Clinical Observation for Prognosis Utilizing Brain Computerized Tomography Findings and Barthel Index In Acute Cerebral Infarction (급성 뇌경색 환자의 Brain-CT 소견과 Barthel ADL을 이용한 예후에 관한 고찰)

  • In a 36-month period, 23 selected Patients with Acute Cerebral Infarction were studied utilizing Computerized Tomography, Barthel Index in an attempt to correlate Brain Computerized Tomography findings with 1week, 4weeks rehabilitation and evaluate the influences of the size and location of the lesion. The study suggested that the size of the lesion had impact on 4 weeks rehabilitation. There was significant different between the patients with Middle Cerebral Artery Pial Territory Infarction(I.P.B.M.C.A.) lesion and the patients with Lacunar Infarction(L.I.), Striatocapsular Infarction(S.C.I.), Internal Watershed Infarction(I.W.I.) lesion, but there was no significant difference between the patients with L.I. lesion and the patients with S.C.I. lesion and the patients with I.W.I. lesion. The size and location of the lesion should be considered together in predicting the functional outcomes of Acute Cerebral Infarction.

  • Acute osteomyelitis is usually shown as a localized area of increased activity (hot uptake) in the infectious focus on bone scintigraphy. In our patient, absence of radioactivity (cold lesion) was noted in the distal metaphyseal and diaphyseal regions of his right femur. Initial x-ray was interpreted as normal except for mild soft tissue swelling in the right thigh. The lesion was confirmed as an acute osteomyelitis with subperiosteal abscess on surgery. Staphylococcus aureus was the etiologic organism. We describe a case of acute osteomyelitis in a one-year-old boy shown as a cold lesion on bone scan.

  • A Morel-Lavallée lesion is a post-traumatic closed degloving soft tissue injury after blunt trauma. Infection and skin necrosis frequently occur if it is not treated properly in the early stages. However, there is no clearly established treatment algorithm. In the acute stage, it is mainly treated with aspiration, simple compression, and incisional drainage. In the chronic stage, sclerotherapy is usually performed. If skin necrosis develops, the necrotic tissue is resected and a skin graft is needed. We describe a case of acute Morel-Lavallée lesion in the buttock region that was treated with limited incisional drainage and negative-pressure wound therapy, and also present a review of the literature.

Gastroduodenal intussusception of a gastrointestinal stromal tumor: a rare cause of acute pancreatitis

  • Patients with symptomatic gastrointestinal stromal tumor (GIST) typically present with gastrointestinal bleeding and abdominal pain. This report presents an unusual case of fundic GIST complicated by gastroduodenal intussusception, manifesting as acute pancreatitis. The patient presented with epigastric pain and pancreatic enzyme elevation; thus, he was diagnosed with acute pancreatitis. Computed tomography showed evidence of pancreatitis and a 4×4.7 cm well-defined hyperdense lesion in the 2nd part of the duodenum, compressing the pancreatic head and pancreatic duct. Esophagogastroduodenoscopy revealed invagination of the gastric folds into the duodenum, causing pyloric canal blockage consistent with gastroduodenal intussusception. Spontaneous reduction of the lesion during endoscopy revealed a 4 cm pedunculated subepithelial mass with central ulceration originating from the gastric fundus. Endoscopic ultrasound demonstrated a heterogeneous hypoechoic lesion originating from the 4th layer of the gastric wall. Laparoscopic-endoscopic intragastric wedge resection of the fundic lesion was subsequently performed, and surgical histology confirmed GIST.

