훈침의 미주신경 실신 측면으로 이해와 적절한 예방과 조치 (Understanding Acupuncture Needle-Associated Vasovagal Syncope for the Purpose of Preventing and Managing Adverse Events)

  • Objectives : Needle sickness is one of the adverse events of acupuncture, although substantial adverse effects during a routine acupuncture treatment seem to be highly unusual. In this work, we propose that an acupuncture-related vasovagal response resembles needle sickness during acupuncture therapy. Methods : In this article, we discussed the general characteristics of vasovagal syncope and went into more detail on vasovagal syncope in people who have a fear of blood injection and injury. We also offer a recommendation for the prevention and management of vasovagal syncope brought on by acupuncture. Results : The vasovagal reaction related to acupuncture is closely associated with needle sickness. The prevention can be done using PEACHES (position, experience, anxiety, constitution, hydration, environment, symptom recognition) principles. The management should be conducted using the RIPCORD (recognize, initiate, position, communicate, order treatments, reassess, document) techniques. Conclusions : It is important to comprehend the characteristics of needle sickness as a vasovagal reaction related to acupuncture. According to the recommendation, practitioners should effectively prevent and manage needle sickness.

Improvement of the Sequelae of Peripheral Facial Nerve Palsy in a Patient with Hemophilia Mainly Treated with Facial Chuna Manual Therapy: A Case Report

  • A patient with sequelae of peripheral facial nerve palsy (FNP) was mainly treated with facial chuna manual therapy (FCMT) for over 6 months. The patient was diagnosed with hemophilia B and C, so invasive treatments, such as acupuncture and pharmacopuncture, were precluded. The facial nerve grading system 2.0 (FNGS) and the House-Brackmann Grading Scale (HBGS), Peitersen, Murata, Mehta, and numeric rating scale (NRS) systems were used to measure treatment effectiveness. Each scale showed improvement: FNGS, 3 to 2; HBGS, 3 to 2; Peitersen scale, 2 to 1; Murata scale, 9 to 4; Mehta scale, 14 to 1; and NRS, 8.5 to 2.5. An overall improvement was evident in facial muscle strength, particularly in synkinesis. If acupuncture and pharmacopuncture are unavailable, FCMT alone may be effective in treating FNP sequelae.

Integrative Korean Medicine Treatment for Low Back Pain with Butterfly Vertebra: Two Clinical Cases

  • The butterfly vertebra is a rare congenital anomaly of the spine, which arises from an asymmetric fusion defect in the embryonic vertebral column. This report delineates an integrative Korean medicine treatment administered to two patients who presented with low back pain associated with asymptomatic butterfly vertebrae. The patients received comprehensive treatment comprising acupuncture, Chuna therapy, pharmacopuncture, herbal medicine, and cupping therapy. The efficacy of the treatments was assessed utilizing diverse parameters, including a numerical rating scale, range of motion of the lumbar spine, Oswestry Disability Index, EuroQoL 5-Dimension 5-Level, and patient global impression of change. After the treatment, a notable improvement was noted in the evaluated indicators. These positive outcomes suggest that integrative Korean medicine treatment is suitable for patients experiencing low back pain associated with asymptomatic butterfly vertebrae.

침과 매선요법 복합시술의 유방 확대 효과 136례 (Effect of Combined Treatment of Needle-embedding Therapy and Acupuncture for Breast Augmentation in 136 Patients)

  • Objectives: The purpose of this study was to evaluate the effect of combined treatment of needle-embedding therapy and acupuncture for breast augmentation. Methods: 136 patients had needle-embedding therapy and acupuncture combined treatments for breast augmentation. The treatments was performed once a week for 10 weeks. We measured bust circumference, underbust circumference, bust circumference and underbust circumference difference, treatments either before or after. Results: The mean of bust circumference was $77.75{\pm}4.93cm$ before the procedure and $80.25{\pm}5.01cm$ after the procedure, $2.50{\pm}1.95cm$ significantly increased. The mean of underbust circumference was $70.01{\pm}3.99cm$ before the procedure and $70.49{\pm}4.08cm$ after the procedure, $0.48{\pm}1.48cm$ significantly increased. the mean of bust circumference and underbust circumference difference was $7.74{\pm}2.24cm$ before the procedure and $9.76{\pm}2.55cm$ after the procedure, $2.01{\pm}1.72cm$ significantly increased. Conclusions: The combined treatment comprising needle-embedding therapy and acupuncture had a significant effect with regard to breast augmentation.

