Objectives: The study aimed to identify the oral health factors that affect the nutritional status of the elderly. Methods: The study was conducted over ten months from September 2013 to June 2014, and included senior citizens who were supported by the visiting health service. The rate of saliva release, the number of remaining teeth, and the ability of the elderly to identify nutritional conditions were evaluated. Statistical analyses were performed using the t-test, ANOVA, and multiple linear regression using SAS 9.4 (SAS Institute Inc., Cary, NC, USA.). Results: The study participants had an average irritation saliva secretion rate of $2.26{\pm}1.11mg$ per minute. The higher the rate of saliva secretion, the higher the mini nutritional assessment (MNA) score (p<0.001). The average number of remaining teeth was $8.21{\pm}9.76$. The MNA scores were highest in groups with 11 or more remaining teeth (p=0.001). The factors that affected the nutritional condition of the elderly were their ability to perform activities of daily living, saliva flow rate, and number of remaining teeth. The highest correlation among them was that of the standardized regression coefficient was - 0.386 by activity daily living, followed by a 0.170 saliva secretion rate and 0.118 remaining teeth in daily life performance. Conclusions: Activities of daily living and rate of saliva secretion showed the highest correlations to nutritional status of the elderly.
Purpose: The aim of this study was to evaluate the effects of active mandibular exercise (AME) in patients with limited mouth opening after maxillomandibular fixation (MMF) release. Methods: The study used a quasi-experimental, nonequivalent control group and a pre test-post test design. Sixty-two patients with Maxillomandibular Fixation Release were assigned to the experimental (n=31) or control group (n=31). The AME was performed in the experimental group for 4 weeks. The exercise AME consisted of maximal mouth opening, lateral excursion and protrusive movement. These movements were repeated ten times a day. After the final exercise of the day, the number of tongue blades used for mouth opening was noted. The effect of AME was evaluated after MMF release at different time intervals: a) immediately, b) after 1 week, c) after 2 weeks, d) after 4 weeks, and e) after 12 weeks. The exercise was assessed using the following criteria: a) mandibular movements, b) pain scores associated with maximal mouth opening, c) discomfort scores associated with range of movement, and d) daily life activities that involve opening the mouth. Results: The experimental group showed significant improvement regarding the range of mandibular movements (maximal mouth opening (F=23.60, p<.001), lateral excursion to the right side (F=5.25, p=.002), lateral excursion to the left side (F=5.97, p=.001), protrusive movement (F=5.51, p=.001)), pain score (F=39.59, p<.001), discomfort score (F=9.38, p<.001). Daily life activities that involve opening the mouth were more favorable compared to those in the control group. Conclusion: The AME in patients after MMF release is helpful for increasing mandibular movement range, decreasing pain and discomfort, and improving day life activities that involve opening the mouth. Therefore, AME is highly recommended as an effective nursing intervention.
Objectives: The purpose of the study is to investigate the related factors of oral health-related quality of life in the severely disabled people. Methods: The subjects were 205 severe disabled people in Busan and Gyeongnam. Data were collected by direct interview with the severe disabled persons from June to August, 2011. The study instruments included oral health impact profile(OHIP)-14 and Korean instrumental activities of daily living(K-IADL). Data were analyzed by SAS version 9.2. The questionnaire consisted of eight questions of the general characteristics of the subjects, five questions of oral health related problem, seven questions of K-ADL, ten questions of K-IADL, fourteen questions of oral health related quality of life. Results: The severely disabled's IADL was $19.9{\pm}7.8$ and the oral health-related quality of life was $17.5{\pm}10.5$. In multiple regression analysis, oral health-related quality of life was closely related to multiple disorders and IADL. Conclusions: The oral health-related quality of life was poor in the severely disabled people. It is necessary to provide the severely disabled people with self-supporting tools that help ADL and IADL.
Purpose: The purpose of this study was to identify from the International Classification of Functioning model, factors influencing quality of life in elderly persons and to describe the concrete pathway of influence and the power of each variable. Methods: The sample included 334 elders who lived in 5 districts of D Metropolitan City. A structured questionnaire was used and the collected data were analyzed for fitness, using the AMOS 18.0 program. Results: This model was concise and extensive in predicting the quality of life of elders. The research verified the factors influencing quality of life for elders as direct factors such as activity of daily living (ADL) (${\beta}$=.13, t=2.47), leisure activity (${\beta}$=.55, t=5.04), social disengagement (${\beta}$=-.25, t=-2.25), and depression (${\beta}$=-.62, t=-10.86). Indirect factors including economic status (${\gamma}$=.17, p=.009), type of residence (${\gamma}$=.19, p=.004), ADL (${\gamma}$=.12, p=.027) were important factors in predicting quality of life for elders. These variables explained 75.6% of variance in the prediction model. Conclusion: The findings indicate a need for the nursing scientific community to develop intervention programs considering these variables to improve the quality of life for elders.
