A Study on the Cause Analysis for the Capsizing Accident in Fishing Vessels (어선의 전복사고 원인별 분석에 관한 연구)

  • In recent 5 years, the 70 percent of the ship's accident was occurred in the fishing vessels, and most of them were the engine troubles and collisions. The capsizing accident was comparatively low portion of occupation, which took only 2 percent, but the scale of accident and the loss of lives and property are known very tremendous. In this paper, it was examined for the stability provisions of fishing vessels in domestic and international to reduce the capsizing of them. Also, it was made an analysis for the case of capsizing accidents to suggest the measures against the capsizing.

  • This study applied the web crawling technique for extracting big data news on water quality accidents in the water supply system and presented the algorithm in a procedural way to obtain accurate water quality accident news. In addition, in the case of a large-scale water quality accident, development patterns such as accident recognition, accident spread, accident response, and accident resolution appear according to the occurrence of an accident. That is, the analysis of the development of water quality accidents through key keywords and sentiment analysis for each stage was carried out in detail based on case studies, and the meanings were analyzed and derived. The proposed methodology was applied to the larval accident period of Incheon Metropolitan City in 2020 and analyzed. As a result, in a situation where the disclosure of information that directly affects consumers, such as water quality accidents, is restricted, the tone of news articles and media reports about water quality accidents with long-term damage in the event of an accident and the degree of consumer pride clearly change over time. could check This suggests the need to prepare consumer-centered policies to increase consumer positivity, although rapid restoration of facilities is very important for the development of water quality accidents from the supplier's point of view.

Text Analytics for Classifying Types of Accident Occurrence Using Accident Report Documents (사고보고문서를 이용한 텍스트 기반 사고발생 유형 및 관계 분석)

  • Recently, a lot of accident report documents have accumulated in almost all of industries, including critical information of accidents. Accordingly, text data contained in accident report documents are considered useful information for understanding accident processes. However, there has been a lack of systematic approaches to analyzing accident report documents. In this respect, this paper aims at proposing text analytics approach to extracting critical information on accident processes. To be specific, major causes of the accident occurrence are classified based on text information contained in accident report documents by using both textmining and latent Dirichlet allocation (LDA) algorithms. The textmining algorithm is used to structure the document-term matrix and the LDA algorithm is applied to extract latent topics included in a lot of accident report documents. We extract ten topics of accidents as accident types and related keywords of accidents with respect to each accident type. The cause-and-effect diagram is then depicted as a tool for navigating processes of the accident occurrence by structuring causes extracted from LDA. Further, the trends of accidents are identified to explore patterns of accident occurrence in each of types. Three patterns of increasing to decreasing, decreasing to increasing, or only increasing are presented in the case of a chemical plant. The proposed approach helps safety managers systematically supervise the causes and processes of accidents through analysis of text information contained in accident report documents.

  • MAAP4 is a computer code that can simulate the response of a light water reactor power plant during severe accident sequences, including actions taken as part of accident management. The code quantitatively predicts the evolution of a severe accident starting from full power conditions given a set of system faults and initiating events through events such as core melt, reactor vessel failure, and containment failure. Furthermore, models are included in the code to represent the actions that could mitigate the accident by in-vessel cooling, external cooling of the reactor pressure vessel, or cooling the debris in containment. A key element tied to using a code like MAAP4 is an uncertainty analysis. The purpose of this paper is to present a MAAP4 based analysis to examine the sensitivity of a key parameter, in this case hydrogen production, to a set of model parameters that are related to a Level 2 PRA analysis. The Level 2 analysis examines those sequences that result in core melting and subsequent reactor pressure vessel failure and its impact on the containment. This paper identifies individual contributors and MAAP4 model parameters that statistically influence hydrogen production. Hydrogen generation was chosen because of its direct relationship to oxidation. With greater oxidation, more heat is added to the core region and relocation (core slump) should occur faster. This, in theory, would lead to shorter failure times and subsequent "hotter" debris pool on the containment floor.

A Study on the Side Collision Accident Reconstruction Using 3-Dimensional Crash Analysis (3차원 충돌해석 정보를 이용한 측면 충돌 사고 재구성)

  • The side collision reconstruction algorithm is developed using three dimensional car crash analysis. Medium size passenger car is modeled for finite element analysis. Total 24 side collision configurations, four different speed and six different angle, are set up for making side collision database. Deformation index and degree index are built up for each collision case. Deformation index is a kind of deformation estimate averaging displacement of side door of crashed car from finite element analysis result. Angle index is constructed measuring deformed angle of crashing car. There are two kinds of angle index, one is measured at driver's side and the other is measured at passenger's side. Also a collision analysis information in side of cars is used for giving a basis for scientific and practical reason in a reconstruction of the car accident. The analysis program, LS-DYNA3D is utilized for finite element analysis program for a collision analysis. Those database are used for side collision reconstruction. Side collision reconstruction algorithm is developed, and applied to find the collision conditions before the accident occurs. Three example collision cases are tried to check the effectiveness of the algorithm. Deformation index and angle index is extracted for the case from the analysis result. Deformation index is compared to the established database, and estimated collision speed and angle are introduced by interpolation function. Angle index is used to select a specific collision condition from the several available conditions. The collision condition found by reconstruction algorithm shows good match with original condition within 10% error for speed and angle. As a result, the calculation from the reconstruction of the situation is reproducing the situation well. The performance in this study can be used in many ways for practical field using deformation index and degree index. Other different collision situations may be set up for extending the scope of this study in the future.

