Abdominal Subcutaneous Fat Thickness Measured by Ultrasonography Correlates with Hyperlipidemia and Steatohepatitis in Obese Children

  • Purpose: The aim of this study is to evaluate the relationship between abdominal subcutaneous fat thickness measured by ultrasonography (US) and serum lipid profile and liver transaminases in obese children. Methods: One hundred and sixty-six children diagnosed with obesity from May 2001 to December 2013 were included in this study. Data on serum lipid profile and liver transaminases were collected from clinical records. Abdominal subcutaneous fat thickness and grade of hepatic steatosis were evaluated by US. Results: Of the 166 children, 107 were diagnosed with hepatic steatosis by US, 46 with grade I, 56 with grade II, and five children with grade III. According to the grade of hepatic steasosis, the average values of midline abdominal subcutaneous fat thickness and right flank abdominal subcutaneous fat thickness measured $2.9{\pm}0.8cm$ and $1.9{\pm}0.7cm$ in the normal group, $3.3{\pm}0.8cm$ and $2.0{\pm}0.7cm$ in grade I, $3.8{\pm}0.8cm$ and $2.3{\pm}0.8cm$ in grade II, and $4.1{\pm}0.8cm$ and $2.8{\pm}1.4cm$ in grade III, respectively. Abdominal subcutaneous fat thickness correlated with grade of hepatic steatosis (p<0.01). In addition, abdominal subcutaneous fat thickness correlated with concentration of serum lipids and liver transaminases in the age group of 12-14 years (p<0.01). Conclusion: Abdominal subcutaneous fat thickness measured by US can be used as a reliable predictor of possible hyperlipidemia and steatohepatitis in children, especially during the adolescent stage.

Inverse Correlation between Cancer Size and Abdominal Obesity in Colorectal Cancer Cases

  • Background: Correlation between colorectal cancer (CRC) and abdominal obesity has been established, but there is a paucity of data on non-obese CRC patients. The aim of this study was to establish the characteristics of CRCs that occur in such patients. Materials and Methods: Consecutive CRC patients without cachexia were included. Unintended body weight loss, T4- or M1-staged CRCs, extensive lymph node involvement, or synchronous malignancy were classified as cachectic conditions. Abdominal fat volumes were measured using a multidetector CT unit with a software (Rapidia, INFINITT, Seoul, Korea). Results: Of the newly-diagnosed CRC patients, 258 non-cachectic and 88 cachectic patients were analyzed. The cancer size (p<0.001) and T stage (p<0.001) were inversely correlated with body mass index (BMI), visceral fat and subcutaneous fat volumes. Cancer size was the only independent factor related to BMI (p=0.016), visceral fat volume (p=0.002), and subcutaneous fat volume (p=0.027). In non-cachectic patients, a significant inverse correlation was found only between the cancer size and visceral fat volume (p=0.017). Conclusions: Non-obese CRC patients tend to have larger CRC lesions than their obese counterparts even under non-cachectic conditions. Such an inverse correlation between cancer size and visceral fat volume suggests that considerable CRCs are not correlated with abdominal obesity.

비만의 한방치료 효과 및 집단 간 효과차이에 관한 연구 (A Clinical Study about the Effects of oriental medical therapy on obesity and different effects between groups)

  • Objectives : The purpose of this study were to evaluate the effects of oriental medical therapy on obesity and different effects according to Body Mass Index(BMI), menopause, obesity treatment experience, age, treatment period in patients. Methods : 46 patients were treated from November 2011 to April 2012 in Oriental Obesity Center, Dong-eui Medical Center. They were measured change of body compositions by bioelectrical impedance analysis every 2~3 weeks and 27 patients out of 46 were checked body compositions every 2 weeks. 46 and 27 patients were divided into two or three groups according to BMI, menopause, obesity treatment experience, age, treatment period. We compared before and after treatment body compositions. Results : Body compositions, except edema index were significantly reduced. Abdominal Visceral Fat(AVF) was significantly reduced in Obese group. Body Weight(BW), BMI, Body Fat(BF), Body Fat Percetage(BFP), Abdominal Visceral Fat Level(AVFL), Abdominal Visceral Fat(AVF), Abdominal Subcutaneous Fat(ASF) were significantly reduced in non-menopause group(43 female patients). But in 26 female patients out of 27, there were no significantly differences between groups in menopause. There were no significantly differences between groups in obesity treatment experience. But in 27 patients, BMI and AVF were significantly reduced in non-experience group and AVFL was significantly reduced in experience group. BW, BMI, BF, BFP, VAF, ASF were significantly reduced in younger age group. But in 27 patients, BW and Muscle were significantly reduced in younger age group. BMI, BF, BFP, Edema, AFVL, Abdominal Visceral Fat Area(AVFA), AVF, ASF were significantly reduced in longer treatment period group. Conclusion : The BF, BMI, BF, BFP, Muscle, AVFL, AVFA, AVF, ASF were almost decreased significantly. It resulted that the effects of oriental medical therapy in obesity was positive. And it was meaningful study to know about different effects between groups.

