전신성 홍반성 낭창(SLE)환자 1예(例)에 대한 증례 보고 (One Case of Systemic Lupus Erythematosus treated with traditional Korean Medicine)

  • After observing a patient diagnosed with Systemic Lupus Erythematosus from the September 21, 2000 to the January 13, 2001, the following results are obtained. Method and Result: We treated one case of Systemic Lupus Erythematosus with the Korean Bee Venom Therapy and the other Oriental Medical treatments. In acupuncture therapy, Korean Bee-venom Therapy is believed to be outstanding way to treat Systemic Lupus Erythematosus. Korean Bee Venom Therapy is treated on the following acupuncture points: ST36(B23(Shinsu:腎兪), BL26(Guanyuanshu:關元兪), ST36(Chok-Samni:足三里), LI4(Hapkok:合谷), LV3(Taechung), SP10(Hyolhae:血海) SP6 (Samumgyo;三陰交). In addition, CFC(Carthami Flos;紅花 and Cervi Pantotrichum Cornu;) Herbal-Acupuncture was applied on the acupucture points of GB20(Pungji: 風池), GB21(Kyonjong:), as well as BUM(It was made of Boviscalculus(牛黃), Moschus and Fel ursi Herbal-Acupuncture was given on CV17(Chonjung). In herbal medication, based on the Sasang Constitution, Taeyumin Chungsimyunja-Tang was given to the patient. As the results of these treatments, general conditions of this patient improved drastically. Conclusion: Based on the clinical results, traditional Korean Medical treatment is believed to be effective for treating Systemic Lupus Erythematosus, and futher studies should be carried out to provide more valuable information.

A Retrospective Chart Review of 122 Inpatients with Knee Osteoarthritis Treated with Korean Medicine: An Analysis of the Effects of Treatment

  • Background: Korean medicine treatment was assessed in patients with knee osteoarthritis (OA) according to subgroups of: sex, age, cause of knee OA, body mass index, hospitalization period, history, OA compartment, phenotype, and comorbidity. Methods: A retrospective review was performed of 122 inpatients who were admitted to the Hospital of Korean Medicine for Korean medicine treatment of knee pain, and were diagnosed with knee OA based on magnetic resonance imaging findings. Analysis of patient subgroups (sex, age, cause of knee OA, body mass index, hospitalization period, history, OA compartment, phenotype, and comorbidity) was carried out and treatments including acupuncture, cupping, pharmacopuncture, herbal medicine, chuna therapy, medicinal steaming therapy, manual therapy, and extracorporeal shock wave therapy were listed. The numeric rating scale (NRS), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), and 5-level EuroQol- 5 Dimension (EQ-5D-5L) scores were measured before and after treatment to assess the effects of treatment on pain and quality of life. Results: Seventeen males and 105 females were included in this study. Most patients were in their 60s. In the total study population, NRS, WOMAC, and EQ-5D-5L scores were improved statistically significant when comparing before and after treatment. The NRS and WOMAC scores improved statistically significant in the medial, patellofemoral, medial + patellofemoral, medial + lateral + patellofemoral compartment. Conclusion: Korean medicine treatment significantly reduced pain, stiffness, and physical dysfunction, and improved the quality of life of patients with knee OA, suggesting that it may be an effective alternative to the current conservative treatments.

퇴행성 슬관절염의 화침 치료에 대한 체계적 문헌 고찰 및 메타분석: 최근 5년간의 호침 치료 비교 연구를 중심으로 (A Systematic Review and Meta-Analysis of Fire Needling Treatment for Knee Osteoarthritis: Focused on Comparative Studies with Manual Acupuncture Treatment during Recent Five Years)

  • Objectives : The purpose of this study is to review the effectiveness of fire needling treatment for knee osteoarthritis in comparison with manual acupuncture treatment. Methods : Through four foreign online databases (PubMed, Cochrane library, EMBASE, and CNKI) and five domestic online databases (NDSL, RISS, KISS, OASIS, and KTKP), we searched for clinical studies that performed fire needling treatment for knee osteoarthritis until May 10, 2019. Only randomized controlled trials were selected and we assessed the risk of bias according to the Cochrane RoB criteria. This review examined the selected studies into first author, publication year, sample size, outcome measurements, results, acupoints, treatment time & period and so on. Results : A total of 7 RCTs were selected in this review and all were conducted in China. Treatment period of more than 4 weeks and treatment visits of 10 to 20 times were the most common. EX-LE4 and ST35 (=EX-LE5) acupoints were most frequently selected in treatment. Among the evaluation indexes, a total efficacy rate was used the most. Most of fire needling groups showed more significant results compared with the manual acupuncture groups statistically. Conclusions : All studies showed that fire needling treatments for knee osteoarthritis were more effective than manual acupuncture treatments statistically. Therefore, the results of this study could be utilized as a preliminary data for another clinical research on fire needling treatment for knee osteoarthritis. However, further well-designed randomized controlled trials will be needed to develop sufficient evidence about the effectiveness and safety of fire needling treatment for knee osteoarthritis in the future.

