Development of an evidence-based clinical imaging diagnostic guideline for implant planning: Joint recommendations of the Korean Academy of Oral and Maxillofacial Radiology and National Evidence-based Healthcare Collaborating Agency

  • Purpose: This study was conducted to develop an evidence-based clinical imaging diagnostic guideline for implant planning, taking into account efficacy, benefits, and risks. Materials and Methods: The guideline development process employed the adaptation methodology used for Korean clinical imaging guidelines(K-CIG). Core databases(Ovid-Medline, Ovid-Embase, National Guideline Clearinghouse, Guideline International Network) and domestic databases (KoreaMed, KMbase, and KoMGI) were searched for guidelines. The retrieved articles were analyzed by 2 reviewers, and articles were selected using well-established inclusion criteria. Results: The search identified 294 articles, of which 3 were selected as relevant guidelines. Based on those 3 guidelines, 3 recommendations for implant planning were derived. Conclusion: We recommend radiography or cone-beam computed tomography (CBCT) scanning for individual patients judged to require a cross-sectional image after reading of a panoramic X-ray image and a conventional intraoral radiological image. Various steps should be taken to raise awareness of these recommendations among clinicians and the public, and K-CIG should be regularly reviewed and revised.

중소규모 화학공장의 압력방출시스템에 대한 안전성 검토 (Review of Safety for Pressure-Relieving Systems of Small to Middle Scale Chemical Plants)

  • A variety of safety issues were investigated for chemical reactors using a toluene solvent in case of a fire at small to middle scale chemical plants. The issues covered the operation of pressure-relieving valves and the subsequent discharges of the toluene to the atmosphere either directly or through an absorber, which represent the current practice at most small chemical plants. It was shown that the safety valve on the reactor may not operate within about twenty minutes after an external fire breaks out, but, once relieved, the toluene vapor released directly to the atmosphere may form a large explosion range on the ground. It was also shown that if the discharge is routed to an existing absorber used for the scrubbing of volatile organic compounds or dusts, the column may not operate normally due to excessive pressure drops or flooding, resulting in the hazardous release of toluene vapors. This study proposed two ways of alleviating these risks. The first is to ruduce the discharge itself from the safety valve by using adequate insulation and protection covers on the reactor and then introduce it into the circulation water at the bottom of the absorber through a dip linet pipe equipped with a ring-shaped sparger. This will enhance the condensation of toluene vapors with the reduced effluent vapors treated in the packing layers above. The second is to install a separate quench drum to condense the routed toluene vapors more effectively than the existing absorber.

식품안전성에 대한 기본인식 조사 - 식품위생 관련 공무원을 중심으로 - (A Study on Attitudes Toward Food Safety Issues in Korea - Focus on the Public Official Related to Food Hygiene -)

  • A survey was conducted to investigate the attitudes of public officials related to food hygiene toward food safety. The official groups were divided into four sub-groups based on their area, agency, gender and age. Response of each group to the survey was statistically analysed. The survey's results showed that most of them (87.0%) were satisfied with a domestic level of food hygiene. But, some of then (29.8%) considered unsatisfactoryly on safety of the food they eat. Their response to food safety did not statistically significant between each group of area, agency and age but, in case of gender, female group showed stastically more negative attitude to food safety than male one (P<0.05). All groups chose residues of chemical substances such as pesticides and food additives as the most potential food risk factor followed by foodborne pathogens, heavy metals and animal drug residues. The results are not consistent with the scientific judgement. Therefore, more education and information were needed fro these groups. They pointed out food manufacturer as a responsible group for poor food hygiene (48.7%). In addition, food manufacture and processing were selected as main business types with the lowest level in the food hygiene but official, working in the area of the central government and Seoul metropolitan city, pointed out food services establishments as the poorest hygiene one (P<0.01). This results suggested that education and information to let mind of responded groups change, working in this part, and governmental financial support are needed to improve hygiene level of food manufacture and processing (70.3%). They also chose HACCP as the most effective way for improving the level of food hygiene followed by Recall, PL (Product Liability), monitoring, labeling and increasing of number of company with good manufacture and processing (GMP).

