Subjective and Functional Results after a Proximal Gastrectomy: Prospective Study for Comparison of Reconstruction Procedures (근위부 위절제술 후 재건 술식에 따른 경과 관찰)

  • Purpose: A proximal gastrectomy is performed for gastric cancer in the upper part of the stomach to preserve the function of the stomach after surgery. An esophagogastrostomy is one of the common reconstruction methods for a proximal gastrectomy, but this method results in a high incidence of reflux esophagitis. This study was undertaken to compare subjective and functional results between esophagogastrostomy and jejunal interposition reconstructions. Materials and Methods: From June 1998 to December 2002, proximal gastrectomies were performed in 33 patients with tumors in the upper third of the stomach; 8 had reconstruction using jejunal interposition between the esophagus and the remnant stomach (JI group) while 25 had reconstruction using esophagogastrostomy (EG group). The postroperative courses of the patients were reviewed in terms of symptoms, weight changes, and endoscopic findings. Results: The mean age of the patients was 59 years; 26 were men and 7 were women. There were no significant differences in general complications, operating times, or histologic features between the two groups. Fifty-two percent (52%) of the EG group complained of dysphagia, and 16% them experienced heartburn and acid belching. Twelve percent (12%) of the JI group complained of dysphagia, but heartburn and acid belching did not occur. Incidences of reflux esphagitis (36%) and balloon dilatation for anastomotic stricture (16%) were more common in the EG group than in the JI group (0% and 12%). Conclusion: To prevent or minimize complications, such as reflux esophagitis and postoperative symptoms, a proximal gastrectomy with a jejunal interposition is an alternative method as an organ-preserving surgical strategy to improve quality of life for patients. Antioxidant Effect according to Extraction Method in Extracts of Dendranthema zawadskii var. yezoense and Cosmos bipinnatus (추출 방법에 따른 남구절초와 코스모스 추출물의 항산화 효과)

  • Top parts of $Dendranthema$ $zawaskii$ var. $yezoense$ (DZY) and $Cosmos$ $bipinnatus$ (CB) are believed to contain substances with antioxidant effects and activity. Present experiments were undertaken to investigate an adequate and efficient extraction method which ensures the highest yield of antioxidant components. Extraction yield was 1.11-1.92 times higher by ultrasonic method than other methods in both species. By 45 minute ultrasonic extraction, total polyphenol contents reached the highest level, $80.70mg{\cdot}g^{-1}$ tannic acid equiv./DW in $Cosmos$ $bipinnatus$ and total flavonoid contents $50.41mg{\cdot}g^{-1}$ naringin equiv./DW in DZY. DPPH radical scavenging activity was almost the same among extraction methods or slightly higher in reflux extraction. The highest DPPH radical scavenging was observed by reflux extract in CB ($RC_{50}=0.135mg{\cdot}mL^{-1}$). ABTS radical scavenging activity was the highest by 15 minute ultrasonic wave in DZY and CB, $RC_{50}=0.159mg{\cdot}mL^{-1}$ and $RC_{50}=0.139mg{\cdot}mL^{-1}$, respectively. High ferrous ion chelating effects were obtained by 30-minute ultrasonic wave in DZY ($RC_{50}=0.803mg{\cdot}mL^{-1}$). Extracts of top parts of DZY obtained by reflux method showed the highest inhibition effect against peroxidation of linoleic acid, and extract of CB obtained by ultrasonic for 45 minutes showed the highest inhibition effect. In conclusion, optimum extraction method should be adjusted according to plant species and antioxidant substances.

  • In this paper, optimum extraction condition for berberine which is the indan companent of Coptidis rhizoma was examined at various conditions in the aspect of temperature, solvent and time followed by quantitation with reversed phase HPLC. The efficiency for extraction has been shown the best at $60^{\circ}C$ with 2~3 hours reflux time in methanol or acetic acid. Any significant effect can not be shown from the addition of SLS to extraction solvent.

