A Review of Trends for Acupotomy (도침요법의 연구동향 고찰)

  • Objectives : The purpose of this review is to overview and evaluate the current state of the usage of acupotomy. Methods : Eight electronic databases(Science direct, Pubmed, five Korean databases, CKNI) were searched for relevant articles and data from January 1999 to May 2014. Results : Thirty-nine articles were included. Acupotomy were used in musculoskeletal diseases(n=34) and other diseases(n=5). Among musculoskeletal diseases, lumbar spine(n=12) neck(n=5) and shoulder(n=3) diseases were mostly often used. Most studies(n=21) had small sample size(under 10 people) and combined with other therapies(acupuncture, herbal medicine, physical therapy, etc.). Conclusion : Acupotomy appears to be effective and useful for musculoskeletal diseases and some other diseases. But there were a few clinical studies for acupotomy and had many limitations to judge its effectiveness. Further modified studies are required.

  • The purpose of this study was to review randomized controlled trials (RCTs) of pes anserinus tendinitis or bursitis (PATB) syndrome in the China National Knowledge Infrastructure database to investigate the efficacy of traditional Chinese medicine treatment for PATB syndrome. There were 20 RCTs published from 2001 to 2021 which were selected for analysis by publication year, number of samples, evaluation criteria, treatment duration, and treatment method. Out of the 142 retrieved RCTs, 20 were relevant to this review, and had performed Chinese medicine treatments including acupuncture (the most common treatment typically using acupoints SP10, ST35, SP9, and LR8), manipulation (typically using acupoints ST35, SP10, and SP9), and external application therapy (typically herbal medicine) in the treatment of PATB syndrome. Chinese medicine treatments were used widely in the treatment of PATB syndrome. We hope in the future, this review may initiate the development of treatments for PATB syndrome using Korean medicine.

The Comparison of Effectiveness between Acupuncture and Its Cotreatment with Wan-Gwa Acupuncture on the Treatment of Low Back Pain (요통 환자의 침치료와 완과침 병행치료에 대한 비교 연구)

  • Objectives : The purpose of this study is to investigate the effectiveness of treating low back pain patients with Wan-Gwa acupuncture. Methods : Clinical study was conducted to 42 patients who were treated in Dept. of Acupuncture and Moxibusion, Woosuk University Hospital from March 1 to December 31, 2010. Subject were randomly divided into two groups. In one group(control group), patients were treated with acupuncture. And in the other group(test group), patients were treated with both acupuncture and Wan-Gwa acupuncture. To estimate the efficacy of treatments, visual analog scale(VAS) and Oswestry disability index(ODI) were checked. 13 patients were excluded and we investigated 14 patients of control group and 15 patients of test group entirely. Results : 1. In both two groups, VAS and ODI of patients were decreased significantly in the statistics 2. In test group, ODI of patients were decreased more significantly in the statistics than ODI of patients in control group. Conclusions : We suggest that cotreatment of acupuncture and Wan-Gwa acupuncture on low back pain patients can be recommended as a useful therapy.

The follow-up study on patients of Amyotrophic lateral sclerosis after 1 year (근위축성 측삭경화증 환자 1년 후 추적 조사)

  • Objectives : This study was to investigate the effects of Oriental medical treatment on ALS. Methods : We investigated 12 ALS patients which were admitted to Gwang-Ju O.M.hospital for 3 months and follow-up at 8 months after discharge. All patients were treated by SAAM-acupuncture, herb medication, Bee venom Pharmacopuncture therapy, Needle-embedding therapy, etc and after discharge self-therapy at home. We evaluated patients using the Amyotrophic lateral sclerosis Functional Rating Scale-Revised(ALSFRS-R), Medical Research Council (MRC) Scale. Results : After 1 month, mean ALSFRS-R score of patients was $29.08{\pm}7.99$, 2 months $28.70{\pm}7.17$, 3 months $28.16{\pm}8.23$, 1 year $21.33{\pm}9.93$ and mean MRC Scale of patients was $25.34{\pm}8.45$, 2 months $25.34{\pm}8.45$, 3months $21.56{\pm}9.20$. But in both cases, the variation was not statistically significant. Conclusions : We think that the results of this case be a pilot study that proves the effect of Oriental Medical treatment on ALS.

  • Objectives : The Purpose of this study is to report in patient with common peroneal nerve palsy, who improved by oriental medical treatament. Methods : We Checked the temperature of a leg by Digital Infrared Thermal Imaging(DITI) at intervals of 10 days, angle of active dorsiflexion and range of active motion for estimating the improvement of symptoms. We used the Visual Analogue Scale(VAS) for estimating the degree of pain, too. Results : After 4 weeks treatment, the movement and power of ankle joint improved to nearly normal range. The degree of active dorsiflexion of the ankle increased from $-40^{\circ}\;to\;15^{\circ}$ and range of active motion increased from Gr III to Gr I. The difference of temperature between the both legs decreased remarkably. Conclusion : In this case we experienced improvement of symptoms by conservative oriental medical therapy, e.g acupuncture stimulation, herbal medication, physical therapy. It should be needed further investigation on common peroneal nerve palsy and its symptoms in order to set up a reasonable standard about a surgical operation.

