Bronchopulmonary Sequestration with Dual Arterial Supply from Celiac Artery and Thoracic Aorta

  • Bronchopulmonary sequestration (BPS) is a rare congenital malformation of the lower respiratory tract. Most intralobar BPSs are provided with an arterial blood via the thoracic or abdominal aorta but such a supply is rarely found in patients older than 50 years. We report a case of an intralobar BPS with a dual arterial supply from the celiac artery and thoracic aorta in a 50-year-old man presenting with a respiratory tract infection and haemoptysis. To our knowledge, this is the first case report of a BPS supplied by the celiac artery and thoracic aorta in a 50-year-old man.

  • A 34-year-old woman experienced a sudden cardiac arrest after complaining of abdominal pain. The cause of that serious event was a hidden hemorrhagic shock. On computed tomography of her pelvic area, we found that her left common iliac artery had been ruptured. No bone fractures were observed. Her angiography showed neither atherosclerosis nor an aneurysm of the artery. Because spontaneous ruptures of the common iliac artery are rare, we suspected, based on her husband's statement, that a hidden blunt trauma to the artery had occurred via an endo-vaginal route due to dildo masturbation. Unfortunately, she died without recovery, in spite of our having controlled the bleeding by using an angiographic endovascular stent-graft.

심방중격결손증을 동반한 관상동맥루 - 1례 보고 - (Coronary Artery fistula Associated with Atrial Septal Defect -Report of one case-)

  • 좌전하행지의 근위부에서 기시하여 주폐동맥으로 연결되는 관상동맥루와 심방중격결손증을 동반한 한 환자를 보고한다. 환자는 56세 남자 환자로 운동성 호흡곤란과 복부팽만을 주소로 입원하였다. 심초음파와 관상동맥조영술상 심방중격결손증에 동반되어있는 관상동맥루를 발견하였으며 이는 좌전하행지와 주폐동맥사이에 존재하였다. 환자는 심실세동하 심폐기 가동상태에서 수술이 시행되었다. 심방중격결손은 자가심낭막을 이용하여 패취 봉합하였고 동맥루의 주폐동맥내 개구부를 직접 결찰 봉합하였다 환자의 술후 경과는 양호하였고 특별한 합병증 없이 퇴원하였다.

The Superficial Inferior Epigastric Artery Flap and its Relevant Vascular Anatomy in Korean Women

  • Background Lower abdominal soft tissue transfer is the standard procedure for breast reconstruction. However, abdominal wall weakness and herniation commonly occur postoperatively at the donor site. To reduce the morbidities of the donor site, the superficial inferior epigastric artery (SIEA) flap was introduced, but inconsistent anatomy of the SIEA has reduced its utility. In the present study, the anatomy of the superficial inferior epigastric vessels in Korean women was determined with regards to breast reconstructive surgery. Methods The vascular anatomies of the SIEA and superficial inferior epigastric vein (SIEV) were evaluated on 32 breast cancer patients receiving free transverse rectus abdominis musculocutaneous flap reconstruction after mastectomy. The existence, pulsation, location, external diameter, and depth of the SIEA and SIEV were measured at the lower abdominal incision level. Results SIEA and SIEV were present in 48/64 (75.00%) and 63/64 (98.44%) hemi-abdomens, respectively. Pulsation of the SIEA was found in 44/48 (91.67%) cases. The mean locations of SIEA and SIEV were +5.79 (${\pm}12.87$) mm, and -8.14 (${\pm}15.24$) mm from the midpoint between the anterior superior iliac spine and symphysis pubis, respectively. The mean external diameters of SIEA and SIEV were 1.20 (${\pm}0.39$) mm and 1.37 (${\pm}0.33$) mm, and they were found at a mean depth of 9.75 (${\pm}2.67$) mm and 8.33 (${\pm}2.65$) mm, respectively. Conclusions The SIEA was absent in 25% of Korean women and had a relatively small caliber. Therefore, careful preoperative assessment of the lower abdominal vasculature is required to achieve successful breast reconstruction using SIEA flaps.

Abdominal-based adipocutaneous advancement flap for reconstructing inguinal defects with contraindications to standard reconstructive approaches: a simple and safe salvage reconstructive option

  • Background Groin wounds occurring after vascular surgical site infection, oncologic resection, or occasionally orthopedic surgery and trauma may represent a surgical challenge. Reconstruction of these defects by the usual workhorse flaps may be contraindicated following previous surgery and in patients with lower limb lymphedema or extreme morbidity. Methods This study included 15 consecutive patients presenting with inguinal wounds after vascular or general surgery that required debridement and soft tissue coverage. All cases had absolute or relative contraindications to conventional reconstructive techniques, including a compromised deep femoral artery network, limb lymphedema, scarring of potential flap harvesting sites, or poor overall condition. Abdominal adipocutaneous excess enabled the performance of adipocutaneous advancement flaps in an abdominoplasty-like fashion. Immediate and long-term outcomes were analyzed. Results Soft tissue coverage was effective in all cases. Two patients required re-intervention due to flap-related complications (venous congestion and partial flap necrosis). All patients fully recovered over a mean±standard deviation follow-up of 2.4±1.5 years. Conclusions Abdominal flaps can be an effective and simple alternative technique for inguinal coverage with reproducible outcomes. In our experience, the main indications are a compromised deep femoral artery network and poor thigh tissue quality. Relative contraindications, such as previous open abdominal surgery, should be considered.

