광범위 수부 손상에서 서혜부 유경피판과 유리 포장주위피판을 이용한 단계적 무지 재건술 (Staged Reconstruction of the Mangled Hand with the Combined Use of Pedicled Groin Flap and Free Wrap Around Flap)

  • Purpose: Despite the free tissue transfer using microsurgical technique being the current trend of soft tissue reconstruction of the hand, the pedicled groin flap has the advantage to provide coverage for the mangled hand without necessitating the use of a damaged arterial system and also providing the benefit of saving the arterial system for later free tissue transfer. This report presents the author's experience using pedicled groin flap in four cases of mangled hands with massive bone and soft tissue defects requiring later thumb reconstruction with the free wrap around flap. Materials and methods: The patients' age ranged from 30 to 51 years; three patients were male and one was female. The causes of mangled hand included two machinery crush injuries, one laboratory explosion and one motor vehicle accident. While evaluating the post-operative results, factors like flap survival, complications, stability in opposition, pinch power and 2 point discrimination were taken into account. Results: All massive soft tissue defects of the hands were completely covered with pedicled groin flap successfully. The reconstructed thumb using free wrap around flap did not have any limitation in opposition. There was no occurrence of post-operative infection and all the flaps survived completely. The average pinch power was 70% of the contralateral intact thumb and average 2 point discrimination was 10 mm. Conclusion: The pedicled groin flap for the reconstruction of the massive soft tissue defects of the hand with subsequent reconstruction of the thumb with a wrap around flap is a very useful procedure. The combined use of pedicled groin flap and wrap around flap allows adequate coverage of sizable soft tissue defects and functional thumb opposition in cases of reconstruction of the mangled hands.

손상된 하지에 시행한 유리조직 이식술 (Free Flap Transplantation for the Injured Lower Extremities)

  • The ability for tissue transfer based on microvascular anastomosis has created a revolution in microsurgical free tissue transplantation. The advantages of cutaneous flaps are that they are soft, durable and provide a good cosmetic reconstruction and muscle flaps have a more vigorous blood supply and a longer, larger vascular pedicle than cutaneous flaps. From June 1992 through May 1997, 68 patients had received reconstructive microsurgery in the lower extremity at Department of Orthopedic Surgery, Chonbuk National University Hospital. The results were as follows. 1. The age distribution was from 15 years of age to 67 and male were 59 cases and female 9 cases. 2. The most common cause was traffic accident(54 cases, 79.4%) and followed chronic osteomyelitis(9 cases, 13.2%), industrial accident(3 cases, 4.4%), burn(1 case, 1.5%) and farm injury(1 case, 1.5%). 3. Latissimus dorsi myocutaneous flap were 25 cases(36.8%), rectus abdominis muscle flap 21 cases(30.9%), gracilis muscle flap 10 cases(14.7%), dorsalis pedis flap 9 cases(13.2%), groin 2(2.9%) and vascularized iliac osteocutaneous flap 1(1.5%). 4. 61 cases(89.7%) of 68 cases were survived and the exposed vital tissues and bones were covered and revealed good cosmetic results.

차량형상과 충돌형태에 따른 보행자 거동 특성에 관한 연구 (Pedestrians Trajectory Characteristic for Vehicle Configuration and Pedestrian Postures)

  • Pedestrians involved in traffic accidents manifest unique trajectory characteristics depending on the collision speed, vehicle configuration, and pedestrian postures. However, the existing analytical models for pedestrian movements do not fully include the rotational characteristics of the pedestrians because they assume a two dimensional parabolic trajectory. This faulty assumption in the development of these models limits their applicability and reliability This study investigated the pedestrians movement at collision by computer simulation. The simulations are carried out by using HADYMO, which is a special simulation software system for dynamic movement analysis. Vehicles and pedestrians are modeled and verified via real crash worthiness experiments. Simulations are performed for various collision speeds, vehicle configuration, and pedestrian postures. Since the simulation uses multi-body dynamics, It can express irregular phenomena of the bodies quite well. The results can be exploited for vehicle design and traffic accident reconstruction.