  • Variable systemic diseases affect larynx and vocal fold and result in voice change. Asthma and chronic obstructive pulmonary disease make increase of intra-abdomimal pressure followed by reflux of gastric acid, which stimulate vagal-bronchopulomary reflex aggravating cough and respiratory disturbance. Fungal laryngitis in the general population is extremely rare, but can occur in immunocompromised AIDS patients. Although, initially, empirical antifungal therapy for candidiasis is often given without biopsy, diagnostic direct laryngoscopy and biopsy is imperative if a substantial clinical response is not rapidly achieved. In the highly active anti-retroviral therapy era, HIV-positive patients are living longer and are at higher risk for developing non-AIDS-defining malignancies. The incidence of head and neck cancer (HNC) which is related with human papilloma virus infection has increased. The survival is significantly lower among the AIDS-HNC patients with CD4 counts ${\leq}200cells/{\mu}L$. Rheumatoid arthritis (RA) cause voice disturbance by developing cricoarytenoid joints fixation or nodule on vocal fold. Post-menopausal voice disorder (PMVD) is caused by decreased secretion of estrogen-progesterone resulting in decrease of fundamental frequency (F0). Hormonal replacement therapy is helpful to reduce F0 decrease. RA and PMVD result in slight voice change, but it could crucial in professional voice user.

  • Although asymptomatic bacteriuria, cystitis, and acute pyelonephritis (APN) have been categorized as urinary tract infections (UTIs), the immunopathogenesis of each disease is different. APN shows an age predilection; the majority of children (over 70-80%) with APN are under 1-2 years of age, with a male predominance. After 1-2 years of age, female predominance has been reported. This finding suggests that the immature immune state of infancy may be associated with the pathogenesis of APN. Escherichia coli is the most common etiologic agent; other uropathogens associated with UTIs originate from the host and comprise normal flora that are continuously altered by environmental factors. Therefore, uropathogens may have characteristics different from those of extraneous bacterial pathogens. Although antibiotic-resistant uropathogens, including extended-spectrum beta-lactamase-producing strains, are increasing in Korea and worldwide, treatment failure is rare in immune-competent children. The immunopathogenesis of APN remains unknown. Intact bacteria may not be the causative substances in renal cell injury; rather, smaller substances produced during bacterial replication may be responsible for renal cell injury and scarring. Moreover, substances from host cells such as proinflammatory cytokines may be involved in renal cell injury. A dimercaptosuccinic acid scan is used to detect the site of bacterial replication in the renal parenchyma, and may be influenced by the size of the focus and the stage of APN. Traditional aggressive studies used to identify vesicoureteral reflux after the first episode of APN have been modified because of rare cases of chronic kidney disease in patients with recurrent UTI.

Optimization of biodiesel production via methyl acetate reaction from cerbera odollam

  • Cerbera Odollam (sea mango) is a proven promising feedstock for the production of biodiesel due to its high oil content. Fatty acid methyl esters (FAME) were produced as the final reaction product in the transesterification reflux condensation reaction of sea mango oil and methyl acetate (MA). Potassium methoxide was used as catalyst to study its reacting potential as a homogeneous base catalyst. The initial part of this project studied the optimum conditions to extract crude sea mango oil. It was found that the content of sea mango sea mango oil was 55%. This optimum amount was obtained by using 18 g of grinded sea mango seeds in 250 ml hexane. The extraction was carried out for 24 hours using solvent extraction method. Response surface methodology (RSM) was employed to determine the optimum conditions of the reaction. The three manipulated variables in this reaction were the reaction time, oil to solvent molar ratio, and catalyst wt%. The optimum condition for this reaction determined was 5 hours reaction time, 0.28 wt% of catalyst and 1:35 mol/mol of oil: solvent molar ratio. A series of test were conducted on the final FAME product of this study, namely the FTIR test, GC-FID, calorimeter bomb and viscometer test.

DISSOLUTION AND BURNUP DETERMINATION OF IRRADIATED U-Zr ALLOY NUCLEAR FUEL BY CHEMICAL METHODS

  • Destructive methods were used for the burnup determination of U-Zr alloy nuclear fuel irradiated in the High-flux Advanced Neutron Application Reactor (HANARO) at KAERI. The dissolution rate of unirradiated U-Zr alloy fuel in $HNO_3$/HF mixtures was investigated for the experimental conditions of a different temperature, and initial concentrations of HF and $HNO_3$. The irradiated U-Zr alloy fuel specimen was dissolved in a mixed acid condition of 3 M HNO3 and 1 M HF at $90^{\circ}C$ for 8 hours under reflux. The total burnup was determined from measurement of the Nd isotope burnup monitors. The method includes U, Pu, $^{148}Nd,\;^P{145}Nd+^{146}Nd,\;^{144}Nd+^{143}Nd$ and total Nd isotopes determination by the isotope dilution mass spectrometric method (IDMS) using triple spikes $(^{233}U,\;^{242}Pu\;and\;^{150}Nd)$. The effective fission yield was calculated from the weighted fission yields averaged over the irradiation period. The results are compared with that obtained by the destructive -spectrometric measurement of the $^{137}Cs$ monitor.