  • Though not very often, a train accident can cause large number of fatality. As a result, public concern about its safety is increasing nowadays. Tn this paper, the structural crashworthiness of Korean High Speed Train trailer was examined through FE analysis. Crash analyses on energy absorbing part and safety zone were carried out to determine each section force. Rollover analysis was performed to observe the amount of intrusion in the passenger's area in case of rollover accident.

UNCERTAINTY AND SENSITIVITY ANALYSIS OF TMI-2 ACCIDENT SCENARIO USING SIMULATION BASED TECHNIQUES

  • The Three Mile Island Unit 2 (TMI-2) accident has been studied extensively, as part of both post-accident technical assessment and follow-up computer code calculations. The models used in computer codes for severe accidents have improved significantly over the years due to better understanding. It was decided to reanalyze the severe accident scenario using current state of the art codes and methodologies. This reanalysis was adopted as a part of the joint standard problem exercise for the Atomic Energy Regulatory Board (AERB) - United States Regulatory Commission (USNRC) bilateral safety meet. The accident scenario was divided into four phases for analysis viz., Phase 1 covers from the accident initiation to the shutdown of the last Reactor Coolant Pumps (RCPs) (0 to 100 min), Phase 2 covers initial fuel heat up and core degradation (100 to 174 min), Phase 3 is the period of recovery of the core water level by operating the reactor coolant pump, and the core reheat that followed (174 to 200 min) and Phase 4 covers refilling of the core by high pressure injection (200 to 300 min). The base case analysis was carried out for all four phases. The majority of the predicted parameters are in good agreement with the observed data. However, some parameters have significant deviations compared to the observed data. These discrepancies have arisen from uncertainties in boundary conditions, such as makeup flow, flow during the RCP 2B transient (Phase 3), models used in the code, the adopted nodalisation schemes, etc. In view of this, uncertainty and sensitivity analyses are carried out using simulation based techniques. The paper deals with uncertainty and sensitivity analyses carried out for the first three phases of the accident scenario.

A Study on Accident Prevention through Analysis of Industrial Drone Accidents and Their Causes (산업용드론 재해발생 특성과 원인분석을 통한 재해예방에 관한 연구)

  • It has been only a few years since drones have been introduced to Korea, and as the use of drones has increased in industrial sites as well as in hobby activities, accidents such as workers' body parts coming into contact with rotating propellers have been occurring since 2016. Industrial accident cases are being filed. In the case of a remote control vehicle, if you release the remote controller in the event of an abnormal situation, it stops in place and no further danger occurs but in case of drones flying over the sky is different. Sudden motor stops during flight, uncontrolled changes in flight posture, loss of radio waves, loose propeller, etc, numerous factors can lead to accidents due to unintentional movements. Therefore, drone operators need safety knowledge as well as high knowledge about drones. However, drones with a weight of less than 12 kg can fly without a pilot's certification, and therefore, a minimum accident prevention measure is required. 10 accidents approved as industrial accidents since 2016, 23 accidents surveyed by general drone operator, 40 accidents surveyed by the Korean Consumer Resources Unfortunately for the purpose of writing this paper, drones are a new species and do not have many parameters for analysis but my experience has shown that the type of disaster does not deviate significantly. In addition, there is no paper that analyzes drone related industrial accidents not only in Korea but also in other countries. However, even if you watch an overseas drone accident video through YouTube, it is not much different from the case that occurs in Korea. I hope that this study will be of little help to all the drone pilots as well as the work safety of industrial drones used in industrial sites.

  • A Recently elevator accidents were occurred and increased due to carelessness of user and violent behavior. The accident of door detachment in elevator platform by electronic motor scooter show a example of safety concerns and insensibility for safety awareness. In this report, we review the cause of accident of door detachment in elevator platform by electronic motor scooter, and show some suggestions of preventing from the safety concerns about this case.

Traffic Accident Models of 3-Legged Signalized Intersections in the Case of Cheongju (3지 신호교차로의 교통사고 발생모형 - 청주시를 사례로 -)

  • This study deals with the traffic accidents at the 3-legged signalized intersections in Cheongu. The goals are to analyze the geometric, traffic and operational conditions of intersections and to develop a various functional forms that predict the accidents. The models are developed through the correlation analysis, the multiple linear, the multiple nonlinear, Poisson and negative binomial regression analysis. In this study, two multiple linear, two multiple nonlinear and two negative binomial regression models were calibrated. These models were all analyzed to be statistically significant. All the models include 2 common variables(traffic volume and lane width) and model-specific variables. These variables are, therefore, evaluated to be critical to the accident reduction of Cheongju.