본 연구는 미충족 의료 경험과 삶의 질 관련 측정지표 간 인과관계를 실증하는 데 목적을 두었다. 이 두 변수간 인과관계 검증을 위해 본 연구는 한국의료패널의 부가조사 원자료를 가공하여 분석을 실시하였다. 삶의 질과 관련된 측정지표로는 운동능력, 자기관리, 일상 활동, 통증/불편, 불안/우울, 주관적 건강상태의 6개 변수를 투입해 선형회귀분석을 실시하였다. 본 연구의 분석 결과 미충족 의료 경험은 본 연구에 투입한 6가지의 삶의 질 하위변수 모두에 통계적으로 유의한 부(-)적 영향을 미치는 것을 확인할 수 있었다. 아울러 선형회귀분석을 통해 도출된 β값을 활용해 상대적 영향력의 크기를 검토한 결과 통증/불편, 주관적 건강상태, 불안/우울, 일상 활동, 운동능력, 자기관리의 순으로 차별적 영향 수준을 나타내는 것을 알 수 있었다. 이러한 분석 결과에 기초해 삶의 질적 수준 향상에 기여하기 위한 미충족 의료에 대한 정책 대응방안으로서 보건과 복지의 실무 연계 강화와 관련한 실무적 시사점을 제언하였다.
Purpose: The purpose of this study was the examination of the correlations between the daily physical pain intensity (DPPI) scale and the numeric rating scale (NRS), and between the DPPI scale and the quality of life (QOL) of short form health survey (SF-36) and beck depression inventory (BDI) questionnaires. The focus of the DPPI scale was the daily-living activities of the individual, and the scale contains three items regarding the pain per movement range, the pain per day, and the pain when touching the pain area. Methods: A total of 241 adults answered the DPPI, NRS, SF-36, and BDI questionnaires. Pearson's correlation coefficients were calculated for the various relations of the DPPI to the other scales. Results: High correlations were shown between the NRS and the DPPI (r=0.809, p<0.05). The DPPI scale (r=0.437, p<0.05) showed "moderate" significant correlations with the SF-36 and the NRS (r=0.370, p<0.05), and it showed "weak" significant correlations with the SF-36. There are no statistically significant correlations between the DPPI, the NRS score, and the BDI score (p>0.05). Conclusion: This study was the first attempt to establish the concurrent validity of a new focus on daily-living activities for the assessment of pain. This study showed promise for the development of activities of daily living focused tool for an assessment of the subjective pain in patients that was more objective.
기혼여성의 사회진출이 증대되고 있는 사회 현실속에서 취업주부는 사적${\cdot}$공적 영역에서 임금노동자로서의 역할과 가사 및 자녀양육이라는 역할을 동시에 수행해야 됨으로써 겪게 되는 부담이 크다. 이러한 취업주부의 문제점을 파악하고 대안을 모색하기 위해서는 일상생활 전반에 대한 고찰이 필요하다. 따라서 본 연구에서는 그들의 일상생활활동에 대한 일반적인 경향을 알아 본 다음, 일상생활에 큰 영향을 미치는 것으로 나타난 가족의 생애주기와 자녀와의 결합제약을 고려하여 일상생활활동의 유형을 구분하고 유형별로 전형적인 세대를 선정하여 세대 및 개인 속성과 관련하여 미시적이고 상세한 분석을 시도했다. 취업주부의 일상생활은 업무활동과 가사활동을 중심으로 이루어지기 때문에, 외출활동의 공간적 범위는 업무가 있는 날에는 직장주변이나 거주지 중심, 업무가 없는 날에는 거주지 중심 또는 친척이 거주하는 장소에서 이루어져 거주지 주변에서 대부분의 활동이 이루어지는 전업주부보다는 비교적 먼거리까지 활동이 이루어지나, 대체적으로 거주지 지향적 활동공간의 특성을 가진다. 이러한 결과는 사회구조적 제약뿐만 아니라, 직주간 거리 및 보육시설 입지 등과 같은 공간적 사상에 의해 더욱 가중되고 있음을 알 수 있었다.
