Serum alanine aminotransferase levels are closely associated with metabolic disturbances in apparently healthy young adolescents independent of obesity

  • Purpose: Liver metabolism plays a pivotal role in the development of metabolic disorders. We aimed to investigate the clinical and laboratory risk factors associated with alanine aminotransferase (ALT) levels in young adolescents from an urban population in Korea. Methods: A population of 120 apparently healthy adolescents aged 12-13 years was included in the cross-sectional design study; 58 were overweight or obese and 62 were of normal weight. We estimated anthropometric and laboratory measurements, including waist-to-height ratio, blood pressure, insulin sensitivity, aspartate aminotransferases (AST), ALT, and lipid profiles. Results: The mean ages of the overweight or obese and normal weight participants were $12.9{\pm}0.3$ and $13.0{\pm}0.3years$, respectively. Height, weight, body mass index, waist circumference, waist-to-height ratio, systolic and diastolic blood pressure, AST, ALT, total cholesterol, low-density lipoprotein-cholesterol, triglyceride, insulin, and the homeostatic model assessment of insulin resistance (HOMA-IR) score were significantly higher and the high-density lipoprotein-cholesterol and quantitative insulin-sensitivity check index were significantly lower in the overweight/obese participants in comparison to the normal-weight participants (all P<0.05). In multivariate linear regression analysis, waist-to-height ratio, systolic blood pressure, and HOMA-IR score were independently and positively associated with serum ALT levels. Conclusion: Screening for ALT levels in adolescents may help to differentiate those at risk of metabolic abnormalities and thus prevent disease progression at an early age.

한국 성인남성에서 Lipid Accumulation Product 지수와 Alanine Aminotransferase와의 관련성 (Relationship between the Lipid Accumulation Product Index and Alanine Aminotransferase in Korean Adult Men)

  • LAP 지수는 공복시 중성지방 농도와 허리둘레의 조합으로 과도한 지방축적을 평가하는 지표이다. 이 연구는 LAP 지수와 ALT간의 관련성과 LAP 지수가 ALT에 미치는 영향에 대해 알아 보고자 간질환의 병력이 없는 건강한 성인남성을 대상으로 횡단면 연구를 실시하였다. 2015년 1월부터 2017년 6월까지 경기지역 종합병원에서 건강검진을 시행한 20세 이상 70세 이하의 성인 남성 13,854명을 연구 대상자로 하였다. LAP 지수는 남성의 경우 [허리둘레(cm)-65]${\times}$[중성지방(mmol/L)]으로 산출하였으며, 혈청 ALT 농도의 이상치는 남성 40 IU/L 이상인 경우를 기준으로 설정하였다. 모든 대상자들은 인체측정학적 지표와 생리학적 검사를 실시하였다. LAP 분위수가 증가할수록 ALT는 통계적으로 유의한 증가를 보였다(p<0.001). LAP 지수는 ALT와 상관관계를 보였으며(r=0.238, p<0.001), 특히 인슐린(r=0.449, p<0.001) 및 HDL-콜레스테롤(r=-0.369, p<0.001)과 상관성이 높게 나타났다. 또한 LAP 지수는 ALT에 영향을 미치는 요인이었으며(p<0.001), LAP 지수가 높을수록 ALT가 상승할 위험이 높게 나타났다(p<0.001). 결론적으로 LAP 지수는 한국 성인남성에서 ALT 상승을 예측하는 지표로 나타났다.

경기도 일개 종합병원에서 건강검진을 받은 한국 성인의 혈청 De Ritis 비율과 당뇨 검사와의 관계 (Relationship between the Serum De Ritis Ratio and Diabetes Tests in Korean Adults Who Underwent Health Screening at a General Hospital in Gyeonggi-do)

  • 본 연구의 목적은 당뇨 검사결과와 간기능검사와의 관계연구이다. 간성당뇨는 제2형 당뇨와는 다르게 간기능 이상으로 기인하는 것이다. 본 연구에서 간기능 검사의 주요 효소검사인 아스파르트산 아미노전이효소(aspartate aminotransferase, AST), 알라닌 아미노전이효소(alanine aminotransferase, ALT) 그리고 AST/ALT ratio (De Ritis ratio)와 당뇨관련 검사와의 관계를 주로 확인하였다. 연구 결과 AST와 글루코스(glucose) (r=0.14, P<0.01); ALT 및 글루코스(r=0.21, P<0.01); AST 및 당화혈색소(HbAlc) (r=0.15, P<0.01); ALT와 HbAlc (r=0.20, P<0.01), 모든 변수는 양의 상관관계를 나타났으며, De Ritis ratio는 글루코스 (r=-0.20, P<0.01)와 당화혈색소 (r=-0.14, P<0.01)와 음의 상관관계를 보였다. AST와 ALT 그리고 De Ritis ratio 를 독립변수로 하고 글루코스 (R2=0.05) 와 HbA1c (R2=0.04) 를 종속변수로 하여 회귀분석한 결과 독립변수는 종속변수에 통계적으로 유의하게 영향을 나타내는 것으로 나타났다. AST는 ALT 보다 혈당과 당화혈색소에서 상관관계가 낮게 나타났으며, ALT가 증가하는 것은 즉, De Ritis ratio 감소의 원인이 된다. 따라서, De Ritis ratio는 당뇨관련 검사와의 관계에서 의미가 있는 것으로 볼 수 있다.

