In this study, we report the industrial application of an efficient technology for the recovery of gallic acid from Chinese nutgall processing wastewater. The recovery of gallic acid by industrial scale extraction and stripping devices was performed, with tributyl phosphate as the extractant and kerosene as the diluent. The results showed that the theoretical extraction stage was four, while the theoretical stripping stage was two. A closed-cycle system was studied for the continuous countercurrent extraction and stripping, with a five-stage extraction device and a three-stage reflux stripping device. The results showed that the multistage extraction-stripping system could steadily run for a long period, the average gallic acid level in the raffinate was $0.85g{\cdot}L^{-1}$, and the gallic acid content recovered in the strip liquor was higher than $120g{\cdot}L^{-1}$. The average extraction yield of gallic acid was 94.14%. When the strip liquor was used as raw material for production, the average production yield increased by 8.64%. In addition, after extraction, the $COD_{Cr}$ in the wastewater decreased by 38.19%, and the biodegradability of wastewater improved by 1.6 times. This study provided a new impetus for the sustainable development of the Chinese nutgall processing industry.
Objectives : This study presents a high performance liquid chromatography methods for the quantitative and qualitative analysis of rosmarinic acid (RA) and caffeic acid (CA) in Perilla frutescens var. japonica and var. acuta. Methods : Chromatographic separation was performed using a mixture of methanol, water and formic acid (35 : 64.2 : 0.8) with a reversed-phase column (Gemini C18, 4.6 ${\times}$ 150 mm, 3 ${\mu}m$). The analyses were detected at UV (280 nm). Results : The samples were extracted with 50% EtOH under reflux for 1 h, and simultaneous determination for RA and CA in hyang-so-san and haeng-so-san was possible without interference peaks Conclusions : According the results, we developed a determination method for RA and CA in Perillae Folium. Owing to Perilla frutescens var. japonica and var. acuta did not show significant difference in contents of RA and CA, both Perilla frutescens could be available as herbal medicine.
Proton pump inhibitors (PPIs), a potent gastric acid inhibitor, are widely used in gastric acid-related diseases such as gastroesophageal reflux disease and peptic ulcer, and are known as the most frequently used drugs worldwide. However, as the frequency of use increases, the number of cases of long-term PPI therapy without clear indications is increasing. Recently, there have been concerns about the risk of gastric cancer in patients with long-term PPI users. Potential mechanisms for the association between PPI and gastric cancer include enterochromaffin-like cell proliferation due to hypergastrinemia caused by gastric acid suppression, progression of atrophic gastritis, and corpus-predominant type through interaction with Helicobacter pylori (H. pylori) infection. Several epidemiologic studies showed controversial results on the issue, and it is difficult to prove a causal relationship between PPI and gastric cancer. Nevertheless, long-term PPI should be administered cautiously based on individual risk-benefit profile, specifically among those with history of H. pylori infection, in high-risk region of gastric cancer.
Esophagocardiomyotomy [modified Heller procedure] is a widely accepted operation for relief of dysphagia in patient with esophageal achalasia. But patients with advanced achalasia were more likely to get poorer results from a modified Heller myotomy because of the dependent pouch that creates an angulation at the junction of thick-walled dilated esophagus with the thin wailed aganglionic segment and hinders complete emptying. Thorbjarnarson[1975] proposed the method including truncal vagectomy and pyloroplasty. Vagectomy and pyloroplasty should lesson the severity of acid-peptic esophagitis, if reflux should occur postoperatively. Here we presented 4 cases esophageal achalasia treated by modified Heller operation of 3 cases and one case of Thorbjarnarson method. All postoperative results are good.

Introduction: This study investigated whether renal and bladder ultrasonography (RBUS) findings performed in children with the first incidence of febrile urinary tract infection (UTI) can predict UTI recurrence, high-grade vesicoureteral reflux (high-grade VUR), or acquired renal scarring (aRS). Methods: In all, 917 children who were admitted to our hospital from January 2001 to October 2010, owing to the first incidence of febrile UTI were enrolled in this study. All children underwent RBUS during admission. The mean follow-up was 7.9 months (standard deviation $[SD]{\pm}13.3$). UTI recurrence rates were calculated according to various clinical parameters. By using bivariate and multiple logistic regression analyses, we determined whether age, sex, abnormal RBUS findings, abnormal dimercaptosuccinic acid renal scan findings, or RBUS findings parameters were predictive of UTI recurrence, high-grade VUR, or aRS. Results: On RBUS, hydronephrosis and congenital anomaly of the kidney and urinary tract significantly predicted UTI recurrence. A small kidney, hydroureter, hydronephrosis, cortical thinning, and increased parenchymal echogenicity significantly predicted high-grade VUR. However, their odds ratios (OR) are low compared to normal RBUS findings (recurrent UTI: OR 0.432 and 0.354 vs. 0.934, respectively, high-grade VUR: .019, 0.329, 0.126, 0.058, and 0.188 vs. 2.082, respectively). No RBUS findings significantly predicted aRS. Recurrent UTI, high-grade VUR, and abnormal RBUS findings significantly predicted aRS (OR of 4.80, 4.61, and 2.58, respectively). Conclusion: RBUS is necessary to exclude severe congenital renal scarring, obstructive uropathy, and renal abscess at the first incidence of febrile UTI and is helpful in determining the need for subsequent clinical imaging.

