As the interest in traffic safety has been increasing recently, social movement is being made to reduce the number of traffic accidents and the view on improving the mobility of the existing roads is being converted into on establishing traffic safety as a priority. The increase of traffic accidents related to an intersection in a state that traffic accidents are decreasing overall may suggests the necessity to investigate the specific causes. In addition, we have to consider them when establishing the measures against traffic accidents in a intersection by investigating and analyzing the influences and factors that may affect traffic accidents. To induce the accident severity model, we collected the factors that affect accidents and then applied the Poisson Regression Model among nonlinear regression analysis by verifying the distribution of variables. As a result of the analysis, it turned out that the volume of traffic on main roads, the right turn ratio on sub-roads, the number of ways out on sub-roads, the number of exclusive roads for a left turn, the signals for a right turn on main roads, and an intersect angle were the factors that affect the accident severity.

In order to reduce the casualties of fishing boats, the author analyzed the fishing boat accident on the 412 cases in Korean maritime safety tribunal for the 2005~2009, and then studied the relation between the weather element and the accidents. According to this studies, the occurring ratio of sea casualty for fishing boat in fog weather was appeared 1 boat per 1.6 days. It means that the restricted visibility condition gives the most influence on the fishing boat accident. The casualties in winter season from November to next January occurred 139(33.7%), and small boats less than 50tons broke out more casualties with 68.4%. From this we can find that small fishing boats are very deeply affected on the sea weather condition. According to the boat types for fishing the casualty of jig boat was ranked first, and collision accident account for first with 77.9% for the types of casualties. As mentioned above, most sea casualties for small fishing boats were resulted from the human factors such as poor watch keeping in invisibility and the bad sea condition, it is necessary for navigation operators and the manager to take more attention to the meteorological factors.
Report documents of industrial and occupational accidents have continuously been accumulated in private and public institutes. Amongst others, information on narrative-texts of accidents such as accident processes and risk factors contained in disaster report documents is gaining the useful value for accident analysis. Despite this increasingly potential value of analysis of text information, scientific and algorithmic text analytics for safety management has not been carried out yet. Thus, this study aims to develop data processing and visualization techniques that provide a systematic and structural view of text information contained in a disaster report document so that safety managers can effectively analyze accident risk factors. To this end, the risk factor map using text mining and self-organizing map is developed. Text mining is firstly used to extract risk keywords from disaster report documents and then, the Self-Organizing Map (SOM) algorithm is conducted to visualize the risk factor map based on the similarity of disaster report documents. As a result, it is expected that fruitful text information buried in a myriad of disaster report documents is analyzed, providing risk factors to safety managers.
This study aimed to analysis factors related to in-hospital death of injured patients by patient safety accident. A total of 1,529 inpatients were selected from Korea Centers for Disease Control and Prevention database(2013-2017). Frequency, Fisher's exact test, t-test, ANOVA, logistic regression analyses by using STATA 12.0 were performed. Analysis results show that the mortality rate was lower for female than male but the mortality rate was higher for the older age, the higher the CCI, head (or neck), multiple, systemic damage sites, internal and others, metropolitan cities based on Seoul and 300-499 based on the bed size of 100-299. Based on these findings, the possibility of using the in-depth investigation of discharge damage from the Korea Centers for Disease Control and Prevention as a data source for the patient safety survey conducted to understand the actual status of patient safety accident types, frequency, and trends should be reviewed. Also, it is necessary to prevent injury and minimize death by identifying factors that affect death after injury by patient safety accident.
Objective: The purposes of this study are to survey needs for changing accident investigation from blaming to systems approach and to briefly summarize systems-based accident analysis techniques. Background: In modern complex socio-technical systems, accidents are caused by a variety of contributing factors including human, technical, organizational, social factors, not by just a single violation or error of a specific actor, but accidents investigation used to be focused on the incorrect action of individuals. A new approach investigating causes of accidents as a symptom of a deficient system is required. Method: This study was mainly based on survey of literatures related to accidents, accidents investigation, which included academic journals, newspapers, etc. Results: This study showed that accidents investigation of Korea focusing on blaming is problematic. This was confirmed by two concepts of migration and hindsight bias frequently found in accident causation studies, and an attribute of accidents having varying causes. This was illustrated with an example of Sewol ferry capsizing accident. Representative systems-based accident analysis models including Swiss cheese model, AcciMap, HFACS, FRAM and STAMP were briefly introduced, which can be used in systematic accidents investigations. Finally, this study proposed a procedure for establishing preventive measures of accidents, which was composed of two steps: public inquiry and devising preventive measures. Conclusion: A new approach considering how safety-critical components such as technical and social elements, and their interactions lead to accidents is needed for preventing reoccurrence of similar accidents in complex socio-technical systems. Application: The results would be used as a reference or guideline when the safety relevant governmental organizations investigate accidents.
