Clinical and Radiologic Outcomes of Acute Acromioclavicular Joint Dislocation: Comparison of Kirschner's Wire Transfixation and Locking Hook Plate Fixation

  • Background: Kirschner's wire (K-wire) transfixation and locking hook plate fixation techniques are widely used in the treatment of acute acromioclavicular joint (ACJ) dislocation. The purpose of this study was to compare the clinical and radiologic outcomes between K-wires transfixation and a locking hook plate fixation technique. Methods: Seventy-seven patients with acute ACJ dislocation managed with K-wire (56 shoulders) and locking hook plate (21 shoulders) were enrolled for this study. The mean follow-up period was 61 months. Results: At the last follow-up, the shoulder rating scale of the University of California at Los Angeles (UCLA) was higher in patients treated with locking hook plate than with K-wires ($33.2{\pm}2.7$ vs. $31.3{\pm}3.4$, p=0.009). In radiologic assessments, coracoclavicular distance (CCD) (7.9 mm vs. 7.7 mm, p=0.269) and acromioclavicular distance (ACD) (3.0 mm vs. 1.9 mm, p=0.082) were not statistically different from contralateral unaffected shoulder in locking hook plate fixation group, but acromioclavicular interval (ACI) was significant difference. However, there were significant differences in ACI, CCD, and ACD in K-wire fixation group (p<0.001). Eleven complications (20%) occurred in K-wire transfixation group and 2 subacromial erosions on computed tomography scan occurred in locking hook plate fixation group. Conclusions: ACJ stabilization was achieved in acute ACJ dislocations treated with K-wires or locking hook plates. Locking hook plate can provide higher UCLA shoulder score than K-wire and maintain CCD, and ACD without ligament reconstruction. K-wire transfixation technique resulted in a higher complication rate than locking hook plate.

Estimation of spatial autocorrelation variations of uncertain geotechnical properties for the frozen ground

  • The uncertain geotechnical properties of frozen soil are important evidence for the design, operation and maintenance of the frozen ground. The complex geological, environmental and physical effects can lead to the spatial variations of the frozen soil, and the uncertain mechanical properties are the key factors for the uncertain analysis of frozen soil engineering. In this study, the elastic modulus, strength and Poisson ratio of warm frozen soil were measured, and the statistical characteristics under different temperature conditions are obtained. The autocorrelation distance (ACD) and autocorrelation function (ACF) of uncertain mechanical properties are estimated by random field (RF) method. The results show that the mean elastic modulus and mean strength decrease with the increase of temperature while the mean Poisson ratio increases with the increase of temperature. The average values of the ACD for the elastic modulus, strength and Poisson ratio are 0.64m, 0.53m and 0.48m, respectively. The standard deviation of the ACD for the elastic modulus, strength and Poisson ratio are 0.03m, 0.07m and 0.03m, respectively. The ACFs of elastic modulus, strength and Poisson ratio decrease with the increase of ratio of local average distance and scale of fluctuation. The ACF of uncertain mechanical properties is different when the temperature is different. This study can improve our understanding of the spatial autocorrelation variations of uncertain geotechnical properties and provide a basis and reference for the uncertain settlement analysis of frozen soil foundation.

  • Three new quaternary tantalum thiophosphates, $K_{0.18}TaPS_6,\;K_{0.28}TaPS_6,\;and\;Rb_{0.09}TaPS_6$ have been synthesized by using reactive alkali metal halide fluxes and structurally characterized by single crystal X-ray diffraction techniques. The crystal structures of $K_{0.18}TaPS_6,\;K_{0.28}TaPS_6,\;and\;Rb_{0.09}TaPS_6$ contain 3-dimensional open framework anions, $[TaPS_6]^{x-}$(x = 0.09, 0.18, 0.28) with the empty channel which disordered alkali metal cations, $K^+\;and\;Rb^+$ are located in. Clinical and Radiological Outcomes of Modified Phemister Operation with Coracoclavicular Ligament Augmentation Using Suture Anchor for Acute Acromioclavicular Joint Dislocation