경도비만 여성에서 생커피두 엑기스의 섭취에 의한 체지방 감소 효과 (Effect of Green Coffee Bean Extract Supplementation on Body Fat Reduction in Mildly Obese Women)

  • In previous studies, we performed joint animal studies and clinical trials between Yonsei University and Oryza Oil & Fat Chemical Co. Ltd. We have shown that coffee bean extract has potent anti-obesity and hypotriglyceridemic activities as well as beneficial effects on body fat reduction.In this study, the effects of coffee bean extract (100 mg/capsule) on body fat reduction were evaluated in overweight/obese women (body mass index of 25~30 $kg/m^2$ or body fat > 30%) not diagnosed with any type of disease. Subjects were randomly assigned to a coffee bean extract group (n=10) or placebo group (n=10). We measured anthropometric parameters, abdominal fat distribution by computed tomography and blood components before and after the 8week intervention period. After supplementation, the coffee bean extract group showed body weight (p=0.08), body mass index (p=0.06), hip circumference (p<0.05), and upper waist circumference (p< 0.01). In addition, after 8 weeks, the coffee bean extract group showed a significant decrease in abdominal internal fat area compared to 0 weeks (0 weeks : $155.8cm^2$; 8 weeks : $145.9cm^2$, ${\Delta}$ change : $-9.9cm^2$, respectively). However, there were no significant differences in lipid profiles or serological measurements between the coffee bean extract group and placebo group. The results of our human study demonstrated that coffee bean extract supplementation for 8 weeks has beneficial effects on reducing abdominal internal fat area as well as hip and waist circumferences.

마황복용이 성인의 체중감소에 미치는 영향에 대한 이중맹검 임상연구 (Effects of Mahuang for Weight Loss in Healthy Adults : A Double-Blind, Controlled, Randomized, Clinical Trial)

  • Objectives : To provide an objective assessment of the effectiveness for weight loss of Mahuang. Methods : Effects were assessed by measuring changes in body weight, body mass index (BMI), body fat (kg, %),body muscle, and abdominal f3t ratio. The subjects of this study were healthy adults who agreed to participate in this study. Eligibility requirements include age between 20 and 50 and BMI more than 18.5 kg/m2. One hundred subjects were randomly assigned to two groups (control and Mahuang group) in a double-blind manner. Mahuang group subjects took 8g of Mahuang capsule/day for two weeks; control subjects took the same amount of indistinguishable placebo. We analyzed the body weight, body mass index, body fat, abdominal fat, body muscle and BMR (basal metabolic rate) before and after the study. Result After two weeks of clinical trial, body weight and BMI decreased in both groups, but the Mahuang group showed more significant decrease. On body fat, the Mahuang group showed significant decrease. while it increased in the placebo group. Although body muscle, body fat ratio and basic metabolic rate showed differences between the two groups, there was no statistical significance. Conclusion : It is suggested that intake of Mahuang may induce weight loss and body f3f decrease in healthy adults.

  • Obesity is considered as a primary health problem over the past century in line with life environmental changes. It is mainly associated with increased risk of numerous chronic diseases which may significantly reduce health-related quality of life. Therefore, efforts to reduce weight should be performed. This study suggested a novel approach to reduce body fat by applying external stimulation which is micro-current stimulation (MCS). In this clinical study, we evaluated the potential effects of MCS for reduction of abdominal fat and weight loss. Prior to the clinical test, computational simulation was conducted to find the proper MCS conditions that allow externally applied stimulation to reach the internal fat section from the external skin. Particularly, the clinical study evaluated the unilateral effects of MCS for body fat loss and lipolysis without any additional limitations such as physical exercise and dietary therapy. The results showed that whole body fat, waist circumferences, and abdominal fat are gradually decreased after intervention in proportion to the time. From the results, we can estimate that MCS can be effective on the body fat loss by activation of lipolysis in human adipose.

근전도, 초음파와 DITI를 이용한 전기자극의 성인여성 복부비만 개선 효과 관찰 (The Effects of Functional Electrical Stimulation on Abdominal Obesity Improvement of Adult Women by EMG, Ultrasound and DITI)

  • In this study, we investigated the improvement effect of obesity by treatment with a developed low frequency electrical stimulation system. Thirty female in their 20's as an experiment subjects divided 3 groups(control, commercialized device, developed device) were treated with electrical stimulation on abdomen for 4 weeks. The body weight, body mass index(BMI), waist-hip ratio(WHR), muscle strength, muscle(transverse abdominis(TrA), internal obliquus abdominis(IO), external obliquus abdominis(EO)), fat thickness and abdominal surface temperature were measured by electromyogram(EMG), ultrasound and digital infrared thermal image(DITI). In the result, the body weight and BMI were decreased. Subcutaneous abdominal fat were significantly reduced after 4 weeks. The muscle strength and TrA muscle thickness was increased 13.2%(p<0.05), and 35.5%, respectively. The fat thickness showed decrease in abdomen (p<0.05). Infrared measurement on abdominal surface temperature as a parameter of improvement in blood circulation was significantly increased(p<0.05). Therefore, the low frequency stimulation showed positive effects on parameters of the obesity improvement.

