침치료가 사용된 임상연구에서의 수기법 활용에 대한 고찰 (Review of Acupuncture Manipulation in Clinical Trials)

  • Objectives : This study aims to review the actual use of acupuncture manipulation in clinical trials and to discuss the directions of studies, to be conducted in the future. Methods : Through five search engines, clinical trial papers were collected, which had involved the use of acupuncture treatment since 2001 when STRICTA was published. The selected papers were classified based on whether acupuncture manipulation and De-Qi were stated. The comparison status of domestic and foreign, status by year, target diseases, and therapeutic interventions were also investigated. Results : 54 % domestic and 69 % foreign papers mentioned acupuncture manipulation or De-Qi. Among them, acupuncture manipulation was used in 40 % domestic and 53 % foreign papers. There was no significant difference around 2010 when the revised edition of STRICTA was published. The use of acupuncture manipulation was the highest in studies that targeted musculoskeletal disorders. Besides that, the use of acupuncture manipulation was higher in cases where acupuncture was used for therapeutic interventions than it was for basic manipulation or combination therapy. Conclusion : Compared to the importance of acupuncture manipulation and De-Qi, there was actually a smaller number of papers that clearly suggested information on acupuncture manipulation and De-Qi. The reason is considered to be a lack of studies to establish the mechanism and effectiveness of each acupuncture manipulation method and studies that objectified the amount of stimulation incurred by the acupuncture manipulation methods. It is necessary to properly utilize acupuncture manipulation by actively conducting these studies.

  • Objectives : Acupuncture manipulation, a kind of sophisticated hand movements, has been considered a fundamental skill for acupuncture practice. In this study, we aimed to develop acupuncture manipulation education system(AMES) using visual feedback of acupuncture manipulation. We also investigated whether or not acupuncture practice-$na{\ddot{i}}ve$ students could enhance their acupuncture manipulation skills after AMES training. Methods : Using AMES and motion sensor, we visualized a time-series motion template(intended motion) and participant's own motion(actual motion) manipulating an acupuncture needle. Ten students were trained with complex lifting/thrusting techniques for 8 training trials. We compared the motion pattern error of the students between the first and the last trials. Results : In our pilot experiment, half of the participants showed significantly improved manipulation skills in complex lifting/thrusting techniques after training with AMES which is developed in this study, while the other half of the participants did not show significant improvements. Conclusions : The AMES could be useful in acupuncture-manipulation training for students. Our findings suggest that novice can improve sophisticated hand movement for acupuncture manipulation with sensorimotor learning using visual feedback.

족관절 염좌에 대한 수기요법(手技療法) 복합치료가 미치는 영향 (Effect of Manipulation Complex Therapy on Ankle Sprain with Ankle Pain)

  • Objectives : This study was to evaluate the effectiveness of manipulation complex therapy on ankle sprain with ankle pain. Methods : We divided ankle sprain with ankle pain patient into 2 groups; one group combined manipulation therapy and acupuncture therapy, another group was only acupuncture therapy. To estimate the efficacy of treatment that applied for two groups, we used visual analog scale(VAS) and ankle-hindfoot scale(AHS). We compared the VAS score and AHS score of two groups statistically. Results : 1. As a result of evaluation by using visual analog scale(VAS) and ankle-hindfoot scale(AHS), treatment score at final was marked more higher than score before treatment on each groups. 2. treatment at final, acupuncture and manipulation therapy group had significant result on visual analog scale(VAS) and ankle-hindfoot scale(AHS) compared with acupuncture therapy group. Conclusions : Manipulation therapy can be used with acupuncture therapy for highly effective treatment for ankle sprain with ankle pain.

침자 보사에서 자침 자극 강도의 특성 (Characterizing the Stimulation Intensity in Acupuncture Manipulation Techniques for Tonification and Sedation Therapy)

  • Objectives : The purpose of this study was to characterize the stimulation intensity in acupuncture manipulation techniques for tonification and sedation therapy. Methods : To describe the level of stimulation used in acupuncture manipulation techniques for tonification and sedation therapy, we reviewed the acupuncture manipulation techniques described in classical medical textbooks. Results : Based on the patients' conditions and pattern identification, acupuncture manipulation strategies for tonification and sedation therapy were chosen. For example, the excess condition was treated with sedative therapy, and the deficiency condition was treated with tonification therapy. For tonification therapy, weak to modest stimulation was applied in acupuncture manipulation techniques, whereas intense stimulation was applied for sedative therapy. Even though the intensity of acupuncture stimulation was chosen based on the clinical examination, deqi sensation is a crucial component of acupuncture treatment, and during acupuncture practice, the practitioner should choose the right intensity of acupuncture stimulation based on deqi response in each patient. Conclusions : We concluded that the tonifying and sedative effects of acupuncture treatment are related to the stimulation intensity of acupuncture manipulation techniques. For individualized medicine, the right amount of acupuncture stimulation should be administered based on the patients' conditions and responses, such as deqi responses.

