퇴행성 슬관절염 환자의 한열 성향에 따른 한약 제제의 유효성과 안전성 비교 (Efficacy and Safety of Herb Medication According to Cold-heat Tendency of Knee Osteoarthritis Patients)

  • Objectives : To demonstrate the importance of syndrome differentiation in clinical research of herb medication, through the comparative study on efficacy and safety of herb medication according to cold-heat tendency of OA knee patients. Methods : During December 2010 to July 2011, 138 knee OA patients were randomly assigned to WIN-34B 600mg(300mg, b.i.d.), 1,200mg(600mg, b.i.d.) and placebo b.i.d. for 8 weeks. Patient were re-classified into cold-heat tendency group according to cold-heat questionnaires. To investigate efficacy and safety, we assessed the 100mm pain VAS at baseline and 8 weeks later, and we monitored adverse event of patients during treatment period. Results : 1. Efficacy study : In WIN-34B 1,200mg group, VAS mean changes of heat tendency group showed slightly increase than those of cold tendency, but no significant difference within two groups. In heat tendency group, WIN-34B 1,200mg group showed a significant decrease of VAS compared to placebo group. but there were no significant difference in cold tendency group. 2. Safety study; In WIN-34B 600mg group, incidence of adverse events of cold tendency group was higher than those of heat tendency, but not in WIN-34B 1,200mg group. Conclusions : This study suggests that WIN-34B tend to have more efficacy in heat tendency-knee OA patients and WIN-34B is safe drug relatively, regardless of cold-heat tendency. In further clinical research on efficacy and safety of WIN-34B, stratification using syndrome differentiation is required.

견관절 석회화 건염으로 진단된 견비통환자(肩臂痛患者) 치험(治驗) 1례(例) 보고(報告) - 단순 방사선 영상의 변화를 중심으로 - (A Case of Calcific Tendinitis)

  • Objective : The purpose of this study is to report the patient diagnosed as calcific tendinitis located at supraspinatus, who improved by Oriental medical treatment, through analysis of radiograph. Methods : The patient was managed by acupuncture, moxibustion and cupping without medication and physical therapy. We measured the shoulder state by radiograph and evaluated with UCLA shoulder rating scale. Results : After 5 week treatment, the pain of patient and The UCLA functional average score was improved. Also we found that the deposits of calcific tendinitis was almost disappeared in radiograph. Conclusions : The results suggest that acupuncture, moxibustion and cupping can be effective in reducing the pain and the deposits of calcific tendinitis. We think that it need the further study and clinical trial for calcific tendinitis.

A Case Report of a Stroke Patient with Dysarthria Treated with Korean Medicine

  • This case study examined the effect of Korean Medicine on a 72-year old male with a habitual history of smoking and drinking, and a family history of hypertension who had suffered an acute stroke. A couple of months later he complained of upper extremity weakness and was diagnosed with dysarthria. The patient received Korean Medicine treatment of acupuncture and herbal medicine for 39 days at the Samse Oriental Hospital, in conjunction with physiotherapy and Western medicine. Articulation accuracy, vowel accuracy, alternation and speed of reading sentences were evaluated every 7days. The results showed that the patient had improvement of articulation (10%) and vowel (37%) accuracy, alternation (12%) and speed of reading sentences (28%) suggesting that Korean Medicine treatment contributed to this progress.

Treatment of Cervical Myelopathy with Acupotomy Combined with Korean Medicine Treatments: Two Clinical Cases

  • The degenerative spinal cord disease cervical spondylotic myelopathy (CSM), and cervical myelopathy caused by trauma, can result in debilitating symptoms affecting quality of life. This study used acupotomy and other Korean medicine treatments (acupuncture, herbal medicine, and physical therapy) to improve the symptoms of CSM and cervical myelopathy. The visual analog scale, the modified Japanese Orthopaedic Association scale (mJOA scale), the Nurick grading system, and the American Spinal Injury Association impairment scale were used as the evaluation criteria to determine the effectiveness of treatment. The functional status of both patients improved from mild to moderate, with improved gait, local sensation, and level of pain. The degree of spinal cord injury remained the same. The findings of this study suggest that combined Korean medicine treatments including acupotomy may be helpful in the treatment of CSM and cervical myelopathy.

  • Fat pad syndrome is a knee joint condition/disease where acute or chronic inflammation occurs in the fat pad of the knee joint, and it is a cause of anterior knee pain. Fat pad syndrome usually occurs concurrently with other conditions/diseases to collectively cause anterior knee pain. No study to date has reported the treatment of anterior knee pain solely due to fat pad syndrome. Here, we report a case of fat pad syndrome of the knee joint as the sole cause of anterior knee pain in a 49-year-old woman who received integrated Korean medicine treatments (pharmacopuncture, acupuncture, herbal medicine, deep fascial meridian therapy, and chuna). Using patient-reported pain scale scores, the level of the patient's pain was relieved, and her mobility improved. Integrated Korean medicine treatments could be effective for patients who have fat pad syndrome of the knee joint.

