In this study 1,079 occupational accident reports were used to examine the characteristics and causes of occupational injuries in the small-sized textile companies with less than 50 employees. These data were analyzed in terms of age of injured person, work experience, accident type, injury type, and agency of accident. The results show that there are some patterns: (1) injuries occur more frequently in the 40~49yr age group; (2) about half of all accident occurred during the first year of employment; (3) there is a higher percentage of sick people leaving in the 29~90day range; (4) 'caught in and between objects' represents the leading accident type; (5) the most common type of incidence is related to the machinery; (6) 'lower back injuries' is the leading type of occupational disorder. These results can be used to develop more effective accidental occupational injury prevention programs for small-sized textile industries.

Penetrating gallbladder injuries are uncommon in the pediatric population. The treatment varies according to the severity of the injury and the patient's hemodynamics. We present the case of an 11-year-old male with an accidental pellet gunshot wound to the right upper abdomen that resulted in a grade III liver laceration and damage to the anterior gallbladder wall. The patient underwent laparoscopic cholecystectomy with drain placement. Postoperative radiography of the surgical specimen confirmed the presence of the pellet in the gallbladder. The patient recovered uneventfully and was discharged home on postoperative day 3. Laparoscopic cholecystectomy is a feasible treatment option for penetrating gallbladder injuries in hemodynamically stable patients.