  • Soft tissue defects in oral & maxillofacial region caused by tumor resection, trauma, congenital deformities have been treated in autogenous soft tissue flap, allogenic material, free dermal graft, fascia graft. Of these methods, autogenous dermis graft had initially been applied in hernia treatment at the beginning of nineteenth century and have been applied in soft tissue augmentation coverage of vital structure, dead space removal and reconstruction of fascia. A fat graft is used in reconstruction of orbit at the enucleation, restoration of facial contours, etc. In this case, patient with chin soft tissue defect in traffic accident was treated in autogenous dermis-fat graft from patient's abdominal and gluteal region. Chin defect was reconstructed favorably. There was no severe atrophy of grafted area 12 months postoperatively. We will report the result that is favorable esthetically with literature review.

  • This is the study of the knee joint injured patients at the orthopaedic surgery clinic where is located in Daejon, who has MCL combine injured ACL reconstruction caused by sport activity and accident during the period from Jan. 2001 to Oct. 2001. By comparing with groups between 7th case of I-group for MCL combined stitch and II-group for ACL reconstruction since 6weeks cast. We have been concluded with that following results. 1. Range of motion for the knee was not limited at 5th case(37%) of I-group, 6th case(42%) of II-group and the cases of Flexion deficit less then 10 -degree were 2nd case(13%) of I-group and II-group 1st case(8%) with no extension deficit more then 5 -degree. 2. The level of activity that tells you whether you are capable of exercise for six month after operation. It han been divided by 3 levels. The case of capable of doing low risk exercise(swimming, cycling, etc.) was 5th case of I-group, the case of capable of doing medium risk exercise(jogging, etc.) was 3rd case of I-group and 4th case of II-group and the case of capable of doing high risk exercise(football, etc.) were 3rd case of I-group and 3rd case of II-group. 3. The timing of the return to their job were average 6.4 weeks for I-group and average 22.9 weeks for II-group(P<.05, statistical difference). 4. There was no statistical difference between I-group and II-group for the timing of the return to their job(P>.05). 5. By using VAS to compare them there was no statistical difference between I-group and II-group of clinical results according to Lysholm scale.

미세수술을 이용한 수부 재건술 (The Reconstruction of Hand with Microsurgery)

  • The authors analyzed the clinical results of the reconstructive surgery for injuried hand with microsurgery in 33 patients, 35 cases at the department of orthopaedic surgery, school of medicine, Kyung Hee university from 1985 to 1992 and the results were as followings. 1. There were 31 men and 4 women who had a mean age of 23 years(range, 3 to 44 years) and the follow up evaluations averaged 19 months. 2. The causes of the injury were machinery injury in 25 cases, traffic accident in 2, frostbite in 4, burn in 3 and fall down in 1. 3. For the reconstructive procedure, scapular free flap was applied in 6 cases, radial forearm flap in 7, dorsalis pedis free flap in 4, neurovascular island flap in 6, gracilis free flap in 1, wrap around flap in 6, toe to thumb in 5. 4. 32 cases(91.4%)were successful in reconstructive surgery with microsurgery exept the failure of scapular free flap in 2 cases and dorsalis pedis free flap in 1. 5. The causes of failure in scapular free flap were infection in 1 case and thrombosis in 1. In dorsalis pedis free flap, the cause of failure was infection. In the analysis of above results, the reconstruction with microsurgery was effective procedure for reconstruction of injuried hand.

