A 20-Year Update on the Practice of Thoracic Surgery in Canada: A Survey of the Canadian Association of Thoracic Surgeons

  • Background: This study provides an update to a landmark 2004 report describing demographics, training, and trends in adherence to thoracic surgery practice standards in Canada. Methods: An updated questionnaire was administered to all members of the Canadian Association of Thoracic Surgeons via email (n=142, compared to n=68 in 2004). Our report incorporates internal data from Ontario Health and the Canadian Partnership Against Cancer. Results: Forty-eight surgeons completed the survey (male, 70.8%; mean±standard deviation age, 50.3±9.3 years). This represents a 33.8% response rate, compared to 64.7% in 2004. Most surgeons (69%) served a patient population of over 1 million per center; 32%-34% reported an on-call ratio of 1:4-1:5 days, and the average weekly hours worked was 56.4±11.9. Greater access to dedicated geographic units per center (73% in 2021 vs. 53% in 2004) has improved thoracic-associated services and house staff, notably endoscopy units (100% vs. 91%), with 73% of respondents having access to both endobronchial and endoscopic ultrasound. Access to thoracic radiology has also improved, particularly regarding positron emission tomography scanners per center (76.9% vs. 13%). Annual case volumes for lung (255 vs. 128), esophageal (41 vs. 19), and mediastinal resections (30 vs. 13), along with hiatal hernia repair (45 vs. 20), have increased substantially despite reports of operating room availability and radiology as rate-limiting steps. Conclusion: This survey characterizes compliance with current practice standards, addressing the needs of thoracic surgeons across Canada. Over 85% of respondents were aware of the 2004 compliance paper, and 35% had applied for resources and equipment in response.

  • Stiffness of the elbow joint is relatively common after trauma, ectopic ossification, bum, postoperative scar, and etc. Mild flexion deformity can be reduced by use of active or passive motion exercise, dynamic sling, hinged distractor device, or turnbuckle orthosis. But these methods have disadvantages of difficulty in gaining acceptable range of motion only with stretching exercise, re-contracture after conservative managements and poor results that flexion contracture remained. The common described operative exposures for treatment of the stiff elbow are anterior, lateral, posterior, and medial approach. Through Anterior, lateral and medial approach each has not access to all compartments of the elbow. But, posterior approach has benefits that access to posterior, medial and lateral aspects of the elbow and as needed, fenestration to the olecranon fossa that produces a communication between the anterior and posterior compartments of the elbow are possible. From June 1991 through April 1997, 11 patients who had posttraumatic stiff elbow, were treated with debridement arthroplasty through the posterior approach. The purpose of this study are to introduce technique of the debridement arthroplasty and to evaluate final outcomes. With regarding to preoperative pain degree, mild degree matches to 3 cases, moderate to 3 cases, and severe to 2 cases. In preoperative motion, flexion was average 85° and extension was 30°. Postoperatively nine patients had got the complete relief of pain and two patients continued to have mild pain intermittentely. Postoperative flexion improved to 127° and extension to 2°, so that elbow flexion had improved by an average of 42° and elbow extension by 28°. On the objective scale all patients had good or excellent results and they all felt that they were improved by operation. Debridement arthroplasty is one of excellent procedures for the intractable stiff elbow if it is not unstable or it has not incongrous. But it need a meticulous operative technique and a well-programmed rehabilitation.

  • PURPOSE: People with physical disabilities such as cerebral palsy usually experience obstacles when interacting with computer through conventional keyboard because of their motor disabilities. The purpose of this study is empirically compare of text entry(alphabet and word) speed and accuracy using the three different keyboard type on four students(male 2 and female 2) with cerebral palsy. METHODS: This research design used a replicated single-case experimental approach to compare the individual performance. An alternating treatments design was used to examine the effectiveness of standard QWERTY keyboard and alternative keyboard(mini and big keyboard) on computer access for students with cerebral palsy. To avoid changes in posture that influence a keyboard character entry training and evaluation was carried out using his sitting in a wheelchair. Compass software program used in this study as an assessment tool to measure speed and accuracy when performance of text entry(alphabet and word). This was repeated until the stable status of reaction time. RESULTS: As a result, the alternative keyboard seems to be the most effective device for students with cerebral palsy to perform text entry. But various factors such as peculiarity of motor disabilities, experience and preferences of the user are heavily related. CONCLUSION: Thus, we must perform the objective and systematic assessment for computer access and if sustained training is accomplished, it could to improve speed and accuracy of text entry(alphabet and word).