Electronic cigarettes are novel tobacco products that are frequently used these days. The cartridge contains liquid nicotine and accidental poisoning, even with a small oral dose, endangers children. We present here a mortality case of a 15-month-old child who ingested liquid nicotine mistaking it for cold medicine. When the emergency medical technicians arrived, she was found to have pulseless electrical activity. Spontaneous circulation was restored after approximately 40 minutes of cardiopulmonary resuscitation. The cotinine level in her urine was 1,716 ng/mL. Despite intensive supportive care, severe anoxic brain injury was found on computed tomography and the child ultimately died. This fatality highlights the need for public health efforts to minimize such accidents.
Purpose: High-pressure injection(HPI) injury is an injury caused by accidental injection of substances by industrial equipment. HPI injury of the hand is a serious injury that can be potentially devastating. There have been a number of publications on the results of its treatment and its functional outcome of these hands. Unfortunately, the clinical outcomes were unsatisfactory following an initial treatment approach of digital expression of the injection material, elevation, soaks, dressing changes, and antibiotics. Methods: A 43-year-old right handed man sustained a high pressure injection injury to the tip of the left index finger. The injected material was industrial paint thinner. Tissue necrosis was noted at the pulp of the finger. Several debridements and irrigation were required. A pedicled chest flap transfer was performed on the eighteenth day after injury as the dorsal nail complex remained viable. This is a retrospective review of our experience with high-pressure finger injection injury caused by paint. A literature review, retrospective chart and radiologic review were presented. Results: Follow-up length was about 1 year. The injuried hand was left nondominant hand, the index. Patient complaints were cold intolerance, paresthesia, contact pain, and impairment of activities of daily living. Conclusion: The outcome of high-pressure injection injuries of the hand is affected by many factors. The time between injury and operative treatment has been regarded as a key determinant by a number of authors. The nature of the injected material is probably more important. It has been noted by many authors that injuries with paints have a worse outcome than those with oil or grease. This study confirms the fact that high-pressure injection injury caused by paint thinner to the hand is a significant problem. Virtually a patient suffers sequelae of this injury. The injury has significant repercussions for future function and reintegration into the work force.
Purpose: To explore the association between sleep and the risk of accidental injury at school among Korean adolescents. Methods: From the database of the Ninth Korea Youth Risk Behavior Web-based Survey (KYRBS), the researcher selected 63,307 adolescents who responded to a survey on sleep hours. We conducted logistic regression with sleep duration and fatigue after sleep as independent variables, the risk of injury at school as a dependent variable, and gender, grade, school type, economic status, parents' education level, number of participations in physical education, and current smoking and drinking as control variables. Results: Using 9 hours of sleep as the reference, the adjusted injury risk (odds ratio) was 1.74 for those sleeping less than 5 hours a day, 1.61 for 5 hours, 1.45 for 6 hours, 1.31 for 7 hours, 1.13 for 8 hours, and 1.40 for 10 hours or longer. The difference between each pair of groups was statistically significant. In this study, injury risk increased as sleep duration decreased and fatigue after sleep increased. Conclusion: The findings suggest that a short nightly duration of sleep and fatigue after sleep can be considered potential risk factorsfor unintentional injuries at school among Korean adolescents.
Objectives: Recently injury has become a major world-wide health problem. But studies in Korea about injuries were very few. Thus, this study was conducted to analyze the trend of major injuries from 1991 to 2006 and to provide basic data for preventing injuries. Methods: This study was based on the National Statistical Office data from 1991 to 2006 and calculated to estimate the burden of major injuries by using the standard expected years of life lost (SEYLL) and total lost earnings equation. Results: For transport accidents, mortality, SEYLL and total lost earnings were increased from 1991 to 1996 and decreased from 2000 to 2006. On the other hand, for suicides, these were increased gradually. Since 2003, falls were included in ten leading causes of death. This study showed that injury causes major social and economical losses. Conclusions: We could reduce injury related premature death through active interest in injury prevention program.
To investigate patterns and severity of hunting-related emergency traumatic injuries in wild boar hunting dogs. One hundred wild boar hunting dogs with emergency traumatic injuries sustained during wild boar hunting as a result of wild boar attack and accidental shooting of ensnarement in a trap. The retrospective study involved 100 dogs brought to CAMC for treatment of emergency traumatic injury sustained during wild boar hunting in Jeon-buk province from August 2007 to April 2008. Medical information obtained from the medical records included signalment; cause of injury; number, location and severity of injuries; and mortality. The 100 patients displayed 136 injuries (single injury in 71 dogs and multiple injuries in 29 dogs). Causes of the emergency traumatic injuries were wild boar attack (n = 92), accidental shooting (n = 7), and entrapment (n = 1). The thoracic area was the most common site of injury. The most common injury severity score (ISS) was code 2. The mortality rate was 9%, and all deaths involved thoracic injury. Emergency traumatic injuries sustained during wild boar hunting are most commonly thoracic injuries caused by prey attack. The nature of the injuries can differ from those typically encountered by small animal veterinarians.
Purpose: During August 2010, a natural gas fuel cylinder on a bus exploded in downtown Seoul, injuring 20 citizens. This kind of blast injury has never been reported in Korea before. Thus, the goal of this study was to review the clinical features of these victims to help physicians manage similar cases and to understand the risk factors associated with blast injuries in everyday life. Methods: Twenty (20) victims who visited nearby emergency departments, and 3 peoples left hospital without care. Seventeen (17) victims were included in this study, and the following factors were investigated: age, sex, type of hospital, diagnosis of injury, injury mechanism, position of victim (in-bus/out of bus), classification of injury severity with START (simple triage and rapid treatment), and classification of injury according to the mechanism of the blast injury. Results: The victims included 8 males (47%), 9 females (53%). The mean age was $37.5{\pm}12$. Thirteen (13) victims were transferred to two tertiary hospitals, and 4 were transferred to two secondary hospitals. The types of injury were 3 fractures, 2 ligaments injuries, 6 contusions, 4 abrasions, and 3 open wounds (one of them was combined fracture). According to START classification, 17 victims were 1 immediate, 11 minor, 5 delayed, and no death. Classifications according to the mechanism of the blast injury were 1 primary injury, 6 secondary injuries (2 of them combined other mechanism), 3 tertiary injuries and 9 quaternary injuries. Conclusion: Trauma care physicians should be familiar with not only the specific types of injuries from blast accidents, but also the potential accidents that may occur in public facilities.
Central venous catheterization through a subclavian approach is indicated for some special purposes but it may cause many complications such as infection, bleeding, pneumothorax, thrombosis, air embolization, arrhythmia, myocardial perforation, and nerve injury. A case involving a mistaken central venous catheterization into the right vertebral artery through the subclavian artery is presented. A 33-year-old man who had deteriorated mentality after head injury underwent an emergency craniotomy for acute epidural hematomas on the right frontal and temporal convexities. His mentality improved rapidly, but he complained of continuous severe pain in the right posterior neck even though he had no previous symptom or past medical history of such pain. Three-dimensional cervical spine computed tomography (3D-CT) was performed first to rule out unconfirmed cervical injuries and it revealed a linear radiopaque material intrathoracically from the level of the 1st rib up to the level of C6 in the right vertebral foramen. An additional neck CT was performed, and the subclavian catheter was indwelling in the right vertebral artery through right subclavian artery. For the purpose of proper fluid infusion and central venous pressure monitoring, the subclavian vein catheterization had been performed in the operation room after general anesthesia induction before the craniotomy. Sufficient anatomical consideration and prudence is essential because inadvertent arterial cannulation at a non-compressible site is a highly risky iatrogenic complication of central venous line placement.

Human lung injury caused by the inhalation of nitrogen dioxide($NO_2$) has been reported in occupational situations other than agriculture, including arc welding, production of nitric acid or explosives, and blasting operations. Nitrogen dioxide reacts with the water in the respiratory tract to form nitric acid. The nitrates and nitrites formed from dissociation of nitric acid cause extensive local and systemic tissue damage. We present two cases of acute lung injury due to accidental inhalation of $NO_2$ gas in occupational situtions with a review of the literature.