Effects of Korean Traditional Medicine Treatment on Spontaneous Osteonecrosis of the Knee: A Case Report

  • Spontaneous osteonecrosis of the knee (SONK) is a common form of osteonecrosis of the knee and mainly affects the medial condyle due to localized vascular insufficiency. We report a case of SONK in a 65-year-old woman who had severe knee pain in her left knee whichimpeded her capacityto walk beyond 10 minutes. Bilateral knee X-rays revealed degenerative osteoarthritis of both knees and magnetic resonance imaging revealed R/O SONK in the lateral aspect of the medial femoral condyle, as well as a medial meniscal posterior horn root tear, and a Grade 1 medial collateral ligament injury. She was hospitalized at Jaseng Hospital of Korean Medicine for 21 days and receivedcombination therapy includingacupuncture, pharmacopuncture, and herbal medicine. Patient-reported scales indicated that her pain and physical functional limitations were alleviated. Combination therapy consisting of Korean traditional medicine may bean alternative nonoperative treatment approach for patients with SONK.

Effect of Traditional Korean Medicine Treatments on the Discoid Meniscus of Knee Joint: Two Clinical Cases

  • The discoid meniscus is an innate modified form of the meniscus, characterized by middle hypertrophy and a larger than the regular diameter, leading to an absence of the characteristic "C" arrangement. Previously, no study has reported the traditional Korean medicine treatment of knee pain mainly due to discoid meniscus. Here, we report two cases of a discoid meniscus of the knee as the cause of knee pain. Patients were diagnosed with magnetic resonance imaging for discoid meniscus and received traditional Korean medicine treatments, including pharmacopuncture, acupuncture, herbal medicine, and chuna. The pain was alleviated when assessed by patient-reported pain scale scores, and the general knee condition was improved. Thus, traditional Korean medicine treatments could be effective for patients who had discoid meniscus of the knee joint in this case report.

Effect of Complex Korean Medical Treatment on Spinal Epidural Hematoma: A Case Report

  • This paper presents the clinical outcome of an 82-year-old female who experienced sudden back pain combined with lower limb paresthesia and weakness after epidural steroid injection. The magnetic resonance imaging of the thoracolumbar spine showed a spinal epidural hematoma (SEH) extending from T8 to L4. She was treated non-operatively in the traditional Korean medicine (TKM) hospital. The patient's progress was assessed using the Numerical Rating Scale (NRS), American Spinal Injury Association (ASIS), Spinal Cord Independence Measure version III (SCIM III), and self-reported symptoms. During the 22-day hospitalization period,the NRS score decreased from 7 to 2 points, the motor score on the ASIS scale increased from 65 to 95 points. The subjective sense of lower extremities was felt by 1 increased to 8. The SCIM III score increased from 32 to 69 points. These results suggest that TKM could effectively reduce pain and aid the rehabilitation of patients with SEH.

백반증의 침구(鍼灸) 치료 효과: 무작위 대조 시험에 대한 주제 범위 문헌 고찰 (Effect of Acupuncture and Moxibustion for Vitiligo: A Scoping Review of Randomized Controlled Trials)

  • Objectives : The aim of this study is to review the effect of acupuncture/moxibustion on vitiligo. Methods : Using 5 databases(Pubmed, EMBASE, Cochrane Library, RISS, OASIS) clinical studies about effect of acupuncture/moxibustion on vitiligo were searched. Only randomized controlled trials(RCTs) were selected to analyze. Results : Total 235 studies were searched. After screening those studies 5 studies were selected from china and Iran. 3 studies used effective rate to measure the effect of acupuncture for vitiligo. 1 study used Vitiligo Area Scoring Index(VASI) and 1 study assessed 4 grade of repigmentation to prove acupuncture's effect on vitiligo. All studies showed repigmentation in acupuncture/moxibustion group. Adverse effects reported in the five studies were temporary erythema, itching, and dryness. Conclusions : These findings suggest that acupuncture and moxibustion can be considered a safe treatment for vitiligo. However, due to the small number of RCTs conducted on acupuncture and moxibustion for vitiligo, more RCTs should be conducted to confirm the effectiveness of acupuncture and moxibustion for vitiligo.

구안와사(口眼喎斜)에 대한 체침(體鍼)과 동씨침(董氏鍼)의 효과(效果) 비교(比較) (The Effect of Acupuncture and Dong's Acupuncture about Bell' palsy)

  • Objective: The aim of this research was to investigate the effect of acupuncture and the effect of Dong's acupuncture on Bell's palsy. Methods: We measured the facial palsy changes of the patients who were admitted for Bell's palsy in the Oriental Medical hospital of Dongeui medical center from 07-01-2003 to 09-30-2003. Bell's palsy patients were divided into two groups. One group(A group) was treated by acupuncture. The other group(B group) was treated by Dong's acupuncture. The effects of these treatment was evaluated by Yanagihara's unweighted grading system. Results : B group was marked more higher than A group in treatment outcome. we discovered that it is significant differences between two groups after 2 week. Conclusion : These results provided that A group is more effective than B group on. For clearly comparing the Effect of Acupuncture and Dong's Acupuncture on Bell's palsy, it is need more sample's number and more treatment's duration.
