노인성 음성의 임상양상 (Clinical Manifestation of Aging Voice)

  • The presbyphonia is a combination of physiological and structural changes due to aging of the larynx in elderly patients with voice problems. Some of these changes are inevitable, while others may be avoidable or reversible. The fatigue of phonation is the most common clinical symptom of the aging voice. The voice problems with aging are produced from variable causes including the organic lesions of the larynx. It is essential that the curers understand physiologic and pathologic changes of aging voice for minimizing glottal incompetence and improving vocal performance and quality of life of the elderly.

한국어판 노인음성지수의 신뢰도와 타당도 (Validity and Reliability of Korean Version of the Aging Voice Index (KAVI))

  • Background and Objectives : Voice disorder is recognized as a major problem because it negatively affects the elderly's social participation and quality of life. The purpose of this study was to examine the validity and reliability of Korean aging voice index (KAVI), which assesses the quality of life related to the voice of the elderly. Materials and Method : This study was conducted on 211 elderly people aged 65 years or older : 111 patients with voice disorder (mean age 69.8, range 65-80 years) and 100 nomorphonic participants (mean age 70.6, range 65-82 years). Aging voice index was translated into Korean and used and Korean voice-related quality of life (KVQOL) was conducted to verify KAVI. The validity (item validity, concurrent validity, and construct validity) and reliability (test-retest reliability and internal consistency reliability) of KAVI. Results : The item validity (ICC=0.895) and construct validity (r=0.765) showed a high correlation, respectively. And concurrent validity (r=0.748), test-retest reliability (0.851), and internal consistency reliability (${\alpha}=0.832$) were statistically significant in voice disorder group. In addition, there was a significant difference between the voice disorder and the nomorphonic group in AVI total score. Conclusion : KAVI is a validated and reliable quality of life tool that will be useful for assessing the presence and effectiveness of interventions in clinical settings.

파킨슨병 환자와 정상 노인의 음성비교 (A Comparison of the Voice Differences of Patients with Idiopathic Parkinson's Disease and a Normal-Aging Group)

  • In view of the hypothesis that the effects of Parkinson disease on voice production can be detected before pharmacological intervention, the voice differences of patients with Idiopathic Parkinson's disease and a healthy aging group were diagnostically analyzed with the long term object of establishing, for clinical purposes, early disease-progression biomarkers. Fifteen patients with Idopathic Parkinson's disease (prior to pharmacological intervention) and a healthy control group of 15 were selected and every voice was recorded three times using praat (ver. 5022) with a headset mic. Relevant parameters - acoustic measure of /a/ phonation, F0 related parameters, MPT related parameters, articulatory ratio, VOT - were then analyzed by MANOVA. Significant differences were found in the F0 related (low F0, high F0, F0 range) and MPT related parameters. There were also significant differences in acoustic measurements (intensity, shimmer, HNR, jitter), AMR (/$t{\Lambda}$/,/$k{\Lambda}$/) and VOT (/ta/), The findings indicated that the voice production of patients with Idiopathic Parkinson's disease have normal pitch but bad quality. In particular, with slow articulatory ratios and VOT values, the tongue tip functioning of patients was lower than for the healthy group.

노인성 음성의 치료 (Treatment of Presbyphonia (Aging Voice))

  • Presbyphonia is defined as voice change caused by aging. Since presbyphonia is one of the natural aging processes, the treatment should be considered for the patients complaining communication difficulties. The treatment should not only target on presbylaryngis, but also on underlying systemic conditions such as lung function, neurological diseases and medications. Therefore, the treatment for the patients with presbyphonia should be multidisciplinary including underlying disease control, voice therapy and surgical treatment. Although various experiments on treatment of presbylaryngis are currently being tried, repeated injection laryngoplasty is still playing an important role because presbyphonia is destined to get worse over time.

노화에 따른 음질과 구어 유창성의 음향학적 특성 변화 (Change in acoustic characteristics of voice quality and speech fluency with aging)

