Prediction model of health-related quality of life in older adults according to gender using a decision tree model: a study based on the Korea National Health and Nutrition Examination Survey (의사결정나무 분석을 이용한 한국 노인의 성별에 따른 건강관련 삶의 질 취약군 예측: 국민건강영양조사 자료 분석)

  • Purpose: The aim of this study was to predict the subgroups vulnerable to poorer health-related quality of life (HRQoL) according to gender in older adults. Methods: Data from 5,553 Koreans aged 65 or older were extracted from the Korea National Health and Nutrition Examination Survey. HRQoL was assessed using the EQ-5D tool. Complex sample analysis and decision-tree analysis were conducted using SPSS for Windows version 27.0. Results: The mean scores of the EQ-5D index were 0.93 ± 0.00 in men and 0.88 ± 0.00 in women. In men, poorer HRQoL groups were identified with seven different pathways, which were categorized based on participants' characteristics, such as restriction of activity, perceived health status, muscle exercise, age, relative hand grip strength, suicidal ideation, the number of chronic diseases, body mass index, and income status. Restriction of activity was the most significant predictor of poorer HRQoL in elderly men. In women, the poorer HRQoL groups were identified with nine different pathways, which were categorized based on participants' characteristics, such as perceived health status, restriction of activity, age, education, unmet medical service needs, anemia, body mass index, relative hand grip, and aerobic exercise. Perceived health status was the most significant predictor of poorer HRQoL in elderly women. Conclusion: This study presents a predictive model of HRQoL in older adults according to gender and can be used to detect individuals at risk of poorer HRQoL.

  • The purpose of this study was to explore the relationship between partner violence and depression in the elderly. This secondary analysis study used the data from 2015 Korean Welfare Panel Study. A total of 2,819 older adults who completed the survey about demographic characteristics, health-related characteristics, satisfaction in the relationship with children, satisfaction in social relationship, partner violence, and depression. Data were analyzed with SPSS 23.0 by using descriptive analyses, independent t-tests, $x^2$-tests, bivariate logistic regressions, and hierarchical logistic regressions. In the results of hierarchical logistic regressions, after controlled demographic and health related characteristics, satisfaction in the relationship with children, and satisfaction in social relationship, partner violence was significantly associated with depression in both male and female older people. In conclusion, it is important to make a prevention program to reduce partner violence and depression for not only female but also male older adults.

Predictors of Chewing Discomfort among Community-dwelling Elderly (지역사회 노인에서의 저작불편감 예측요인)

  • Purpose: The purpose of this study was to identify associated factors of chewing discomfort among community-dwelling elderly. Methods: The study was cross-sectional design and secondary data analysis using the 6th Korea National Health and Nutrition Examination Survey. Among the total of 7,550 participants, data was analyzed with 1,126 adults aged 65 years and over. Chewing discomfort was assessed by the perceived chewing discomfort. Multivariate logistic regression analysis was used to find the associated factors of chewing discomfort. Results: Along with 61.7% of the participants reported having chewing discomfort, 85.2% reported to perceive poor oral health and 35.0% had oral pain. In multivariate logistic regression, perceived oral health (OR 3.22, 95% CI 2.24~4.63), oral pain (OR 2.46, 95% CI 1.76~3.43), activity limitation (OR 1.71, 95% CI 1.05~2.80), teeth requiring treatment (OR 1.61, 95% CI 1.14~2.26), number of remaining teeth (OR 1.60, 95% CI 1.22~2.10) and educational level (OR 1.56, 95% CI 1.15~2.12) were the significant predictors of chewing discomfort. Conclusion: The prevalence in chewing discomfort was high in elderly Koreans and various factors were associated with chewing discomfort. To improve chewing ability, it is suggested that the national level of policies offer strategical oral health programs in this population.

Expected Family Involvement of Family of Elderly Residents in Nursing Homes (요양시설 입소 노인 가족의 가족참여 요구도에 관한 연구)

  • Purpose: The purpose of this study was to investigate the expected family involvement of family members of elderly residents in nursing homes. Methods: This study utilized a descriptive design with a survey research method. Data were collected using the Expect Family Involvement Questionnaire Scale, which was completed by 112 family caregivers of older adults in nursing homes in Seoul and Gyeong-gi Province. The collected data were analyzed using descriptive statistics. Results: Responses to the questionnaire indicated the highest value was recorded for the item, 'use of a room for family privacy' with $2.41{\pm}0.43$ points, followed by the item, 'staff calling family for advice with resident problems' with $2.38{\pm}0.38$ points and 'education about nursing home programs' with $2.32{\pm}0.42$ points. Conclusion: In order to promote family involvement in long-term care facilities, various approaches, interventions, and government supports with the respective of the family are required. The nursing home placement process should include understanding discuss on the importance of the role of family, and should help promote positive relationships between the elderly residents and their families. Staffs and administrators of nursing home are to consider the family as a partner rather than visitor.