The Effects of Injury and Accidents on Self-rated Depression in Male Municipal Firefighters

  • Objectives: The present study aims to determine the causal relationship between self-rated depression and experiences of injury and accidents in municipal firefighters. Methods: A panel survey of 186 municipal firefighters measured with depressive symptoms according to the Beck's depression index (BDI) was conducted. The effects of job-related injuries and accidents were evaluated using self-administered questionnaires that were taken once in a 12-month period from 2005 to 2006. Firefighters were classified into the Depression Group or Control Group based on follow-up BDI results with a cutoff level that was set to having "over mild depression." Results: The depression Group was comprised of 17 (9.1%) workers, including 9 firefighters who met had sufficient BDI scores twice in the 2-year test period and newly sufficient BDI scores in the follow-up test. A significantly higher number of subjects in the Depression Group experienced injuries and accidents in the 2-year test period as compared to the Control Group (15.4% vs. 1.5%, p=0.04). Firefighters who experienced injuries and accidents in the 2-year test period had a 7.4 times higher risk of being in the Depression Group than those who had not. As compared to accidents, near-miss accidents revealed stronger risks related to being classified as in the Depression group (adjusted odds ratio [OR] = 4.58, 95% confidence interval [CI] = 1.15-18.18 vs. Adjusted OR = 4.22, 95% CI = 1.08-16.58). Conclusion: The above results suggest that we should establish an effective program to promote mental health for groups at high risk for self-rated depression, including persons who have experienced consecutive injuries and accidents as well as near-miss injuries.

Quantitative and Semiquantitative Health Risk Assessment of Occupational Exposure to Styrene in a Petrochemical Industry

  • Background: Styrene is one of the aromatic compounds used in acetonitrile-butadiene-styrene (ABS) producing petrochemicals, which has an impact on health of workers. Therefore, this study aimed to investigate the health risks of styrene emitted from the petrochemical industry in Iran. Methods: Air samples were collected based on NIOSH 1501 method. The samples were analyzed by the Varian-cp3800 gas chromatograph. Finally, risk levels of styrene's health effects on employees were assessed by the quantitative method of the U.S. Environmental Protection Agency (U.S. EPA) and the semiquantitative way by the Singapore Occupational Safety and Health Association. Results: Based on the results, the employees had the highest average exposure to styrene vapors (4.06 × 10-1mg.(kg - day)-1) in the polybutadiene latex (PBL) unit. Therefore, the most top predictors of cancer and non-cancer risk were 2.3×10-4 and 7.26 × 10-1, respectively. Given that the lowest average exposure (1.5 × 10-2mg.(kg - day)-1) was in the dryer unit, the prediction showed a moderate risk of cancer (0.8 × 10-6) and non-cancer (2.3 × 10-3) for the employees. The EPA method also predicted that there would be a definite cancer risk in 16% and a probable risk in 76% of exposures. However, according to the semiquantitative approach, the rate of risk was at the "low" level for all staff. The results showed that there was a significant difference (p < 0.05) between the units in exposure and health risk of styrene (p < 0.05). Conclusion: Given the high risk of styrene's health effects, appropriate control measures are required to reduce the exposure level.

Panel Session toward Improved Communication and Engagement with the Public after the Fukushima Daiichi Nuclear Power Plant Accident: Study Reports and Discussion with Specialists from Relevant Fields

  • Background: From 2018 to 2020, the Expert Study on Public Understanding after the Fukushima Daiichi Nuclear Power Plant Accident (the Expert Study Group) identified and analyzed activities designed to promote public understanding of science and radiation since the Fukushima accident, and held discussions on how to achieve public understanding in the situation where public confidence has been lost, and how experts should prepare for dealing with the public. This panel session was held at the 53rd meeting of the Japan Health Physics Society on June 30, 2020. Materials and Methods: First, three subgroup (SG) leaders reported their research methods and results. Then, two designated speakers, who participated as observers of the Expert Study Group, commented on the activities. Next, the five speakers held a panel discussion. Finally, the rapporteur summarized. Results and Discussion: SG leaders presented reports from researchers and practitioners in health physics and environmental risks who provided information after the Fukushima accident. During the discussion, experts in sociology and ethics discussed the issues, focusing on the overall goals of the three groups, local (personal) and mass communication, and ethical values. Many of the activities instituted by the experts after the accident were aimed at public understanding of science (that is, to provide knowledge to residents), but by taking into account interactions with residents and their ethical norms, the experts shifted to supporting the residents' decision-making through public engagement. The need to consider both content and channels is well known in the field of health communication, and overlaps with the above discussion. Conclusion: How to implement and promote the public engagement in society was discussed in both the floor and designated discussions. Cooperation between local communities and organizations that have already gained trust is also necessary in order to develop relationships with local residents in normal times, to establish an information transmission system, and to make it work effectively.