Purpose: This study aimed to investigate clinical and radiological factors that may predict high-grade vesicoureteral reflux (VUR) in patients with febrile urinary tract infection (UTI). Methods: We retrospectively analyzed medical records of 446 patients diagnosed with febrile UTI from March 2008 to February 2017. All patients underwent renal-bladder ultrasonography (RBUS), 99mTc dimercaptosuccinic acid (DMSA) renal scan, and voiding cystourethrography (VCUG), and were divided in to 3 groups: a high-grade VUR group (n=53), a low-grade VUR group (n=28), and a group without VUR (n=365). Results: The recurrence and non-Escherichia coli infection rates in febrile UTI were significantly higher in the high-grade VUR group than in the other two groups (P<0.05). RBUS showed that hydronephrosis and ureter dilatation were more frequent in the high-grade VUR group than in the other groups (P<0.05). In the high-grade VUR group, a renal cortical defect was more likely to appear as multiple defects, and the difference in bilateral renal scan uptake between both kidneys was larger than in the other two groups (P<0.001). Conclusion: Recurrent UTI, non-E. coli UTI, abnormal findings on RBUS such as hydronephrosis and ureter dilatation, and abnormal findings in the DMSA renal scan such as multiple renal cortical defects and greater uptake difference were associated with high-grade VUR. VCUG should be selectively performed when RBUS and/or DMSA renal scan reveal significant abnormalities.
Objectives: Wild ginseng pharmacopuncture is widely used in oriental medicine. However, there is no standard method for efficiently extracting the active ingredient. In this study, in order to determine an efficient extraction method, wild ginseng was extracted by the distillation and 70% ethanol reflux methods, respectively. In comparing each extract, the index compounds were analyzed, and antioxidant activity was measured. Methods: The index compounds, ginsenoside Rg1 and ginsenoside Rb1, were detected using high performance liquid chromatography (HPLC). Antioxidative activities of total phenolic compounds, DPPH (${\alpha}$, ${\alpha}$-diphenyl-${\beta}$-picrylhydrazyl), ABTS (2,2'-azino-bis(3-ethylbenzothiazoline-6-sulphonic acid)) and FRAP (ferric reducing antioxidant power) were measured to compare their bioactivities. Since saponin is known to be hemolytic, the hemolytic activity of each extract was compared. Results: The index compounds were analyzed. Nothing was detected in the wild ginseng distilled extracts (WGDE). In the wild ginseng 70% ethanol reflux extracts (WGEE), ginsenoside Rg1 was 3.66 mg/g, and ginsenoside Rb1 was 16.70 mg/g. WGEE showed higher levels than WGDE in all antioxidative activities. In the hemolytic test, the extracts showed almost no toxicity, but WGEE showed lower toxicity than WGDE. Conclusions: In this study, it was concluded that WGEE is more advantageous than WGDE in the detection of index compounds and bioactivity. However, additional studies of additional extraction methods and other bioactivity tests are needed.
Objectives: The purpose of this study was to investigate the efficacy of Lijin-tang-gamibang on refractory gastroesophageal reflux disease (GERD). Methods: This before-and-after study compared the first medical examination and examination at the end of medical treatment. Twelve refractory GERD patients who visited the Department of Digestive Diseases of Kyung Hee University Korean Medicine Hospital from August 14th, 2017 to October 14th, 2017, were treated with Lijin-tang-gamibang for four consecutive weeks. The clinical characteristics of refractory GERD and the efficacy of herbal medical treatment was assessed by questionnaires, the Nepean Dyspepsia Index-Korean version (NDI-K), the Korean Gastrointestinal Symptom Rating Scale (KGSRS), the Numerical Rating Scale (NRS), the Korean version of the Perceived Stress Scale (KPSS-10), the Qi Stagnation Questionnaire (QSQ), the Spleen Qi Deficiency Questionnaire (SQDQ), and the Patient Global Impression of Change (PGIC). Prolonged effects were reported upon the follow-up telephone survey two weeks after treatment. Results: After treatment with the herbal medicine Lijin-tang-gamibang, the clinical characteristics of 12 refractory GERD patients were improved, especially in terms of the most common symptoms of acid regurgitation and heartburn. Each symptom score of NDI-K, KGSRS, NRS, SQDQ, and PGIC showed significant advances. Prolonged effects were reported in NDI-K, NRS and PGIC questionnaires two weeks after treatment. Conclusions: These results suggest that Lijin-tang-gamibang is an effective treatment for refractory GERD.
영아때 방광요관역류 환아로 진단된 남아중 2세가 넘어 후천적으로 신반흔이 급격히 악화되는 경우는 드물다. 본 증례는 생후 4개월 때 첫 열성 요로감염으로 입원하여, 왼쪽 5등급 방광요관역류로 진단되었고, 부모가 수술을 원치 않아 2년간 예방적 항생제 요법만 시행받았다. 생후 29개월 때 열성 요로감염이 한 차례 재발하였고, 이후 3년간 추적관찰되지 않았다. 이 후, 5세 때 본원을 방문하여 신장초음파, DMSA 신스캔, 배뇨성 방광요도조영술을 재시행받았다. 추적 DMSA 신스캔에서 왼쪽 신상대섭취율이 생후 2세경 38%였으나, 8%까지 떨어졌으며, 왼쪽 방광요관역류는 4등급으로 남아있었다. 현재 방광요관역류의 수술적 요법이 신기능의 예후와 관련이 없다고 알려져 있으나, 이런 증례를 볼 때, 아직은 신반흔을 동반한 고등급의 방광요관역류 환자들에게 수술적 요법을 권유하는 것이 임상의들에겐 타당하다고 볼 수 있으며, 어떤 치료요법이 선택되어지든 상관없이 첫 요로감염 이후 환자를 장기적인 추적 관찰하는 것이 중요하다고 사려된다.