심장의 수축운동과 관상동맥 혈류와의 위상차가 관상동맥 혈관의 동맥경화 민감성에 미치는 영향 (Influence of Cardiac Contraction and its Phase Angle with Coronary Blood flow on Atherosclerosis of Coronary Artery)

  • 관상동맥은 체순환 동맥과 비교하여 매우 다른 유동조건을 갖는다. 본 연구에서는 이러한 관상동맥의 유동조건이 다른 혈관과 비교하여 동맥경화의 생성과 발전에 어떤 영향을 미치는지를 협착된 혈관 모델의 혈류역학적 전산유체해석을 통해 분석하였다. 연구 결과, 협착된 관상동맥 모델의 유동장과 벽면전단응력의 분포는 복부 대동맥 및 고동맥과 비교하여 큰 차이점을 보였다. 관상동맥은 혈관 지름이 혈류량에 비해 상대적으로 작으므로 큰 벽면전단응력값을 보이는 반면에, 혈관 협착부 목 후방부에 하나의 보텍스만 보이는 단순한 유동장 분포를 보였다. 한편, 복부대동맥과 고동맥 모델은 협착부 목 전반부와 후방부에 여러 개의 보텍스를 나타내는 복잡한 유동장을 형성하였다. 관상동맥 모델에서는 임피던스 페이즈앵글이 음의 큰 값을 가질 때에 벽면전단응력의 크기가 증가하고 재순환영역이 증가함이 관찰되었다. 그러므로 심장의 수축운동과 -110。에 이르는 관상동맥의 큰 음의 임피던스 페이즈앵글 값은 동맥경화를 촉진시키는 유동환경을 조성함을 알 수 있었다.

  • Pulmonary sequestration is an uncommon congenital pulmonary malformation characterized by presence of nonfunctioning lung tissue which receives its blood supply mostly from the anomalous systemic arteries - descending thoracic aorta or abdominal aorta, subclavian artery, innominated artery and internal mammary artery, etc. In our country, 39 cases were reported previously. The patient was a 40 years old woman and admitted due to productive cough for 1 year. The chest X-ray and chest C-T showed a dense mass containing a large cavity with air-fluid level and multiple radiolucent cysts in the right lower lung field. On the operative field, we could identify an aberrant large artery [ $\phi$7mm which arose directly from the descending thoracic aorta at eighth thoracic spinal level and fed the sequestrated portion of the right lower lobe. The aberrant artery was double ligation after division. Only sequestrated lobe on the superolateral lesion of the right lower lobe was resected because of nonseparated lobes in all the right lobes. An abnormal vein and bronchiole were ligated with black silk. The patient`s postoperative course was unevenful.

External Iliac Artery Transection Managed by Iliofemoral Bypass Grafting Using Temporary Balloon Occlusion

  • Traumatic abdominal vessel injury is rare, but difficult to manage. Approaching the injured vessel and controlling the bleeding is very hard. We experienced the right iliac artery transection managed by iliofemoral bypass grafting using temporary balloon occlusion. Proximal occlusion of an iliac artery with a temporary balloon cab be an option or bridge technique for a definite operation in case of iliac artery rupture. So, we present our case.

  • A 74-year-old patient presented with recurrent aneurysms in the infrarenal abdominal aorta and right common iliac artery 6 years after endovascular aortic repair using endografts in the same location. The patient underwent an aorto-bi-iliac replacement with removal of the stent graft. Two holes measuring 2 mm each were found in the removed graft, and they appeared to have been caused by wear from continuous friction between the endograft and the aortic wall.

Successful nonoperative management of a simultaneous high-grade splenic injury and devascularized kidney in Australia: a case report

  • Severe blunt injuries to isolated solid abdominal viscera have been previously managed nonoperatively; however, management algorithms for simultaneous visceral injuries are less well defined. We report a polytrauma case of a 33-year-old man involved in a motorbike collision who presented with left-sided chest and abdominal pain. Initial imaging demonstrated multiple solid organ injuries with American Association for the Surgery of Trauma (AAST) grade V splenic injury and complete devascularization of the left kidney. The patient underwent urgent angioembolic coiling of the distal splenic artery with successful nonoperative management of simultaneous grade V solid organ injuries.