LED와 CMOS 이미지 센서 기반 간 기능 분석용 소형 광학장치 (A Compact Optical System using LED and CMOS Image Sensor for Liver Function Analysis)

  • This paper presents a portable and compact optical device which can conveniently be used to perform a functional analysis of human liver function. The proposed system employed red/green LEDs, as a light source, and CMOS image sensor, which is commonly used in cellular phones. With this system, several blood serum samples have been evaluated for liver functional analysis by measuring light absorption level through the blood serum samples depending on aspartate aminotransferase (AST), alanine aminotransferase (ALT), and total bilirubin concentration. The light absorption through the blood serum samples containing AST, ALT, or total bilirubin can provide their concentrations. The green light absorption is more sensitive to the concentration of AST or ALT, and the red light absorption is more sensitive to the total bilirubuin concentration. Additional calibration steps were performed by using a MATLAB program in order to eliminate the light scattering effects from the extraneous particles existing in each blood serum sample. From the blind test, three standard light intensity curves through each enzyme have been obtained and the enzyme concentration values have been compared to those obtained from a commercially available biochemistry analyzer (Toshiba 200 FR). The average percent difference in the obtained concentrations from two systems for AST, ALT, and total bilirubin concentration came out to be 7.79%, 7.98%. and 7.56%, respectively, with the adjusted coefficient of determination (R2) higher than 0.98. This system can possibly lead to a low-cost and simple system that can be used as a point-of-care (POC) system in a condition without advanced equipments.

Analysis of copy number variation in 8,842 Korean individuals reveals 39 genes associated with hepatic biomarkers AST and ALT

  • Biochemical tests such as aspartate aminotransferase (AST) and alanine aminotransferase (ALT) are useful for diagnosing patients with liver disease. In this study, we tested the association between copy number variation and the hepatic biomarkers AST and ALT based on 8,842 samples from population-based cohorts in Korea. We used Affymetrix Genome-Wide Human 5.0 arrays and identified 10,534 CNVs using HelixTree software. Of the CNVs tested using univariate linear regression, 100 CNVs were significant for AST and 16 were significant for ALT (P < 0.05). We identified 39 genes located within the CNV regions. DKK1 and HS3ST3B1 were shown to play roles in heparan sulfate biosynthesis and the Wnt signaling pathway, respectively. NAF1 and NPY1R were associated with glycoprotein processes and neuropeptide Y receptor activity based on GO categories. PTER, SOX14 and TM7SF4 were expressed in liver. DPYS and CTSC were found to be associated with dihydropyrimidinuria and Papillon-Lefevre syndrome phenotypes using OMIM. NPY5R was found to be associated with dyslipidemia using the Genetic Association Database.

비파식초의 사염화탄소에 의한 간독성 보호 효과 (Protective Effect of Eriobotrya japonica Lindl. on Hepatotoxicity by Carbon Tetrachloride)

  • 비파 식초가 사염화탄소에 의한 간독성에 대한 보호효과를 흰 쥐를 이용하여 확인하고자 연구하였다. 4주간 SD rat 암컷을 정상군, 음성대조군, 양성대조군, 비파 식초군으로 나누어 샘플을 처리하였고, 29일째 되는 날 사염화탄소를 처리하였다. 혈청 AST 및 ALT 효소 활성 수치 변화에서 비파 식초가 사염화탄소를 단독으로 투여한 군보다 유의한 수치로 각각 49.77%, 75.88%로 감소하였다(p < 0.001). 간 조직에서는 CAT 및 SOD 효소 활성은 비파 식초군이 사염화탄소를 단독으로 투여한 음성대조군보다 유의한 차이로 높은 활성 수치를 보였다. 조직학적 관찰에서는 비파식초군이 사염화탄소를 단독으로 투여한 음성대조군에 비해 지방변성이 적게 분포하고, 조직의 섬유화가 진행되지 않았으며, 정상군과 비슷한 간세포의 구조가 관찰 되었다. 이와 같은 결과를 보아 비파 식초는 항산화 효소의 증가로 활성산소를 제거하여 간 손상을 억제할 뿐아니라 혈청의 AST 및 ALT 효소 활성을 감소시켜 간 보호 효과가 있는 것으로 건강기능식품으로써 긍정적인 가능성이 있다고 사료된다.