Correlation of Ultrasound and Scintigraphic Findings in Scrotal Disease (음낭 질환의 초음파 및 씬티그라피에 관한 방사선학적 연구)

  • Recently ultrasound and scintigraphic study are most useful modality for diagnosis of scrotal diseases. They are able to determine the nature, anatomic extent of the lesion, and possible to differential diagnosis of the scrotal lesion. The ultrasound and scintigraphic findings were analyses in 59 patients of confirmed scrotal disease at Seoul Red Cross Hospital from June 1986 to Aug. 1989. The results were as follows. 1) Most patients were below the 30 years old age (69.4%), and most prevalent age was $11\sim30$ Yrs. in trauma, $31\sim40$ Yrs. in inflammation, and $51\sim60$ Yrs. in mass lesion of scrotum. 2) The diagnostic accuracy of scrotal disease was 94.9% in ultrasound and 72% in scintigraphic study. The ultrasound study showed high accuracy in all kind of scrotat disease, but scintigraphy showed only high detection rate in inflammatory disease. 3) Scrotal ultrasound is first choice of study for the diagnosis of scrotal lesion, and useful determine the cystic or solid lesion and anatomic extent of disease. However it can't be differentiate the acute epididymitis with acute testicular torsion. 4) Scrotal scintigraphy is useful study for the diagnosis of acute painful scrotal disease, and showed specific findings in acute epididymitis and acute torsion for differential diagnosis. 5) In consideration of age, history and clinical findings of patient, appropriate study will be chosen firstly in ultrasound and scintigraphy, and combined all of findings of these studies will be helpful for the accurate diagnosis and adequate treatment of scrotal disease.

Differentiation of Bone Metastases and Fractures using 24 hour/3 hour Radio-uptake Ratio in Bone Scintigraphy (골신티그라피의 3시간과 24시간 방사능 섭취비를 이용한 골 전이와 골절의 감별)

  • Purpose: We evaluated the usefulness of 24 hour/3 hour radio-uptake ratio, lesion to non-lesion ratio, in differentiating bony metastases from acute (<2 months) and healing (${\geq}2$ months) fractures. Materials and Methods: Sixty-three patients (age range: 26-81, 32 males, 31 females) having 90 lesions (30 bone metastases, 30 acute fractures, 30 healing fractures) were included. Bone scans were obtained 3 and 24 hours after administration of 740 MBq of $^{99m}Tc$-MDP. The ratio of radio-uptake in the lesion to normal area was measured as 24/3 hour radio-uptake ratio ([lesion/non-lesion RUR at 24 hour]/[lesion/non-lesion RUR at 3 hour], 24/3 RUR) and analyzed clinical significance in differentiating bone metastases from acute or healing fractures. Results: Mean 24/3 RUR were $1.22{\pm}0.18$ for bone metastases, $1.25{\pm}0.14$ for acute fractures, and $0.99{\pm}0.15$ for healing fractures. 24/3 RUR values of bone metastases and acute fractures were not significantly different. But 24/3 RUR values of bone metastases and healing fractures, and those of acute and healing fractures were found to be significantly different (p<0.001). When 24/3 RUR of 1.0 was considered as the cut off point separating metastases from fracture, a sensitivity of 100% (30/30) was obtained. The specificity was 0% (0/30) in separating metastases from acute fractures, and 47% (14/30) in separating metastases from healing fractures. When 24/3 RUR of 1.2 was considered as the cut off point, sensitivity of 53% (16/30) in the diagnosis of bone metastasis, and specificity of 37% (11/30) in separating metastases from acute fractures, and 100% (30/30) in separating metastases from healing fractures were obtained. Conclusion: 24/3 RUR is useful in differentiating bone metastases from healing fractures, but not in differentiating bone metastases from acute fractures. A 24/3 RUR of less than 1.0 suggests healing fractures. A 24/3 RUR of more than 1.2 suggests bone metastases or acute fractures.

  • In acute pancreatitis group, all the dogs are showed increase of amylase and lipase after the 1st day of surgery, and amylase and lipase activity were significantly more increased than those of control group. The methemalbumin was increased significantly after the 2nd day of surgery in the acute pancreatitis group. In pancreatitis group, ultrasonographic findings included thickened duodenal wall and poorly circumscribed hyperechoic lesion of pancreatic mass after the 2nd day of surgery. And the lesion was exacerbated until the 4th day and reduced after the 6th day of surgery. To identify the lesion of pancreas, it is considered that transverse view is more useful Than sagittal view. Gross findings showed increase of pancreatic parenchymal consistency, surface nodule, and extensive pancreatic necrosis. Necrosis of peripancreatic fat tissue was also a prominent feature. The microscopic appearance of the pancreas was characterized by pancreatic acinar cell necrosis, hemorrhage, infiltration of the inflammatory cell and fat necrosis and saponification were also observed.

