Toceranib therapy for hepatic fibrosarcoma in a dog

  • A 12-year-old neutered male Maltese was brought to our hospital with loss of appetite, generalized alopecia, anemia, and a palpable abdominal mass. Ultrasonography revealed multiple abdominal masses in the liver and spleen. Subsequently, splenectomy and liver biopsy were performed, and the masses were histologically diagnosed as metastatic fibrosarcomas of unknown primary origin. The owner refused further investigations such as computed tomography and hepatectomy, therefore, we decided to initiate chemotherapy in the form of metronomic therapy with toceranib phosphate, which has several advantages, such as better tolerability, low cost, and convenience. The size and number of hepatic fibrosarcomas continued to increase despite continued administration of toceranib (10 mg/dog, PO, q48h). The dog died approximately 43 days after initiation of the toceranib treatment. To the best of our knowledge, this is the first report on toceranib therapy for metastatic hepatic fibrosarcoma in a dog.

Nature of Lesions Undergoing Radical Nephrectomy for Renal Cancer

  • Aim: The aim of the present study was to evaluate retrospectively histopathologically-diagnosed lesions that were detected in the kidney after radical nephrectomy for a preoperative diagnosis of kidney cancer. Methods: The medical records of 83 patients (51 male, 32 female) were included. Preoperative staging was accomplished by various methods including physical examination, blood hemography and biochemistry, abdominal ultrasonography (US), chest x-ray, abdominal computed tomography (CT) and abdominal magnetic resonance imaging (MRI). Results: Totals of 70 patients underwent radical nephrectomy and 13 nephron sparing surgery. Of the 83 patients, 70 had malignant lesions (renal cell carcinoma, squamous cell carcinoma or other malignancies) 13 had a variety of benign lesions, the most frequently detected being oncoytoma (6), angiomyolipoma (3), xanthogranulamatous pyelonephritis (2), cortical cyst (1) and chronic pyelonephritic change (1). Conclusion: It was concluded that in spite of great technological developments regarding radiological imaging modalities such as US, CT and MRI, benign lesions might still be detected pathologically in patients who undergo radical nephrectomy with the preoperative diagnosis of renal cancer. But, all renal masses should be regarded as malignant and should be managed surgically otherwise proven benign.

A case of midgut volvulus due to mesenteric lymphangioma, not associated with malrotation in a 13-year-old boy (13세남아에서 발견된 창자 회전이상이 동반되지 않은 낭성 림프관종에 의한 창자꼬임 1례)

  • A 13-year-old boy, complained of an intermittent suddenly aggravated severe abdominal pain and diarrhea, was diagnosed as a small bowel volvulus without an intestinal malrotation, due to mesenteric lymphangioma. He took abdominal ultrasonography, abdominal CT scanning, upper gastrointestinal study and got an operation. The small bowel volvulus with cystic lymphangioma was confirmed by gross and pathologic findings.

  • A spayed female, 8-year-old, maltese, weighing 1.84 kg with a history of acute severe vomiting, anorexia, depression for 2 day was referred. An abdominal mass was palpated with abdominal pain on physical examination. Radiographic findings included two soft tissue density masses in the mid-abdominal cavity and gas dilated colon. Ultrasonographs showed one heterogeneous hypoechoic round mass including intestinal wall. There were loss of normal wall layering and motility and absence of internal lumen of the intestinal wall. And another heterogeneous hyperechoic mass was identified to the medial side of the intestinal mass. Based on clinical signs, image findings, and the result of fine needle aspiration, intestinal obstruction due to alimentary lymphoma was diagnosed. On surgery, a cecal lymphoma and mesenteric lymphadenopathy were confirmed and the cecal mass was resected.

Development of Pneumoperitoneum after Endoscopic Retrieval of Gastric Foreign Body in a Dog (개에서 위내 이물의 내시경적 제거 후 발생한 기복증 증례)

  • An 1-year-old, intact female Yorkshire Terrier weighing 1.83 kg was referred for vomiting and anorexia. Survey abdominal radiographs revealed marked gastric distention. Ultrasonography showed a hyperechoic material with acoustic shadowing near the pylorus. A foreign body and ulceration were identified during gastroscopic examination. After endoscopic retrieval of gastric foreign body, unexpectedly abdominal distention was developed. Abdominal radiographs revealed pneumoperitoneum, but no leakage of contrast media was observed in gastrointestinal contrast study. Three days after hospitalization with supportive care, no abnormal findings were detected, and then the dog was discharged. This case describes the development of peumoperitoneum during endoscopic retrieval of gastric foreign body.

Colorectal Cancer in Children - 2 Cases Report - (소아에서 발생한 결장암)

  • A 12-year-old boy with severe periumbilical pain visited the emergency room. Physical examination, abdominal ultrasonography, colonoscopy and CT, identified a lesion of sigmoid colon. Endoscopic biopsy showed a signet ring cell carcinoma of the sigmoid colon. On explorative laparotomy, cancer invasions of the adjacent structures and metastases on peritoneal wall were noticed. We performed palliative loop-ileostomy. He underwent chemotherapy and radiotherapy for 3 months. The second case was a 16-year-old boy with abdominal pain and hematochezia, transferred to our hospital with the diagnosis of acute appendicitis with periappendiceal abscess. Although he underwent appendectomy, the abdominal pain persisted. Digital rectal examination revealed a lumen-obstructing fungating mass in the rectum. Endoscopic biopsy revealed a adenocarcinoma. Cancer invasion of the adjacent structures and metastases involving the mesentery of the small intestine were found at laparotomy. A palliative procedure, a Hartmann's operation and end-colostomy at the sigmoid colon were performed. The patient died 8 month later due to pneumonia and sepsis. Chemotherapy was not applied.