  • The article discusses the results of a quantitative analysis of open educational resources in the field of information technology. Study is based on a study of the content of ten platforms that provide access to open resources (OPs). To achieve this goal, we used the following methods: theoretical analysis and generalization of Internet sources to determine the popularity of educational platforms and resources on them; quantitative data analysis to determine the relative proportion of IT courses in various parameters: the relative weight of courses in the IT field in general and on each platform in particular, the language of instruction, the quantitative content by thematic areas. The following platforms providing access to open educational resources were subjected to quantitative analysis: Coursera, Edx, Udemy, MIT OpenCourseWare, OpenLearn, Intuit, Prometheus, UoPeople, Open Learning Initiative, Maidan Open University.

Investigating the Relationship Between Accessibility of Green Space and Adult Obesity Rates: A Secondary Data Analysis in the United States

  • Objectives: In spite of the importance of green space for reducing obesity-related problems, there has been little exploration of whether access to green space (e.g., parks and recreational facilities) influences the obesity rate of adults in the United States. The purpose of the study was to investigate the relationships among accessibility of green space, obesity rates, and socioeconomic and demographic variables among adults living in the State of Indiana, United States. Methods: We conducted a secondary data analysis to investigate the relationships among accessibility to green space, obesity rates, and socio-demographic variables with employing Geographic Information System in order to measure the accessibility of green space. Results: This study found that accessibility of green space served as a strong predictor of reduced obesity rates among adults (β=-2.478; p<0.10). In addition, adults with higher education levels, as well as better access to green space, were found to have even lower obesity rates (β=-0.188; p<0.05). Other control variables such as unemployment rates, food security, and physical inactivity are additional factors that influence obesity rates among adults. Conclusions: Accessibility of green space may play an important role in facilitating physical activity participation and reducing obesity rates.

Prostatic Artery Embolization for Lower Urinary Tract Symptoms via Transradial Versus Transfemoral Artery Access: Single-Center Technical Outcomes

  • Objective: To evaluate the safety and feasibility of prostatic artery embolization (PAE) via transradial access (TRA) compared with transfemoral access (TFA). Materials and Methods: This retrospective study included 53 consecutive men with lower urinary tract symptoms (LUTS) who underwent PAE between September 2018 and September 2021. Thirty-one patients (mean age ± standard deviation: 70.6 ± 8.4 years) were treated with TFA, including 14 patients treated before adopting TRA. Since December 2019, TRA has also been attempted with the procedure's selection criteria of patent carpal circulation and a height ≤ 172 cm, with 22 patients treated via TRA (69.1 ± 9.6 years). Parameters of technical success (defined as successful bilateral embolization), clinical success (defined as LUTS improvement), procedural time, radiation dose, and adverse events were compared between the two groups using the Fisher's exact test, independent sample t test, Wilcoxon signed-rank test, or Mann-Whitney test. Results: All patients received at least one-side PAE. Technical success of PAE was achieved in most patients (TRA, 21/22; TFA, 30/31; p > 0.999). No technical problem-related conversion from TRA to TFA occurred. The clinical success rate was 85% (11/13) in patients with TRA, and 89% (16/18) in patients with TFA for follow-up > 2 weeks post-PAE (median, 3 months) (p > 0.999). The median procedure time was similar in both groups (TRA, 81 minutes vs. TFA, 94 minutes; p = 0.570). No significant dose differences were found between the TRA and TFA groups in the dose-area product (median Gycm2, 95 [range, 44-255] for TRA and 84 [34-255] for TFA; p = 0.678) or cumulative air kerma (median mGy, 609 [236-1584] for TRA and 634 [217-1594] for TFA; p = 0.551). No major adverse events occurred in either of the groups. Conclusion: PAE via TRA is a safe and feasible method comparable to conventional TFA. It can be safely implemented by selecting patients with patent carpal circulation and adequate height.