오행화침법(五行和鍼法)을 적용한 다발성 근염 치료 1례 (A Case Report on Polymyositis Treatment with Ohaeng-Hwa Acupuncture in Korean Medicine)

  • Objectives : Polymyositis is non-hereditary, autoimmune rheumatic disease, and one of the idiopathic inflammatory myopathy. It is characterized by progressive symmetrical proximal muscle weakness. Major clinical signs and symptoms of polymyositis are chronic non-suppurative inflammatation of skeletal muscle, elevation of muscle enzyme, and electromyographical changes. By use of Korean medicine treatments, muscle weakness and normalize muscle enzyme can be improved in case of polymyositis. Methods : A Patient, 32 years old, whom diagnosed with polymyositis on muscle biopsy and electromyographical changes, had medical treatments with Corticosteroids and Immunosuppressants, but no improvement was noted. She was treated with Korean Medicine, especially Ohaeng-Hwa Acupuncture(五行和 鍼法) and Ssanghwa-Tang(雙和湯) for three months period. Results : With Korean Medicine treatment of polymyositis for three months, most of her clinical signs and symptoms have been improved significantly, especially her muscle weakness and muscle enzyme level. Conclusion : This case has been reported to show that Korean medicine treatments have positive effects on polymyositis.

전신성 홍반성 낭창(SLE) 환자 1례 증례보고 (A Clinical Report of Systemic Lupus Erythematosus)

  • Objectives: The purpose of this study is to report the effect of Korean medicine treatments on a systemic lupus erythematosus (SLE). Methods: We treated 1 case of systemic lupus erythematosus patient with Korean herbal medicine, acupuncture, moxibustion and pharmaco-acupuncture. Patient is a 44-year-old woman, who suffered from general body weakness, arthralgia and foamy urine. She was treated with Samilshinki-hwan-gami and Oryeong-san-gami. Patient is taken acupuncture at GV20, CV12, LI4, LR3, ST36, BL23, BL25 and moxibustion at CV4. We injected pharmaco-acupuncture at BL23, BL25. Results: After treatments the patient recovered from foamy urine, reduced general body weakness, arthralgia. Conclusions: This study suggests that Korean medicine treatments have significant effect on reducing symptoms of systemic lupus erythematosus.

퇴행성 슬관절염의 한의학적 치료에 관한 연구동향 (Research Trends on the Treatment of Knee Osteoarthritis in Korean Medicine)

  • Objectives : To research trends of studies on treatments of knee osteoarthritis in Korean medicine. Methods : Domestic theses, published from 2000 to 2010, mentioning the treatments of knee osteoarthritis in Korean medicine were reviewed and analyzed. This study researched 79 theses and classified again according to the fields of the treatment. Results : The results were obtained as below ; 1. Studies on pharmacoacupuncture account for 41% of the total (32cases/79cases). The target of treatments had changed from pain alleviation and anti-inflammation to cartilage-protection since 2006. 2. Herbal Drug, Bee Venom, Cervi Cornu Parvum pharmacoacupuncture, Taping Therapy and Tai-Chi Exercise program were more effective than others. Moreover research on exercise program such as Tai-Chi has been increasing. 3. Some Herbal Drug (Glycyrrhizae Radix, Angelicae Gigantis Radix, Achyrantis Radix, Paeoniae Radix, Eucommiae Cortex and so on) were mainly used to investigate the effect on cartilage protection, pain relief and anti-inflammation in laboratory test. 4. Acupuncture including individualized, local acupoints and constitution acupuncture were effective than others in pain alleviation and symptom relief. 5. Pharmacoacupuncture Bee Venom were effective in recovery of joint function and pain relief while Buthus martensii Karsch, Homnis Placenta and Cervi Cornu Parvum were effective in cartilage protection and recovery. 6. Moxibustion was efficient to reduce joint pain and partly increase ROM (range of motion) of knee joint, but was not efficient to recover the function of knee joint. Conclusions : These results indicate that research trends on knee osteoarthritis were changed from symptomatic treatment (pain alleviation and anti-inflammation) to conservative treatment (cartilage protection and recovery).