The Usefulness of Deconvolution Perfusion CT in Patients with Acute Cerebral Infarction : Comparison with Diffusion MRI (급성 뇌경색 환자에서 Deconvolution perfusion CT의 유용성 : Diffusion MRI와 비교)

  • This study was performed to evaluate the usefulness of Deconvolution perfusion CT in patients with acute cerebral infarction. Nine patients with acute cerebral infarction underwent conventional CT and cerebral perfusion CT within 23 hours of the onset of symptoms. The perfusion CT scan for each patient was obtained at the levels of basal ganglia and 1cm caudal to the basal ganglia. By special imaging software, perfusion images including cerebral blood volume(CBV), cerebral blood flow(CBF), and mean transit time(MTT) maps were created. The created lesions were evaluated on each perfusion maps by 3 radiolocical technician. MTT delay time was measured in the perfusion defect lesion and symmetric contralateral normal cerebral hemisphere. Lesion sire were measured on each perfusion map and compared with the value obtained by diffusion weighted MR imaging(DWMRI). All perfusion CT maps showed the perfusion defect lesion in all patients. There were remarkable CT delay in perfusion defect lesion. In comparison of lesion size between each perfusion map and DWMRI, the lesion on CBF map was the most closely correlated with the lesion on DWMRI(7/9). The size of perfusion defect lesion on MTT map was larger than that of lesion on DWMRI, suggesting that m map can evaluate the ischemic penumbra. Deconvolution Perfusion CT maps make it possible to evaluate not only ischemic core and ischemic penumbra but also hemodynamic status in perfusion defect area. These results demonstrate that perfusion CT can be useful to the diagnosis and treatment in the patients with acute cerebral ischemic infarction.

  • Vestibular evoked myogenic potentials (VEMP) have been known to useful in documenting abnormality in patients with various vestibular disorders but the studies of VEMP in stroke patients are rare. We recorded VEMP in 17 consecutive patients with acute ischemic stroke in the brainstem lesions. All patients underwent magnetic resonance imaging and we compare VEMP results with the lesion documented by brain imaging. VEMP were defined to be abnormal when they were very asymmetrical (one is 2 times of more as large as the other), or absent in one side. VEMP abnormalities were found in 71%(12/17) of acute infarction patients with brainstem lesions. Most abnormalities found in the ipsilateral side of the lesion(9/12) but abnormalities in contralateral side of lesion were found in 25%(3/12) of patients.VEMP would be considered a useful complementary neurophysiological tool for the evaluation of brainstem dysfunction in acute stroke patients.

A Case Report on Korean Medicine Treatment for a Patient Diagnosed with Acute Gastric Mucosal Lesion on Ultrasound Imaging (초음파 영상에서 급성 위점막 병변으로 진단된 환자에 대한 한의치료 치험 1례)

  • Objectives: This study reports a clinical case that showed improvement in the symptoms of a patient with acute gastric mucosal lesion (AGML) through ultrasound. Methods: A 90-year-old woman with AGML was treated with Korean medicine treatments, including Hyangsapyeongwisan Soft Ext, acupuncture, and moxibustion. The responses before and after treatment were evaluated using ultrasound imaging and the numerical rating scale (NRS). Results: After treatment, ultrasound imaging showed improvement in gastric mucosa thickness, from 1.45 cm to 0.53 cm at the anterior wall and from 1.25 cm to 0.36 cm at the posterior wall. The NRS scores for epigastric pain, nausea, and dizziness all decreased, and the symptoms of diarrhea disappeared. Conclusion: This study suggests that Korean medicine treatment can be a therapeutic option in treating the symptoms of patients with AGML.