  • 나이가 들면서 발생하는 음성 문제는 사회적, 정서적으로 영향을 미칠 수 있으며, 나아가 고립감과 우울증으로 이어질 수 있다. 이에 본 연구에서는 노화로 인한 음향학적 특성 변화를 음질과 구어 유창성의 변화를 알아보고자 한다. 이를 위해 노년층 남성 20명과 청년층 남성 20명이 산출한 연장발성과 구절 읽기 과제를 녹음하여 분석하였다. 음질 분석 변수로 기본주파수(F0), 주기 변동률(jitter), 진폭 변동률(shimmer), 켑스트럼 정점(cepstral peak prominence, CPP) 값을 분석하였으며 구어 유창성 분석 변수로는 평균 음절 길이(average syllable duration, ASD), 조음 속도(articulation rate, AR), 구어 속도(SR)를 분석하였다. 연구결과, 음질 측정에서 노년층의 경우 F0가 높게 나타났으며 jitter, shimmer, CPP의 결과값을 통해 음질이 저하된 것으로 나타났다. 구어 유창성 분석 결과, 노년층은 ASD, AR, SR의 결과값을 통해 느리게 발화하는 것으로 나타났다. 음질과 구어유창성 간 상관관계 분석 결과, shimmer와 CPP 값과 각각 ASD와 SR에서 높은 상관관계가 나타났다. 본 연구결과를 통해 노화에 따른 음성과 구어 유창성 변화를 조기에 발견하고 이에 대한 적절한 훈련법을 제공할 수 있을 것으로 기대된다.

3온스 물 삼킴검사 이후 정상 노년층의 음질 변화: 음향학적 분석 (Voice quality of normal elderly people after a 3oz water-swallow test: An acoustic analysis)

  • The elderly are at increased risk of developing dysphagia due to aging and illnesses. The aim of the current study was to analyze, via an acoustic study, the change in the voice quality of normal elderly people after a 3oz water-swallow test. Subjects included a group of 60 normal elderly people (age: $mean{\pm}SD=76.9{\pm}6.66$) and 60 healthy young adults (age: $mean{\pm}SD=25.1{\pm}2.36$). Every participant produced a five-second /a/ phonation pre- and post-swallowing, and the fractioned two-second sections were analyzed using the MDVP (multi dimensional voice program) analysis. The elderly group demonstrated a post-swallowing increase in the following related acoustic parameters: fundamental frequency, fundamental frequency variation, amplitude-variation, and noise in both two-second sections. However, the younger group showed an increase only in frequency related acoustic parameters (i.e., STD ) in the first two-second section. The significant changes in values in the post-swallowing parameters might indicate temporary irregularities in pitch and amplitude along with higher amounts of noise in the voice. The results could be attributed to water residues in the vocal fold and vocal tract, as well as a deterioration of the motor and sensory functions caused by anatomical and physiological changes that result from aging.

노인성 음성에 대한 최신 연구동향 (Current Researches on Vocal Fold Aging)

  • Aging causes a variety of changes in the structure of the vocal fold (VF), resulting in aging-induced dysphonia (presbyphonia). Several studies have investigated the structure of the VF of elderly people from autopsy as well as animal studies. There is an increasing evidence on correlation of structural changes of VF with deteriorated voice in elderly. Although the cellular mechanisms of aging VF have only partially been elucidated, there are many recent advances in biological treatment on aging VF using bioactive molecules such as growth factors. In this study, I'd like to address aging-related structural, biological, and physiological changes in previous literature about human and rodent aging VFs, to provide further insight into the mechanisms responsible for presbyphonia and to translate the basic researches for future clinical trials.

노화 쥐 성대의 조직학적 분석 (Histologic Analysis of Vocal Folds in Aging Rats)

  • Background and Objectives Presbyphonia is characterized by hoarse, breathy, weak vocal intensity. Extracellular matrix (ECM) in lamina propria (LP) of the vocal folds play an important role in voice production, and change of ECM according to the aging leads to the presbyphonia. The aim of this study was to investigate the histologic analysis of aging vocal fold of rat. Materials and Method Six and twenty two months old Sprague-Dawley rats (n=8, each group) were used and classified into young (six months old rats) and old (twenty two months old rats) group. Histologic analysis and immunohistochemical staining for ECM of LP were performed. Results Overall cellular density was significantly decreased in old rat group. Elastin fibers of LP were significantly decreased in old rat group. Type I collagen was significantly increased in old rat group. Type III collagen did not show significant difference. Hyaluronic acids did not show significant difference in Alcian blue staining and immunohistochemical staining. Conclusion Decreased general cellular density and elastin fiber and increased type I collagen were observed in the LP of vocal folds of aging rats. These ECM changes might to contribute the aging voice.

  • 4차 산업혁명 시대에 살고 있는 현대인들은 혼자 사는 1인 가구의 증가와 고령화 사회의 진입으로 인한 실버세대가 증가하는 추세이다. 외로움, 소외감, 우울증을 겪는 1인 가구 및 실버세대의 문제점을 해소시켜 주고 더 나아가 실버세대의 스마트폰 활성화를 위해 본 논문에서는 인공지능 기반 음성인식 기능을 탑재한 'Voice' 어플리케이션을 제안하고자 한다.