  • The inertness of eight-coordinate tetrakis(picolinato)tungsten(IV) complex has been investigated by spectroscopic method in nonpolar solvent at 60$^{\circ}C$, 75$^{\circ}C$, 90$^{\circ}C$, 120$^{\circ}C$, and reflux condition. Until 2 days, showed the inertness whether existance of oxygen or not. After 2 days, showed the oxidation of W(IV) complex much more rapidly than substitution, which had $k_{obs}(sec^{-1}) {\varpropto} 1/[ligand]^2$. On sealing the sample under Ar gas, showed the inertness until 50 days, which had $k_{obs}(sec^{-1}) = 8.8{\times} 10^{-7}$. The$^1H$-NMR peak of $H_6$ in complex was diminished, otherwise that of free ligand was grown up. This result indicated that this complex was very inert.

Epidemic Trends of Upper Gastrointestinal Tract Abnormalities: Hospital-based study on Endoscopic Data Evaluation

  • Purpose: To understand the epidemiology of different upper gastrointestinal (UGI) tract related abnormalities through endoscopic data analysis. Materials and Methods: A retrospective study of three years from January 2009 to December 2011 was conducted with data from endoscopic surveillance of upper GI tract problems, collected from the Gastroenterology Unit, Osmania General Hospital, Hyderabad. MS excel and Medcalc software (comparison of proportions) were used for data analysis. Results: A total of 10,029 (6,468 in males and 3,561 in females) endoscopies were performed during this three-year period. The male to female ratio was 1.8:1. Overall, ~30% of endoscopies evaluated showed patients with acid peptic disorders, 13.6% with vascular-related abnormalities, 10.6% showed structural abnormalities, followed by 6.3% with malignancies. Burden of malignancies was mostly observed in the older age group (60-69 years). Esophageal cancer cases decreased (p=0.0001) whereas stomach cancers increased over this period (p=0.0345). We also observed an increased incidence of acid peptic disease (APD) (p=0.0036) and gastroesophageal reflux disease (GERD) (p=0.0002) cases during this period. Conclusions: Endoscopic diagnosis is useful for early detection of UGI anomalies and helpful for physicians to manage and treat varied kinds of UGI disorders. Analysis of data revealed changing trends in the incidence of various pathologies of the UGI tract. Functional dyspepsia and GERD definitely reduce the quality of life of the individual. The role of our diverse dietary habits and lifestyle associated with these problems have not yet been established, though there have been reports on the effect of coffee, spicy food, wheat-based diet, screening of UGI pathologies along with collection of complete personal and medical history details, can h elp in correlating the patients' condition with various aspects of lifestyle and diet.

Synthesis and Biological Activities of Some New 3,6-Disubstituted 1,2,4-Triazolo[3,4-b]1,3,4-thiadiazole Derivatives

  • A series of aromatic hydrazides 3a-j were prepared by refluxing esters 2a-j with hydrazine hydrate in methanol, which were prepared by the esterification of 1a-j. Acetohydrazides 3a-j upon treatment with carbon disulfide and methanolic potassium hydroxide yielded potassium dithiocarbazate salts 4a-j, which on refluxing with hydrazine hydrate yielded substituted 4-amino-5-aryl-3H-1,2,4-triazole-3-thiones 5a-j. The target compounds 6a-j were synthesized by condensing furan-3-carboxylic acid in the presence of polyphosphoric acid under reflux. The structures of newly synthesized compounds were characterized by IR, $^1H$ NMR, $^{13}C$ NMR, elemental analysis and mass spectrometric studies. All the synthesized compounds were screened for their urease, acetylcholine esterase inhibition, antioxidant and alkaline phosphatase inhibition activity. Almost all of the compounds 6a-j showed good to excellent activities against urease and acetylcholine esterase more than the reference drugs. Compounds 6f and 6g were more potent scavenger of free radicals than the reference n-propyl gallate. Compound 6b and 6h showed excellent activities of alkaline phosphatase as compare to the reference $KH_2PO_4$.