  • Background: Modified Phemister operation has been widely used for the treatment of acute acromioclavicular (AC) joint dislocation. Additionally, the use of suture anchor for coracoclavicular (CC) fixation has been reported to provide CC stability. This study was conducted to evaluate the clinical and radiological results of a modified Phemister operation with CC ligament augmentation using suture anchor for acute AC joint dislocation. Methods: Seventy-four patients underwent the modified Phemister operation with CC ligament augmentation using suture anchor for acute AC joint dislocation and were followed-up for an average of 12.3 months. The visual analogue scale (VAS), range of motion, Constant score, and Korean shoulder scoring system (KSS) were used for clinical assessment. Acromioclavicular interval (ACI), coracoclavicular distance (CCD), and acromioclavicular distance (ACD) were obtained to evaluate the radiological assessments. Results: At the last follow-up, the mean VAS Score was 1.7 points, the mean joint range of the forward flexion was $164.6^{\circ}$, external rotation at the side was $61.2^{\circ}$ and internal rotation to the posterior was a level of T12. The mean Constant score and the mean KSS was 82.7 points and 84.2 points, respectively. At the mean ACI, CCD, and ACD, significant differences were found preoperatively and at the last follow-up. When the ACI, CCD, and ACD were compared with the contralateral unaffected shoulder at the last follow-up, the affected shoulders had significantly higher values. Conclusions: The modified Phemister operation with CC ligament augmentation using suture anchor is clinically and radiologically effective at acute AC joint dislocation.

Effect of spatial variability of concrete materials on the uncertain thermodynamic properties of shaft lining structure

  • The thermodynamic properties of shaft lining concrete (SLC) are important evidence for the design and construction, and the spatial variability of concrete materials can directly affect the stochastic thermal analysis of the concrete structures. In this work, an array of field experiments of the concrete materials are carried out, and the statistical characteristics of thermophysical parameters of SLC are obtained. The coefficient of variation (COV) and scale of fluctuation (SOF) of uncertain thermophysical parameters are estimated. A three-dimensional (3-D) stochastic thermal model of concrete materials with heat conduction and hydration heat is proposed, and the uncertain thermodynamic properties of SLC are computed by the self-compiled program. Model validation with the experimental and numerical temperatures is also presented. According to the relationship between autocorrelation functions distance (ACD) and SOF for the five theoretical autocorrelation functions (ACFs), the effects of the ACF, COV and ACD of concrete materials on the uncertain thermodynamic properties of SLC are analyzed. The results show that the spatial variability of concrete materials is subsistent. The average temperatures and standard deviation (SD) of inner SLC are the lowest while the outer SLC is the highest. The effects of five 3-D ACFs of concrete materials on uncertain thermodynamic properties of SLC are insignificant. The larger the COV of concrete materials is, the larger the SD of SLC will be. On the contrary, the longer the ACD of concrete materials is, the smaller the SD of SLC will be. The SD of temperature of SLC increases first and then decreases. This study can provide a reliable reference for the thermodynamic properties of SLC considering spatial variability of concrete materials.

Study on the Correlation between Refractive error and Components of Eye's Optical system in Children and Teenagers in an Optometric Practice (안과 병원을 내원한 소아 청소년의 굴절이상과 안광학 성분간의 상관관계 연구)

  • Purpose: The purpose of this study to evaluate visual acuity and refractive state and measure and analyze the components of eye's optical system in children and teenagers. Methods: With subjects of 124 (230 eyes) children and teenagers who had no eye diseases, correlation between the correlation between refractive errors and component's of eye's optical system was investigated. The spherical equivalent power of cycloplegic clinical refraction or manifest clinical refraction(SE), corneal power(CP), corneal radius(CR), axial length(AL), anterior chamber depth(ACD) and axial length to corneal radius (AL/CR) ratio were measured and analysed. Results: the SE was negatively correlated with the AL(r = -0.80, p = 0.00), the ACD(r = -0.35, p = 0.00) and the CR(r = -0.11, p=0.00) and positively correlated with the CP(r = +0.11, p=0.00). The AL was positively correlated with the AL/CR ratio (r = +0.84, p = 0.00), the ACD(r = +0.47, p=0.00) and the CR(r = +0.38, p = 0.00) and negatively correlated with the CP(r = -0.38, p=0.00). The CR was negatively correlated with CP(r = -1.00, p = 0.00), the AL/CR ratio(r = -0.19, p = 0.00) and the ACD(r = -0.06, p = 0.39). The CP was positively correlated with the AL/CR ratio(r = +0.19, p = 0.00) and the ACD(r = +0.06, p = 0.39). The ACD was positively correlated with the AL/CR ratio(r = 0.53, p = 0.00). Conclusions: the highest change of refractive errors was shown that the AL/CR ratio was a very important indicator for diagnosing the refractive errors of the children and teenagers.