The Relationship of Health-related Variables with Regular Walking Exercise in the Elderly based on: The Korea National Health and Nutrition Examination Survey(KNHNES) (노인들의 걷기 운동과 건강관련 변인과의 관계)

  • Purpose: The purpose of this study was to determine factors related to regular walking exercise in the elderly using KNHNES data. Method: The study subjects, a total of 1437 adults over age 65, were classified into two groups based on their type of walking exercise: regular walking exercise group or irregular walking exercise group. The study variables were measured using scale physiological function, mental health status, chronic disease and quality of life. Results: The data was analyzed by $x^2$-test, t-test, and logistic regression using the SPSS 18.0 program. The factors related to regular walking exercise were male (OR=0.628, 95%CI=0.503-0.783), subjective health state (OR=0.832, 95%CI=0.751-0.921), thoughts of suicide (OR=0.753, 95%CI=0.586-0.967), hypoHDL-cholesterolemia (OR=0.742, 95%CI=0.591-0.930), and stroke (OR=0.604, 95%CI=0.366-0.997). Conclusion: The findings indicate that regular walking exercise in the elderly is related to physiological function, mental health status, and chronic disease. Therefore, providing for the education and awareness of health promotion for the elderly will may improve the quality of life in old age.

Effects of Bibliotherapy of Dotard Dementias Old Adults (독서요법이 경증 치매노인에게 미치는 효과)

  • This study was conducted to investigate the effectiveness of bibliotherapy on the levels of cognitive function, depression, and social behavior in the elderly with mild dementia. For this quasi-experimental research, a total of 166 subjects the age of from 90 to 65 were selected of the 166, were divided into the experimental group and control group for six months. These findings confirmed that the dementia Bibliotherapy program made a contribution to improve cognitive function and social behavior, and to reduce depression in the elderly with mild dementia. Therefore, it is recommended that this program be used in clinical practice as an effective Bibliotherapy program for the elderly with mild dementia.

  • This study aimed to suggest the way to support flood disaster older survivors with analysing how relief services and their human resources are used. For this study, the data was collected from 184 elderly aged over 65 years from Inje and Pyungchang in Gangwon province where lots of flood damages were done. The results of the study was elderly used human resources of public servant/military soldiers, volunteers as public or official services than as private resources. These results provide the evidence that public or official human resources are very helpful to control their emergency situations because there is hardly any use of their private human resources except for assistance from their family. And it shows that older people are willing to use services of life rescue and information services of their family members safety rather than basic supplies, medical care or medicine providing. With this findings we suggest informing the news of family safety including basic necessaries are highly signigicant. Thus, it is useful for disaster planners to understand building immediate life rescue and accurate information delivery systems. These are relevant to older adults' psychological well-being, thus, providing news of family safety including offering material resources are highly needed for older disaster survivors.

Strength-dexterity Complementariness: Comparison between Left and Right Hands in Older Female Adults (근력-기민성 보완: 여성 노인의 오른손과 왼손 비교)

  • Objective: The purpose of this study was to in this study. The maximum grip force of the elderly hand was measured using a custom-designed grasping apparatus mounted with five three-component force transducers. The Jebsen-Tayler hand function test and Purdue Pegboard test were performed to evaluate the dexterity of the hand. Method: Twenty-six elderly women participated in the left hand between the maximum grip force and the Jebsen-Taylor hand function test results (r=-.513, p=.007). A significant correlation was also shown in the hand maximum grip force and the hand Purdue Pegboard results (r=.514, p=.007). However, no significant correlation was found in the right hand. Results: We found a significant correlation investigate the relationship between hand grip strength and hand dexterity in the elderly. Conclusion: Our findings in the current study support the theory of 'Strength-dexterity complementariness' which states that improvement in dexterity is associated with the grip force strength.

  • As the socialization of care is progressing rapidly, the necessity of community-level mental health intervention for older adults with severe social isolation and depression is also increasing. In line with the reorganization of the Individualized Support Services for Older Adults in 2020, the social relations revitalization project for the elderly living alone, which had been conducted as a pilot project in 2014~19, was expanded and reorganized into a separate specialized project within the Individualized Support Services for Older Adults. The purpose of this study is to enhance understanding of the specialized service and to clarify its conceptual framework. The characteristics and conceptual framework of the specialized service were examined through a review of the process of institutionalization of the specialized service, which included analysis of related literature and the pilot projects. Finally, it discussed what to consider in order for the specialized service to proceed as intended in the future, focusing on a couple of situations that occur at the fields.

Analysis of the Characteristics of the Older Adults with Depression Using Data Mining Decision Tree Analysis (의사결정나무 분석법을 활용한 우울 노인의 특성 분석)

  • Purpose: The purpose of this study was to develop a prediction model for the characteristics of older adults with depression using the decision tree method. Methods: A large dataset from the 2008 Korean Elderly Survey was used and data of 14,970 elderly people were analyzed. Target variable was depression and 53 input variables were general characteristics, family & social relationship, economic status, health status, health behavior, functional status, leisure & social activity, quality of life, and living environment. Data were analyzed by decision tree analysis, a data mining technique using SPSS Window 19.0 and Clementine 12.0 programs. Results: The decision trees were classified into five different rules to define the characteristics of older adults with depression. Classification & Regression Tree (C&RT) showed the best prediction with an accuracy of 80.81% among data mining models. Factors in the rules were life satisfaction, nutritional status, daily activity difficulty due to pain, functional limitation for basic or instrumental daily activities, number of chronic diseases and daily activity difficulty due to disease. Conclusion: The different rules classified by the decision tree model in this study should contribute as baseline data for discovering informative knowledge and developing interventions tailored to these individual characteristics.
