. We conclude that there is no clinically detectable advantage for the routinely supplementing of albumin to the priming solution of bypass circuit in cardiac operations. Whether this practice can be of value in selected cases needs to be further studied.

  • 한외여과분리를 이용해 albumin과 lysozyme의 혼합용액 중 두 단백질을 분리하는데 대한 전기장의 효과를 조사하였다. albumin 용액을 pH7에서 분리한 결과, 전기장의 의해 투과유속은 증가하나 albumin의 여과계수비는 감소하였다. lysozyme 용액을 pH7에서 분리한 결과, 전기장의 의해 투과유속이 소폭 감소하며 여과계수비는 증가함을 볼 수 있었다. 한외여과에서의 lysozyme의 albumin에 대한 선택도는 전기장에 의해 대폭 향상되었다. 전기한외여과법을 사용하여 lysozyme과 albumin을 효율적으로 분리하기 위한 기초를 확립하였으며 기존 한외여과법의 단점을 크게 개선하였다.

Protein Electrophoresis Fraction in Serum of the Rural Elderly Patient

  • We analyzed total protein (TP), albumin (AL), aspartic aminotransferase (AST), alanine aminotransferase (ALT), urea nitrogen (UN), creatinine (CRE) and serum protein electrophoretic fractions in sera of rural elderly patients to evaluate the health status in rural elderly patients. We observed that the frequencies of patients showed lower level of total protein, albumin, and both total protein and albumin than them of reference range were 20.3%, 22.8% and 19.0%, respectively. The rates of patients showed higher level of AST, ALT, both of AST and ALT, UN, creatinine and both of UN and creatinine than them of reference range were 33.8%, 40.0%, 30.0%, 17.7%, 15.2% and 7.9%, respectively. Comparison of protein fractions of each patient to reference range showed that 77.2% of patients showed normal in all of patterns. Few of patients showed abnormal pattern in albumin (13.6%), ${\alpha}1-globulin (0.0%),\;{\alpha}_2-g10bulin(1.1%),\;{\beta}-globulin(1.9%)\;and\;{\gamma}-globulin(6.2%)$. These data suggest that many of rural elderly patients may suffer from heart, liver and kidney diseases. The serum protein fractions are not typical criterion to evaluate the disease, but production of proteins in rural elderly patients may be affected by liver disease and kidney disease.

Inhibitory Effects of Bovine Serum Albumin on Cytotoxicity and Mutagenicity of 6-Sulfooxymethylbenzo[a]pyrene

  • A 6-sulfooxymethylbenzo[a]pyrene (SMBP), the ultimate metabolite of methyl-substituted benzo[a]pyrene (BP), has been found to be carcinogenic in mice. These properties may be attributable to its strong reactivity with cellular macromolecules such as DNA. However, serum and its major constituent albumin attenuated significantly the cytotoxicity and mutagenicity of 5MBP in bacterial and mammalian cell systems. This inhibitory activity of serum against 5MBP-induced cytotoxicity and mutagenicity in Chinese hamster V79 cells appears to be caused by the reduced macromolecular adducts such as DNA and proteins, but serum failed to reduce 5MBP binding to naked calf thymus DNA. A number of proteins in the serum could act as nucleophiles that are able to intercept reactive chemicals through covalent binding. Albumin present in the plasma seems to be one of major components responsible for direct binding with 5MBp, thereby reducing its reactivity to genetic materials. We here determined which fraction is preferential for 5MBP binding through fractionation of 5MBP-treated serum with ammonium sulfate. The albumin-containing fraction had slightly more affinity for 5MBP than the immunoglobulin-containing fraction. Our results indicate that the covalent modification of plasma proteins may reduce 5MBP-induced damage.

혈액투석중인 만성신부전환자의 영양상태에 관한 연구 (Nutritional Assessment of the Hemodialysis Patients)

  • This study investigated the nutritional status of 33 hemodialysis patients. Their weight, height, triceps and mid-arm circumference were measured and their dietary intake and blood profiles evaluated. The subjects were 57.1$\pm$11.9 years old. The energy intakes of men and women were 61% and 68% of RDA, respectively. The men and women's intake of protein was 0.93g/IBWkg/d and 0.99g/IBWkg/d, respectively. According to the distribution of BMI, 22.2% of the men and 73.3% of the women were underweight. A total of 5.6% of the men were overweight, versus none of the women. The serum albumin levels of the men and women were 3.56 and 3.52g/dl, respectively. The serum cholesterol levels of the men and women were 134.1 and 148.5mg/dl, respectively. The subjects were divided into three groups according to the level of albumin, and their intakes of nutrients were compared with one another. The group with high levels of albumin did not show higher energy and protein intake than the other groups but the serum total protein level was significantly higher. When the nutritional status of the patients was evaluated by weight and serum albumin level, 6.0% of them showed kwashiorkor-type malnutrition and 75.8% of them showed mild malnutrition.

  • To date, various strategies have been studied to increase specific productivity in Chinese hamster ovary (CHO) cell cultures. Also, albumin-fusion platform is being applied to other important bioactive peptides with short half-lives. Here, we investigated the effects of silkworm gland hydrolysate (SGH) on the production of albumin-erythropoietin (Alb-EPO) in transgenic CHO cells. The viable cell density of CHO cells was increased by 13% in the medium containing 1 mg/mL SGH higher than in the control medium without SGH. In addition, the production of Alb-EPO was also 1.26- fold enhanced by reducing the early apoptosis of CHO cells. In conclusion, SGH could be used as a useful supplement for the enhancement of recombinant protein production.