Effect of Hapgok Needling and Bee Venom Acupuncture Complex Treatment on Patients with Carpal Tunnel Syndrome (합곡자(合谷刺)와 봉약침(蜂藥鍼)으로 치료한 수근관 증후군 환자 4례)

  • Objectives : This study was to observe the effect of Hapgok needling and Bee Venom Acupuncture on a patient with neuralgia caused by Carpal Tunnel Syndrome. Methods : The patient were treated by Hapgok needling and Bee Venom Acupuncture at acupuncture points following diagnosed as Carpal Tunnel Syndrome. Improvement of the symptoms was evaluated by VAS. Results : Carpal Tunnel Syndrome related symptoms of the patient were remarkably improved by Hapgok needling and Bee Venom Acupuncture therapy. Conclusions : These results suggested that Hapgok needling and Bee Venom Acupuncture should be one of the useful treatment methods for relieving the symptoms of Carpal Tunnel Syndrome.

Comparative Study of Acupuncture, Bee Venom Acupuncture, and Bee Venom Pharmacopuncture on the Treatment of Herniation of Nucleus Pulpous (요추 추간판 탈출증 환자의 침 치료와 봉독침, 봉약침 병행치료에 대한 비교연구)

  • Objectives : Herniation of Nucleus Pulpous (HNP) of Lumbar is one of the most common causes of low back pain. The aim of this study is to investigate the effectiveness of acupuncture, bee venom acupuncture (BVA), and bee venom Pharmacopuncture (BVP) therapy for HNP. Methods : We separated 35 patients with HNP of L-spine into three groups; Acupuncture treated group, BVA treated group, and BVP treated group, and monitored for 30 days. 4 grades of recovery degree, score of Visual Analog Scale (VAS), and Oswestry Disability Index (ODI) were examined for comparison. Results : 1. Visual Analogue Scale of low back pain and radiating pain showed significant decrease in BVA and BVP groups compared to the Acupuncture group at VAS0_2, VAS0_3 evaluations. 2. ODI of disability of daily activities showed significant decrease in BVA and BVP groups compared to the Acupuncture group at VAS0_1, VAS0_2, and VAS0_3 evaluations. 3. Compared to the Acupuncture group, BVA and BVP groups were improved in S.L.R.T and R.O.M of L-spine as confirmed by the physical examination. 4. Treatment efficacy was more apparent in acute cases as ODI was significantly lower than chronic cases. Conclusion : BVA and BVP therapies can be used in addition to the acupuncture therapy for highly effective treatment of HNP. Further clinical studies are required to verify these findings.

A Clinical Study on One Case of Exotropia Caused by Unilateral-internuclear- ophthalmoplegia due to Pontine Infarction (뇌교 경색에 의한 일측성핵간마비로 유발된 외사시 환자에 대한 증례보고)

  • Objectives : This study is to report one case of the patient who has exotropia caused by unilateral internuclear ophthalmoplegia due to pontine infarction, which is a rare disease so there has few research about the effect of acupuncture therapy on it, treated with the SAAM(舍巖) Acupuncture Sojangjeonggyeok(小湯正洛) Methods & Results : In the point of differentiation of Syndrome, these subjects were diagnosed as deficiency of Yin and blood(陰血不足). We treated her with SAAM Acupuncture Sojangjeonggyeok(小湯正洛), herb medication(淸肝湯, 六味地黃湯), and the symptoms were resolved clearly by above treatment during about a week. Conclusion : This case shows proper oriental medical treatment based on the exact differentiation of symptoms has good effect on this disease as one of conservative therapies.

The Study of Oriental Medicine Music Therapy Applied to the Acupuncture of Pathogenic Fire (화열병(火熱病) 침구치료 시 적용 가능한 한방음악치료에 관한 연구)

  • Objectives : The purpose of this study is to introduce oriental medicine music therapy during the acupuncture of pathogenic fire. Methods : We investigated the acupuncture points of pathogenic fire and the methodology of oriental medicine music therapy. Results : During the acupuncture of pathogenic fire, patients listen to proper music and after withdrawing of acupuncture, patients play the musical instrument. The patients with the case of fire due to affection by exopathogen, the method of dispelling superficial fire can be used through playing percussion instruments. The case of patients with the fire of five organ, the Five Phases music can be used properly. Conclusions : This study describes oriental medicine music therapy has a good possibility as a treatment of pathogenic fire with acupuncture.

The Influence of Moxibustion and Basic Compound Therapy on Peripheral Facial Paralysis (말초성안면신경마비(末梢性顔面神經麻痺) 구치료(灸治療) 및 복합치료(複合治療)가 미치는 영향(影響))

  • Thirty cases were observed among the peripheral facial paralysis patients who admitted. the Department of Acupuncture and Moxibustion of Seoul Oriental Hospital between Feb. 2nd, 1995 and Aug. 14th, 2000. These patients were divided into two groups; the first group was treated with indirect facial moxibustion., while the second with basic oriental-westem medicine treatment. Object : To improve the therapeutic rate of peripheral facial paralysis by applying basic oriental-western medicine treatment and indirect facial moxibustion. Method : Inpatients suffering from peripheral paralysis of facial nerve were divided into two groups; both groups recieved basic oriental-western medicine treatment; the experimental group additionally received indirect facial moxibustion onto Tuyu(頭維) and Chich'ang(地倉) once or twice daily. Result : In the therapeutic result from moxibustion treatment group, 3 excellent, 7 good, 3 fair, and 1 poor case were observed. From compound treatment group, 2 excellent, 5 good, 5 fair and 4 poor cases were observed. Among the excellent and good patients of both groups, therapeutic rate of moxibustion treatment group (71.4%) was higher than that of compound treatment group(43.8%). Conclusion : The overall therapeuric rate of the moxibustion treatment group was 92.8%, which was higher than that of the compound treatment group(75.1%).