A Web-based Survey Research on Clinical Practice Patterns of Korean Medicine Treatment for Degenerative arthritis of hip and finger joints

  • Objectives: This study aimed to understand the current status of clinical practice patterns of Korean medicine in the treatment of degenerative arthritis of the hip and finger joints using web-based survey. Methods: An e-mail questionnaire survey was conducted with the Korean Medicine Doctor(K.M.D) registered in the Korean Medicine Association. The survey data were analyzed using Microsoft Excel. Results: Although the response rate of the survey was low, the characteristics of the respondents generally reflected the characteristics of the study population. A total of 35.2% of the respondents had heard of the guidelines; however, they did not use them for medical treatment. The reason as answered by 32.8% of the respondents was "not enough treatment time to check the manual and apply it." More than half (78.80%) answered that they were treating patients with both degenerative hip and finger arthritis; however, the average monthly number of first-time patients was ≤5. A total of 379 (60.9%) respondents answered that "acupuncture is very important," which was the only one that exceeded the majority compared to other treatments. As a result of the safety investigation, "Acupuncture, herbal medicine, cupping" were considered high in safety, while "Pharmacopuncture and Bee venom acupuncture" were considered low in safety. Conclusions: This survey helped to determine the current clinical practice patterns of Korean medicine treatments, further providing basic data for CPGs for degenerative arthritis of the hip and finger joints.

악관절장애(顎關節障碍)의 개구장애(開口障碍)와 통증(痛症)에 대한 첩대요법(貼帶療法)의 효과(效果) (The Effects of Spiral Taping Treatment on Temporomandibular Disorder)

  • Objective : The purpose of this study is to estimate the effects of spiral taping treatment on temporomandibular disorder. Methods : 22 temporomandibular disorder patients were treated with spiral taping, and no other treatments such as herbal medicine, acupuncture, herbal acupuncture, and chiropractic therapy were added. We evaluated the improvement by temporomandibular pain and mouth opening range. Results : 11 patients felt no pain or inconvenience of daily life and 4 patients showed improvement of pain or symptom after 1 month of spiral taping theraphy. 3 patients had normal opening size of mouth and convenience of daily life, and 2 patient showed improvement of opening size of mouth as 35mm-40mm after 1 month of spiral taping theraphy. Conclusion : These results suggest spiral taping treatments contribute to the improvement of temporomandibular disorders. Further study is needed for the confirmation of this effect of spiral taping treatments on temporomandibular disorders.

북한이탈주민의 심신증상에 대한 M&L 심리치료와 한의치료 치험2례 보고 (Case Reports: Effect of M&L Psychotherapy and Korean Medical Treatment on Psychiatric & Physical Symptoms of North Korean Defectors)

  • Objectives: This study was undertaken by the department of Korean neuropsychiatry, to evaluate the effects of M&L psychotherapy and Korean Medical Treatments on hospitalized North Korean defectors.Methods: We treated two patients with indigenous Korean medical therapies (acupuncture and herb-medicine) and M&L psychotherapy. Beck's Depression Inventory (BDI), Insomnia Severity Index (ISI), Geriatric Depression Scale (GDS), Mini-mental state examination Korean Version (MMSE-K), and Visual Analogue Scale (VAS) were used to evaluate the patients.Results: Following observations were made after treatments. Case 1: the patient had significant improvement in Beck's Depression Inventory (BDI), Insomnia Severity Index (ISI), and Visual Analogue Scale (VAS). Case 2: similar improvements were observed. Here, the patient had undergone Geriatric Depression Scale (GDS), Mini-mental state examination Korean Version (MMSE-K), and Visual Analogue Scale (VAS). In addition, noticeable improvement was observed in other accompanying symptoms.Conclusions: These results suggest that M&L Psychotherapy and Korean medical treatments might be effective for treating the psychiatric and physical symptoms of hospitalized North Korean Defectors.

