Cystic lymphangioma in the submandibular triangle in the adult (성인의 악하부에 발생한 낭성 림프관종)

  • Cystic lymphangioma is an uncommon benign developmental tumor of the lymphatic system, seen rarely in adults. A 23-year-old man visited DKUDH complaining of the painless swelling in the right submandibular area. It was reported that swelling had increased since 5 months ago. Clinically, fltriangle uctuant and mobile mass was palpated at the right submandibular area. CT scan showed a large, well-defined, homogenous low density mass, measuring 5.0 × 2.5 × 4.0 cm. T1W MRI scan demonstrated a large, multiseptated homogenous low signal mass with septum and rim enhancement. Proton-density and T2W MRI scan showed mass of high signal intensity. MRI scan was able to delineate better the enhanced multi septation and extent of the lesion. Histopathologically, a lobulated cystic mass lined by a layer of flattened endothelium was observed.

  • We have experienced three cases of congenital bronchoesophageal fistula which is rare and usually has an insidious clinical course. The patients included a thirty year old man and thirty six, thirty eight year old women respectively. Bronchiectatsis was found in all three cases, and bronchoesophageal fistula was found in one case preoperatively by esophagography and esophagoscopy, and other two cases operative field. The fistula was found between right lower esophagus and right lower lobe in all cases and esophageal diverticulum in one case. So they belonged to type I[1 case , II[2 cases of Braimbridge and Keith`s classification of congenital bronchoesophageal fistula . The fistulectomy was performed in all cases and concomitant lobectomy [2 cases and bilobectomy [1 case were done. There were toxic hepatitis in two cases and prolonged air leakage in one case postoperatively. They were discharged on recovered state and have continued to do well.

  • The Wnt signaling pathway plays pivotal roles in embryonic development and oncogenesis through various signaling molecules inculding Frizzled receptor, recently characterized LRP5/6 and Dickkopf protein. Although Wnt signaling has been characterized in both developmental and oncogenic processes, little is known about its function in the normal adult. The ability of LRP5 to bind apolipoprotein E(apoE) and the abundant expression of LRP5 transcripts in hepatocytes, raise the possibility that LRP5 plays a role in the hepatic clearance of ApoE-containing chylomicron remonants, a major plasma lipoprotein carrying diet-derived cholesterol.   • The objectives of the study are to analyze the impacts of rural road project on the social and economic activities of community people, offer basic data to perform evaluation of the project, and suggest future directions of the project. Research methodology adopted is a questionnaire method. There were interviewed 281 adult residents selected from 34 villages in Yeongdong-gun(mountainous), Miryang-si(middle), and Gochang-gun(plain). Results of the study are as follows: 1) Increase of income and land prices in mountainous and middle regions by the rural road project is higher than in plain; 2) Most inconvenient road expressed by the interviewee is the one linking farm and residence and 3) The problems of rural roads derived are narrowness, unpavement and partial pavement, unreliable maintenance, and long construction time. In order to meet the community needs on rural roads, it may be necessary that community people participate in the process of the project. And it is desirable to put priority on expanding and paving existing roads rather than constructing new one.

  • Using a relationship between step length and accelerometer output, step length can be estimated. In this paper, we propose a new step length estimation algorithm, which can be used both for the plane and the slope by compensating the slope angle. The slope angle is estimated using a barometer and the slope angle is compensated by observing how the slope affects the step length estimation. The proposed algorithm is verified using five adult man walking data, where the average length error is about 3% regardless of the slope.

  • A human case of Centrocestus armatus (Heterophyidae) infection was proved by identifying an adult worm collected after treatment with praziquantel in Korea. The case is 42-year old man who resides in a rural area in Sanchung-gun, Kyeongsangnam·do. The case was concomitantly infected with Clonorchis sinensis and had the history of eating raw (reshwater fishes including Zacco platypus, which are known to be the second intermediate host of C. armatus in Korea. This is the first report of natural human infection by C. armatus in the literature.

  • A 69-year-old Korean man was admitted to emergency room with complaints of abdominal pain, vomiting, and diarrhea, Laboratory tests revealed eosinophilia, anemia, hypoproteinemia, and hyponatremia. The gastric mucosa showed whitish mottled and slightly elevated lesions on the body angle of antrum. Microscopically, chronic gastritis with incomplete intestinal metaplasia was observed. Many adult worms, larvae, and eggs in cross sections were located in the crypts. Furthermore, the filariform larvae of Strongyloides stercoralis with a notched tail were detected through the culture.

Effects of gamma-irradiation on the infectivity and chromosome aberration of Clonorchis sinensis

  • Effects of gamma irradiation on the worm survival and chromosomal aberration of Clonorchis sinensis were studied. The metacercariae irradiated with various amounts of gamma radiation (ranging from 5 Gy to 50 Gy) were fed to rats, and the effects were compared with those of non-irradiated controls. Recovery rates of adult worms in irradiated groups were reduced gradually as increasing of the irradiation doses. No worm was recovered from rats which were fed with 50 Gy irradiated metacercariae. The chromosome number was 2n = 56 in all worms from all experimental groups. However, the groups irradiated with 20 Gy, 25 Gy or 30 Gy showed variations in the chromosome number, depending on different cells in the same individual. Radiation doses used in this study did not appear to induce chromosome aberrations, however, irradiation with 30 Gy showed slightly reduced chromosome size.

  • We present the rare case of solitary xanthogranuloma in the upper cervical column mimicking a Brown-Sequard syndrome. A 29-year-old man complained with right hemiparesis and left hypoesthesia after a car accident. Computed tomography and magnetic resonance images revealed a lobulated homogenously well-enhancing mass in between posterior arch of the atlas (C1) and spinous process of the axis (C2) resulting in a marked spinal canal narrowing with cortical erosions. The patient was managed by complete resection of the tumor with partial laminectomy with lower half of C1 posterior arch and upper half of C2 spinous process. The authors advise complete removal of the xanthogranuloma and consideration as a differential diagnosis of lesions among upper cervical lesions.

Isolated Lateral Sinus Thrombosis Presenting as Cerebellar Infarction in a Patient with Iron Deficiency Anemia

  • As a rare cerebrovascular disease, cerebral venous thrombosis (CVT) is caused by various conditions including trauma, infection, oral contraceptive, cancer and hematologic disorders. However, iron deficiency anemia is not a common cause for CVT in adult. Posterior fossa infarction following CVT is not well demonstrated because posterior fossa has abundant collateral vessels. Here, we report a case of a 55-year-old man who was admitted with complaints of headache, nausea, and mild dizziness. The patient was diagnosed with isolated lateral sinus thrombosis presenting as cerebellar infarction. Laboratory findings revealed normocytic normochromic anemia due to iron deficiency, and the patient's symptoms were improved after iron supplementation.