Development and Validation of a Deep Learning System for Segmentation of Abdominal Muscle and Fat on Computed Tomography

  • Objective: We aimed to develop and validate a deep learning system for fully automated segmentation of abdominal muscle and fat areas on computed tomography (CT) images. Materials and Methods: A fully convolutional network-based segmentation system was developed using a training dataset of 883 CT scans from 467 subjects. Axial CT images obtained at the inferior endplate level of the 3rd lumbar vertebra were used for the analysis. Manually drawn segmentation maps of the skeletal muscle, visceral fat, and subcutaneous fat were created to serve as ground truth data. The performance of the fully convolutional network-based segmentation system was evaluated using the Dice similarity coefficient and cross-sectional area error, for both a separate internal validation dataset (426 CT scans from 308 subjects) and an external validation dataset (171 CT scans from 171 subjects from two outside hospitals). Results: The mean Dice similarity coefficients for muscle, subcutaneous fat, and visceral fat were high for both the internal (0.96, 0.97, and 0.97, respectively) and external (0.97, 0.97, and 0.97, respectively) validation datasets, while the mean cross-sectional area errors for muscle, subcutaneous fat, and visceral fat were low for both internal (2.1%, 3.8%, and 1.8%, respectively) and external (2.7%, 4.6%, and 2.3%, respectively) validation datasets. Conclusion: The fully convolutional network-based segmentation system exhibited high performance and accuracy in the automatic segmentation of abdominal muscle and fat on CT images.

Gene Expression of Heart and Adipocyte Fatty Acid-binding Protein in Chickens by FQ-RT-PCR

  • This study was to detect the expression of heart fatty acid-binding protein (H-FABP) and adipocyte fatty acid-binding protein (A-FABP) gene mRNA in different tissues of Rugao and Luyuan chickens at 56 d and 120 d by real-time fluorescence quantitative reverse transcription polymerase-chain reaction (FQ-RT-PCR). The primers were designed according to the sequences of HFABP, A-FABP and GAPDH genes in Gallus gallus, which were used as target genes and internal reference gene, respectively. The levels of H-FABP and A-FABP gene expression were detected by SYBR Green I FQ-RT-PCR. The relative H-FABP and A-FABP gene mRNA expression level was calculated with 2-$^{{\Delta}Ct}$. Melting curve analysis showed a single peak of three genes. Intramuscular fat (IMF) content in breast muscle and leg muscle of the two chicken breeds at 120 d was higher than at 56 d. IMF content in breast muscle and leg muscle at 56 d and 120 d in Luyuan was significantly higher than in Rugao, however, abdominal fat of Luyuan was significantly lower than that of Rugao. The relative H-FABP gene mRNA expression level in cardiac muscle was the highest in both chicken breeds. The relative H-FABP and A-FABP gene expression of different tissues in Luyuan was higher than in Rugao. H-FABP gene mRNA expression had a negative effect on IMF of leg and breast muscles, and was significantly negatively correlated with IMF content. The relative A-FABP gene mRNA level in abdominal fat was higher than in liver. The A-FABP gene mRNA was not expressed in leg, breast and cardiac muscles. A-FABP gene mRNA expression level was significantly positively correlated with abdominal fat and had a significant effect on abdominal fat but not IMF content.

Effect of Dietary Conjugated Linoleic Acid (CLA) on Abdominal Fat Deposition in Yellow-feather Broiler Chickens and Its Possible Mechanism

  • A total of 60 one-day-old Yellow-feather broiler chickens were allotted into treatment and control groups. The treatment group was fed with the diet supplemented with 3% conjugated linoleic acid (CLA) for 48 d, while control group was fed with the diet supplemented with 3% rapeseed oil. Chickens were slaughtered in each group at the age of 49 d, and the blood and the abdominal adipose tissue were sampled. Serum cLeptin and serum cAdiponectin were measured by ELISA. The total RNA was extracted from adipose tissue to measure the abundance of the chicken growth hormone receptor (cGHR), insulin-like growth factor 1 (cIGF-1), insulin-like growth factor I receptor (cIGF-IR), peroxisome proliferator-activated receptor gamma ($cPPAR{\gamma}$), cAdiponectin and cAdipoIR mRNA by RT-PCR using ${\beta}$-actin as an internal standard. Results showed that the CLA decreased the abdominal fat index by 20.93% (p<0.05). The level of serum cLeptin but not serum cAdiponectin was significantly increased by CLA treatment (p<0.05). CLA down-regulated the relative abundance of cGH-R mRNA and $cPPAR{\gamma}$ mRNA in abdominal adipose tissue by 24.74% (p<0.05) and 66.52% (p<0.01) respectively. However, no differences were found between CLA treatment group and control group (p>0.05) in the relative abundance of cIGF-1, cIGF-IR, cAdiponectin, and cAdipoIR mRNA in abdominal adipose tissue. The data suggested that CLA inhibited abdominal fat deposition in broiler chicken may be determined by decreasing the GHR available for GH, and by inhibiting the differentiation of preadipocytes via down-regulation of $PPAR{\gamma}$, but independent of IGF and (or) GH-IGF pathway or adiponectin action.