Railroad shunting movements connecting and disconnecting train sets are very susceptible to human errors since they depend on human decision-making and action procedure that are variable to situation to situation. Nevertheless, in the investigation of railroad accidents, all the accident causes related with human factors have merely been categorized as 'careless treatment' of the workers without any systematic approach of behavioral sciences or the analysis of human errors. In this research, therefore, 137 accident cases occurred during railroad shunting movements and 435 accident cases occurred during driving were analyzed with a special interest of human errors. According to results, the traditional accident investigation scheme used for last several decades did not seem to be appropriate for catching up true accident causes with respect to human errors. In addition, both signal men and locomotive drivers made many mistakes in judgement/action stage while the former mainly commit judgement tasks where as the latter mainly commit cognition tasks. Ant those tasks such as 'confirmation of signal and route', 'location check-up of connected train sets', and 'route identification for a shift of track' ranked highly for accident susceptibility.
As Korean government and safety-related organizations make continuous efforts to reduce the number of industrial accidents, accident rate has steadily declined since 2010, thereby recording 0.48% in 2017. However, the number of fatalities due to industrial accidents was 1,987 in 2017, which means that more efforts should be made to reduce the number of industrial accidents. As an essential activity for enhancing the system safety, accident analysis can be effectively used for reducing the number of industrial accidents. Accident analysis aims to understand the process of an accident scenario and to identify the plausible causes of the accident. Accident analysis offers useful information for developing measures for preventing the recurrence of an accident or its similar accidents. However, it seems that the current practice of accident analysis in Korean manufacturing companies takes a simplistic accident model, which is based on a linear and deterministic cause-effect relation. Considering the actual complexities underlying accidents, this would be problematic; it could be more significant in the case of human error-related accidents. Accordingly, it is necessary to use a more elaborated accident model for addressing the complexity and nature of human-error related accidents more systematically. Regarding this, HFACS(Human Factors Analysis and Classification System) can be a viable accident analysis method. It is based on the Swiss cheese model and offers a range of causal factors of a human error-related accident, some of which can be judged as the plausible causes of an accident. HFACS has been widely used in several work domains(e.g. aviation and rail industry) and can be effectively used in Korean industries. However, as HFACS was originally developed in aviation industry, the taxonomy of causal factors may not be easily applied to accidents in Korean industries, particularly manufacturing companies. In addition, the typical characteristics of Korean industries need to be reflected as well. With this issue in mind, we developed HFACS-K as a method for analyzing accidents happening in Korean industries. This paper reports the process of developing HFACS-K, the structure and contents of HFACS-K, and a case study for demonstrating its usefulness.
Accident analyses are used to identify common factors contributing to occupational accidents and to give recommendations for accident prevention. In this study we developed a human error analysis system that can be used easily at the industries. This accident analysis system can be used to develop accident prevention programs to reduce human initiated accidents.

Them traffic accidents have been increased every year due to increasing of vehicles numbers as well as the gravitation of the population. The carelessness of drivers, many road weather factors have a great influence on the traffic accidents. Especially, the number of traffic accident is governed by precipitation, visibility, humidity, cloud amounts and temperature. The purpose of this paper is to analyse the effect of road weather factors on traffic accident. We use the data of traffic accident, AWS weather factors (precipitation, existence of rainfall, temperature, wind speed), time zone and day of the week in 2013. We did statistical analysis using logistic regression analysis and decision tree analysis. These prediction models may be used to predict the traffic accident according to the weather condition.
Objectives This study was designed to find out influential factors associated with Hospitalization of the patients from traffic accident by Groups. Methods Based on the medical charts, we analyzed statistical study of 486 patients putting them into two groups to identify the factors associated with the duration of hospitalization involving age, sex, elapse day (Interval between time of injury and visit to hospital), and the types of accident. Patients in group A were diagnosed with a mere 'sprain', and patients in Group B had other symptoms accompanied by 'sprain'. Results 1. In group A, cervical sprain was the most and male patients with the symptom outnumbered female patients. In group B, headache was most where female patients had the symptom 1.7 times more than male patients. 2. Female patients in group B were found to stay for a longer period of time in the hospital than the male counterparts (p<0.05). 3. Both group A and B have positive correlation between length of hospital stay and age though group B exhibited stronger correlation. 4. Patients who had accident while riding bus in group A and those who were pedestrians in group B were found to stay in the hospital for a significantly longer period of time than those who had any other types of accident (p<0.05). Conclusions We noticed that hospital stay lengthened when the patients were older and when they had accidents while riding bus in Group A whereas in Group B, patients stayed longer when they were females and older as well as when they were pedestrians in accidents.