부자 및 천오를 포함한 한약이 간 손상 수치에 미치는 영향 (The Consequences of Long-period Dosages of Herbal Medicine Including Boo-ja and Cheon-o on Liver Injury)

  • Objectives : Boo-ja and Cheon-o are not commonly prescribed, but are necessary for some clinical conditions, dispite the fact that some negative effects have been known to occur with these medicines. Hence, consequences for aspartate aminotransferase(AST), alanine aminotransferase(ALT) due to herbal medicines, including boo-ja and cheon-o are here reported. Methods : From January 2005 to July 2005, results were analyzed for 36 inpatients belonging to the sixth internal medicine department of Dongeui Oriental Medical Hospital who took herbal medicine, including boo-ja and cheon-o, over 20 days. The study is about the comparison and the investigation of LFT, and all results were taken upon their hospitalization and upon their departure. The standard of liver injury was used as a standard for their examinations. Results were as follows : No Change in normal limit : 28 of the 36 Increased AST. ALT from when they were hospitalized : 2 of 36 Normalized AST. ALT from when they were hospitalized with results higher than normal : 6 of 36 These results show no side-effects, therefore suggest that these herbal medicines, including boo-ja and cheon-o, do not cause adverse side-effects for patients of this kind if administered as was done in this study.

A Fruit Extract of Paeonia anomala Attenuates Chronic Alcohol-induced Liver Damage in Rats

  • Prolonged alcohol consumption causes alcoholic liver damage due to the generation of reactive oxygen species, the accumulation of fatty acids, and an increase in inflammatory cytokines in the liver. In this study, the protective effect of a fruit extract of Paeonia anomala (FEPA) against chronic alcohol-induced liver damage was evaluated in Sprague-Dawley rats fed an ethanol or a control Lieber-DeCarli diet for 5 weeks to induce alcoholic liver damage. FEPA (50, 25, and 10 mg/kg body weight/day) as well as the reference control silymarin (25 mg/kg body weight/day) were administered along with the ethanol diet. FEPA protected against increases in alanine aminotransferase and aspartate aminotransferase in serum and attenuated alcohol-induced increases in triglycerides, tumor necrosis factor alpha, thiobarbituric acid-reactive substances, and cytochrome P450 2E1 enzyme activity in the liver compared with the group treated with ethanol only. Anti-oxidative defenses such as the total glutathione level and glutathione peroxidase activity were increased by FEPA treatment. These results suggest that FEPA exerts protective effects against chronic alcohol-induced liver damage by attenuating hepatosteatosis and pro-inflammatory cytokine production and enhancing anti-oxidative defense mechanisms in the liver.

Clinical characteristics of obese boys and girls in a high school: focused on abdominal fat indices, fatty liver and carotid intima-media thickness

  • Purpose: Our study aimed to evaluated sex differences in clinical features of obese high school students. Methods: One hundred three obese high school students (body mass index [BMI]${\geq}$85th percentile) and 51 control students (BMI<85th percentile) were enrolled in this study. Anthropometric measurements were performed. Fasting serum glucose, insulin, aspartate aminotransferase, alanine aminotransferase, total cholesterol, triglyceride, low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), and high-sensitive C-reactive protein were measured. Abdominal fat thickness, degree of fatty liver, and carotid intima-media thickness were measured by ultrasound. Results: In control and obese groups, waist circumference was significantly longer in boys but body fat mass was Significantly higher in girls. In the control group, total cholesterol and LDL-C were higher in girls. In the obese group, however, aspartate aminotransferase, alanine aminotransferase and triglyceride were higher and HDL-C was lower in boys. Preperitoneal fat thickness was significantly higher in obese girls. In obese group, the degree of fatty liver was significantly higher in boys. Carotid intima-media thickness was not significantly different between boys and girls. Conclusion: Obese adolescents had distinguishable sex differences in body measurements, metabolic abnormalities, abdominal fat thickness and fatty liver. We can infer that these characteristics may extend into adult obesity.

애엽 에탄올 추출물이 사염화탄소로 유발된 흰쥐의 간 손상 보호효과 (Protective Effect of Ethanol Extract of Artemisiae vulgaris L. on hepatic injury Induced by Carbon tetrachloride In Rat.)

  • Dawley 계 숫컷 흰쥐(200~210g)를 정상군, CCl4-대조군, CCl4-실험군으로 나누어 실험군은 애엽 에탄올 추출물을 1,000mg/kg,b.w의 용량으로 1일1회7일간 경구 투여 후 사염화탄소를 0.6mg/kg,b.w의 용량으로 복강내 주사 후 다음날 개복하여 혈청내의 Alanine aminotransferase(ALT), Aspartate aminotransferase(AST),Alkaline phosphatase(ALP), Glutamyltranspeptidase(γ-GT), Lactate dehydrogenase(LDH)의 활성도와 중성지방, 콜레스테롤 함량을 측정한 결과 애엽 추출물 투여군에서 CCl4-대조군과 비교하여 유의적인 감소를 나타내었으며, HDL-콜레스테롤은 유의적인 증가를 나타내었다. 간조직중의 지질과산화 함량은 감소를 glutathione함량은 유의적인 증가를 나타내었다 이 실험 결과 애엽 에탄올 추출물은 사염화탄소 투여에 의한 간 손상을 억제하는 보호물질을 함유하고 있음을 알 수 있었다.