Quantitative study of acupuncture manipulation of lifting-thrusting using an needle insertion-measurement system in phantom tissue

  • Objectives: Quantification, objectification, and standardization of lifting-thrusting manipulation are important issues in traditional Chinese medicine (TCM). The purpose of this study was to quantitatively investigate the difference in the amount of stimulation according to range and frequency among parameters of lifting-thrusting manipulation with the use of a needle insertion-measurement system. Methods: For quantification of lifting-thrusting manipulation, an acupuncture needle insertion-measurement system was used in phantom tissue. The motor and force sensors of the needle insertion device were connected to the control software. This enabled operation of the lifting-thrusting manipulation and measurement of the acupuncture needle force. The measurement of the acupuncture needle force according to various frequencies (0.25, 0.50, 0.75, and 1 Hz) and ranges of movement (2, 4, 6, 8, and 10 mm) was repeated 10 times. Results: At a constant frequency of movement, acupuncture needle force according to range of movement (2, 4, 6, 8, and 10 mm) increased with increasing range of movement (p < 0.05). At a constant range of movement, acupuncture needle force according to frequency of movement (0.25, 0.50, 0.75, and 1.0 Hz) increased with increasing frequency of movement (p < 0.05). Conclusion: In this study, we conducted a quantitative comparison of the amount of stimulation according to range and frequency, the main parameters of lifting-thrusting manipulation, by using an acupuncture needle insertion-measurement system. Future studies on various manipulations and parameters are warranted to quantify and objectify the amounts of stimulation by acupuncture manipulation.

오행침법효과(五行鍼法效果)의 정량적(定量的) 연구(硏究) (The Quntitative Study of Effects Of O-Haeng Reinforcing-Reducing Manipulation Acupuncture Method)

  • It is very important to understand objectively the Qi variation induced by the reinforcing-reducing manipulation method in the therapy of acupuncture. The Qi variation in the meridian treated by O-Haeng Reinforcing-Reducing Manipulation Acupuncture Method and the recovery of Five Phase deviation were measured by choosing complex acupoints. By using O-Haeng Reinforcing-Reducing Manipulation Acupuncture Method, we increased or decreased the Qi of the phase which caused the unbalance state. We observed whether the Qi of the treated meridian can be increased or not and if the state of unbaIance can be recovered. To achieve the effect of reinforcing-reducing, we need a correct choice of treating method and a selection of a proper meridian in advance. We discovered that the effects of reinforcing-reducing by each manipulation method could be superposed each other, when two counteracting O-Haeng Reinforcing-Reducing Manipulation Acupuncture Method were treated at the left and the right side of human body. We found that the Qi variation of the traeted meridian, which was induced by O-Haeng Reinforcing-Reducing Manipulation Acupuncture Method, is lineary proportioned to the reduction of Five Phase deviation.

부침 이해를 위한 문헌 비교: 경근, 근막동통이론, 완과침 그리고 부침 (Comparative Literature Review of Floating Acupuncture: Compared to Meridian Muscle, Myofascial Pain Syndrome and Wrist-Ankle Acupuncture)

  • Objectives : Floating acupuncture (FA) is a kind of newly developed acupuncture technique that contains its own apparatus. The technique has unique points that the body of the acupuncture needle stays intradermal space and manipulation is performed by shaking the needle horizontally; after manipulation, part of the needle remained in the intradermal space for 2~3 days. FA is not a common acupuncture methodology while various clinical study claims its efficacy on musculoskeletal disorders. In this study, the authors aimed to enhance the use of FA by comparing related theories. Methods : The authors reviewed classics, books, and articles related to FA, Meridian Muscle (MM), Myofascial Pain Syndrome (MPS), and Wrist-Ankle Acupuncture (WAA), and compared its characteristics by related theory, related symptoms, apparatus, and performing procedures. Results : FA was related and had various common parts with MM, MPS, and WAA, however, there were unique parts in the manipulation, apparatus, and stimulation location. FA is based on MM and MPS which pointing 'ashi points' or 'trigger points' as a treating target while FA does not stimulate the points directly. FA also targets subcutaneous space by inserting the needle horizontally as WAA does, while FA remains the needle handle part in the subcutaneous area for 2~3 days for more stimulation. Conclusions : FA has a unique manipulation procedure and potential benefit for musculoskeletal disorders despite the crude theological base written by Fu himself. Thus, developing a new explanation and patient-friendly methodology/apparatus is required for further down-to-earth studies.

The Clinical Effect of Manipulation of Acupuncture to Shen-Men and Nei-Kuan on Autonomic Nervous Function of Healthy Subjects.

