Total Occlusion of the Abdominal Aorta Caused by Detachment of Cardiac Myxoma

  • Abdominal aortic occlusion (AAO) caused by detachment of cardiac myxoma (CM) is a very rare complication in patients with CM. Although the nature of CMs has been well established, detachment of CM may cause unexpected serious complications such as vicious embolic events. Actually, in several cases of AAO caused by detachment of CM, it has been reported that CM fragments easily migrated to the brain, heart, and lungs, and caused lifelong neurological complications despite appropriate surgical therapy. Herein, we report a case of a patient with AAO caused by detachment of CM who underwent CM excision and abdominal aortic thromboembolectomy. Additionally, we have presented the preoperative and postoperative images using 64-multidetector computed tomography.

급성 마비성 장폐색 환자 치험 1례 (A Case Report on Patient of Acute paralyic ileus with past history of mild depressive episode)

  • Objective : The purpose of this case is to report the improvement of a patient with acute paralytic ileus with past history of mild depressive episode after acupuncture therapy and herbal medicine. Methods : We treated the patient, who had acute abdominal pain and were diagnosed as paralytic ileus, with acupuncture, herbal medicine and manipulation therapy. We observed changes of chief symptoms and abdominal states. Results : We treated the patient who had severe abdominal pain, constipation and anorexia caused by acute paralytic ileus. For about one month of the treatment, we had the improvement of the symptoms. Conclusions : This study suggests that Conservative korean medical treatment might be useful for abdominal pain caused by acute paralytic ileus, and make a patient feel psychological stability.

  • We developed a portable urodynamics monitoring system using personal digital assistance (PDA) and proposed a new method for measuring the abdominal pressure in non invasive mode using surface EMG signals. This system is consisted of a signal conditioning and control module and could measure the vesical, abdominal and detrusor pressure. The result showed a high correlation coefficient between the practical abdominal pressure, using a conventional rectal catheter and the estimated values, by our proposed algorithm (r=0.81). This system should prove a useful tool in the future evaluations of ambulatory urodynamics monitoring study.

마그네슘 부족식이가 수술받은 쥐의 Stress 반응에 미치는 영향 (Effect of Dietary Magnesium on Stress Reactions in Rats with Abdominal Surgery)

  • This study was performed to investigate the effect of dietary magenesium on stress reactions in rats having abdominal surgery. Sixty three male rats of sprague-dawley strain were blocked into 3 groups : rats fed regular magnesium (0.05% Mg: control) rats receiving regular magnesium with surgery(Mg-adeq : S) Five weeks after feeding abdominal surgery was performed and randomly chosen 7 rats from each group were sacrificed on 1, 3 and 5 days after surgery. Te following were found ; 1) Rats fed marginal magnesium showed significantly elevated urinary urea nitrogen urinary potassium and plasma glucose compared controls only one day after abdominal surgery but not 3 days or 5 days after surgery 2) Rats fed adequate magnesium did not show any significant change in metabolic stress indicator after surgery. 3) Plasma free fatty acid and cortisol level were not different among groups. 4) Decreased plasma magnesium and potassium level were found in rats fed marginal magne-sium and sacrificed one day and three days after surgery.

흉, 복부 전산화 단층촬영이 정상인 외상 환자에게 척추 단순촬영이 필요한가? (Is There a Need for Conventional Spine Radiographs Following a Negative Chest and Abdominal CT in Trauma Patients?)

  • Purpose: This was conducted to assess the need for conventional radiographs of the thoracolumbar spine for routine screening of trauma patients who revealed no spinal trauma on chest and abdominal computed tomography (CT). Methods: We performed a retrospective review of the medical records of trauma patients who underwent conventional radiographs of the thoracolumbar spine following a chest and abdominal CT that revealed no spinal trauma. Results: Two hundred seventy-five trauma patients underwent conventional radiographs of the thoracolumbar spine following a chest and abdominal CT that revealed no spinal trauma. In 274 of the cases, the thoracolumbar spine series was also negative. Conclusion: CT of the chest and abdomen is an adequate evaluation of the thoracolumbar spine in trauma patients who require routine thoracolumbar spine screening, making subsequent conventional radiographs of the thoracolumbar spine unnecessary.

  • Paragonimiasis is a parasitic disease caused by the lung fluke, Paragonimus spp. Lung flukes may be found in various organs, such as the brain, peritoneum, subcutaneous tissues, and retroperitoneum, other than the lungs. Abdominal paragonimiasis raises a considerable diagnostic challenge to clinicians, because it is uncommon and may be confused with other abdominopelvic inflammatory diseases, particularly peritoneal tuberculosis, and peritoneal carcinomatosis. Also, subcutaneous paragonimiasis does not easily bring up clinical suspicion, due to its rarity. We herein report 2 cases of abdominal paragonimiasis and 1 case of subcutaneous paragonimiasis in Korea.

  • Henoch-Scholein purpura is a systemic vasculitis of small blood vessels. It is characterized by nonthrombocytopenic purpura, abdominal pain, arthralgia and renal involvement. In Henoch-Scholein purpura, severe abdominal pain may be relived by steroid, but occasionally unresponsive to steroid and conventional analgesics therapy. We tried IV-gamma globulin for severe abdominal pain, unresponsive to steroid, analgesics, and antispasmodics therapy in HSP, and experienced dramatic symptomatic improvement. So we report three cases of IVIG therapy against severe abdominal pain in HSP with a brief review of literatures

Reconstruction of a Large Infected Midline Abdominal Wall Defect Using a Latissimus Dorsi Free Flap

  • Managing large infected midline abdominal defects are clinically challenging and technically demanding. The alloplastic materials, regional flaps, and component separation are usually infeasible because of the size, location, depth, and state of the defects. In these cases, the free flap is the only option with a large well-vascularized tissue that is free to inset regardless of the location. Herein, we report a case of 44-year-old man with a large infected midline abdominal wall defect who was completely treated with a latissimus dorsi myocutaeous free flap followed by negative pressure wound therapy.

  • Aspergillosis, caused by infection with Aspergillus fumigatus and less commonly by other Aspergillus species, is a prevalent and costly respiratory disease of poultry, In a flock of chicks, the number of birds, 4∼5 months old, had become gradually emaciated and subsequently died. Gross necropsy revealed multiple granulomatous masses on the abdominal serosa. The masses, 4∼15 mm in size, were attached on mainly intestinal wall. Also, the smaller masses in size were on mesentery and pancreas. However, only a few small white nodules were scattered throughout liver and lung in few samples. Microscopically, the mass were granulomatous with a central area of necrosis containing numerous septate, branched fungal hypae consistent with Aspergillus sp. These were surrounded by macrophages, giant cells, lymphocytes and fibrous tissues. Nodular lesions of liver and lung were seemed to spread hemotogenously from intestine and the possible route of infection was speculated by oral. This report is a Aspergillus-induced granuloma limited to the serosa of abdominal cavity, especially of intestinal wall.

Successful Endovascular Management of Intraoperative Graft Limb Occlusion and Iliac Artery Rupture Occurred during Endovascular Abdominal Aortic Aneurysm Repair

  • For high-risk patients, endovascular aortic aneurysm repair (EVAR) is a good option but may lead to serious complications, which should be addressed immediately. A 75-year-old man with a history of abdominal surgery underwent EVAR for an aneurysm of the abdominal aorta and iliac arteries. During EVAR, iliac artery rupture and graft limb occlusion occurred, and they were successfully managed by the additional deployment of an iliac stent graft and balloon thrombectomy, respectively. We, herein, report a rare case of the simultaneous development of the two fatal complications treated by the endovascular technique.