다중추돌사고의 재구성 해석: 경험적/해석적 방법과 영상사고기록장치 활용 (Reconstruction Analysis of Multi-Car Rear-End Collision Accidents: Empirical/Analytical Methods, and Application of Video Event Data Recorder)

  • 다중추돌사고의 형태는 연쇄 추돌이라고 불리우는 뒤로부터의 순차적인 추돌과 앞으로부터의 순차적인 추돌, 그리고 순서가 섞여 있는 추돌형태로 구분된다. 다중추돌사고에 대하여 몇가지 효과적인 재구성 해석 방안을 제시한다. 브레이크 다이브 등으로 인한 차량 파손과 승객 부상을 활용하는 전통적인 경험적 수법과 강체역학 범주내에서의 이론적인 해석 결과를 구하여 당장에의 실용적인 적용이 가능하도록 하였다. 국내에 보급이 확산되고 있는 영상사고기록장치를 활용하고 시뮬레이션 프로그램 등을 병행 활용하여 다중추돌사고를 효과적으로 해석하는 방안을 제시하였다. 저장 동영상에 대한 단순 직관적인 조사를 넘어설 수 있도록 동영상 분석 결과에 근거한 시뮬레이션 해석을 수행하여 다중추돌사고에 대한 구체적이고 다양한 정보의 획득을 가능하게 하여 원인 규명 및 책임 소재 등을 명확하게 구분할 수 있도록 하였다.

  • The most commonplace of collisions that directly affect people is that of vehicles. Safety studies have noted a correlation between vehicle occupant injury severity and velocity changes. Methods for estimating collision velocity changes are discussed here. This topic is part of what is referred to as accident reconstruction. Only planar collisions are considered. When a vehicle collides with another, impact dynamics with friction should be considered. This paper presents a general analysis methodology of impact. must dynamics incorporating friction. The presence of friction between sliding contacts during the impact makes the problem difficult since the events such as reverse sliding or sticking, which may occur at different times throughout the impact, must be determined. This paper uses the results of RICSAC experiments for verifying the developed methodology. The analysis and experimental results agree well.

  • During these 2-3 years, there were many big and small power system black-out including america-canada black out(2003.08). It is indicated that cause of accident is not only obsolete equipment but also dispatcher's improper reaction. therefore, it raised its head for the importance of dispatcher training. Korea power exchange(KPX) which is splitted fromKEPCO as part of reconstruction of power industry(2001.04), is in chagre of stable and efficient power system operation. also, KPX created 'dispatcher training team' which is benchmarked from foreign advanced Power corporation. In this paper, it will be introduced KPX's dispacher training program and various training procedure.

교통사고 재현 프로그램 PC-CRASH의 주요 충돌 인자 설정 (Establishment of Important Impact Parameters of Traffic Accident Reconstruction Program "PC-CRASH")

  • 현재 국내에서는 교통사고 재현 방법이 노면 흔적물에 의존하여 이루어지고 있고, 노면 흔적물이 없는 경우 교통사고 재현은 불가능하게 된다. 이러한 단점을 보완하기 위해 외국에서는 충돌모델을 이용한 교통사고 재현프로그램을 활용하고 있는 추세이다. 현재 우리나라에서 가장 많이 사용하는 교통사고 재현 프로그램 PC-CRASH를 활용하기 위해서는 각 사고마다 알맞은 충돌특성 인자 값을 사용자가 직접 입력하여 사용하여야 하나, 이를 활용할 수 있는 활용자료가 부족한 실정이다. 본 연구는 국내에서 실제 발생된 교통사고 사례에 대해 교통사고 재현 프로그램인 PC-CRASH와 충돌에 영향을 미치는 충돌 인자들로 교통사고를 재현하였고, 차량의 최종위치간 거리와 자세에 어떠한 영향을 미치는가를 알아보았다. 또한 실제 역 해석을 하기 위해 차량의 최종위치와 자세만으로 각각의 사례에 적절한 인자값을 추정할 수 있는 회귀식을 구성하였고, 통계적으로 신뢰성을 검증하였다. 사고 재현에 필요한 주요 충돌인자들의 초기치 설정시 추정식을 이용할 경우, 사고 재현 프로그램 활용 시, 시간 단축 효과를 프로그램 내부에 있는 유전자 알고리즘의 반복횟수로 추정식의 통계적 검증을 하였다.