월경통의 한의학적 치료에 대한 국내 임상 연구 경향 : 체계적 문헌 고찰 (Review of Domestic Clinical Research about Dysmenorrhea: A Systematic Review of Clinical Studies in Korean Medical Journals)

  • Objective: The aim of this review was to investigate clinical studies on Korean medical treatments for dysmenorrhea. Method: Using the keywords "dysmenorrhea," "Korean medical treatment," "acupuncture," and "herbal medicine," searches were conducted using domestic databases, including the National Discovery for Science Leaders (NDSL), the Research Information Sharing Service(RISS), and the Oriental Medicine Advanced Searching Integrated System (OASIS). The keywords were combined in various ways, instead of being searched individually. Papers that did not cover clinical studies or were not matched with the subject, absent of the abstract and text, were excluded. Results: Using the above searching method, 20 studies were found. Of these, 15 were in the form of case reports and five were in the form of randomized controlled trials. Acupuncture, herb medicine, pharmacopuncture, moxibustion, and cupping were used as treatments for dysmenorrhea. All of these studies reported that the Korean medical treatment was effective. Conclusion: According to the results, Korean medical treatments can provide an effective treatment for dysmenorrhea. However, more clinical studies on dysmenorrhea should be conducted for further clinical applications.

진정수면제를 음독한 노인 입원 환자에서 발생한 섬망에 대한 한의복합중재 증례보고 (Korean Medicine Treatments for Delirium in an Elderly Patient Caused by Sedative-Hypnotics Overdose)

  • Objectives: The purpose of this study was to report the effectiveness of the combination of Korean medicine therapy on elderly patients with delirium due to overdose of sedative-hypnotics. Methods: The patient was diagnosed with delirium, which occurred after an overdose of sedative-hypnotics. The patient received Korean Medicine treatment, including herbal medicine, acupuncture, moxibustion and psychotherapy, and other treatments. The evaluation variables to check the effectiveness of the interventions were the Korean Version of the Delirium Rating Scale-Revised-98 (K-DRS-R-98) Mini-Mental State Examination-Korean (MMSE-K) Clinical Dementia Rating (CDR) and Manual Muscle Test (MMT). Results: During the hospitalization period, the K-DRS-R-98 score decreased from 31 to 4, MMSE-K score increased from 18 to 26. CDR score decreased from 2 to 0. MMT in the Right lower extremity was improved. Normal daily activities were possible. Conclusions: Combining Korean Medicine treatments, including herbal medicine, acupuncture, moxibustion, psychotherapy, and other treatments, may help alleviate delirium.

The study of Sports Injuries treatment with Korean Medicine : A survey of World Martial Arts Masterships' athletes and officials

  • Objectives: The objective of this study was to report the general characteristics, satisfaction, and changes before and after treatment to foreign athletes and officials who were received Korean Medicine at the 2019 Chungju World Martial Arts Masterships Korean Medical Clinic. Methods: We investigated age, gender, nationality, type of sports, and, onset through medical records. Besides, we developed the patient satisfaction survey to determine how satisfied the patients were with the treatment they received. The survey included type of injury, location of the injury, cause of injury, Numeric Rating Scale (NRS) before and after treatment, and treatment satisfaction. Result: This study showed that knee and lower leg injuries (S80-S89) were the most frequent injuries in the Korean Medicine(KM) clinic of the 2019 Chungju World Martial Arts Masterships. We provided KM treatments including acupuncture, cupping therapy, Chuna manual therapy, herbal medicine, dressing, and taping. Acupuncture was the most frequently performed and the most satisfactory treatment. The average NRS after treatment significantly decreased, suggesting that KM is effective in controlling pain in sports injuries. The overall satisfaction was also high with KM treatments.