A Case of Alcoholic Liver Cirrhosis Treated with Injinoryeong-san (인진오령산(茵蔯五苓散)을 투여한 알코올성 간경변 환자 치험 1례)

  • Objectives: To introduce the effect of traditional Korean medicine (TKM) treatment with Injinoryeong-san on alcoholic liver cirrhosis (ALC).Methods: A 37-year-old man with a history of alcohol ingestion for the past five years was diagnosed with ALC based upon his clinical symptoms, sonogram results, and laboratory studies. The patient received TKM treatment with acupuncture, moxibustion, and Injinoryeong-san in parallel to Western medicine for two weeks. The clinical assessment was monitored based upon changes in the Child-Pugh score and ultrasonography, body weight, and abdominal circumference for measuring the amount of ascites.Results: The patient’s clinical symptoms improved with decreases in abdominal circumference and body weight. Ultrasound revealed a regression of the amount of ascites in the abdominal cavity. Laboratory results also improved significantly, and the Child-Pugh score increased from class B to A.Conclusions: This case report showed a significant improvement of ALC with relatively simple treatment for only a short period and therefore supported the potential of TKM treatment in ALC.

Soft Tissue Sarcomas Originating from Retroperitoneal Cavity in a Maltese Dog (후복강에서 기원한 비혈관성 연부조직 육종 증례)

  • An 11-year-old spayed female maltese was presented for abdominal distention, dysuria and dyschezia. Panting and heart murmur was found and abdominal palpation was difficult due to increased abdominal pressure. A soft tissue mass, $6{\times}3cm$ in size, was identified radiographically in pelvic canal, displacing the descending colon to the medioventral direction and the urinary bladder cranially. On ultrasonography, the mass consisted of homogeneous hypoechoic parenchyma containing the focal hyperechoic region ($1.6{\times}1.5cm$). The mass had distinct margin and no connection with adjacent organs. It was considered as a mass originating from the retroperitoneal cavity. Additional diagnostic procedures were not advanced because of the owner's request, and only a surgical excision of the mass was performed to alleviate the dysuria and dyschezia. Histopathologic examination and immunohistochemistry determined the mass as a soft tissue sarcoma and excluded hemangiosarcoma and osteosarcoma, both are the most common types of the retroperitoneal tumors. This report described non-vascular soft tissue sarcoma originating from the retroperitoneal cavity in a maltese dog.

Subvalvular Pulmonic Stenosis with Congestive Heart Failure in a Yorkshire terrier (요크셔테리어종에서의 울혈성 심부전을 동반한 판막하형 폐동맥판 협착증)

  • A two-month-old female Yorkshire terrier was referred to the Veterinary Teaching Hospital, College of Veterinary Medicine, Kyungpook national University. The patient was presented with a history of dyspnea, cough, exercise intolerance and abdominal distension, but she was appetence. In physical examination the puppy was coughed on slight tracheal compression. Rectal temperature, pulse and respiratory rate were normal, and grade 3/6 systolic murmur heard at the left heart base. The murmur was crescendo-decrescendo. Electrocardiography showed sinus arrhythmia, right-ventricular hypertrophy pattern, and right axis deviation. Thoracic radiography revealed cardiomegaly, bulging of the main pulmonary artery, and enlarged left side heart. Abdominal radiography revealed abdkominal distention. Echocardiography showed hypertrophy of right ventricle and turbulence in the pulmonary artery in parasternal oblique view. Subvalvular pulmonic stenosis was diagnosis based upon the clinical signs, physical examination, electrocardiography, radiography and echocardiography. We treated the patient with furosemide, enalapril and $\beta$-blocker. After the clinical signs of cough, abdominal distension and dyspnea were disappeared, she was on just $\beta$-blocker for prevention of occurrence of congestive heart failure. Now she was recovered her health, and she is not on any medication.

A Case of Ileoileocolic Type Intussusception Presented with Hematemesis Due to Meckel's Diverticulum (토혈이 동반된 멕켈 게실에 의한 회회맹장형 장중첩증 1례)

  • Intussusception is the most common cause of intestinal obstruction in early childhood and characterized by periodic colicky abdominal pain or irritability, vomiting, current jelly stool, and sausage-like abdominal mass. Meckel's diverticulum is common intestinal anomaly presenting with painless rectal bleeding during first 2 year of age. It is recognized as a common leading point of intussusception in childhood. Hematemesis is the rare clinical manifestation of both intussusception and Meckel's diverticulum. A 7-year-old girl presented with hematemesis was diagnosed as having intussusception by abdominal ultrasonography. Meckel's diverticulum was the leading point of intussusception in this case.