  • Objectives : With an assumption of the traditional oriental medical theory, we were to evaluate the effect of tonification/sedation-manipulated acupuncture on autonomic nervous activity in healthy subjects. Methods : This study is a randomized cross-over trial. We enrolled healthy subjects, and assigned them randomly into group A or group B. Group A received acupuncture therapy on Shen-Men (He-7) and Nei-Kuan (EH-6) with tonification-manipulation, while Group B received it with sedation-manipulation. Their heart rate variability was monitored for 2 hours. After 3 days of washout period, the subjects were crossed over to the other manipulation method, and the same procedures were performed. Results : There was no statistical significance in the change of RR, SDNN, LF, HF, and LF/HF ratio between the tonification-manipulated and the sedation-manipulated groups. However, HF curve in the tonification manipulated group showed increasing tendency for 15 minutes after acupuncture. Conclusions :We suggest that the traditional tonification-manipulated acupuncture on Shen-Men and Nei-Kuan could increase parasympathetic activity shortly after acupuncture, but further evaluation on a larger sample size is needed to confirm this suggestion.

심정격(心正格) 혈위(穴位)에 시술한 침자(鍼刺)와 침습(侵襲)및 비침습(非侵襲) 레이저침요법(鍼療法)이 뇌허혈(腦虛血)에 미치는 영향(影響)에 대한 비교(比較) 연구(硏究) (Comparative studies of effects of manual acupuncture, invasive laser acupuncture and laser skin irradiation at the Heart Jeonggyeok acupoint on the focal ischemia induced by intraluminal filament insertion in rats)

  • Objectives: The purpose of this study was to identify the effectiveness of neuronal activities for the acupuncture and laser acupuncture application. Methods: The subject were divided into 7 groups as control group without acupuncture, acupuncture treatment with tonify manipulation with the direction of channel at HT9, LR1(AT-A), acupuncture treatment with purge manipulation against the direction of channel at HT3, Kl10(AT-B), acupuncture treatment with tonify manipulation with the direction of channel at HT9, LR1 and purge manipulation against the direction of channel at HT3, KI10(AT-C), laser acupuncture treatment with red light 658 nm at HT9, LR1(LAT-A), laser acupuncture treatment with green light 532 nm at HT3, KI10(LAT-B), laser acupuncture treatment with red light 658 nm at HT9, LR1 and green light 532 nm at HT3, KI10(LAT-B). Antiapopotic effect of acupuncture was observed by Bax, Bcl-2 and cytochrome C. Neuroprotective effect of acupuncture was observed by cresyl violet and ChAT. Results: AT-A, AT-B, AT-C, LAT-A, LAT-B and LAT-C groups were significantly increased comparing the control groups in expression ChAT and in neuroprotective effect by cresyl violet. AT-A, AT-B, AT-C, LAT-A, LAT-B and LAT-C groups were significantly decreased comparing the control groups in expression Bax. AT-C, LAT-A, LAT-B and LAT-C groups were significantly increased comparing the control groups in expression Bcl-2. AT-A, AT-B, AT-C, LAT-A, LAT-B and LAT-C groups were significantly decreased comparing the control groups in Bax/Bcl-2 ratio. LAT-B and LAT-C groups were significantly decreased comparing the control groups in expression cytochrome C. Conclusions: The acupuncture with tonify and purge manipulation and laser acupuncture with red and green light could be effective for antiapopotic and neuroprotective effect in focal brain ischemia.

한조절(汗調節) 혈위(穴位) 합곡(合谷) 복류(復溜)의 보사(補瀉)에 따른 침자(鍼刺)가 지한(止汗)에 미치는 영향(影響) (The effect of acupuncture using tonification and sedation manipulation at LI4(Hegu) and KI7(Fuliu) on the control of sweating)

  • Objectives : The purpose of this study was to exam the effectiveness of acupuncture using the tonification & sedation manipulation for LI4(Hegu) and KI7(Fuliu) which was known to promote sweating for lack sweating and inhibit for excessive sweating. Methods : The subject of this study was 60 people who agreed with participating. The participants were divided into 6 groups as control group without acupuncture, acupuncture group with needle inserted perpendicularly at LI4, KI7 without manipulation(AT-A), acupuncture group with needle inserted and twisted at LI4 using sedation technique and KI7 using tonificaton technique(AT-B), acupuncture group with needle inserted and twisted at LI4 using tonificaton technique and KI7 using sedation technique(AT-C), acupuncture group with needle inserted and twisted at LI4 and KI7 using tonificaton technique(AT-D) and acupuncture group with needle inserted and twisted at LI4 and KI7 using sedation technique(AT-E). We obtained baseline data at a stable condition with the temperature of $25^{\circ}C$. After having the participant stay at $40^{\circ}C$ for 10 minute and treated with acupuncture for 10 minute, we measured the parameters such as sweating rate, water content in subcutaneous, water content in skin surface, systolic blood pressure, diastolic blood pressure, pulse rate, oxygen saturation and body temperature. Results : After the tonification & sedation manipulation at LI4 and KI7, the sweating rate was significantly decreased in AT-B, AT-D and AT-E groups. The water contents in skin surface was significantly decreased in AT-B and AT-E groups. The water contents in subcutaneous was significantly decreased in AT-B, AT-C, AT-D and AT-E groups. However, there were no significant changes in systolic blood pressure, diastolic blood pressure, pulse rate, oxygen saturation and body temperature. Conclusions : Acupuncture at LI4 and KI7 is effective in controling sweating, and the effect varies according to the tonification & sedation manipulation technique.