양방 항암치료와 병행하여 산삼약침과 봉약침 치료를 받은 전립선암 환자들의 PSA 수치 변화 관찰 보고 (A case report of monitoring PSA level changes in two prostate cancer patients treated with Mountain Ginseng Pharmacopuncture and Sweet Bee Venom along with western anticancer therapy)

  • Objectives: The purpose of this report is to find out how Mountain Ginseng Pharmacopuncture(MGP) and Sweet Bee Venom(SBV) treatments are effective on prostate cancer patients by monitoring Prostate specific antigen(PSA) values. Methods: We treated two prostate cancer patients with MGP and SBV from October 2008 to April 2011. One patient had localized prostate cancer, the other was in the terminal stage of prostate cancer with lung and bone metastasis and both had been receiving western anticancer therapy. We had monitored the changes of PSA value. Results: In case 1, MGP and SBV treatments seemed to be helpful in preventing the recurrence of localized prostate cancer. In case 2, PSA value was decreased by MGP treatment. Conclusions: It is conceivable that MGP and SBV are effective treatments for patients with prostate cancer.

A Review of Research on Acupuncture and Moxibustion Treatment for De Quervain's Stenosing Tenosynovitis

  • Objectives : This study was performed to review articles concerning acupuncture and moxibustion treatment for De Quervain's Stenosing Tenosynovitis. Methods : On-line databases including Cochrane Library, Pubmed, CNKI, NDSL and OASIS were searched to identify articles concerning acupuncture and moxibustion treatment for De Quervain's Stenosing Tenosynovitis (DQST). Several duplicated articles and those not relevant to this topic were excluded, as were review articles and commentaries. Results : Fifty-one studies were identified, which included 28 clinical case studies with 1,227 patients and 23 randomized controlled trials with 2,040 patients. In these studies, acupuncture, acupotomy, moxibustion, pharmacopuncture, and laser acupuncture were used as DQST interventions. Although DQST is a common disease seen in clinical practice, only four of the 51 studies we identified in our search were published in Korean academic journals, all of which used pharmacopuncture. Conclusion : Although the results of the studies to date provide evidence that acupuncture and moxibustion are effective treatments for De Quervain's Stenosing Tenosynovitis, the absence of a standard, objective evaluation tool, and a lack of reporting on the negative side-effects associated with treatment remain important factors that should be addressed in future studies.

A Review of Research on the Treatment of Peroneal Nerve Palsy by Acupuncture and Moxibustion

  • Background: This was a retrospective review of published articles reporting acupuncture and moxibustion treatment of peroneal nerve palsy. Methods: On-line database searches were carried out using; Cochrane Library, Pubmed, CNKI, NDSL and OASIS to find articles reporting acupuncture and moxibustion treatment for peroneal nerve palsy. Duplicate articles and studies that were not relevant to the topic were excluded, along with review articles and commentaries. Results: 20 studies were selected, 18 clinical case studies (47 patients) and 2 randomized controlled trials (154 patients). Intervention treatments included acupuncture, moxibustion, bee-venom (BV), pharmacopuncture, electroacupuncture and acupotomy. Surprisingly, although peroneal nerve palsy is not a very rare disease, only 2 studies out of 20 carried out a randomized controlled trial. Conclusion: Although studies to date report the efficacy of acupuncture and moxibustion treatment in peroneal nerve palsy patients, the absence of objective evaluation and the absence in the reporting of side-effects remains an issue.

척추전방전위증의 한의학적 치료에 대한 국내 연구 동향 (The Domestic Trends of Korean Medicine Treatments of Spondylolisthesis)

  • Objectives The purpose of this study is to investigate the trends of Korean medicine treatments of spondylolisthesis. Methods We investigated the studies on Korean medicine treatments for spondylolisthesis via searching 4 Korean web databases (OASIS, KTKP, RISS, DBpia). We used "Spondylolisthesis", "Lumbar spondylolisthesis" as keyword. Limitations were as follows; Domestic studies, mentioning the treatments of spondylolisthesis in Korean medicine. This study researched and classified the papers according to type of study, published year, titles of journals, published institution, the number of cases, and types of treatments. Results 14 research papers were found to be analyzed according to their published year, the titles of journals, published institution, the types of study, the number of cases, and the types of treatments. 14 papers were published since 2000. The studies on Korean medicine treatments about spondylolisthesis were mainly published in the journal of Korean acupuncture & moxibustion medicine. 7 case reports, 6 case series, 1 RCT had been under research. In 14 cases of the studies, various Korean medicine treatments were used to treat the symptoms. Numeric rating scale (NRS) was used as primary assessments. Conclusions In this study, we analyzed the trends of Korean medicine treatments of spondylolisthesis. Reviewing the domestic trends of studies on Korean medicine treatments of spondylolisthesis and examining the strong and weak points of those treatments are essential for the further studies.