청강 김영훈 진료기록 복원연구 (Study of Young-Hoon Kim's Medical Chart Restoration)

  • This study is a report on the restoration process and future projects involving the restoration of the medical records from 1914 to 1974. Cheong-Gang Young-Hoon Kim was born in 1882 and passed away in 1974. His times were the times when Korean Traditional Medicine was being neglected due to the introduction of Western Medicine through Japan. During this time Young-Hoon Kim put much effort into the Korean Traditional Medicine Restoration Movement and left over 150,000 medical records while consistently examining patients. Currently, this data can be found at the College of Oriental Medicine, Kyunghee University and is being compiled into a database as a part of the 2007 Knowledge of Oriental Medicine Web Service Project. The Preface and Chapter 1 introduce the author and the contents of the Cheong-Gang Medical Records, and Chapter 2 briefly discusses the necessity of providing digitalization and modernization to the medical records. Chapter 3 discusses the preservation process of the original medical records, chapter 4 describes the process of restoring and providing web access to the contents of the medical records, and chapter 5describes the main purpose of the medical records as well as future projects and an outlook involving the Knowledge of Oriental Medicine Web Service Project.

  • The purpose of this study was to search non-invasive and reproductive pulp test. Temperature of the crown surface was measured using the infrared thermography, and the pulp test was investigated with difference of crown temperature of the vital and the non-vital tooth in vitro and in vivo. Twenty extracted human maxillary central incisors were used in this study. Two sample teeth after access cavity preparation were arranged setting with one pair.   • Objectives SCAT (Sasang Constitution Analysis Tool) is a system designed to provide the information needed to diagnose a sasang constitution diagnosis expert diagnosis. In order to determine the sasang constitution diagnosis requires the following four ways. This system provides objective information to the constitution diagnostic expert to collect such information.Methods SCAT was constructed by considering the user UI/UX to be easy to enter information. To provide a state of the subject in Table to make it easier to grasp the information of the subject also in constitution diagnostician.Results Provides objective data for experts to determine the constitutional history of the subject.Conclusions Professionals and ordinary users through the SCAT system can easily access the sasang constitution diagnosis. In the future through the advancement of the SCAT system will improve the user's convenience stability.

Can central venous access device care bundles and regular feedback reduce central line-associated complications in pediatric patients?

  • Background: The use of indwelling central venous access devices (CVADs) in children can result in complications such as infection, occlusion, and dislodgement. Purpose: To evaluate whether reinforcing CVAD care bundles by using a regular direct feedback system could reduce such complications in children. Methods: The intervention in this retrospective interrupted time-series study was initiated in January 2019. The study was divided into the preintervention (October-December 2018), 3-month postintervention (January-March 2019), and 6-month postintervention (April-June 2019) phases. Risk difference and Poisson regression analyses were used to illustrate the effectiveness of the intervention. Results: The hospital-wide central line-related bloodstream infection rate decreased from 10.0/1,000 catheter-days to 4.5/1,000 catheter-days at 3-month postintervention (P=0.39) and to 1.4/1,000 catheter-days at 6-month postintervention (P=0.047). The central line occlusion rate significantly decreased from 30% to 12.8% (P=0.04) and 8.3% (P=0.002) at 3 and 6 months, respectively. Approximately 7% of CVADs became dislodged during the preintervention phase versus 8.5% (P=0.364) and 3.3% (P=0.378) at 3 and 6 months, respectively. Conclusion: Reinforcing CVAD care bundles with direct feedback could significantly decrease CVAD-associated complications in terms of infection at 6-month postintervention, and occlusion at 3- and 6-month postintervention. Thus, reinforcement and regular direct feedback might improve care quality in children with CVADs.