  • 현재 웹 환경은 정보 공유와 비즈니스 수행을 위해 보편적으로 사용되고 있는 영역으로 개인 정보 유출이나 시스템 장애 등을 목표로 하는 외부 해킹의 공격 타켓이 되고 있다. 기존의 사이버 공격 탐지 기술은 일반적으로 시그니처 기반 분석으로 공격 패턴의 변경이 발생할 경우 탐지가 어렵다는 한계가 있다. 특히 웹 취약점 기반 공격 중 삽입 공격은 가장 빈번히 발생하는 공격이고 다양한 변형 공격이 언제든 가능하다. 본 논문에서는 웹서버 로그에서 정상상태를 벗어나는 비정상 상태를 탐지하는 이상상태 탐지 기법을 제안한다. 제안된 방법은 웹서버 로그 내 문자열 항목을 머신러닝 기반 임베딩 기법으로 벡터로 치환한 후 다수의 정상 데이터와 상이한 경향성을 보이는 비정상 데이터를 탐지하는 머신러닝 기반 이상상태 탐지 기법이다.

회전익 항공기 착륙장치에 대한 퍼지 FMEA (Fuzzy FMEA for Rotorcraft Landing System)

  • 군수품은 품질 보증을 위해 개발과 양산단계에서 위험 식별을 수행해야 한다. 위험 식별은 부품, 구성품, 계통 등에 대한 고장 요소를 분석하는 것으로, 다양한 신뢰성 기법 중에서 고장 모드 영향 분석(FMEA)을 이용하고 있다. FMEA는 위험 식별 중 고장 요인에 대하여 분석하는 방법으로, 위험도(RPN)를 통해 관리할 수 있다. FMEA는 심각도, 발생도, 검출도가 같은 중요도로 평가되기 때문에 단점을 가진다. 퍼지 FMEA는 FMEA의 단점을 보완하기 위해 퍼지이론을 이용한 것이다. 퍼지 이론은 현상의 불확실한 상태를 표현해주는 방법으로, 정량적인 값을 제공한다. 본 논문에서, 퍼지 FMEA는 회전익 항공기 착륙장치의 고장 모드에 대한 객관적인 평가를 위해 적용되었다. 착륙장치에 대한 위험도 분석을 위해, 퍼지 규칙과 소속 함수를 구성하였다. 퍼지화 모델은 심각도, 발생도, 검출도의 크리스프(crisp) 값을 이용하였고, 위험도를 도출하였다. 착륙장치에 대한 퍼지 FMEA 결과는 위험도와 우선순위를 분석할 수 있다. 퍼지 FMEA는 회전익 항공기의 품질 보증 활동에서 기초자료로 활용할 수 있음을 확인하였다.

Surveying the Impact of Work Hours and Schedules on Commercial Motor Vehicle Driver Sleep

  • Background: Given the long hours on the road involving multiple and interacting work stressors (i.e., delivery pressures, irregular shifts, ergonomic hazards), commercial drivers face a plethora of health and safety risks. Researchers goal was to determine whether and to what extent long-haul trucker work schedules influence sleep duration and quality. Methods: Survey and biometric data collected from male long-haul truck drivers at a major truckstop in central North Carolina over a six month period. Results: Daily hours worked (mean = 11 hours, 55 minutes) and frequency of working over government-mandated daily HOS regulations (23.8% "frequently or always") were statistically significant predictors of sleep duration. Miles driven per week (mean = 2,812.61), irregular daily hours worked (63.8%), and frequency of working over the daily hour limit (23.8% "frequently or always") were statistically significant predictors of sleep quality. Conclusion: Implications of findings suggest a comprehensive review of the regulations and operational conditions for commercial motor vehicle drivers be undertaken.

European Approaches to Work-Related Stress: A Critical Review on Risk Evaluation

  • In recent years, various international organizations have raised awareness regarding psychosocial risks and work-related stress. European stakeholders have also taken action on these issues by producing important documents, such as position papers and government regulations, which are reviewed in this article. In particular, 4 European models that have been developed for the assessment and management of work-related stress are considered here. Although important advances have been made in the understanding of work-related stress, there are still gaps in the translation of this knowledge into effective practice at the enterprise level. There are additional problems regarding the methodology in the evaluation of work-related stress. The European models described in this article are based on holistic, global and participatory approaches, where the active role of and involvement of workers are always emphasized. The limitations of these models are in the lack of clarity on preventive intervention and, for two of them, the lack of instrument standardization for risk evaluation. The comparison among the European models to approach work-related stress, although with limitations and socio-cultural differences, offers the possibility for the development of a social dialogue that is important in defining the correct and practical methodology for work stress evaluation and prevention.