도시교통 수요는 활동 수행으로부터 유발된다. 개인의 활동 의사결정에 의한 일상활동의 개인 간 총합은 집합적 공간행동으로 관찰되며, 활동간 서로 다른 공간의 극복을 위해 유발된 통행은 활동 간의 구조적인 상호연쇄관계에 의해 그 구체적 형태를 부여받는다. 개인의 하루 일상을 통한 시공간적 의사결정 및 사회적 실행과 사회 공간적 환경간의 상호작용을 탐구하는 활동기반접근법은 도시민의 일상과 통행을 분석하는데 중요한 이론 틀을 제공한다. 이 연구는 도시민의 일상활동을 활동기반접근법에 근거하여 대표적인 유형으로 분류하고, 분류된 유형의 프로파일과 관련 있는 활동 주체 특성과 활동 당시의 상황 특성을 분석하였다. 분석 결과 도시민의 일상활동은 소수의 대표적 활동패턴 집단으로 분류 가능하며, 각 집단의 특성은 다차원 프로파일에 의해 유의하게 요약되었다. 또한 각각의 프로파일은 서로 다른 사회경제적, 상황적 특성과 상관되어 있음을 확인하였다. 연구는 도시민의 일상활동 원리를 밝힘으로써, 도시교통 정책수단에 대한 도시민의 개별 반응 양식과 그 집합적 행동을 예측하기 위한 이론적 기초를 제공한다.
목적 : 본 연구는 지역사회 거주 노인의 인지기능, 일상생활활동, 참여 및 삶의 질의 관계를 확인하고자 하였다. 연구방법 : 지역사회 거주 노인 144명을 대상으로 인지기능, 일상생활활동, 참여 및 삶의 질을 측정하였다. 인지기능은 한국어판 간이정신상태검사, 일상생활활동은 한국형 일상생활활동 측정도구, 참여는 한국형 활동분류카드, 삶의 질은 노인 삶의 질 척도를 사용하였다. 결과 : 인지기능은 참여의 사회활동과 삶의 질 3개 영역(신체적 건강, 심리적 건강, 사회관계 및 경제수준)과 유의한 상관이 있었다. 일상생활활동은 참여(수단적 일상생활활동, 여가활동)와 삶의 질의 모든 하위영역에서 유의한 상관이 있었다. 참여의 하위영역들 중 수단적 일상생활활동과 여가활동은 삶의 질의 모든 하위영역과 상관이 있었고, 참여의 사회활동은 신체적 건강을 제외한 삶의 질의 모든 영역과 유의한 상관이 있었다. 여러 변인들 중 삶의 질과 가장 상관이 큰 변인은 참여의 여가활동인 것으로 나타났다. 결론 : 참여, 일상생활활동, 인지기능의 순서로 삶의 질과 상관이 있었고, 참여의 하위영역 중 여가활동이 삶의 질과 상관관계가 가장 큰 것으로 나타났다. 본 연구는 지역사회 거주 노인의 인지기능, 일상생활활동, 참여와 삶의 질의 상관관계를 확인하여 성공적인 노년기 삶을 위한 기초자료를 제공하였다는데 의의가 있다.
Being able to monitor the heart will allow the diagnosis of heart diseases for patients during daily activities, and the detection of burden on the heart during strenuous exercise. Furthermore, with the help of U-health technology, immediate medical action can be taken, in the case of abnormal symptoms of the heart in daily life. Therefore, it appears to be necessary to develop the corresponding technology to monitor the condition of the heart daily. In this study, a novel wearable smart system was proposed, to monitor the activity of the heart in daily life, and to further evaluate the rhythm of arrhythmia. The wearable system includes three modified bipolar conductive fiber electrodes in the chest part, which can resolve the reduction problem of the magnitude of the signal, by magnifying the signal and removing the noise, to obtain high affinity and validity for medical-type usage (<0.903%). The biological signal acquisition and data lines, and the signal processing engine and communication consist of a conductive ink, and the pic18 and ANT protocol nRF24AP2, respectively. The proposed algorithm was able to detect a strong ECG, signal and r-point passing over the noise. The confidence intervals were 96 %, which could satisfy the requirement to detect arrhythmia under the unconstrained conditions.
